Diabetic nephropathy (DKD), as a common microvascular complication of diabetes mellitus (DM), is a major cause of end-stage renal disease (ESRD). Its clinical manifestations include increased urinary protein excretion, thickening of the glomerular basement membrane, and renal tubulointerstitial fibrosis. The pathogenesis of DKD is complex and involves multiple factors, including disordered glucose metabolism, hemodynamic alterations, and oxidative stress. Although modern medical approaches can alleviate certain symptoms, they still have limitations such as insufficient therapeutic targeting and prominent adverse effects. The transforming growth factor-β/Smad (TGF-β/Smad) signaling pathway is not only a tissue fibrosis pathway that has attracted considerable attention in recent years, but also regulates multiple protein molecules, including the glomerular podocyte slit diaphragm protein Podocin, interleukin-1β (IL-1β), and superoxide dismutase (SOD), thereby participating in various pathological processes and ultimately mediating renal injury. Flavonoid compounds, owing to their sustained pharmacological effects, broad spectrum of action, and high safety profile, have become ideal candidates for targeted therapy research in DKD. Existing studies have shown that these compounds can exert inhibitory effects on renal fibrosis, alleviate inflammatory responses, protect podocytes, and reduce oxidative stress by regulating the interactions between the TGF-β/Smad signaling pathway and the aforementioned protein molecules, thereby maintaining renal structure and function, reducing proteinuria, and significantly improving DKD lesions. This review briefly outlines the composition and functions of the TGF-β/Smad signaling pathway, elucidates the mechanisms by which this pathway regulates DKD, and focuses on summarizing major studies from the past decade on flavonoid-based interventions in DKD through targeted inhibition of the TGF-β/Smad signaling pathway. Furthermore, it discusses the considerable therapeutic potential of flavonoids in the treatment of this disease, aiming to provide a scientific basis for future clinical prevention and treatment of DKD and to promote the development of targeted drugs.

Journal of Environmental and Occupational Medicine 2026;43(2):242-246
Occupational hazards pose a major public health challenge that seriously endangers the health of the working population. As a statutory key component of occupational health work, occupational health risk assessment urgently requires more attention. Currently, commonly used occupational health risk assessment methods exhibit notable deficiencies such as insufficient comprehensive assessment capabilities and limited applicability. The absence of assessment methods that accurately align with occupational health practices has constrained the depth and breadth of occupational health risk assessment in China, resulting in inadequate risk identification and significant deficiencies in social service provision, decision-making support, and health protection. Meanwhile, under the current assessment system, the occupational health big data obtained from risk assessments, occupational health examinations, biological monitoring, and other sources suffer from issues such as fragmented resources and low utilization rates. As a result, existing systems are unable to effectively support critical operational needs, including accurate risk early warning, timely risk response, and refined risk management. This study systematically reviewed the common occupational health risk assessment methods and the current status of occupational health risk assessment and big data applications in China, identified existing shortcomings, proposed a comprehensive risk assessment and risk prediction-early warning scheme based on the application of occupational health big data. The scheme is expected to provide theoretical foundations and methodological references for advancing comprehensive risk assessment of occupational health through big data analytics, and to enhance the level of occupational health management, thereby safeguarding worker health and promoting sustainable socioeconomic development.

BackgroundPersistent cognitive impairment is prevalent among patients with stable schizophrenia. While serum total bile acid (TBA) level in acute-phase patients are known to be associated with cognitive dysfunction, the relationship between serum TBA and multi-dimensional cognitive functions in stable phase patients remains unclear. ObjectiveTo investigate the correlation between serum TBA level and cognitive function in patients with stable schizophrenia, and to evaluate its predictive value for cognitive impairment, thereby providing a serological biomarker for the timely identification and objective assessment of cognitive dysfunction. MethodsA cross-sectional study was conducted on 137 inpatients with stable schizophrenia at The Fourth People's Hospital of Yancheng from March to December 2024. All participants met the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5). Cognitive function was evaluated using the Chinese Brief Cognitive Test (C-BCT), patients were categorized into four groups: normal cognition (n=28), mild impairment (n=28), moderate impairment (n=47), and severe impairment (n=34). Fasting venous blood samples were collected, and serum TBA level was quantified using an enzymatic cycle assay. Spearman correlation analysis was ultilized to determine the relationship between serum TBA level, overall cognitive function, and specific cognitive domains. Binary Logistic regression model was used (adjusting for covariates such as age, gender, and disease duration) to analyze the impact of serum TBA level on overall and individual cognitive functions. The predictive value of serum TBA level for overall cognitive impairment was evaluated using receiver operating characteristic (ROC) curve. ResultsSerum TBA levels differed significantly among the four groups (H=18.677, P<0.01). Specifically, serum TBA levels in both the moderate and severe cognitive impairment groups were significantly higher than those in the normal cognitive group (adjusted P<0.01). Serum TBA level was positively correlated with the severity grading of overall cognitive impairment (rs=0.354, P<0.05), and negatively correlated with T-scores on the trail making test (rs=-0.328, P<0.05), continuous performance test (rs=-0.247, P<0.05), digit span (rs=-0.265, P<0.05), and symbol coding (rs=-0.221, P<0.05). Binary Logistic regression analysis identified serum TBA level as an independent risk factor for overall cognitive impairment (OR=1.322, 95% CI: 1.021 - 1.713, P=0.034), with a particularly robust predictive ability for impaired information processing speed (OR=1.325, 95% CI: 1.057 - 1.661, P=0.015). The area under ROC curve (AUC) for serum TBA level in predicting overall cognitive impairment was 0.738, with a sensitivity of 60.61% and a specificity of 78.64%. ConclusionIn patients with stable schizophrenia, elevated serum TBA levels are associated with worse overall cognitive function, as well as deficits in information processing speed, attention, working memory, and executive function. Serum TBA serves as an independent risk factor and exhibits moderate predictive value for overall cognitive impairmen, particularly in the domain of information processing speed. BACKGROUND:Cannabidiol is effective in ameliorating the body's inflammatory response, but no clear mechanistic studies have been conducted to ameliorate skeletal muscle inflammation induced by exhaustive exercise. OBJECTIVE:To explore the mechanism by which cannabidiol improves skeletal muscle inflammation during exhaustive exercise by using transcriptome sequencing technology. METHODS:Thirty-six Sprague-Dawley rats were randomly divided into six groups:blank control group, exercise coconut oil group, exercise control group,50 mg/kg cannabidiol group,60 mg/kg cannabidiol group, and 70 mg/kg cannabidiol group, with six rats in each group.Except for rats in the blank control group, rats in each group were subjected to swimming exercise for 9 days to produce the exhaustive exercise model. At the end of each swimming exercise, rats in the cannabidiol groups were given 2 mL of fat-soluble cannabidiol at different concentrations(50,60,and 70 mg/kg)by gavage;rats in the exercise coconut oil group were given the same volume of coconut oil by gavage until the end of the exercise on the 9th day;and rats in the blank control group and the exercise control group were not given any special treatment. The levels of inflammatory factors and differentially expressed genes in the skeletal muscle of rats in each group were determined using ELISA and transcriptome sequencing techniques.Differentially expressed genes obtained were subjected to KEGG analysis, and the accuracy of the sequencing data was verified by fluorescence quantitative PCR. RESULTS AND CONCLUSION:The results of ELISA showed that the contents of interleukin-6(P<0.05),tumor necrosis factor-α(P<0.01),interleukin-10 and other inflammatory factors in the exercise group increased significantly compared with the blank control group and the coconut oil group. After cannabidiol intervention, the mass concentrations of interleukin-6 and tumor necrosis factor-α showed a sequential decrease with increasing cannabidiol concentration.By comparing GO and KEGG databases, the functional properties of differentially expressed genes were analyzed, and the results showed that the differentially expressed genes were mainly involved in the tumor necrosis factor signaling pathway and the Toll-like receptor signaling pathway.RT-qPCR results showed that the trends of five randomly selected differentially expressed genes were in agreement with the transcriptome sequencing results. To conclude, cannabidiol can improve skeletal muscle inflammation caused by exhaustive exercise.

Objective To explore the association between dynamic indexes of maternal progesterone in early pregnancy and the average level and growth rate of fetal head circumference(HC)in mid-and late pregnancy. Methods This study adopted a retrospective cohort design and included 255 singleton pregnant women in the maternal and infant cohort of The First Affiliated Hospital of Xi'an Jiaotong University from January 2015 to December 2024.The progesterone levels of their early venous blood were detected and divided into two groups of progesterone trajectories, namely, fluctuating type and increasing type,by morphology. The dynamic indicators of progesterone in early pregnancy were constructed:cumulative dynamic deviation index in the first trimester(CDDI-P1T),gestational age at progesterone peak(GA-PP),and maximal relative progesterone decline in the first trimester(MRD-P1T).The average head circumference level and linear growth rate of the fetuses in the middle and late pregnancy were calculated.Generalized linear regression(GLM)was used to analyze the linear association between progesterone dynamic indicators and head circumference development. The key gestational weeks of progesterone affecting fetal head development were explored by linear regression of gestational weeks.Ordinary least squares(OLS)regression and restricted cubic spline(RCS)plots were used to draw the nonlinear association between progesterone dynamic indicators and head circumference. Results Among the 255 pregnant women included,92.5%of the progesterone trajectories in early pregnancy were fluctuating, and 7.5%were increasing. The growth rates of the increasing progesterone trajectory group were higher in the second and third trimesters than in the fluctuating group, but the differences were not statistically significant(all P>0.05).GLM analysis showed that for every 1 unit increase in CDDI-P1T,the head circumference in the middle and late pregnancy increased significantly by 1.574 cm and 1.193 cm(Z=3.714,2.885,P<0.01).The delay of GA-PP was negatively correlated with the head circumference in the middle pregnancy(β=-0.190 cm,95%CI:-0.339--0.041,P=0.010)but positively correlated with the head circumference growth rate in the late pregnancy(β=0.022 cm/week,95%CI:0.003-0.041,P=0.025).A 10%decrease in the decline of CDDI-P1T increased the head circumference in the middle pregnancy by 0.200 cm(95%CI:0.016-0.384,P=0.033),and a 100%decrease in the decline increased the head circumference growth rate in the late pregnancy by 0.201 cm/week(95%CI:0.002-0.399,P=0.048).The analysis of the key time window showed that for every 20 nmol/L increase in progesterone during 9.5-13 weeks of pregnancy, the mid-term head circumference increased by 0.035-0.166 cm(Z=2.452-3.517,allP<0.05),and the late-term head circumference increased by 0.767 cm during 9-13 weeks of pregnancy(Z=2.452-3.517,all P<0.05).When progesterone increased during 9.5-10.5 weeks of pregnancy, the growth rate of mid-term head circumference increased by 0.013-0.023 cm/week(Z=2.074-2.243,all P<0.01).When progesterone increased during 8.5-10.5 weeks of pregnancy, the growth rate of late-term head circumference increased by 0.010-0.026 cm/week(Z=2.061-3.137,all P<0.05).Conclusion Progesterone dynamic index is a new sensitive tool for evaluating fetal head circumference development. There is a stage-specific window period for progesterone regulation.9.5-13 weeks of pregnancy is the critical period for progesterone to affect head circumference growth, and 9.5-10.5 weeks of pregnancy is the core window for regulating the growth rate of head circumference. Therefore,it is necessary to combine progesterone dynamic index and time window for individualized intervention to promote the transformation of prenatal care from pregnancy maintenance to eugenics intervention.

AIM:This study aims to investigate the molecular mechanism by which tubeimoside I(TBMS1)inhibits Snail expression in pancreatic cancer cells(PANC-1).METHODS:Human pancreatic cancer PANC-1 cells were cultured in vitro. The inhibitory effect of TBMS1 on PANC-1 cells was assessed using the MTT assay, and the data were analyzed based on the IC50 value of TBMS1.The impact of TBMS1 on the clonal formation ability of PANC-1 cells was evaluated through colony formation assays. The Transwell assay was employed to assess the effect of TBMS1 on the migrato-ry capability of PANC-1 cells.Apoptosis and cell cycle alterations in PANC-1 cells were analyzed using acridine orange staining and flow cytometry. The expression of Snail protein in pancreatic cancer and its relationship with survival of the patients were analyzed using the GEPIA database and Kaplan-Meier Plotter data.Immunofluorescence staining was con-ducted to investigate the effect of TBMS1 on Snail expression, while Western blot was used to evaluate the expression of poly(ADP-ribose)polymerase(PARP),E-cadherin and Snail in the cells. The ubiquitination of Snail protein was mea-sured using immunoprecipitation techniques.RESULTS:As the concentration of TBMS1 increased, the survival rate and number of clones formed by PANC-1 cells progressively decreased, leading to apoptosis, cleavage of PARP,and cell cycle arrest in the G1 phase. There was also a reduction in the proportion of cells in the S phase and a decrease in cell migration ability. The expression of Snail protein,a critical factor in cell migration, was inhibited, while E-cadherin protein levels were increased.Treatment with the proteasome inhibitor MG132 was able to reverse the suppression of Snail protein ex-pression caused by TBMS1.Immunoprecipitation results indicated that TBMS1 enhances the ubiquitination and subse-quent degradation of Snail protein.CONCLUSION:TBMS1 effectively inhibits the malignant progression of pancreatic cancer cells by promoting the ubiquitination and degradation of Snail protein in PANC-1 cells.
