1.Adrenal Suppression During Biologic-Facilitated Steroid Tapering in Severe Asthma: A Real-World Tertiary-Centre Experience

Introduction:
A significant proportion of severe asthma patients require
oral corticosteroids (OCS) to maintain symptom control.
Prolonged OCS predisposes patients to hypothalamicpituitary-adrenal axis suppression which can be lifethreatening. Biologic therapies are effective steroid-sparing
options suitable for select severe asthma patients. We
evaluated steroid tapering outcomes and adrenal function
during biologic therapy in a tertiary severe asthma clinic.
Results:
A total of 43 patients were included. Median age was 66
years (interquartile range [IQR] 60–74), and 65% were male.
Mepolizumab was the most used (17 patients), followed
by benralizumab (15), tezepelumab (6), dupilumab (4),
and omalizumab (1). Median baseline prednisolone dose
was 10 mg (IQR 5–10). Median lowest dose achieved was
3 mg (IQR 0–3), sustained for at least 4 weeks in 90.7%
of patients. Median reduction in prednisolone dose was
5 mg (IQR 5–7.25). In all, 42% of patients successfully
discontinued maintenance steroids. Adrenal function was
assessed using morning serum cortisol (n = 37) and/or SST
(n = 28). Evidence of adrenal insufficiency was identified
in 19 patients (44%), including 15 patients with failed SST
and four with low morning cortisol suggestive of adrenal
suppression. Morning serum cortisol was indeterminate
in four patients. A total of 17 patients were referred to
endocrinology. During steroid tapering, 11 asthma-related
hospital admissions and eight emergency department
presentations occurred, involving 10 patients.
Conclusion
Biologics facilitate the reduction of the OCS burden in
severe asthma. Tertiary adrenal suppression is common
during steroid tapering and requires prompt biochemical
assessment and endocrinology referral. Structured
monitoring of adrenal function is essential during biologicfacilitated steroid tapering in routine clinical practice.

2.Metastatic Extra-Ovarian Steroid Cell Tumor Presenting with Hyperandrogenism and Transaminitis Post Oophorectomy

Introduction:
Steroid cell tumors are rare sex cord-stromal tumors,
accounting for <0.1% of ovarian neoplasms. Extraovarian
steroid cell tumors are exceptionally rare, often androgensecreting, and pose significant diagnostic challenges. Early
recognition is essential to prevent prolonged morbidity
from hyperandrogenism.
Case:
A 60-year-old female, 15 years after total hysterectomy
and bilateral salpingo-oophorectomy for a large ovarian
mass with massive ascites, presented with deranged liver
function tests on routine follow-up. Ultrasonography
and computed tomography imaging revealed multiple
hypervascular lesions in the liver, retroperitoneum,
and peritoneum, suggestive of metastatic disease, with
normal-appearing adrenal glands. Biopsy of a liver lesion
demonstrated a metastatic neoplasm with morphology and
immunoprofile favoring a steroid cell tumor. However,
metastasis from the adrenal cortex or an ovarian primary
could not be excluded.
Given the prior bilateral oophorectomy, metastatic adrenocortical carcinoma was initially suspected, prompting
endocrine evaluation. Further history revealed a 1-year
history of progressive virilization, including increased
facial hair and frontal balding. Hormonal studies
demonstrated elevated testosterone (12.2 mmol/L and
reference range 0.1–1.42) and dehydroepiandrosterone
sulfate (15.9 µmol/L and reference range 0.510–5.560)
and the adrenocorticotropic hormone level of 11.7
pmol/L (reference range 1.60–13.9) with suppressed
gonadotrophins. Additional workup for catecholamine,
cortisol, and aldosterone excess was unremarkable. The discordance between androgen excess and normal adrenal
imaging, despite absent ovarian tissue, suggested an extraadrenal androgen-secreting steroid cell tumor. A second
histopathology review and multidisciplinary discussion
with radiology, gynecologic oncology, and pathology teams
were undertaken. As the disease was deemed inoperable,
repeat retroperitoneal lesion biopsy confirmed metastatic
steroid cell tumor and guided palliative chemotherapy.
She was subsequently referred to gynecologic oncology
for systemic chemotherapy.
Conclusion
Extra-adrenal steroid cell tumors, though rare, should be
considered in patients with hyperandrogenism long after
bilateral oophorectomy, especially when adrenal imaging
is normal. Multidisciplinary evaluation and repeat biopsy
are often crucial for establishing the diagnosis and guiding
treatment.

3.Steroid-Responsive Encephalopathy Associated With Thyroiditis

Introduction:
Steroid-responsive encephalopathy associated with
thyroiditis (STREAT) is a rare clinical entity. Clinical
presentations of STREAT can range from a strokelike presentation to psychiatric symptoms. STREAT is
diagnosed based on four major criteria which include
altered cognitive function, new or worsening psychiatric
symptoms, elevated antithyroid antibodies, and exclusion
of infectious, toxic, metabolic, or neoplastic causes. Corticosteroids are the cornerstone of treatment, with reported
excellent response in neurocognitive symptoms resolution.
Case:
This was a case of a 48-year-old male with Graves’s disease
who underwent radioiodine therapy 4 months earlier
and was started on levothyroxine 100 mcg 2 weeks prior
to presentation. He presented with 4 days of behavioral
symptoms, irritable mood with auditory and visual
hallucinations. There was no history of fever, headache, limb
weakness, or seizure. On general appearance, the patient
was agitated, talking incoherently, and disorientated with
normal vitals. There was no delayed relaxation of the reflex. The examination of the cranial nerve, motor, sensory, and
cerebellar system was normal. A lumbar puncture showed
normal opening pressure with a high protein CSF content
of 1,596 mg/L. The thyroid-stimulating hormone (TSH) was
elevated at 72.7 uIU/mL and T4 at 9.56 pmol/L. Serum antithyroid peroxidase was elevated at 877.42 IU/mL, and antiTSH receptor antibody at 21.30 IU/L. CSF oligonal band,
serum aquaporin-4, and viral screening were negative; noncontrasted computed tomography brain revealed normal
findings. Based on the clinical history and examination,
a diagnosis of STREAT was made at the emergency
department. The patient was initiated on hydrocortisone
100 mg three times a day. Within 24–48 hours of steroid
therapy, marked improvement and subsequent resolution
of the mental status and behavioral symptoms were seen.
Conclusion
Hashimoto’s thyroiditis can rarely lead to STREAT, a
condition with diverse neurological manifestations, where
early recognition and prompt corticosteroid therapy are
essential for favorable outcomes.

4.Perioperative complications associated with routine preoperative glucocorticoid use among patients undergoing pituitary surgery with normal preoperative HPA axis: A retrospective cohort study

OBJECTIVE
This study determined the incidence of perioperative complications associated with routine preoperative glucocorticoid use in patients undergoing pituitary surgery with normal preoperative hypothalamo-pituitary-adrenal axis (HPA axis).
METHODOLOGYFrom 2011-2021 retrospective chart review, 243 patients undergoing pituitary surgery with normal preoperative HPA axis were analyzed into 2 groups: 1) with preoperative steroids and 2) without preoperative steroids. Development of postoperative complications was subsequently evaluated.
RESULTSIncidence of primary composite postoperative complications of in-hospital mortality, postoperative infection and postoperative diabetes insipidus (DI) was significantly increased among those who had preoperative steroids compared to those without (58.33% versus 33.33%, p-value 0.004) with an adjusted odds ratio of 2.90 (CI 1.29 to 6.53, p-value 0.010). Among the components of the composite outcome, post-operative DI was statistically higher among those who were given preoperative steroids (52.45% versus 28.21%, p-value 0.006) with an adjusted OR of 3.31 (CI 1.43 to 7.67, p-value 0.005). The incidence of postoperative adrenal insufficiency was similar between the 2 groups (20.15% with steroids versus 8.70% without steroids, p-value 0.258).
CONCLUSIONAmong patients with normal preoperative HPA axis, the routine use of preoperative steroids is associated with an increased risk of composite postoperative complications (in-hospital mortality, postoperative infection and postoperative DI). Steroid-sparing protocol is not associated with an increased risk of postoperative AI. The findings will encourage more rational use of steroids and minimize preventable complications.

5.Steroids combined with anticoagulant in acute/subacute severe cerebral venous thrombosis.

BACKGROUND:
Inflammation plays a critical role in severe cerebral venous thrombosis (CVT) pathogenesis, but the benefits of anti-inflammatory therapies remain unclear. This study aimed to investigate the association between steroid therapy combined with anticoagulation and the prognosis of acute/subacute severe CVT patients.
METHODS:
A prospective cohort study enrolled patients with acute/subacute severe CVT at Xuanwu Hospital (July 2020-January 2024). Patients were allocated into steroid and non-steroid groups based on the treatment they received. Functional outcomes (modified Rankin scale [mRS]) were evaluated at admission, discharge, and 6 months after discharge. Serum high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), cerebrospinal fluid (CSF) IL-6, and intracranial pressure were measured at admission and discharge in the steroid group. Fundoscopic Frisén grades were assessed at admission and 6 months after discharge. Univariate and multivariate logistic regression were used to evaluat associations between steroid use and favorable outcomes (mRS ≤2) at the 6-month follow-up. Paired tests assessed changes in hs-CRP and other variables before and after treatment, and Spearman's correlations were used to analyze relationships between these changes and functional improvements.
RESULTS:
A total of 107 and 58 patients in the steroid and non-steroid groups, respectively, were included in the analysis. Compared with the non-steroid group, the steroid group had a higher likelihood of achieving an mRS score of 0-2 (93.5% vs . 82.5%, odds ratio [OR] = 2.98, P = 0.037) at the 6-month follow-up. After adjusting for confounding factors, the result remained consistent. Pulsed steroid therapy did not increase mortality during hospitalization or follow-up, nor did it lead to severe steroid-related complications (all P >0.05). Patients in the steroid group showed a significant reduction in serum hs-CRP, IL-6, CSF IL-6, and intracranial pressure at discharge compared to at admission, as well as a significant reduction in the fundoscopic Frisén grade at the 6-month follow-up compare to at admission (all P <0.001). A reduction in serum inflammatory marker levels during hospitalization positively correlated with improvements in functional outcomes ( P <0.05).
CONCLUSION:
Short-term steroid use may be an effective and safe adjuvant therapy for acute/subacute severe CVT when used alongside standard anticoagulant treatments, which are likely due to suppression of the inflammatory response. However, these findings require further validation in randomized controlled trials.

7.Identification of GSK3 family and regulatory effects of brassinolide on growth and development of Nardostachys jatamansi.

This study identified 8 members including NjBIN2 of the GSK3 family in Nardostachys jatamansi by bioinformatics analysis. Moreover, the phylogenetic tree revealed that the GKS3 family members of N. jatamansi had a close relationship with those of Arabidopsis. RT-qPCR results showed that NjBIN2 presented a tissue-specific expression pattern with the highest expression in roots, suggesting that NjBIN2 played a role in root growth and development. In addition, the application of epibrassinolide or the brassinosteroid(BR) synthesis inhibitor(brassinazole) altered the expression pattern of NjBIN2 and influenced the photomorphogenesis(cotyledon opening) and root development of N. jatamansi, which provided direct evidence about the functions of NjBIN2. In conclusion, this study highlights the roles of BIN2 in regulating the growth and development of N. jatamansi by analyzing the expression pattern and biological function of NjBIN2. It not only enriches the understanding about the regulatory mechanism of the growth and development of N. jatamansi but also provides a theoretical basis and potential gene targets for molecular breeding of N. jatamansi with improved quality in the future.

8.The supraclavicular artery island flap: A practical approach for reconstruction of extensive cervicofacial defects following giant cavernous hemangioma resection

o demonstrate the utility of the supraclavicular artery island flap (SCAIF) as a viable alternative to free tissue transfer for reconstruction of large cervicofacial defects.
METHODSDesign:Case Report

RESULTSA 43-year-old woman with a giant cavernous hemangioma measuring 21.43 x 9.91 x 20.75cm underwent tumor resection and immediate reconstruction using SCAIF. Following preoperative embolization and complete tumor excision, an 11 x 15cm cervicofacial defect was successfully reconstructed using SCAIF. The patient experienced post-operative complications including multiple wound dehiscences and hypertrophic scar formation but achieved significant functional and aesthetic improvement. At three months follow up, the patient reported cessation of bleeding episodes, improved feeding capacity, and enhanced quality of life despite persistent tracheostomy dependence. Plans for secondary surgery include scar revision, commissuroplasty, and intralesional steroid injections.
CONCLUSIONThe supraclavicular artery island flap presents a practical, cost-effective alternative to free tissue transfer for extensive cervicofacial reconstruction, particularly in resource-limited environments or in patients where free tissue transfer is contraindicated. Advantages include good color match, wide arc of rotation, and ease of harvest. Despite potential complications, it offers excellent functional and aesthetic outcomes with acceptable donor site morbidity.

9.Isolation and identification of steroids from Cyanotis arachnoidea and their anti-HSV-1 activities in vitro.

The steroid constituents in the 75% ethanol extract of Cyanotis arachnoidea and their anti-HSV-1 activities in vitro were investigated in this study. The ethyl acetate fraction of the extract was isolated and purified using silica gel, ODS, Sephadex LH-20 column chromatography, and semi-preparative HPLC. Structural identification was conducted based on spectroscopic data. Ten steroid compounds were isolated and identified, namely makisterone A 3-acetate(1), makisterone A 2-acetate(2), makisterone A(3), ajugasterone C 2-acetate(4), ajugasterone C(5), 20-hydroxyecdysone 2-acetate(6), 20-hydroxyecdysone 3-acetate(7), podecdysone C(8), rubrosterone(9), and poststerone(10). Compounds 1 and 2 are new steroid compounds. The anti-HSV-1 activities of compounds 1-10 were evaluated using the CPE inhibition method. Vero cells were used in the experiment, with acyclovir as the positive control. The results showed that compounds 9 and 10 showed anti-HSV-1 activity, with EC_(50) values of 83.11 and 103.62 μg·mL~(-1), respectively.

10.Difference in liver injury induced by dictamnine between males and females: based on untargeted metabolomics.

In recent years, reports of adverse reactions related to traditional Chinese medicine(TCM) have been on the rise, especially some traditionally considered "non-toxic" TCM(such as Dictamni Cortex). This has aroused the concern of scholars. This study aims to explore the metabolomic mechanism underlying the difference in liver injury induced by dictamnine between males and females through the experiment on 4-week-old mice. The results showed that the serum biochemical indexes of liver function and organ coefficients were significantly increased by dictamnine(P<0.05), and hepatic alveolar steatosis was mainly observed in female mice. However, no histopathological changes were observed in the male mice. Furthermore, a total of 48 differential metabolites(such as tryptophan, corticosterone, and indole) related to the difference in liver injury between males and females were screened out by untargeted metabolomics and multivariate statistical analysis. According to the receiver operating characteristic(ROC) curve, 14 metabolites were highly correlated with the difference. Finally, pathway enrichment analysis indicated that disorders of metabolic pathways, such as tryptophan metabolism, steroid hormone biosynthesis, and ferroptosis(linoleic acid metabolism and arachidonic acid metabolism), may be the potential mechanism of the difference. Liver injury induced by dictamnine is significantly different between males and females, which may be caused by the disorders of tryptophan metabolism, steroid hormone biosynthesis, and ferroptosis pathways.
