1.Application of multi-dimensional nursing based on empowerment theory in patients undergoing thoracoscopic and laparoscopic esophagectomy

Objective:To explore the effects of multi-dimensional nursing based on empowerment theory in patients undergoing thoracoscopic and laparoscopic esophagectomy.Methods:A convenience sampling method was used to select 110 esophageal cancer patients who were admitted to Henan Provincial People's Hospital from August 2022 to April 2024. Patients were randomly divided into two groups using a random number table: the control group and the observation group, with 55 patients in each group. The control group received routine care, while the observation group received multi-dimensional nursing based on empowerment theory in addition to routine care. The two groups were compared in terms of stress responses (heart rate, systolic blood pressure, diastolic blood pressure) , self-care ability, nutritional status (hemoglobin, albumin) , and nursing satisfaction.Results:After the intervention, the observation group had lower heart rate, systolic blood pressure, and diastolic blood pressure compared to the control group, with statistically significant differences ( P<0.01) . The self-care ability score in the control group was lower than that in the observation group, with a statistically significant difference ( P<0.01) . The observation group also had higher hemoglobin and albumin levels compared to the control group, with statistically significant differences ( P<0.01) . The nursing satisfaction rate in the observation group was higher than that in the control group, with a statistically significant difference ( P<0.05) . Conclusions:Multi-dimensional nursing based on empowerment theory can effectively reduce stress responses, improve self-care ability, enhance nutritional status, and increase nursing satisfaction in esophageal cancer patients.

4.Risk factors for prolonged ICU stay after surgery in patients with infective endocarditis

Objective Analyzing risk factors for prolonged ICU stay after cardiac surgery of Infective Endocarditis(IE)provides a basis for preventing extended ICU durations in postoperative IE cases.Methods From January 1,2019,to March 31,2021,a total of 223 patients with infective endocarditis who underwent cardiac surgery in the cardiac surgery department of Guangdong Provincial People's Hospital were included.Patients were divided into non-prolonged group(<3 days)and prolonged group(≥3 days)based on postoperative ICU stay duration.There were 156 cases in the non-prolonged group and 67 cases in the prolonged group.Single-factor analysis of risk factors for prolonged ICU stay was conducted using t-tests or rank-sum tests.Variables with P<0.05 in the single-factor analysis were further subjected to binary logistic regression for multivariate analysis.The accuracy of the model was evaluated using the ROC curve.Results Among the 223 patients,67 experienced prolonged ICU stay,with an incidence rate of 30%.Single-factor analysis results included gender,age,history of coronary heart disease,history of stroke,preoperative heart failure,aortic valve regurgitation area,left ventricular end-diastolic diameter,left ventricular ejection fraction(LVEF)<60%,extracorporeal circulation time,aortic cross-clamp time,use of Intra-Aortic Balloon Pump(IABP),endotracheal tube reintubation,pulmonary infection,use of Continuous Renal Replacement Therapy(CRRT),and prolonged mechanical ventilation time(>24 hours),among others.Multivariate analysis results revealed that preoperative LVEF<60%(OR=3.004,P=0.041),postopera-tive use of IABP(OR=31.686,P=0.008),and mechanical ventilation time>24 hours(OR=8.135,P<0.001)were independent risk factors for prolonged ICU stay after cardiac surgery.The model's AUC value for predicting risk factors for prolonged ICU stay was 0.858(95%CI:0.806~0.901,P<0.001).Conclusion Preoperative left ventricular ejection fraction(LVEF)<60%,the use of IABP,and mechanical ventilation time>24 hours were identified as independent risk factors for prolonged ICU stay after infective endocarditis(IE)surgery.In clinical practice,it is important to comprehensively address and manage various risk factors with the aim of reducing ICU stay duration and improving the overall success rate of the surgery.

6.Towards wearable and implantable continuous drug monitoring:A review

Continuous drug monitoring is a promising alternative to current therapeutic drug monitoring strategies and has a strong potential to reshape our understanding of pharmacokinetic variability and to improve individualised therapy.This review highlights recent advances in biosensing technologies that support continuous drug monitoring in real time.We focus primarily on aptamer-based biosensors,wearable and implantable devices.Emphasis is given to the approaches employed in constructing biosensors.We pay attention to sensors'biocompatibility,calibration performance,long-term characteristics stability and measurement quality.Last,we discuss the current challenges and issues to be addressed in continuous drug monitoring to make it a promising,future tool for individualised therapy.The ongoing efforts are expected to result in fully integrated implantable drug biosensing technology.Thus,we may anticipate an era of advanced healthcare in which wearable and implantable biochips will automatically adjust drug dosing in response to patient health conditions,thus enabling the management of diseases and enhancing individualised therapy.

8.Effects of Miller's pyramid principle on training for newly enrolled nurses in operation room

Objective? To explore the effects of Miller's pyramid principle on training for newly enrolled nurses in operation room. Methods? Totally 25 newly enrolled nurses in operation room who attended training for new nurses from July 2016 to June 2018 were included into the observation group. Miller's pyramid principle was incorporated into conventional training methods, and progressive and multilayer training was provided based on its theoretical framework. Another 22 newly enrolled nurses in operation room who attended training for new nurses from July 2014 to June 2016 were included into the control group who were trained using conventional methods. Theoretical performance, operation skill, case discussion, core competence self-evaluation and core competence evaluated by others were compared between the two groups. Results? The operation skill, case discussion, core competence self-evaluation and core competence evaluated by others of the observation group scored (88.72±3.10), (52.72±2.11), (144.44±23.87) and (133.44±18.32), respectively, higher than those of the control group, which were (85.27±3.95), (50.82±1.97), (122.64±22.61) and (115.68±14.01), respectively (t=-3.35, -3.18, -3.20, -3.69;P< 0.05). There was no statistically significant difference in theoretical performance score between the two groups (P>0.05). Conclusions? The effects of Miller's pyramid principle were better than those of conventional methods, which can improve the overall quality and core competency of newly enrolled nurses in operation room.

Aim To investigate the effect of Ding''s herb enema prescription on intestinal tissue related target in rat colitis induced by dextran sulfate sodium(DSS), and to elucidate the mechanism of Ding''s herb enema prescription in improving the intestinal inflammation and intestinal fibrosis.Methods Rats were fed with 3.5% DSS.The rats were randomly divided into positive drug group, model group, Control group, and Ding''s herb enema prescription group.The positive drug group was treated with mesalazine enema, and Ding''s herb enema prescription group was treated with Ding''s herb enema prescription.The colon mucosa was taken once a day for 6 weeks.The changes of intestinal inflammatory response and intestinal fibrosis related proteins were detected by GSR-CAA-67 antibody protein array, and the differentially expressed proteins were screened out.Results Eight proteins showed statistical differences, including IFN-γ, erythropoietin(EPO), TIMP-2, TIM-1, IL-6, TIMP-1, TNF-α, IL-22 (P<0.05).On the other hand, Ding''s herb enema prescription and mesalazine significantly antagonized the effect of IL-6 and TIMP-1 (P<0.05).The antagonized effect of Ding''s embolization enema on TNF-α and IL-22 was also significant(P<0.05), but mesalazine had no similar effect (P>0.05).Conclusions Ding''s herb enema prescription has the effect of multiple targets, which may improve the intestinal inflammatory response and intestinal fibrosis to achieve the purpose of treatment of ulcerative colitis(UC).
10.Application of STR-PCR in monitoring the effect of allogeneic hematopoietic stem cell transplantation

Objective To establish multiple short tandem repeat (STR) amplification by fluorescence labeling polymerase chain reaction (PCR) for monitoring the effect of allogeneic hematopoietic stem cell transplantation .Methods Sixty‐five patients were analyzed .DNA from peripheral blood of donors and recipients in pre transplantation and post transplantation were extracted , 15 STR loci and sexual loci were amplified by PCR .Results After allo‐HSCT ,54 patients obtained type of donors ,but 3 patients did not ;eight patients showed mixed chimerism .Two cases of type of donor converted into mixed chimerism and two cases of mixed chimerism converted into type of donors after some time .The time of earliest detection ,duration and transformation of each state was different .The earliest detection showed on the 16th day after surgery ,and the last one showed five months later .As to the du‐ration ,the shortest and longest were two months and seven months ,respectively .Conclusion The key factors that significantly in‐fluence the application of STR‐PCR in monitoring the effect of allogeneic hematopoietic stem cell transplantation were samples ,STR loci and proper monitoring time .So to establish the suitable method for this region could truly take maximum advantage of the PCR‐STR method .While a appropriate detecting time and STR loci should be chose .
