1.Predictive value of contrast-enhanced ultrasound in evaluating delayed graft function in kidneys from donation after brain death

Objective To investigate the predictive value of quantitative parameters of contrast-enhanced ultrasound (CEUS) in evaluating kidneys from donation after brain death (DBD) for the occurrence of delayed graft function (DGF) in recipients. Methods The clinical data of 134 DBD donors and 202 corresponding kidneys and recipients were retrospective analyzed. The recipients were divided into DGF group (n=39) and non-DGF group (n=163) according to the renal function after kidney transplantation. Conventional ultrasound, CEUS parameters, and clinical data were compared between the two groups. Receiver operating characteristic (ROC) curves were used to determine the optimal cut-off values for predicting DGF using CEUS parameters, clinical parameters, and their combination, based on the highest Youden index. The predictive ability of different parameters for DGF was evaluated. Results There were statistically significant differences in cortical peak intensity (PIc), medullary peak intensity (PIm), donor albumin (ALB), serum creatinine (Scr) after admission, and the Na+ concentration of recipients between the two groups (all P<0.05). The area under the curve (AUC) for predicting DGF using the combination of CEUS parameters PIc and PIm was 0.711, with an optimal cut-off value of 0.193 and a Youden index of 0.382. The AUC for predicting DGF using the combination of CEUS parameters PIc, PIm and clinical parameters was 0.808, with an optimal cut-off value of 0.191 and a Youden index of 0.517. The sensitivity and specificity were 0.769 and 0.613 for the former, and 0.769 and 0.748 for the latter, respectively. The AUC for predicting DGF using CEUS parameters PIc and PIm combined with clinical parameters was significantly higher than that using CEUS parameters PIc and PIm (P<0.05). Conclusions The CEUS quantitative parameters PIc and PIm have good predictive value in assessing kidneys from DBD donors for DGF in recipients, and the diagnostic efficacy is better when combined with clinical parameters.
2.Seroprevalence and influencing factors of low-level neutralizing antibodies against SARS-CoV-2 in community residents

ObjectiveTo understand the seropositivity of neutralizing antibodies (NAb) and low-level NAb against SARS-CoV-2 infection in the community residents, and to explore the impact of COVID-19 vaccination and SARS-CoV-2 infection on the levels of NAb in human serum. MethodsOn the ground of surveillance cohort for acute infectious diseases in community populations in Shanghai, a proportional stratified sampling method was used to enroll the subjects at a 20% proportion for each age group (0‒14, 15‒24, 25‒59, and ≥60 years old). Blood samples collection and serum SARS-CoV-2 NAb concentration testing were conducted from March to April 2023. Low-level NAb were defined as below the 25th percentile of NAb. ResultsA total of 2 230 participants were included, the positive rate of NAb was 97.58%, and the proportion of low-level NAb was 25.02% (558/2 230). Multivariate logistic regression analysis indicated that age, infection history and vaccination status were correlated with low-level NAb (all P<0.05). Individuals aged 60 years and above had the highest risk of low-level NAb. There was a statistically significant interaction between booster vaccination and one single infection (aOR=0.38, 95%CI: 0.19‒0.77). Compared to individuals without vaccination, among individuals infected with SARS-CoV-2 once, both primary immunization (aOR=0.23, 95%CI: 0.16‒0.35) and booster immunization (aOR=0.12, 95%CI: 0.08‒0.17) significantly reduced the risk of low-level NAb; among individuals without infections, only booster immunization (aOR=0.28, 95%CI: 0.14‒0.52) showed a negative correlation with the risk of low-level NAb. ConclusionsThe population aged 60 and above had the highest risk of low-level NAb. Regardless of infection history, a booster immunization could reduce the risk of low-level NAb. It is recommended that eligible individuals , especially the elderly, should get vaccinated in a timely manner to exert the protective role of NAb.
3.A Randomized Controlled,Double-Blind Study on Huaban Jiedu Formulation (化斑解毒方) in the Treatment of Psoriasis Vulgaris with Blood-Heat Syndrome

ObjectiveTo evaluate the clinical efficacy and safety of Huaban Jiedu Formulation (化斑解毒方, HJF) in treating psoriasis vulgaris with blood-heat syndrome. MethodsA randomized, double-blind, placebo-controlled study was conducted with 60 patients diagnosed with psoriasis vulgaris of blood-heat syndrome. Patients were randomly assigned to either a treatment group or a control group, with 30 cases in each. The treatment group received HJF granules orally, one dose a day, combined with topical Qingshi Zhiyang Ointment (青石止痒软膏), while the control group received placebo granules, one dose a day, combined with the same topical ointment. Both groups were topically treated twice daily of 28 days treatment cours. Psoriasis area and severity index (PASI), visual analogue scale for pruritus (VAS), traditional Chinese medicine (TCM) syndrome scores, dermatology life quality index (DLQI), and psoriasis life stress inventory (PLSI) were assessed before treatment and on day 14 and day 28. Response rates for PASI 50 (≥50% reduction) and PASI 75 (≥75% reduction), as well as overall clinical efficacy, were compared between groups. Serum levels of interleukin-6 (IL-6) and interleukin-17 (IL-17) were measured before and after 28 days of treatment. Adverse reactions during treatment were recorded. ResultsAfter 28 days of treatment, both groups showed significant reductions in PASI total score, lesion area score, erythema, scaling, and infiltration scores, pruritus VAS score, TCM syndrome score, DLQI, PLSI, and serum IL-6 and IL-17 levels (P<0.05). Compared to the control group, the treatment group had significantly greater improvements in PASI total score and erythema score, TCM syndrome score, serum IL-6 and IL-17 levels, and PASI 50 response rate after 28 days (P<0.05). Between-group comparisons of score differences before and after 28-day treatment revealed that the treatment group showed significantly better improvements in PASI total, lesion area score, erythema score, TCM syndrome score, DLQI, PLSI, and inflammatory markers (P<0.05 or P<0.01). The total effective rate on day 14 and day 28 was 40.00% (12/30) and 83.33% (25/30) in the treatment group, versus 6.90% (2/29) and 41.38% (12/29) in the control group, respectively. The clinical efficacy in the treatment group was significantly superior to that in the control group (P<0.05). Mild gastric discomfort occurred in 3 patients in the treatment group and 1 in the control group. ConclusionHJF can effectively improve skin lesions and TCM symptoms relieve pruritus, enhance quality of life, and reduce inflammatory markers IL-6 and IL-17, in patients with blood-heat syndrome of psoriasis vulgaris, with a good safety profile.
4.Relationship between SERPINE1 gene related to lactic acid metabolism and clinical prognosis of gastric cancer patients

Objective To explore the relationship between SERPINE1 gene related to lactic acid metabo-lism and clinical prognosis of gastric cancer(GC).Methods A total of 89 patients with GC who underwent surgical resection in the hospital from January 2018 to December 2020 were enrolled in the study.In the pre-liminary experiment,the levels of 5 genes related to lactic acid metabolism(F5,MTTP,SERPINE1,CYP19A1,SLC52A3)in 24 serum samples from GC patients with stage Ⅰ(n=12)and stage Ⅳ(n=12)were analyzed.In the second step,the candidate genes with significant changes in serum of GC patients with stage Ⅳ and GC patients with stage Ⅰ in the preliminary experiment were verified in a larger independent co-hort,which included serum samples of 65 GC patients and 22 healthy controls.Results Among the genes re-lated to lactic acid metabolism(F5,MTTP,SERPINE1,CYP19A1,SLC52A3)analyzed in the preliminary ex-periment,SERPINE1 was significantly up-regulated in the serum of patients with stage Ⅳ compared with that in the serum of patients with stage Ⅰ(P<0.001).The expression level of SERPINE1 in serum of GC pa-tients was significantly lower than that of healthy controls(P<0.000 1).In addition,the expression level of SERPINE1 increased with the progress of TNM staging(P<0.05).Based on the receiver operating charac-teristic curve,the maximum predictive value of SERPINE1 on overall survival(OS)rate was analyzed(area under the curve=0.725,cut off value=0.002 1),and GC patients were divided into high expression group(n=39)or low expression group(n=26).Compared with the SERPINE1 low expression group,the cases of venous infiltration,T3/T4 stage,lymph node metastasis and distant metastasis in the SERPINE1 high expres-sion group increased,and the differences were statistically significant(P<0.05).The OS rate of GC patients with low expression of SERPINE1 was significantly higher than that of patients with high expression of SER-PINE1(P<0.05).Multivariate analysis showed that the high expression of SERPINE1 in serum(HR=4.51,95%CI=1.23-23.69,P=0.021)was an independent prognostic marker for predicting the poor prog-nosis of GC patients.Conclusion SERPINE1 gene related to lactic acid metabolism in serum could be used as a reliable biomarker for the prognosis of GC.

6.Advances in non-surgical animal models of vascular cognitive impairment

Vascular cognitive impairment(VCI)includes a range of illnesses from mild cognitive impairment to dementia,attributable to cerebrovascular factors.Although appropriate animal models are needed to allow clinicopathological research and drug development,there are currently no animal models that can perfectly simulate the pathogenesis of VCI.At present,carotid artery and vertebral artery stenosis or occlusion are the main method for VCI modeling;however,increasing numbers of non-surgical method have recently emerged,providing new ideas and prospects for the study of this disease.In this paper,we consider the construction method,model mechanisms,and model characteristics of non-surgical animal models of VCI,to provide a reference to help researchers choose the most suitable animal model.
7.Sedative effect of ciprofol combined with oxycodone on elderly patients undergoing endoscopic gastric mucosal dissection

Objective To investigate the safety and efficacy of ciprofol combined with oxycodone in elderly patients who underwent endoscopic gastric mucosal dissection.Methods A total of 204 elderly patients in the outpatient department of the Northern Theater General Hospital who were to undergo endoscopic gastric mucosal dissection from March 2022 to December 2022 were selected as study participants.They were aged 60-75 years,with a body mass index of 18-30 kg/m2,and ASA grade Ⅱ or Ⅲ,regardless of sex.They were randomly divided into propofol(group P),ciprofol(group C),and ciprofol+oxycodone(group CO)groups,with 68 patients in each group.During anesthesia induction,group P was given propofol(1-1.5 mg/kg);group C,ciprofol(0.2-0.5 mg/kg);and group CO,oxyco-done hydrochloride(0.1-0.2 mg/kg)and ciprofol(0.2-0.5 mg/kg).The injection time of the three groups was>30 s.During anesthesia maintenance,ciprofol[1-1.5 mg/(kg·h)]was continuously injected intravenously in groups C and CO,and propofol[2-5 mg/(kg·h)]was continuously injected intravenously in group P.The modified observer's assessment of alertness/sedation score was evaluated at 3 min after anesthesia induction.If the score was≤1,endoscopy was started.The mean arterial pressure(MAP),heart rate(HR),and blood oxygen saturation(SpO2)at times T0(before drug injection),T1(when eyelash reflex disappeared),T2(when endoscopy began),and T3(when endoscope was withdrawn);visual analogue scale(VAS)scores at 30 min and 1 h after resuscitation;and induction time,recovery time,intravenous pain,respiratory depression,and other adverse reactions were recorded in the three groups.Results There were no significant differences in MAP,HR,and SpO2 at T0 and T3 among the three groups(all P>0.05).At T1 and T2,compared with those in group P,MAP,HR,and SpO2 in groups C and CO were significantly increased(all P<0.05).MAP,HR,and SpO2 in the CO group were slightly lower than those in the C group;however,the differences were not statistically significant(all P>0.05).Compared with that in group P,the incidence of respiratory depression and injection pain in groups C and CO was significantly reduced(P<0.05),but there was no statistically significant difference between groups C and CO(P>0.05).The VAS score at 30 min and 1 h after awakening and inci-dence of body movement in the CO group were lower than those in the P and C groups(all P<0.05);however,there was no statistically significant difference between the P and C groups(all P>0.05).Conclusion Ciprofol combined with oxycodone had definite sedative and analgesic effects in elderly patients undergoing endoscopic gastric mucosal dissection.Compared with the use of propofol or ciprofol alone,with combination therapy,the respiratory cycle is more stable,patients have fewer adverse reactions,and it is worthy of clinical application.
8.Clinical outcomes of allogeneic hematopoietic stem cell transplantation from matched sibling donor for myelofibrosis

Objective:To evaluate the efficacy and safety of matched sibling donor allogeneic hematopoietic stem cell transplantation (allo-HSCT) for the treatment of myelofibrosis (MF).Methods:In this case series, the clinical data of 18 patients with MF who received allo-HSCT in the Department of Hematology, Peking University People′s Hospital from December 2008 to December 2023 were retrospectively studied. Kaplan-Meier survival analysis and competitive risk model were used to evaluate the probabilities of 3-year overall survival (OS), disease-free survival (DFS), cumulative incidence of relapse (CIR), and transplant related mortality (TRM). The transplant related complications were also analyzed.Results:Among the 18 patients included, there were 12 males and 6 females, with a median age of 50 (range: 28-64) years. All 18 patients achieved neutrophil engraftment, and the time of neutrophil engraftment [ M ( Q1, Q3)] was 16.0 (11.8, 18.0) days. Twelve patients achieved platelet engraftment, and the platelet engraftment time was 21.0 (16.2, 43.2) days. Six patients had grade Ⅱ to Ⅳ acute graft-versus-host disease (GVHD), and six patients had chronic GVHD. The 3-year OS rate and DFS rate after transplantation were 62.2% and 52.2%, respectively. The 3-year CIR and TRM were 29.7% and 24.6%, respectively. Four patients died during follow-up, with the main cause of death being infections. Conclusion:Matched sibling allo-HSCT is a feasible option for the treatment of MF.
9.Research progress of cyclic guanosine monophosphate in inherited retinal degeneration

Inherited retinal degeneration (IRD) is a group of fundus diseases characterized by a high degree of genetic heterogeneity and clinical heterogeneity, and more than 300 genetic mutations have been identified in association with IRD. Dysregulation of the intracellular second messenger cyclic guanosine monophosphate (cGMP) plays an important role in the development of IRD. cGMP participates in phototransduction process in photoreceptors. Abnormally elevated cGMP over-activate protein kinase G and cyclic nucleotide-gated channel, causing protein phosphorylation and Ca 2+ overload, respectively, and these two cGMP-dependent pathways may individually or collectively drive photoreceptor degenerative lesions and death; therefore, reducing cGMP synthesis and blocking downstream signaling can be considered as treatment strategies. Investigating the molecular mechanisms of cGMP dysregulation in photoreceptor degeneration may provide a more comprehensive picture of the pathogenesis of IRD, as well as ideas for finding new therapeutic targets and designing therapeutic programs.

The sewage treatment station is a critical pro-environment facility in hospitals.This study presents an overview of the sewage and waste gas treatment processes in a general hospital's sewage treatment station,outlining the current operational status and identifying existing issues.By implementing targeted management measures,including outsourcing services,specializa-tion of operations and maintenance staff,standardization of work protocols,digitization of data,and enhanced inspection and o-versight,the hospital has achieved improvements in the station's operational efficiency and management quality.These advance-ments provide essential support for the hospital's uninterrupted medical services.
