Purpose: This study was performed to predict individualized treatment strategies in ruptured abdominal aortic aneurysm (rAAA) by estimating the survival benefit of endovascular aneurysm repair (EVAR) and open surgical repair (OSR) based on anatomical and physiological features using a causal inference model. Methods: This retrospective study included 45 patients with de novo rAAA who underwent EVAR or OSR between 2012 and 2024. Thirty-three variables were analyzed. The model estimated individualized treatment effects (ITE) for 30-day survival.Model interpretability was assessed using Shapley Additive Explanations (SHAP) analysis. Five-fold cross-validation, receiver operating characteristic (ROC) analysis, and calibration plots were used for model evaluation. A clinical decision tree was developed to derive simplified decision rules. Results: The mean ITE was 0.22 ± 0.42, with 33% of patients classified as OSR-benefit candidates. SHAP analysis revealed that suprarenal angle, infrarenal angle, iliac anatomy, and proximal neck characteristics strongly influenced treatment effects. However, some predictors, such as low hemoglobin and systolic blood pressure favoring OSR, conflicted with clinical intuition. ROC analysis showed an area under the curve of 1.00, but calibration suggested overfitting due to a small sample size. Treatment-matched patients had a higher 30-day mortality rate than mismatched patients, suggesting potential bias or unmeasured confounding. The decision tree identified clinically relevant features but displayed structural inconsistencies and impractical cutoff values due to the limited sample size. Conclusion The X-learner model demonstrated the feasibility of individualized treatment prediction in rAAA but suffered from overfitting and limited generalizability. Validation with larger multicenter cohorts is necessary to confirm clinical applicability.

Objective: Understanding the specific fears associated with coronavirus disease-2019 (COVID-19), particularly within different cultural contexts, is crucial for developing effective mental health interventions. This study aims to develop and validate the COVID-19 Infection Fear Scale (CIFS) in a collectivist cultural context such as Korea. Methods: A total of 1,002 adults aged 19 to 70 participated in an online survey in May 2020. The CIFS was developed through a multidisciplinary approach, categorizing public fears into two domains: fear of infection and fear of negative outcomes post-infection. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were conducted to validate the factor structure. Reliability and construct validity were assessed through correlations with anxiety (Generalized Anxiety Disorder-7), depression (Patient Health Questionnaire-9), suicidal ideation, and coping strategies. Results: The CIFS demonstrated high internal consistency. EFA and CFA supported a two-factor model. The Rasch analysis confirmed good item fit, with infit and outfit indices within the acceptable range. Differential item functioning analysis indicated minor sex and age biases, addressed without removing items. Construct validity was supported by significant correlations with anxiety, depression, suicidal ideation, and coping strategies. Fear of negative consequences post-infection showed a stronger correlation with psychological distress than fear of infection. Conclusion The CIFS is a reliable and valid tool for measuring fear related to COVID-19 infection and its consequences, particularly within a collectivist cultural context. This scale can aid in identifying individuals at higher risk of psychological distress and inform targeted interventions.

Objective: Chordoma, a rare malignant tumor originating from embryonal notochord remnants, exhibits high resistance to conventional treatments, making surgical resection imperative. However, the factors influencing prognosis specifically for cervical spine chordoma have not been clearly identified. We investigate the prognosis of cervical spine chordoma with factors influential in a nationwide multicenter retrospective study. Methods: This study included all patients diagnosed with cervical spine chordoma at 7 tertiary referral centers from January 1998 to March 2023, excluding those with clivus and thoracic spine chordomas extending into the cervical spine. Local recurrence (LR) was identified through follow-up magnetic resonance imaging, either as reappearance in completely resected tumors or regrowth in residual tumors. The study assessed LR and overall survival, analyzing factors influencing LR and death. Results: Forty-five patients with cervical spine chordoma had a mean age of 46.4 years. Over a median follow-up of 52 months, LR and distant metastasis were observed in 21 (46.7%) and 4 patients (8.9%), respectively, and 16 patients (36%) were confirmed dead. The 5-year and 10-year cumulative LR rates were 51.3% and 60%, respectively, while the 5-year and 10-year survival rates were 82% and 53%. Age was the only significant factor affecting mortality (hazard ratio, 1.04; 95% confidence interval, 1.04–1.07; p=0.015). Notably, the degree of resection and adjuvant therapy did not statistically significantly impact local tumor control and mortality. Conclusion This study, the largest multicenter retrospective analysis of cervical spine chordoma in Korea, identified age as the only factor significantly affecting patient survival.
