1.Clinical research on the impact of transcranial direct current stimulation combined with music therapy on cognitive impairment after stroke

Objective To observe the effects of transcranial direct current stimulation(tDCS)combined with music therapy on cognitive function and activities of daily living in patients with post-stroke cognitive impairment(PSCI).Methods This prospective study enrolled subacute stroke(2 weeks to 6 months after onset)patients with cognitive impairment admitted to the Rehabilitation Department of the First Affiliated Hospital of University of Science and Technology of China(Anhui Provincial Hospital)from January 2023 to December 2024.Using the random number table,patients in the subacute early stage(2 weeks to<3 months after stroke)and subacute late stage(3 months to 6 months after stroke)were respectively assigned to the experimental group and the control group.Baseline clinical data were collected,including gender,age,stroke type(intracerebral hemorrhage,cerebral infarction),lesion side(left,right),disease course,educational attainment,history of smoking and alcohol consumption,stroke area(basal ganglia,temporal lobe,frontal lobe),and National Institutes of Health stroke scale(NIHSS)score at admission.Both groups received conventional rehabilitation treatment.The experimental group additionally received tDCS treatment combined with music therapy intervention,which included active and passive music therapy.In contrast,the control group received only tDCS treatment in conjunction of conventional rehabilitation intervention.Both groups received treatment for 2 weeks.The Montreal cognitive assessment scale(MoCA),Rivermead behavioural memory test(RBMT),and digit span test(DST)were used to evaluate the overall cognitive function,memory,and attention of PSCI patients before and after treatment.The modified Barthel index(MBI)was used to evaluate the daily living activity ability of PSCI patients 2 weeks post-treatment.Results A total of 40 PSCI patients were included in this study,with 20 cases in each group.Among the participants,17 in the experimental group and 18 in the control group were in early stage of subacute stroke,while 3 in the experimental group and 2 in the control group were in the late stage of subacute stroke.(1)There were no statistically significant differences in the baseline clinical data of the two groups(all P>0.05).(2)Before treatment,the MoCA,RBMT,and DST scores in the experimental group were(15.00±3.71),(2.90±1.07),and(4.05±1.47)points,respectively,while the corresponding scores in the control group were(13.45±2.61),(2.75±0.77),and(3.35±0.99)points.There were no statistically significant differences in the MoCA,RBMT,and DST scores between the two groups before treatment(all P>0.05).Two weeks after treatment,the MoCA,RBMT,and DST scores in the experimental group were(19.05±4.35),(5.15±1.50),and(5.85±1.66)points,respectively,compared to(15.90±2.73),(3.45±1.15),and(4.35±1.18)points in the control group.The interaction effect of time and group(Ftime×group)was 15.716,45.762 and 14.140,respectively,all of which were statistically significant(all P<0.01);the group main effect(Fgroup)was 4.876,7.140,and 7.074,respectively,all of which were statistically significant(all P<0.05);the time main effect(Ftime)was 259.370,165.762,and 173.209,respectively,all of which were statistically significant(all P<0.01).(3)Before treatment,the MBI scores of the experimental group and control group were(56.00±7.18)and(55.25±5.73)points,respectively,with no statistically significant difference between the two groups(P>0.05).Two weeks after the treatment,the MBI scores of the experimental group and control group were(63.00±6.77)and(60.50±5.36)points,respectively.No statistically significant differences were observed in the interaction effect of time and group,or the main effect of group between the two groups after treatment(Ftime ×group=1.677,P>0.05;Fgroup=0.751,P>0.05).While a statistically significant difference was found in the main effect of time on the MBI scores between the experimental group and control group(Ftime=82.196,P<0.01).Conclusion The combination of tDCS and music therapy can improve the cognitive function and daily living activity ability in patients with PSCI,and offering superior outcomes compared to tDCS treatment alone.
2.Case of stiff skin syndrome treated with acupuncture and cupping therapy.

Stiff skin syndrome is a very rare non-inflammatory reactive skin disease, characterized by skin sclerosis and limited joint mobility. The paper reports one case of child with stiff skin syndrome and treated with combined therapy of acupuncture and cupping. Acupuncture was used at the lateral line 1 of vertex (MS8) on the right side, Jiaji (EX-B2) of L2 to L4, Huantiao (GB30), ashi point, Juliao (GB29), Fengshi (GB31), Weizhong (BL40), and etc. on the left side. After deqi, the electrodes of KWD-808Ⅰimpulse electronic therapeutic device were attached to Jiaji (EX-B2) of L4 and Huantiao (GB30), Fengshi (GB31) and Yanglingquan (GB34) on the left side respectively, at disperse-dense wave, a frequency of 2 Hz/100 Hz, and a current of 2 mA. The needles were retained for 20 min. Acupuncture was operated once every 2 days, 3 interventions a week. When acupuncture was completed in each intervention, moving cupping was followed till the skin turned to be red, along the distribution of the governor vessel, foot-shaoyang gallbladder meridian and foot-taiyang bladder meridian on the left side, of the lumbar region and leg. Moving cupping was delivered once every 2 days, 3 times a week. Once a week, after moving cupping, the cups were retained on the areas with skin stiffness for 8 min to 10 min. One course of the combined therapy of acupuncture and cupping was composed of 6 treatments. After 2 courses of treatment, the skin stiffness on the left buttock region and the lateral side of the lower limb was ameliorated, the swelling on the left lower limb relieved and the walking improved; and the patient could walk continuously for 2 000 m. The combined therapy of acupuncture and cupping provides a new idea for the clinical treatment of stiff skin syndrome.

3.Effect Analysis of the Informatization of the Prescription Review Center and the Homogeneity of Pharmacist Service in Promoting the Rationality of Outpatient and Emergency Department Prescriptions

Objective To discusse the application effects of the informatization of the review center and the homogeneity of pharmacists on the rationality of emergency department prescriptions.Methods Based on the system rules of the rational drug use management system and manually set custom rules,the changes in pharmacist's review quality,efficiency homogeneity,and prescription rationality were compared before(February 2023 to July 2023)and after(August 2023 to January 2024)the construction of the review center,according to the informatization and process standardization management.Results After the establishment of the review center,analysis of variance showed that the approval rate of pharmacist's review significantly increased compared to before the establishment of the review center(P<0.05),while the average time consumption increased significantly(P<0.01).The average review time,average approval time,and average review return time have been extended from(4.50±0.58),(4.50±0.58),and(4.75±0.96)s to(11.67±1.03),(8.50±0.55)and(13.17±0.98)s,respectively.The trend chi-square test showed that the irrationality rate of emergency department prescriptions decreased monthly from 6.27%in August 2023 to 0.93%in January 2024(P<0.01).Correlation analysis between the number of intervention system rules since the establishment of the review center and the irrationality rate of emergency department prescriptions revealed a significant correlation(P=0.004 4).Conclusions By utilizing the platform of the review center,establishing dedicated review pharmacists and an information pharmacist team,and implementing informatization and standardized management processes,it can contribute to improving the quality and efficiency of prescription review,increasing the qualification rate of prescriptions,ensuring rational drug use,and enhancing the management level and medical quality of hospitals.

6.Teaching reform of Traditional Chinese Medicine(TCM)acupuncture course in Nursing College of Peking Union Medical College

Objective To reform the teaching content of acupuncture and moxibustion for undergraduate of Nursing College and to evaluate the effectiveness of the teaching reform,so to provide constructive information for optimizing the curriculum.Methods To revise the teaching content of Traditional Chinese Medicine(TCM)acu-puncture course based on the results of previous questionnaires,including reducing boring theoretical knowledge,increasing commonly used acupuncture techniques and clinical case analysis,organizing all undergraduates atten-ding the TCM teaching in the Nursing College of Peking Union Medical College to fill out the"Acupuncture Course Content Questionnaire"in 2023.The content covers four aspects:students'basic cognition of acupuncture and moxibustion before the course,students' learning feedback,students'learning effectiveness after the course and students' suggestions for further optimization of acupuncture and moxibustion course.Results Totally 149 students participated in the questionnaire survey and 73.15%of them were interested in the acupuncture course.There were 62.42%of the students thought that the most difficult part of the course was the"Acupuncture Points",and 71.81%of them complained the most constraining factor to the learning effectiveness was the diffi-culty of memorizing the meridians and acupoints.The most interesting part of the course was"Overview of Acu-puncture Treatment and Acupuncture Treatment of Common Diseases",which accounted for 44.3%of the students.Through the study,88.59%of the students were willing to recommend acupuncture treatment to pa-tients with indications.Students' suggestions for improvement of the acupuncture course was increase of classroom practice or learning by observation of operation videos.Conclusions The reformed acupuncture course highlights the practicability,improves the learning interest and subjective initiative,but the training of practice skill still needs to be strengthened.
7.Clinical data analysis of 279 inpatients with acupuncture consultation in a comprehensive hospital.

OBJECTIVETo analyze the current status of acupuncture consultation in Peking Union Medical College Hospital, which could provide references and suggestions for the development of acupuncture at comprehensive hospitals and the treatment of diseases.
METHODSThe source of departments, the number of patients, the number of consultation, treatment frequency, consultation reasons and western medicine diagnosis of 279 inpatients who received acupuncture consultation were analyzed.
RESULTSA total of 26 departments, including department of internal medicine, department of surgery, gynecology department, etc. and emergency department, requested for acupuncture consultation, in which the request from neurology department was the highest. The distribution of diseases was quite wide, mainly gastrointestinal dysfunction, cerebrovascular diseases, urinary retention, hiccup and pain-related symptoms.
CONCLUSIONSThe indication of acupuncture is extensive, which could supplement the disadvantages of modern medicine and play a positive role in the adjuvant treatment of comprehensive hospital. It is suggested the communication and cooperation with other departments should be strengthened to promote development of acupuncture and improve the understanding and application of acupuncture for western medicine doctors.
8. Feasibility of the 2014 European guidelines risk prediction model for sudden cardiac death in hypertrophic cardiomyopathy in Chinese patients

Objective:
Exploring the feasibility of the 2014 European Society of Cardiology(ESC)guideline′s risk prediction model for sudden cardiac death in hypertrophic cardiomyopathy (HCM Risk-SCD) in Chinese patients.
Methods:
The study population consisted of a consecutive cohort of 172 Chinese patients with HCM without prior sudden cardiac death (SCD) event who were in patients in Nanjing Drum Tower Hospital from December 2010 to October 2015.The endpoint event was a composite of SCD and appropriate implantable cardioverter-defibrillator (ICD) therapy.Clinical data were collected to calculate the 5-year SCD risk using the HCM Risk-SCD formula and to observe the actual risk during the follow-up.Receiver operating characteristic curves (ROC) and the area under curve (AUC) were calculated for the HCM Risk-SCD and risk stratification methods of the 2011 American Heart Association (AHA) guideline.
Results:
During follow-up of (2.69±1.36) years, five patients achieved the endpoint event.The predicated rate of SCD event using HCM Risk-SCD was (2.36±1.73)%, (1.93±0.78)%, (5.18±0.65)%, (8.77±2.38)% for all patients, low-risk group, medium-risk group and high-risk group respectively.However, the actual rate of SCD event was 2.91%, 1.27%, 25.00% and 14.29%, respectively.The AUC of 2014 ESC guideline and 2011 AHA guidelinewas 0.93(95%
9.Application of ultrasound-guided modified Seldinger technique for upper arm PICC insertion in ;infant patients

Objective To explore the application of ultrasound- guide modified Seldinger technique for upper arm PICC insertion in infant patients. Methods Use the ultrasound-guide modified Seldinger technique to inserted PICC for 27 infant patients. Results All 27 cases were inserted successfully, success rate of the one puncture reached 92.6%. Conclusions By adequate preparation for infants, effective immobilization and good cooperation of operators, and combined with techniques which prevent PICC misplacement, overcome the shortcoming of bad vessel and non-compliance of infant patients, develop the advantage of ultrasound, could improve the rate of successful catheterization of upper arm PICC insertion in infant patients, and protect the vessels.
10.Construction of eukaryotic expression vector for IL-37 and EGFP fusion protein and its expression in 293T cells

Objective To construct the eukaryotic expression vector for IL-37 fused with the enhanced green fluorescent protein (EGFP) and express the fusion protein IL-37-EGFP in 293T cells. Methods The IL-37 and EGFP gene were amplified by PCR, respectively. Then the gene fragments of IL-37 and EGFP were in-serted into the pcDNA3.1 vector to construct the recombinant eukaryotic expression vector pcDNA3.1/IL-37-EGFP. The pcDNA3.1/IL-37-EGFP recombinant expression vector was identified by enzymatic digestion , DNA sequencing, PCR identification. Then the recombinant plasmid pcDNA3.1/IL-37-EGFP was used to transfect 293T cells , the expression of EGFP in 293T cells was detected by fluorescence microscope , the expression of IL-37 in 293T cells was detected by RT-PCR and Western blot. Results The results of enzymatic digestion and DNA sequencing showed that the fusion gene of IL-37 and EGFP linked with (G4S)3 was cloned correctly into pcDNA3.1 vector. The green fluorescence was observed in 293T cells transfected with pcDNA3.1/IL-37-EGFP by fluorescence microscope. RT-PCR and Western blot showed that the expression with high level of IL-37 mRNA and IL-37 protein in 293T cells transfected with pcDNA3.1/IL-37-EGFP , respectively. Conclusion The recom-binant eukaryotic expression vector for IL-37 fused with EGFP, pcDNA3.1/IL-37-EGFP, was constructed suc-cessfully and expressed effectively in 293T cells.
