1.Clinical observation of Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of attention deficit hyperactivity disorder co-induced by epilepsy

Objective:To analyze the clinical effect of Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of children with attention deficit hyperactivity disorder (ADHD) and its influence on serum dopamine transporter (DAT), interleukin-6 (IL-6) and 8 isoprostaglandin F2α[8-iso-PGF-(2α)] levels.Methods:A total of 120 children with ADHD co-induced by epilepsy diagnosed and treated in Handan Second Hospital from May 2022 to June 2023 were retrospectively selected and divided into observation group and control group according to treatment method, with 60 cases in each group. The control group was treated with methylphenidate hydrochloride tablets and tiapride hydrochloride, and the observation group was treated with Jingling oral liquid on the basis of the control group. Both groups were treated continuously for 16 weeks. The clinical efficacy and number of seizures of the two groups were compared, the clinical symptoms of the two groups were assessed by the Conner Parent Symptom Questionnaire (PSQ) and the Swanson Nolan and Pelham Version Ⅳ(SNAP-Ⅳ), and the intelligence level of the two groups was assessed by the Wechsler Intelligence Scale. The serum levels of DAT, IL-6, 8-iso-PGF-(2α), neurotrophic factors before and after treatment and the occurrence of adverse reactions were compared between the two groups.Results:After treatment, the effective rate in the observation group was higher than that in the control group: 93.33% (56/60) vs. 80.00% (48/60), there was statistical difference ( χ2 = 4.62, P<0.05). After treatment, the number of seizures in the observation group was less than that in the control group at 3, 4 to 6, 7 to 12 months: (10.78 ± 1.45)times vs.(14.18 ± 1.56) times, (4.86 ± 0.53) times vs.(8.63 ± 0.89) times, (2.64 ± 0.32) times vs. (4.11 ± 0.45) times, there were statistical differences ( P<0.05). After treatment, the scores of PSQ and SNAP-Ⅳ in the observation group were lower than those in the control group: (13.67 ± 1.48)scores vs. (15.18 ± 1.59) scores, (22.12 ± 2.35) scores vs. (25.37 ± 2.68)scores; while the scores of Wechsler intelligence scale was higher than that in the control group: (93.65 ± 9.77)scores vs.(89.42 ± 8.89) scores, there were statistical differences ( P<0.05). After treatment, the serum levels of DAT, IL-6 and 8-iso-PGF-(2α) in the observation group were lower than those in the control group: (0.46 ± 0.05) μg/L vs. (0.52 ± 0.06) μg/L, (8.26 ± 0.89) ng/L vs. (8.74 ± 0.92) ng/L, (60.38 ± 6.46) ng/L vs.(79.25 ± 8.14) ng/L, there were statistical differences ( P<0.05). After treatment, the serum levels of S100βprotein, brain-derived nerve growth factor and prolactin in the observation group were higher than those in the control group: (41.55 ± 4.28) ng/L vs. (37.26 ± 3.87) ng/L, (498.33 ± 54.26) ng/L vs.(442.15 ± 45.78) ng/L, (10.18 ± 1.14) μg/L vs. (9.69 ± 0.97) μg/L, there were statistical differences ( P<0.05).There was no significant difference in gelatinous-derived nerve growth factor level between the two groups after treatment ( P>0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of ADHD co-induced by epilepsy has good efficacy, and can significantly improve their clinical symptoms, regulate the levels of serum DAT, IL-6, 8-iso-PGF-(2α). It is safe and reliable.
2.Research on the Causes of DRG High-Rate Cases and Their Control Pathways

Objective To analyze the causes of high-rate DRG cases and to propose effective management countermeasures to address the challenges posed by high-rate cases to the economy of hospitals and the quality of medical services.Methods Data of 6 937 high-rate cases and 120 782 normal-rate cases were analyzed using Excel 2021,and the causes of high-rate cases were analyzed in conjunction with the results of medical quality inspections.Results High-rate cases exhibit significantly higher average benchmark points and per capital hospitalization costs compared to normal cases,especially those with benchmark points exceeding 300,where the average cost reaches up to 151 906.70 yuan.The high-rate cases were concentrated in the group of complex diseases such as invasive ventilator support ≥96 hours or ECMO or total artificial heart transplantation.Conclusion The main causes of high-rate cases include complex and severe cases,operational errors of doctors and coders,over-treatment,and the practice of"under-coding with over-charging".To effectively control the number of high-rate cases,improve the quality of medical services,and alleviate the economic pressure on hospitals,the following countermeasures were proposed:improving medical insurance supporting policies,strengthening the training of medical teams,promoting the construction of information technology,formulating clinical pathways,and reinforcing medical supervision.
3.Analysis on the current status of clinical trial registration of the TCM treatment for functional dyspepsia

Objective:To analyze the registration status of clinical trials of TCM treatment for functional dyspepsia (FD); To provide references for relevant clinical trial registration and implementation.Methods:The clinical registration trials of TCM for the treatment of FD were retrieved from ChiCTR and clinicaltrials.gov until January 1st, 2025. Excel 2019 was used to extract and analyze study characteristics, including basic information (registration time, country/region, institution, funding source), study features and quality (study type, trial design, phase, number of centers, sample size, interventions, outcome measures, methodological quality, reporting quality), as well as recruitment status and ethical review.Results:A total of 89 registered studies were included. Registration began in 2008 with 3 studies, peaking in 2021 with 14 studies. The studies involved 6 countries, with domestic trials covering 18 provincial-level regions in China and 52 clinical trial institutions. Primary funding sources were national funding and pharmaceutical company support. Most studies were interventional with randomized parallel-controlled designs, commonly using simple randomization. Post-marketing drug trials were the most frequent phase, and single-center trials predominated, involving a total sample size of 19 776 cases. Common interventions included acupuncture/massage and Chinese patent medicines. Frequently used outcome measures were effectiveness rate, symptom evaluation, and quality of life assessment, though issues existed with non-standardized reporting and lack of TCM-specific indicators. Methodological quality needed further improvement.Conclusions:Although the number of registered clinical trials of TCM treatment for FD has increased, the overall quantity remains insufficient. Researchers should emphasize optimization of registration details and study protocols to further enhance registration quality.

6.The Research on the Construction of Medical Key Discipline Based on Application/Estimation/ Documents of Constructive Task

Objective To determine key medical disciplines construction elements and specific contents.Methods The information of national and provincial level construction standards in medical key discipline was collected,and the key indexes and specific requirements in the standards were analyzed.Results There were 10 one-class indexes with a frequency of five and plus,including the overall situation,academic leaders,scientific research performance and support conditions which have emerged to 10 times.There are 4 first-class indexes set,4~14 second-class indexes respectively,and the frequencies of the second-class indexes were from 1 ~ 10.Conclusions Research on a number of the construction and evaluation criteria from different levels and regions was the best way to discover and identify priority disciplines and the best way to assess the contents.It is an important guidance for build a scientific,sustainable development key medical disciplines.It had important guiding significance to how to construct and develop the medical key disciplines.
7.The Current Situation and Countermeasures of Three Key Construction Projects in the Field of Medicine and Health in Shandong Province

Objective To analyze the current situation and existing problems in the construction of key disciplines of medicine and health in 20 years in Shandong province and to explore the development countermeasures.Methods The data of all key disciplines of medicine and health certified by former Shandong Provincial Health Department from 1993 to 2013 were collected and analyzed.Results A total of 99 key professionals,71 key disciplines and 115 key laboratories were constructed in 20 years in the provincial medical and health institutions.The key projects in general hospitals had accounted for 74.38%.The numbers of key projects in Jinan,Qingdao,Weifang City were over the provincial average.Provincial Hospital and Qilu Hospital stayed top with 25 key projects among medical and health institutions.63.16% of the key projects take the three grade discipline name as the standard name.There were ten discipline names emerged in the three key projects conjointly,including the names of 71 items.Conclusions The construction of three key projects of medicine and health in 20 years in Shandong province had gain the marked achievement.However,the problems of regional inequality,the lack of public health discipline,discipline name confusion and other issues still existed.To amendthe management approach,unified specification,strengthening process supervision,balanced development are the countermeasures for the constructions of key projects in Shandong Province.
