1.Clinical Advantages of Traditional Chinese Medicine in Treatment of Childhood Simple Obesity: Insights from Expert Consensus

Childhood simple obesity has become a significant public health issue in China. Modern medicine primarily relies on lifestyle interventions and often suffers from poor long-term compliance, while pharmacological options are limited and associated with potential adverse effects. Traditional Chinese Medicine (TCM) has a long history in the prevention and management of this condition, demonstrating eight distinct advantages, including systematic theoretical foundation, diversified therapeutic approaches, definite therapeutic efficacy, high safety profile, good patient compliance, comprehensive intervention strategies, emphasis on prevention, and stepwise treatment protocols. Additionally, TCM is characterized by six distinctive features: the use of natural medicinal substances, non-invasive external therapies, integration of medicinal dietetics, simple exercise regimens, precise syndrome differentiation, and diverse dosage forms. By combining internal and external treatments, TCM facilitates individualized regimen adjustment and holistic regulation, demonstrating remarkable effects in improving obesity-related metabolic indicators, regulating constitutional imbalance, and promoting healthy behaviors. However, challenges remain, such as inconsistent operational standards, insufficient high-quality clinical evidence, and a gap between basic research and clinical application. Future efforts should focus on accelerating the standardization of TCM diagnosis and treatment, conducting multicenter randomized controlled trials, and fostering interdisciplinary integration, so as to enhance the scientific validity and international recognition of TCM in the prevention and treatment of childhood obesity.
2.Qualitative study on the process evaluation of tobacco control intervention in junior high school

Objective:
To evaluate the implementation process of a school-family collaborative "online+offline" tobacco control intervention program in junior high school in Beijing and to explore the execution status, influencing factors and sustainability potential of the intervention, so as to provide evidence for optimizing youth tobacco control strategies.
Methods:
In November 2024, using the random number table method, four first year junior high school classes were selected from three schools each in Fengtai District, Tongzhou District, and Fangshan District of Beijing. One class served as the control group, while the other three classes were designated as intervention groups (one each for online intervention, offline intervention, and combined online offline intervention). The control group received only conventional education.The online intervention group was engaged in WeChat push interventions, including watching micro videos, viewing promotional materials, participating in online quizzes and mini games; the offline intervention group attended knowledge lectures, played peer games, and participated in offline knowledge competitions; the combined online offline intervention group integrated all the aforementioned online and offline intervention measures. The intervention period was from November 2024 to June 2025, spanning a total of 7 months. Based on the Practical, Robust Implementation and Sustainability Model(PRISM) framework, a qualitative research design was employed to conduct semi structured interviews with 48 participants (12 in each of the intervention groups and 12 organizational staff members) from the Centers for Disease Control and Prevention (CDC) in 3 districts and 3 sampled schools. The interview outlines were designed according to the intervention plan. Data was managed using Nvivo 12.0 software and analyzed following Colaizzi s seven step phenomenological analysis method. Theoretical saturation was assessed using a reserved subset of transcripts.
Results:
Four core themes were identified in the tobacco control intervention process. Overall fidelity of intervention implementation was largely consistent with the original plan, and students showed strong willingness and positive evaluations toward interactive formats such as knowledge contests and peer games, though occasional breakdowns in school-family communication and blurred boundaries between online and offline components were observed; the participants showed a polarized response in terms of satisfaction and participation, most students and parents recognized the significance of the activity, and some parents observed a reduction in smoking behavior; the implementation of internal tobacco control policies in the school was strict, and the atmosphere was favorable, but there was still room for improvement, such as the scarcity of community tobacco control activities and the difficulty in implementing smoke free units; implementation and sustainability infrastructure were preliminarily established, such as through homeroom teacher supervision and training student assistants to assisted in activities, while the sustainability support system required further refinement.
Conclusion
The school-family collaborative "online+offline" tobacco control intervention has demonstrated significant positive effects, but further optimization of activity design, enhancement of community reward mechanisms, and standardized training are required to improve the efficacy and sustainability of the intervention.
3.Progress and prospect of modern research methods for safety analysis of animal traditional Chinese medicine

Exogenous harmful residues and endogenous toxic components are the main contents of safety analysis for animal traditional Chinese medicine. This review summarizes the inspection methods for exogenous harmful residues, as well as the research methods for the toxic effects and mechanisms of endogenous toxic components. The strategies for enhancing efficacy and reducing toxicity of toxic animal drugs and quality control, and prospects the development trend of safety analysis for animal drugs were also discussed. In the detection of exogenous harmful residues in animal drugs, traditional methods such as atomic absorption spectrometry and inductively coupled plasma mass spectrometry are widely used, and new methods such as high-resolution mass spectrometry and biochemical analysis are continuously developing. In the study of endogenous toxic components, the toxic components and mechanisms of some animal drugs including cantharidin and toad venom have been revealed through chemical composition analysis, toxicity tests and multiomics technologies, and some strategies for enhancing efficacy and reducing toxicity have been proposed based on this. In the future, it is necessary to strengthen multidisciplinary integration to innovate detection technologies, clarify toxic mechanisms to achieve efficacy enhancement and toxicity reduction, and improve the biosafety research system, so as to enhance the quality and safety of traditional Chinese medicine animal drugs and promote the internationalization process of traditional Chinese medicine.
4.Correlation and Clinical Significance of Intestinal Microbiota Dysbiosis and Abnormal Cytokine Expression in Patients with Gastric Cancer

Objective To explore the correlation between alterations in intestinal microbiota composition and abnormal cytokine expression in patients with gastric cancer. Methods Fecal and peripheral blood samples were collected from 81 patients with gastric cancer (gastric cancer group) and 85 healthy subjects (healthy control group). Fecal bacterial DNA was sequenced through 16S rRNA gene amplicon sequencing, and the serum levels of 17 cytokines were measured with the AimPlex multiplex assay technology: IL-1β, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12p70, IL-15, IL-17A, TNF-α, TNF-β, GM-CSF, G-CSF, IFN-γ, IP-10, MCP-1, and VEGF-A. Results In the gastric cancer group, the abundances of 11 butyrate-producing bacteria, including certain genera of the Lachnospiraceae family (such as Lachnospira and Roseburia) and Faecalibacterium, considerably decreased. By contrast, the abundances of nine bacterial taxa, including Escherichia–Shigella, Streptococcus, Veillonella, and Parabacteroides, significantly increased. Correlation analysis showed that the decrease in the abundance of butyrate-producing bacteria were significantly negatively correlated with the high expression levels of pro-angiogenic and immunosuppressive cytokines in the gastric cancer group, including IP-10, IL-10, MCP-1, IL-8, IL-6, and VEGF-A. However, the decrease was significantly positively correlated with the downregulation of antitumor immune-related cytokines, including IL-2, IL-17A, IL-1β, TNF-β, and GM-CSF. Conversely, the enriched bacterial taxa exhibited opposite correlation patterns with these cytokines. Conclusion A close correlation is observed between intestinal microbiota dysbiosis and abnormal cytokine expression in patients with gastric cancer. The findings suggest that restoring the intestinal microbiota through fecal microbiota transplantation is a promising strategy for regulating cytokines and is a potential research direction for the adjuvant therapy of gastric cancer.
5.Effect of luteinising hormone levels on the outcome of in vitro fertilization and embryo transfer in an infertile population with normal ovarian reserve

ObjectiveTo investigate the effects of luteinizing hormone (LH) levels on in vitro fertilization and embryo transfer (IVF-ET) outcomes in an infertile population with normal ovarian reserve (NOR) undergoing controlled ovarian hyperstimulation (COH) with gonadotropin-releasing hormone antagonist protocol, and to explore the reasonable level intervals of LH during the follicular phase. MethodsA retrospective analysis was conducted on 911 NOR infertility patients undergoing IVF-ET treated with gonadotropin-releasing hormone antagonist protocol. Patients were grouped based on baseline LH (bLH) levels, LH levels on the day of antagonist addition, and LH levels on the day of human chorionic gonadotropin (hCG) injection. According to bLH levels, patients were divided into Group A2 (n=391) and Group B2 (n=391). Based on LH levels on the day of antagonist addition, patients were divided into Group A3 (n=600) and Group B3 (n=311). According to LH levels on the day of hCG injection, patients were classified into Group A4 (n=225), Group B4 (n=227), Group C4 (n=231), and Group D4 (n=228). The clinical characteristics, COH outcomes, and pregnancy outcomes of patients in each group were compared. Results① The bLH level could be used as a predictor of assisted reproduction outcomes in an infertile population with NOR, and going for assisted reproduction at bLH≥4.51 IU/L was more favorable for the development of pregnancy outcomes; ② Addition of antagonist at LH≥5 IU/L on the day of antagonist addition improved pregnancy outcome ;③ On the day of hCG injection, there was no statistically significant difference in pregnancy outcomes among the four groups; ④ Analyses using generalized linear models revealed that basal follicle-stimulating hormone (bFSH), basal estradiol (bE2), Gn days, Gn volume and 2PN affected the number of good quality embryos. ConclusionIn an infertile population with NOR using gonadotropin-releasing hormone antagonist, bLH and gonadotropin-releasing hormone antagonist addition day LH levels may influence IVF outcomes. LH levels should be controlled at 5 IU/L and above on antagonist addition days, and bLH levels should be controlled at 4.51 IU/L and above.
6.Efficacy and safety of Chinese herbal compounds for pulmonary nodules: A systematic review and meta-analysis

Objective To systematically evaluate the efficacy and safety of traditional Chinese medicine (TCM) compound in treating pulmonary nodules, providing evidence-based medical evidence for TCM intervention in pulmonary nodules. Methods Computer search of PubMed, CNKI, Wanfang, VIP, and SinoMed was conducted to select randomized controlled trials (RCTs) of TCM compound intervention in pulmonary nodules, with the retrieval time from the inception to November 29, 2023. The Cochrane bias risk assessment tool was used to evaluate the quality of the included studies, and Review Manager 5.4 was used for Meta-analysis. Results A total of 18 RCTs were included, covering 8 provinces across the country, with a total sample size of 1301 patients. The TCM compounds used in the included studies all incorporated the method of dissolving phlegm and dissipating nodules. There was a high risk of bias uncertainty in the included studies. Meta-analysis results suggested that TCM compound could significantly reduce the diameter of pulmonary nodules [MD=−1.41, 95%CI (−1.70, −1.13), P<0.001], decrease the number of nodules [MD=−0.37, 95%CI (−0.73, −0.01), P=0.05], alleviate clinical symptoms [MD=−4.84, 95%CI (−6.04, −3.64), P<0.001], and improve lung function [forced expiratory volume in one second (FEV1), MD=0.55, 95%CI (0.09, 1.01), P=0.02; FEV1/forced vital capacity, MD=6.12, 95%CI (4.47, 7.78), P<0.001]. However, there was no statistically significant difference in the probability of malignancy between the experimental group and the control group [MD=−0.01, 95%CI (−0.01, 0.00), P=0.09]. Conclusion TCM compound can significantly reduce the diameter of pulmonary nodules, decrease the number of nodules, alleviate clinical symptoms, and improve lung function, but future multicenter, large-sample, high-quality RCTs are still needed to further explore and verify this conclusion.
7.Intravesical instillation of urokinase plus heparin sodium for bladder clot irrigation:preliminary experience with 3 cases

Objective To evaluate the feasibility of intravesical instillation of urokinase and heparin sodium in the treatment of bladder clot irrigation.Methods This report discusses the diagnosis and treatment of three patients with bladder blood clot tamponade confirmed by abdominal and pelvic computed tomography(CT)scans enrolled from our hospital during Jun. to Sep. 2024, and discuss the relevant literature.Results A solution containing urokinase(20 000 IU)and heparin sodium(125 000 U)was instilled through a three-way catheter and retained for 30 minutes before drainage. Complete clot dissolution was achieved in all cases, with immediate relief of hypogastric distension and pain. Post-procedural CT showed restored bladder lumen patency. Hemoglobin levels decreased by 2-5 g/L without complications.Conclusion This preliminary trial demonstrated that intravesical instillation of urokinase-heparin provides a safe and effective minimally invasive alternative for bladder clot in irrigation cases.
8.Development and validation of a blood-based differential diagnosis model for pulmonary tuberculosis and community-acquired pneumonia

ObjectiveTo develop and validate a diagnostic prediction model for differentiating pulmonary tuberculosis from community-acquired pneumonia based on routine blood parameters. MethodsA total of 642 patients with pulmonary tuberculosis and 503 patients with community-acquired pneumonia were retrospectively enrolled from the Seventh Hospital of Tangshan. They were randomly divided into a training set and an internal validation set at a 7∶3 ratio. Additionally, 218 patients from the 981st Hospital were independently included as an external validation set. The Boruta algorithm and recursive feature elimination were employed to select predictors from 82 blood parameters, sex, and age. A multivariate Logistic regression model was established and evaluated using receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis. ResultsThe final model incorporated eight predictors, namely uric acid, urea nitrogen, alkaline phosphatase, monoamine oxidase, alanine aminotransferase, glutathione, neutrophil percentage, and age. The model achieved areas under the curve (AUCs) of 0.800 (95%CI: 0.769-0.830), 0.787 (95%CI: 0.738-0.836), and 0.736 (95%CI: 0.667-0.835) in the training, internal validation, and external validation sets, respectively. The model demonstrated good calibration, and decision curve analysis showed clinical net benefit within the threshold probability range of 10%-80%. ConclusionThe developed model exhibits good discriminative ability and clinical utility, serving as an effective early screening tool for primary healthcare institutions.
10.Cytotoxic effects of the novel photosensitizer PEG-MTPABZ-PyC-mediated photodynamic therapy on gastric cancer cells.

OBJECTIVES:
The application of photodynamic therapy in solid tumors has attracted increasing attention in recent years, and the efficiency of photosensitizers is a crucial determinant of therapeutic efficacy. This study aims to evaluate the cytotoxic effects of a novel photosensitizer, PEG-MTPABZ-PyC, in photodynamic therapy against gastric cancer cells.
METHODS:
Gastric cancer MKN45 cells were treated with PEG-MTPABZ-PyC. A high-content live-cell imaging system was used to assess the cellular uptake kinetics and subcellular localization of the photosensitizer. The cytotoxic effects of PEG-MTPABZ-PyC-mediated photodynamic therapy were examined using the cell counting kit-8 (CCK-8) assay and flow cytometry, while the intrinsic cytotoxicity of the photosensitizer alone was verified by the CCK-8 assay. Intracellular reactive oxygen species (ROS) generation after photodynamic therapy was detected using 2'-7'-dichlorodihydrofluorescein diacetate (DCFH-DA).
RESULTS:
PEG-MTPABZ-PyC alone exhibited no cytotoxicity toward MKN45 cells, indicating excellent cytocompatibility. The compound efficiently entered cells within 6 hours and localized predominantly in lysosomes. Upon light irradiation, PEG-MTPABZ-PyC-mediated photodynamic therapy induced significant cytotoxicity compared with the control group (P<0.05) and generated abundant intracellular ROS.
CONCLUSIONS
The novel photosensitizer PEG-MTPABZ-PyC demonstrates potent photodynamic cytotoxicity against gastric cancer cells, showing promising potential for further development in gastric cancer photodynamic therapy.
