1.Effects of Tuina at "Weizhong (BL 40)" on Mechanical Pain Hypersensitivity in Chronic Dorsal Root Ganglion Compression Model Rats and the Characterization of Acupoint-Related Neural Afferents and Central Projections

ObjectiveTo determine the effects of tuina at "Weizhong (BL 40)" on mechanical pain hypersensitivity in a rat model of chronic dorsal root ganglion (CCD), and to preliminarily explore the anatomical distribution characteristics of neural afferents associated with "Weizhong (BL 40)" acupoint and their central projections. MethodsThirty SD rats were randomly assigned to a sham operation group, a model group, and a tuina group, with 10 rats in each group. In the model group and the tuina group, a chronic compression of CCD model was established by implanting stainless-steel rods into the right L4/5 and L3/4 intervertebral foramina. Rats in the sham operation group underwent surgical exposure only. From day 4 to day 28 after modeling, rats in the tuina group received daily manipulation at the right "Weizhong (BL 40)" (pressure: 5 N; frequency: 2 Hz; duration: 10 min/session). Rats in the sham operation group and the model group were subjected only to fixation. Mechanical withdrawal threshold (MWT) was assessed before modeling and on day 3, 7, 14, 21, and 28 after modeling. On day 29 after modeling, pseudorabies virus (PRV), a retrograde trans-multisynaptic tracer, was injected into the right "Weizhong (BL 40)" acupoint in rats from the model group. After 120 h, rats were perfusion-fixed through the heart, and local tissues surrounding the "Weizhong (BL 40)" acupoint, corresponding lumbar dorsal root ganglia, lumbar spinal cord, and brain tissues were collected for fluorescence imaging of PRV-positive signals. ResultsCompared to the sham operation group, rats in the model group and the tuina group showed significantly reduced MWT on day 3, 7, 14, 21, and 28 after modeling (P<0.05). Compared to the model group, rats in the tuina group exhibited significantly increased MWT on day 21 and 28 after modeling (P<0.05). Fluorescence imaging revealed PRV-positive signals in the deep fascial layer of local muscle tissue at "Weizhong (BL 40)", the L4 and L5 dorsal root ganglia, the dorsal horn and central canal region of the spinal cord, as well as the primary somatosensory cortex (S1), secondary motor cortex (M2), and anterior cingulate cortex (ACC). Among the examined cortical regions, M2 contained the largest number of PRV-positive neurons. ConclusionTuina at "Weizhong (BL 40)" alleviates mechanical hyperalgesia in rats with the chronic compression of CCD model. A traceable neuroanatomical connection exists between the linking deep tissues within "Weizhong (BL 40)" region and the corresponding lumbar dorsal root ganglia, the spinal dorsal horn, and specific cortical areas.
2.Effect of manual therapy based on surface electromyography on knee osteoarthritis for older people:a random-ized controlled trial

Objective To investigate the effect of manual therapy based on surface electromyography on knee osteoarthritis(KOA)in the older people. Methods A total of 106 outpatient with unilateral KOA were selected from Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine,from August,2023 to June,2024,and were randomly divided into control group(n=53)and experimental group(n=53).The control group accepted routine manual therapy,and the experimental group accepted manual therapy based on the analysis of average electromyography(AEMG).They were assessed with Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),Visual An-alogue Scale(VAS)for pain,Tinetti Balance and Gait Score,and 6-minute walk test(6-MWT)distances before and after treatment. Results One case dropped down in each group.Before treatment,AEMG decreased in the rectus femoris,medial femoris and medial head of gastrocnemius on the affected side in the experimental group(|Z|>8.647,P<0.001),and it increased in the lateral femoris,semitendinosus and biceps femoris(|Z|>4.808,P<0.001).The scores of WOMAC,VAS,Tinetti Balance and Gait Score,and distances of 6-MWT improved in both groups after treat-ment(|t|>3.987,P<0.001),and improved more in the experimental group than in the control group,except the VAS score(|t|>2.213,P<0.05). Conclusion Manual therapy focusing on activation of rectus femoris,medial femoris and medial head of gastrocnemius,inhibition of the lateral femoris,semitendinosus and biceps femoris,and releasing the tension of the medial and lateral collateral ligaments,according to the results of surface electromyography,can alleviate the pain of the KOA in the older people and improve the mobility of the knee.

4.Clinical effects of bi-level positive airway pressure and heated humidified high flow nasal cannula ventilation as initial treatment for premature infants with respiratory distress syndrome

Objective:To compare the efficacy and safety of bi-level positive airway pressure (BiPAP) ventilation and heated humidified high flow nasal cannula (HHHFNC) ventilation as initial respiratory support for premature infants with respiratory distress syndrome (RDS).Methods:From January 2019 to June 2021, premature infants [gestational age (GA) 28~35 weeks)] with grade Ⅰ to Ⅲ RDS admitted to Suining County People's Hospital were prospectively enrolled. The infants were randomly assigned into BiPAP group and HHHFNC group. The clinical characteristics, ventilation efficacy and complications were analyzed.Results:A total of 33 infants were in BiPAP group and 32 in HHHFNC group. No significant differences existed between the two groups in the following items: the frequency of apnea within 24 h of ventilation, FiO 2 and PaCO 2 at 24 h, the use of pulmonary surfactant (PS), the incidence of non-invasive ventilation failure within 72 h, non-invasive ventilation duration and the age achieving total enteral nutrition. HHHFNC group had lower score in premature infants pain profile (PIPP) than BiPAP group at 24 h of non-invasive ventilation [4 (3, 6) vs. 8 (6, 11), P<0.001]. No significant differences existed in nasal injury, pneumothorax, intraventricular hemorrhage, necrotizing enterocolitis, bronchopulmonary dysplasia and mortality rate between the two groups ( P>0.05). Conclusions:As the initial treatment for premature infants with grade Ⅰ to Ⅲ RDS, BiPAP and HHHFNC has similar rates of non-invasive ventilation failure within 72 h,non-invasive ventilation duration and adverse events. HHHFNC may ease the pain of the infants.
5.Risk factors for recurrence of lower-extremity arteriosclerosis obliterans in patients after interventional surgery

Objective:To investigate the risk factors for the recurrence of lower-extremity arteriosclerosis obliterans in patients after interventional surgery.Methods:The clinical data of 98 patients with lower-extremity arteriosclerosis obliterans who received treatment in Lishui Municipal Central Hospital from January 2021 to January 2022 were retrospectively analyzed. They were randomly divided into a recurrent group ( n = 36) and a non-recurrent group ( n = 62). The immediate efficacy of interventional therapy and the follow-up were determined in each group. Clinical data, triglyceride, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol levels, neutrophil/lymphocyte ratio, and fasting blood glucose level were compared between the two groups. The risk factors for the recurrence of lower-extremity arteriosclerosis obliterans after interventional surgery were further analyzed using logistic regression analysis. Results:The success rate of interventional surgery for aortoiliac artery, superficial femoral artery, and inferior genicular artery were 95.35% (41/43), 97.73% (43/44), and 95.83% (46/48), respectively. Twelve-month follow-up results showed that the patency rate of lower-extremity vessels decreased with time ( P < 0.05). There was a significant difference in Trans-Atlantic Inter Society Consensus (TASC) classification between the two groups (11/26/18/7 vs. 4/5/10/17, χ2 = 18.33, P < 0.05). Ankle-brachial index (ABI) in the recurrence group was significantly lower than that in the non-recurrence group [(0.77 ± 0.12) vs. (0.40 ± 0.02), t = 18.31, P < 0.05]. There were no significant differences in triglyceride, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, and fasting blood glucose levels between the two groups (all P < 0.05). The neutrophil/lymphocyte ratio (NLR) in the non-recurrent group was significantly lower than that in the recurrent group [(1.98 ± 0.28) vs. (13.18 ± 0.50), t = 15.25, P < 0.05]. Logistic regression analysis results showed that ABI ( OR = 1.459, 95% CI: 1.163-1.832), TASC classification ( OR = 1.344, 95% CI: 1.090-1.658), surgical method ( OR = 1.392, 95% CI: 1.140-1.700) and NLR ( OR = 1.474, 95% CI: 1.184-1.836) were the influential factors of recurrence of lower-extremity arteriosclerosis obliterans (all P < 0.05). Conclusion:ABI, TASC classification, surgical methods, and NLR are the influential factors of lower-extremity arteriosclerosis obliterans after interventional surgery. The above indexes should be closely monitored in clinical practice to maintain the effects of interventional surgery.

7.Effect of intervention model based on empowerment theory on self-efficacy and self-acceptance of patients with permanent enterostomy

Objective? To explore the effect of empowerment-based intervention model on self-efficacy and self-acceptance of patients undergoing permanent enterostomy. Methods? By purposive sampling, a total of 100 colorectal cancer patients undergoing permanent enterostomy operation in the First Affiliated Hospital of Zhengzhou University from January to December of 2018 were selected as the research objects. The 50 patients admitted from January to June of 2018 were taken as the control group while the other 50 patients admitted from July to December of 2018 were taken as the experimental group. During intervention, there were 7 patients lost in the control group and 43 valid questionnaire retrieved; in the experimental group, 2 patients were lost and 48 valid questionnaires were retrieved. The control group was given conventional nursing care while the experimental group was given the nursing intervention formulated based on the empowerment theory. Stoma Self-Efficacy Scale(SSES) and Self-Acceptance Questionnaire(SAQ) were used to assess the patients. Results? After intervention, the total score of SSES, the dimension scores of "stoma care efficacy" and "social self-efficacy"of the experimental group were higher than those of the control group, with statistical significances (P<0.05). After intervention, the total score of SAQ and the score of "self-acceptance" dimension in the experimental group were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusions? The intervention based on empowerment theory can improve the self-management efficacy and self-acceptance level of patients with permanent enterostomy. It is suggested that it should be popularized in clinical practice.

Objective To investigate the clinical application of the 2011 edition of China Nanjing Persistent Vegetative State (NPVSS 2011).Methods Three hundred and one patients with vegetative state (VS),persistent vegetative state (PVS) and minimally conscious state (MCS) admitted into hospital from February 22,2011 to May 30,2014 were chosen for the study.Patient conditions and treatment efficacy were assessed by using the NPVSS 2011.Results With regard to conscious state at admission into hospital,109 cases were in VS,65 were in PVS and 127 were in MCS.When discharged from hospital,there were 8 VS cases,72 PVS cases,91 MCS cases,and 130 Ⅲ cases that could carry out more complicated instructions.Of all the cases,164 cases had prominent efficacy,31 were improved,106 were invalid,and 130 cases that had consciousness recovery to the state of Ⅲ.Conclusions The CNPVSS could reflect the changing processes of patient conditions,having the features of being simple and clear,conforming to clinical practice,and above all being easy to master and operate.With this reason,it could be applied to clinical practice.

10.The Relationship between Homocysteine and Carotid Intima-Media Thickness in Patients with Brain Infarction

Objective To investigate the relationship between total homocysteine (tHcy) and carotid intima-media thickness (CIMT) in brain infarction patients. Methods Sixty patients with fasting plasma tHcy levels ≤10μmol/L (non-Hhcy group), 60 patients with fasting plasma tHcy levels>10μmol/L and≤15μmol/L (H1 group), and 60 patients with fast-ing plasma tHcy levels>15μmol/L (H2 group) were chosen in hospitalized patients with acute cerebral infarction. Values of CIMT were detected in three groups of patients. The clinical biochemical indicators including triglyceride (TG), total choles-terol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), fasting blood sugar (FBS), folic acid (FA), Vitamin B12 (VitB12) and glycated hemoglobin (HbA1c) were also detected. Results There was signifi-cant difference in CIMT between three groups (P<0.01). The value of CIMT increased in H2 group [0.98(0.90, 1.05)mm] com-pared with that of non-Hhcy group [0.85(0.80, 0.95)mm]. The value of CIMT increased in H2 group compared with H1 group [0.98(0.90, 1.05)mm vs 0.85(0.85, 0.95)mm], P<0.05). There were significant differences in tHcy, FA and VitB12 between three groups. Based on the log-transformed values of CIMT as the dependent variable, multiple stepwise linear regression showed significant associations of the following variables with increased CIMT: increasing age, the history of smoking, the history of diabetes, higher LDL-C and tHcy levels. Conclusion Brain infarction in patients with higher tHcy level often have lower levels of FA and VitB12, and increased CIMT. When the level of tHcy >15 μmol/L, there is more significantly higher level of CIMT. The increased CIMT level was associated with some cerebrovascular risk factors in patients with brain infarction.
