1.Diagnostic Accuracy of Serological Tests for Mycoplasma pneumoniae Infections in Children with Pneumonia, Based on Symptom Onset

Background:
Mycoplasma pneumoniae is a major cause of community-acquired pneumonia (CAP) in children, with a rising incidence of macrolide resistance. Early diagnosis is crucial for reducing the disease burden; however, current diagnostic tools have limitations.We evaluated the diagnostic accuracy of serological assays and their performance based on symptom onset in children with CAP.
Methods:
From September 2023 to September 2024, we prospectively enrolled children with CAP, classified as M. pneumoniae pneumonia (MPP) or non-MPP, from 16 hospitals in Korea. Serological testing included chemiluminescence immunoassay (CLIA) and ELISA for detecting IgM and IgG, along with particle agglutination (PA) for total antibody measurements. Serological responses were analyzed at different times after symptom onset (0–4, 5–9, and 10–21 days).
Results:
Among 472 children with CAP (362 MPP, 110 non-MPP), 138 (29.2%) underwent PA testing, and 334 (70.8%) underwent IgM testing. PA at a 1:640 cutoff showed 48.0% sensitivity and 100% specificity. CLIA and ELISA showed comparable sensitivities (69.1% vs. 69.2%) and specificities (76.9% vs. 66.7%) for IgM testing. Seropositivity increased significantly with time since symptom onset (P for trend < 0.001), reaching 97.9% for IgM, 62.5% for IgG, and 94.7% for PA at 10–21 days.
Conclusions
The time post-symptom onset significantly influenced the diagnostic utility of serological tests for pediatric MPP, which showed limited value during the early stage of illness. These findings emphasize the importance of symptom onset-based interpretation of serological test results and their utility in complementing PCR when optimizing MPP diagnosis in children.
4.Cohort profile: Korean Varicella Immunization Monitoring (K-VIM) Scheme: a national cohort of children born 2011–2022

Varicella, caused by the varicella-zoster virus, was once nearly universal in childhood before the advent of vaccination and may lead to severe complications and even fatalities. Monitoring varicella vaccine effectiveness is crucial yet often overlooked in settings with limited surveillance infrastructure. The Korean Varicella Immunization Monitoring (K-VIM) Scheme was established to address this gap by assembling a national, insurance-based birth cohort of children born between 2011 and 2022 (n=4,505,165). This cohort leverages comprehensive healthcare databases in Korea to capture vaccination records, medical visits, and varicella infection outcomes for virtually all children within the target birth years. We describe the enrollment and key characteristics of the K-VIM cohort, including vaccination coverage, demographic features, and varicella incidence to date. The structure and completeness of Korea’s national health insurance and immunization registries enable robust longitudinal tracking of varicella infections among vaccinated versus unvaccinated children. Early findings demonstrate distinct patterns in infection rates and disease severity according to vaccination status. The K-VIM Scheme provides a foundation for ongoing epidemiological studies of varicella vaccine effectiveness and public health impact in Korea. Future plans include evaluating the long-term effects of varicella vaccination, including potential impacts on herpes zoster incidence, using diverse study designs to strengthen causal inference and inform immunization policy.
5.Sequential Mediation by Maternal Weight Gain and Late Blood Metal Concentration in the Association Between Heavy Metal Exposure in Early Pregnancy and Fetal Growth

Background:
To assess the mediation effect of gestational weight gain (GWG) and blood heavy metal levels in the late pregnancy affecting the association between early pregnancy metal exposure and fetal growth.
Methods:
We analyzed data from Korean Children’s Environmental Health Study cohort comprising 4,712 full-term births. Blood levels of lead (Pb), mercury (Hg), and cadmium (Cd) during pregnancy, and newborn birthweights were retrieved. We applied single-exposure model to assess the association between heavy metal levels and newborn anthropometric measures and conducted path analyses to examine the direct and indirect effects of heavy metal levels in early pregnancy on newborn measures.
Results:
Exposure to Pb (β = −15.470, P = 0.036) and Cd (β = −24.150, P = 0.004) during late pregnancy were associated with lower birthweight. Early exposure to Pb was not directly associated with lower birthweight, however, mediation of late exposure to Pb (β = −7.417, P = 0.049) resulted in lower birthweight (β = −14.787, P = 0.036). Early exposure to Hg was associated with higher birthweight, however, indirect paths via GWG (β = 3.626, P = 0.048) and late Hg levels (β = −12.929, P = 0.111) countered each other, resulting in the nullification of the total effect (β = −8.519, P = 0.301).
Conclusion
The effect of Pb exposure in early pregnancy on birthweight was mediated by GWG and Pb exposures during late pregnancy. When assessing the impact of heavy metal exposure on birthweight, it is important to note that GWG and late heavy metal levels act as mediators, rather than confounders.
6.Natural History of Gastric Subepithelial Lesions Measuring 1 to 2 cm

Objectives:
Management strategies for subepithelial lesions (SELs) measuring 1–2 cm remain undefined in most guidelines due to limited data. We aimed to establish a practical clinical approach by analyzing the long-term outcomes of SELs in this size range.
Methods:
We retrospectively reviewed cases of gastric SELs measuring 1–2 cm between 2011 and 2023. We reviewed esophagogastroduodenoscopy and endoscopic ultrasound images, alongside data from abdominal computed tomography, biopsy results, and long-term follow-up records.
Results:
Gastric SELs were initially identified in 648 patients. Of these, 88 patients (mean age, 57.1±10.6 years; 60% women) had lesions measuring 1–2 cm and underwent further evaluation or surveillance. The mean lesion size determined by endoscopy was 13.7±3.3 mm. Among the 52 patients with histological confirmation, 24 (46%) were diagnosed with gastrointestinal stromal tumors (GISTs), whereas the remainder had benign lesions. Most gastric SELs were located in the fundus and upper body. Pathological analysis indicated that the majority of GISTs were low-risk. Three GISTs were stratified as moderate-to-high risk; notably, one measured 0.9 cm upon pathological examination. The median surveillance period for the 28 patients without histological confirmation was 55 months (range, 43–115 months). Two of these patients underwent endoscopic or wedge resection due to tumor enlargement; both lesions were histologically confirmed as low-risk.
Conclusions
Approximately half of the gastric SELs measuring 1–2 cm were GISTs, representing an intermediate- to high-risk tumors. Given that GISTs in the upper stomach were associated with elevated risk even when smaller than 2 cm, we recommend strict short-term surveillance for such lesions.
7.Risk of cardiovascular disease associated with use of tumor necrosis factor inhibitors in ankylosing spondylitis

Objective:
To examine the risk of cardiovascular disease (CVD) associated with use of tumor necrosis factor inhibitors (TNFi) in patients with ankylosing spondylitis (AS).
Methods:
This study used data from the Korean National Health Insurance database. Patients aged ≥18 years who were newly diagnosed with AS between 2010 and 2018 and did not suffer prior CVD were included. The primary outcome was a composite endpoint of myocardial infarction (MI) or stroke. The secondary outcomes were specific endpoints of MI, stroke, or congestive heart failure. TNFi use was evaluated as a time-dependent variable. Cox proportional hazard regression was used to examine the association between TNFi use and risk of incident CVD.
Results:
Of 19,775 patients (mean age, 36.1 years; 75% male), 5,978 (30.2%) were exposed to TNFi treatment during the study period. During a follow-up of 4.8 (interquartile range, 2.9~7.2) years, 522 events of MI or stroke occurred. TNFi use was associated with a lower risk of MI or stroke than no TNFi use after adjusting for traditional CV risk factors and medications (adjusted hazard ratio [aHR], 0.72; 95% confidence interval [CI], 0.55~0.94). The lower risk of MI or stroke associated with TNFi use was consistently observed across subgroups. Risk of congestive heart failure did not differ between users and non-users of TNFi (aHR, 0.94; 95% CI, 0.75~1.18).
Conclusion
In a real-world AS cohort, TNFi treatment was associated with a reduced risk of atherosclerotic CVD. However, TNFi use was not associated with risk of congestive heart failure.
8.Risk Assessment for Carotid Atherosclerosis in Asymptomatic Patients with Metabolic Dysfunction-Associated Steatotic Liver Disease

Background/Aims:
Cardiovascular disease remains a major cause of mortality in patients with metabolic dysfunction-associated steatotic liver disease (MASLD). This study evaluated the association between subclinical carotid atherosclerosis (SCA) and MASLD or MASLD and increased alcohol intake (MetALD) in asymptomatic individuals.
Methods:
This cross-sectional study included 56,889 adults undergoing health check-ups in South Korea. Hepatic steatosis was diagnosed by ultrasound, and SCA was defined by carotid plaques or increased intima-media thickness. Liver fibrosis was evaluated using the fibrosis-4 index and elastography.
Results:
SCA was identified in 13.5%. MASLD and MetALD were significantly associated with SCA in models adjusted for demographic and lifestyle factors (adjusted odds ratio [aOR], 1.26;95% confidence interval [CI], 1.19 to 1.33; aOR, 1.43; 95% CI, 1.30 to 1.58; respectively, p<0.001for both). However, these associations attenuated and lost statistical significance when metabolic risk factors were further adjusted. The risk of SCA increased with greater hepatic steatosis and liver fibrosis severity. In patients with MASLD, aORs were 1.70 (hepatic steatosis index >36),1.23 (fibrosis-4 index ≥1.3), and 1.78 (liver stiffness measurement ≥5.6 kPa), compared to indi-viduals without MASLD. Similar trends were observed in the MetALD group. Additionally, hyper-tension and clustering of ≥3 cardiometabolic risk factors were significantly associated with SCA inthe MASLD group, supporting the role of metabolic burden in SCA development.
Conclusions
MASLD and MetALD were associated with increased SCA risk, particularly in individuals with hepatic steatosis and fibrosis. These findings suggest that metabolic burden and liver disease severity jointly contribute to subclinical atherosclerosis risk.
9.Thelazia callipaeda (Nematoda: Spirurida) infections in 2 wildlife species, Prionailurus bengalensis and Nyctereutes procyonoides, in Korea

Thelazia callipaeda (Spirurida, Thelaziidae), a parasitic eye worm causing thelaziasis has been reported in humans and dogs in Korea. However, its occurrence in other potential reservoir hosts, including wild animals, remains unclear. In the present study, we described the 2 cases of thelaziasis from both of feral and captive wild animal in Korea. In August and November 2017, 2 cases of the parasitic infections were found in the third eyelid of rescued leopard cat Prionailurus bengalensis and reared raccoon dog Nyctereutes procyonoides at the Chungnam Wild Animal Rescue Center. A total of 20 and 24 worms were detected from the left and right eyes of leopard cat, respectively. In the left eye of the raccoon dog, 5 worms were recognized. Male worms were 969–11,860 μm long (10,600 μm on average) and 300–320 μm width (315 μm on average). Female worms were 13,430–15,330 (14,480) μm long and 320–370 (344) μm wide in size. They commonly had a characteristic scalariform buccal cavity and short esophagus. The vulva openings were located at the anterior of esophago-intestinal junction in females. The thelaziasis is reported in 2 species of wildlife, P. bengalensis and N. procyonoides, for the first time in Korea.
10.Enhancing recurrent laryngeal nerve localization during transoral endoscopic thyroid surgery using augmented reality: a proof-of-concept study

Purpose:
During transoral endoscopic thyroidectomy, preserving the recurrent laryngeal nerve (RLN) is a major challenge because visualization of this nerve is often obstructed by the thyroid itself, increasing the risk of serious complications.This study explores the application of an augmented reality (AR) system to facilitate easier identification of the RLN during transoral endoscopic thyroidectomy.
Methods:
Three patients scheduled for transoral endoscopic thyroidectomy were enrolled in this proof-of-concept study. Preoperative computed tomography scans were used to create an AR model that included the thyroid, trachea, veins, arteries, and RLN. The model was overlaid onto real-time endoscopic camera images during live surgeries.Manual registration of the AR model was performed using a customized controller. The model was aligned with surgical landmarks such as the trachea and common carotid artery. Manual registration accuracy was assessed using the Dice similarity coefficient (DSC) to evaluate the alignment between the real RLN and the RLN of the AR model.
Results:
The 3 patients included were female (mean age, 33.3 ± 15.7 years), and the mean tumor size was 1.0 ± 0.3 cm. All patients underwent transoral endoscopic thyroidectomy of the right lobe. Final histopathological diagnoses comprised 2 papillary thyroid carcinomas and one follicular adenoma. The manual registration accuracy was 0.60, 0.70, and 0.57 for patients 1, 2, and 3, respectively, with a mean value of 0.6 ± 0.1.
Conclusion
The application of an AR system during transoral endoscopic thyroidectomy proved feasible and demonstrated potential for improving the localization of anatomical structures, particularly the RLN, as indicated by a moderate DSC.
