1.MRI radiomic for differentiating benign and malignant lesions with prostate imaging reporting and data system(PI-RADS)3 points

Objective To investigate the value of MRI radiomics for differentiating benign and malignant prostate lesions with prostate imaging reporting and data system(PI-RADS)3 points.Methods Data of 107 patients with PI-RADS scoring of 3 points lesions were retrospectively analyzed.The patients were randomly divided into training set and testing set at a 7∶3 ratio.Clinical factors related to prostate cancer(PCa)were screened with regression analysis,and lesions'radiomics features were extracted with the least absolute shrinkage and selection operator algorithm,and then clinical models,single sequence models,multiple sequence model and combined model(multiple sequence+clinical)were constructed,respectively.Then receiver operating characteristic curves were drawn,and the area under the curve(AUC)was calculated,the efficacy of each model for differentiating PI-RADS 3 points benign and malignant lesions were assessed,and their performance and clinical applicability were evaluated using the calibration curve and clinical decision curve.Results The total prostate-specific antigen(PSA)was the clinical factor related to PCa,based on which a clinical model was established.Four apparent diffusion coefficient(ADC)features,10 diffusion-weighted imaging(DWI)features and 14 T2WI features of lesions were used to construct single sequence models,while 2 ADC features,3 DWI features and 3 T2WI features of lesions were selected to construct multiple sequence model.AUC of the combined model for differentiating benign and malignant PI-RADS 3 points lesions in training and test sets was 0.940 and 0.906,respectively,indicating that the combined model had good predictive performance and clinical net benefit.Conclusion The combined model established based on multiple sequence radiomics features and total PSA was helpful to differentiating PI-RADS 3 points benign and malignant prostate lesions.

3.Pyrrolidine dithiocarbamate alleviated rat cervical intervertebral disc degeneration via reducing tumor necrosis factor α and matrixmetalloproteinase 9 expression and its underlying mechanism

Aim To observe the effect of pyrimidine dithiocarbamate (PDTC) on the variance of disc morphology and the expressions of TNF-α, MMP-9 in the cervical disc in cervical dynamic equilibrium rat models, and to investigate the roles of PDTC in the process of intervertebral disc degeneration and the mechanism involved.Methods Fifty-four SD rats were divided into three groups randomly, then the dynamic equilibrium rat model was established by cutting the nuchal superficial and deep muscle of the rats.The dynamic equilibrium rats with PDTC solution intraperitoneal injection after operations were defined as PDTC group (group A), the models with saline intraperitoneal injection after operations as saline group (group B), the rats of fake operation with saline intraperitoneal injection as blank group (group C), and the animals were sacrificed in batches 10 weeks, 12 weeks, 16 weeks respectively after operation.The C5, C6 vertebrae and C5/6 discs were harvested, and the disc morphology was observed.TNF-α, MMP-9 mRNA expressions were detected by q-PCR and protein expression was observed by Western blot.Results Compared with the saline group, the morphology of disc in PDTC group was destructed slightly and fiber ring arranged orderly.TNF-α, MMP-9 gene and protein expressions had no obvious changes (P>0.05) in blank group (group C) at each time point.The expressions of IL-6, MMP-9 mRNA increased with time in group B, but the amount increased fast firstly, and slow lately, reaching peak in 12 weeks.The expression of TNF-α, MMP-9 protein became steady in group B from 10 weeks compared with other time points(P>0.05).TNF-α, MMP-9 genes and proteins expression decreased obviously in PDTC group (group A) compared with saline group (group B) (P<0.01) at each time point, but higher than blank group C(P<0.01) at each time point.Conclusions TNF-α and MMP-9 are important inflammatory factors involved in rat cervical disc degeneration, PDTC relieves the degeneration of rat cervical disc by reducing the expression of TNF-α and MMP-9 via disturbing the NF-κB signal pathway probably, and PDTC may become potential medicine for disc degeneration.
4.Feasibility of non-contrast enhanced magnetic resonance angiography for diagnosis of renal artery stenosis in elderly patients.

OBJECTIVETo evaluate the diagnostic efficacy of IFIR-FIESTA technique in detecting renal artery stenosis in elderly patients.
METHODSTwenty-seven aged patients underwent both IFIR-FIESTA and 3D CE-MRA examinations. The imaging quality and renal artery stenosis grades were evaluated. Kappa test was used to assess the consistency between the two methods. With CE-MRA as the reference, the diagnostic sensitivity, specificity, accuracy, PPV and NPV for IFIR-FIESTA were calculated in detecting renal artery stenosis.
RESULTSThe images by the two methods were 100% qualified for diagnosis, although the image quality of CE-MRA was significantly better. IFIR-FIESTA and CE-MRA showed excellent consistency in detecting renal artery stenosis. With CE-MRA as the reference, the diagnostic sensitivity, specificity, accuracy, PPV and NPV for IFIR-FIESTA were 97.1%, 100%, 98.1%, 100%, and 95% in detecting renal artery stenosis, respectively.
CONCLUSIONIFIR-FIESTA is feasible as a routine examination for detecting renal artery stenosis in elderly patients.
