1.The Trends in Research on the Health of North Korean Refugees.

PURPOSE: This study aims to identify the general characteristics, subjects, and methods of research on the health of North Korean refugees through a systematic literature review. METHODS: A total of 140 studies on health were reviewed using the analytical framework developed by the researchers. RESULTS: The quantitative research comprised 90.7% of the studies, whereas the qualitative research were 7.9% of them. Approximately 81.4% of those reviewed have publication dates spanning 2006 to 2015. Only 13.6% of the studies were conducted with the approval of an institutional review board. The subjects of studies were psycho-social health (151.8%), behavioral health (28.5%), cognitive health (15.0%), physical health (12.2%), integrated health (7.8%), and spiritual health (2.8%). Within the quantitative studies reviewed, the most commonly used study design was survey research (86.0%). The two sampling methods used most often were convenience sampling (53.5%) and snowball sampling (19.4%), and the most commonly used data-collection method were questionnaires (94.6%). As for the qualitative studies, the most commonly used study design was phenomenology. CONCLUSION: The results suggested that the reviewed studies focused on physical health more than psycho-social health, and physiological measurement more than questionnaires. Ethical considerations need to be expanded.

2.Primary Adenocarcinoma with Focal Choriocarcinomatous Differentiation in the Sigmoid Colon.

Primary colorectal choriocarcinoma is a rare neoplasm. Only 19 cases have been reported worldwide, most of which involved adenocarcinomas. The prognosis is usually poor, and the standard therapy for this tumor has not been established. A 61-year-old woman presented with constipation and lower abdominal discomfort. She was diagnosed with primary adenocarcinoma with focal choriocarcinomatous differentiation in the sigmoid colon and liver metastasis. Because the serum beta-human chorionic gonadotropin level was not significantly elevated, and because only focal choriocarcinomatous differentiation was diagnosed, we selected the chemotherapy regimen that is used for the treatment of metastatic colorectal adenocarcinoma. The patient survived for 13 months after the initial diagnosis. This is the first case in Korea to assess the suppressive effects of the standard chemotherapy for colorectal adenocarcinoma against coexisting colorectal choriocarcinoma and adenocarcinoma.

3.Comparison of the Results among Automated Treponema pallidum Latex Agglutination, Rapid Plasma Regain and Treponema pallidum Particle Agglutination, Rapid Plasma Regain Card Test in Blood Donors.

BACKGROUND: The Korean Red Cross blood laboratory centers use Treponema pallidum particle agglutination assay on the PK7300 instrument as a primary donor screening test for syphilis, and semi-quantitative TPPA and RPR card as supplementary tests. We compared the results of Treponema pallidum latex agglutination and RPR tests on the automated analyzer with those of TPPA and RPR card tests. METHODS: A total of 1,000 samples with negative TPPA results and 103 samples with positive TPPA results (> or =1:80 titers) were evaluated in this study. HiSens Auto TP, RPR (HBI, Anyang, Korea) and Mediace TPLA, RPR (Sekisui, Tokyo, Japan) reagents were used on the automated analyzer. FTA-ABS test was performed as a confirmatory test to evaluate the sensitivity and specificity of HiSens Auto TPLA, RPR and Mediace TPLA, RPR reagents. RESULTS: The concordance rate between HiSens Auto TP, Mediace TPLA and TPPA was 95.5% and 95.4%, respectively. The concordance rate between HiSens Auto RPR, Mediace RPR and RPR card was 79.6% and 80.6%, respectively. Sensitivity of HiSens Auto TP and Mediace TPLA was 87.7% and 90.8%, respectively, and specificity was 99.5% and 99.0%, respectively. CONCLUSION: Despite the high concordance rate between TPLA and TPPA, there were negative TPLA results which were positive for both TPPA and FTA-ABS tests. Therefore, changing the primary donor screening test for syphilis from current TPPA to TPLA on the automated analyzer requires further investigation.

4.A Historical Trends of Doctoral Nursing Education in Korea.

PURPOSE: The aim of this study was to identify historical backdrop leading to the introduction of the doctorate degree of nursing in Korea, and to explore trends of doctoral nursing education program. METHODS: The research design was a descriptive study adopting a historical approach. Documentation data were collected through web sites and mail survey. The semi-structured interviews were conducted with 6 professors who were involved in the introduction of the doctorate degree of nursing. The outcomes of doctoral nursing education program were evaluated with a total of 1,153 dissertations' titles published from 1982 to 2007. RESULTS: First introduced in Korea in 1978, doctoral nursing education program had steadily increased totaling 21 doctoral program in 2007. This resulted in a rapid increase in the number of doctoral students, but the number of faculty and the quality were not as satisfactory as expected. Many doctoral program had the missions or goals that fostered nursing scholars, theorists, and researchers, a trend that seems set to continue. The majority of dissertations utilized the experimental design (39.9%), others were qualitative design (21.6%), and survey design (19.0%). CONCLUSION: Doctoral education that is the hallmark of nursing scholarship is further elaborated in terms of academic tradition of nursing school in Korea.

5.Harmonic ultrasonography for the detection of microlithiasis in the gallbladder.

PURPOSE: To validate the use of harmonic ultrasonography (US) in the detection of gallbladder microlithiasis. METHODS: From November 30, 2012, to January 18, 2014, fundamental US (FUS) and harmonic US with a high background noise (HUS-N) were performed for evaluation of gallbladder during the routine abdomen US. During the US, a dot-like stone (or stones) with Brownian motion was regarded as a positive finding of microlithiasis. Fifty-five patients with microlithiasis in the gallbladder detected on US were enrolled as the subjects of a retrospective review. With respect to the obtained images, two abdominal radiologists independently scored the conspicuity of gallbladder microlithiasis on FUS and HUS-N by using a 4-grade scale. The statistical analysis employed a kappa test and a Wilcoxon rank-sum test. RESULTS: For FUS, the conspicuity grades of gallbladder microlithiasis were G1 in 25 and 37, G2 in 21 and 9, G3 in 6 and 6, G4 in 3 and 3 patients, while HUS-N showed G1 in 0 and 0, G2 in 3 and 2, G3 in 12 and 15, and G4 in 40 and 38 patients, respectively, by each of the two radiologists. The kappa value was 0.633 for FUS between the two radiologists and 0.708 for HUS-N. HUS-N showed better conspicuity of gallbladder microlithiasis than FUS with significant P-values of less than 0.001 and 0.001 for the two radiologists, respectively. CONCLUSION: Compared with FUS, HUS-N enables better detection of microlithiasis in the gallbladder.

6.Three-dimensional finite element analysis of stress distribution for different implant thread slope and implant angulation.

PURPOSE: The purpose of this study was to find an inclination slope of the screw thread that is favorable in distributing the stresses to alveolar bone by using three dimensional finite element analysis. MATERIALS AND METHODS: Three types modelling changed implant thread with fixed pitch of 0.8 mm is the single thread implant with 3.8degrees inclination, double thread implant with 7.7degrees inclination and the triple thread implant with 11.5degrees inclination. And three types implant angulation is the 0degrees, 10degrees and 15degrees on alveolar bone. The 9 modelling fabricated for three dimensional finite element analysis that restored prosthesis crown. The crown center applied on 200 N vertical load and 15degrees tilting load. RESULTS: 1. The more tilting of implant angulation, the more Von-Mises stress and Max principal stress is increasing. 2. Von-Mises stress and Max principal stress is increasing when applied 15degrees tilting load than vertical load on the bone. 3. When the number of thread increased, the amount of Von-Mises stress, Max principal stress was reduced since the generated stress was effectively distributed. 4. Since the maximum principal stress affects on the alveolar bone can influence deeply on the longevity of the implants. When comparing the magnitude of the maximum principal stress, the triple thread implant had a least amount of stress. This shows that the triple thread implant gave a best result. CONCLUSION: A triple thread implant to increase in the thread slope inclination and number of thread is more effective on the distribution of stress than the single and double thread implants especially, implant angulation is more tilting than 10degrees on alveolar bone. Thus, effective combination of thread number and thread slope inclination can help prolonging the longevity of implant.

7.Job Analysis of Clinical Research Coordinators Using the DACUM Process.

PURPOSE: This study was done to analyze the job of clinical research coordinators (CRCs). METHODS: Through the "developing a curriculum (DACUM)" workshop, the definition of CRCs' role was described and CRCs' duties and tasks were identified. Finally, the developed duties and tasks were validated for importance, difficulty, and frequency. RESULTS: A CRC is defined as the one who coordinates and performs tasks related to clinical research/trials among investigators, participants, and sponsors according to the Good Clinical Practice at institutions conducting clinical trials. Twelve duties and 78 tasks were identified on the DACUM chart which represented the importance, difficulty, and frequency of tasks represented as A, B, and C respectively. Based on determinant coefficient (DC) of the task, the highest ranked task was confirming the eligibility of participants for research (DC=8.03) and the lowest was inventory management for clinical study materials (3.95). CONCLUSION: In this study, the job of a CRC was analyzed through the DACUM process and it was found that CRCs were doing various duties and tasks. Based on these results, it is suggested that it is necessary to develop CRC education programs considering the career ladder of CRCs.

8.Calcified Carcinoma of the Gallbladder with Calcified Nodal Metastasis Presenting as a Porcelain Gallbladder: A Case Report.

Porcelain gallbladder is regarded as a risk factor of gallbladder cancer. A porcelain gallbladder with calcified regional lymph nodes was found using computed tomography (CT) and magnetic resonance imaging (MRI) in a 43-year-old man who presented with nausea, vomiting, and abdominal pain. His cholecystectomy specimen showed diffuse wall thickening and contained small gallstones. Histological examination revealed diffuse infiltrative adenocarcinoma with extensive intratumoral calcification (calcified carcinoma). The majority of the calcified material was located within or replaced the tumor glands, and was not found in the stroma. A lymph node was totally replaced with a calcified metastatic adenocarcinoma. To the best of our knowledge, only one case of calcified lymph node metastasis from a calcified carcinoma of the gallbladder has been previously reported in the literature. We herein add a case of calcified carcinoma of the gallbladder with calcified lymph node metastasis, presenting as a porcelain gallbladder on CT and MRI.

9.Pregnancy-induced Osteoporosis Combined with Multiple Compression Fractures: A Case Report.

Pregnancy associated osteoporosis (PAO) is a rare condition. It may affect women during pregnancy or after the delivery and it can induce severe back pain. Physicians can find multiple compression fractures on the plain images of these patients. However, little is known about PAO, including the prevalence, the cause, the risk factors and the prognosis. Herein we report on a case of PAO in a 38-year-old female who suffered from severe back pain induced by multiple vertebral compression fractures. After excluding the possibility of unknown malignancy, the patient underwent vertebroplasty to improve the clinical symptom. The bone biopsy results confirmed multiple benign acute compression fractures. The patient was treated with oral bisphosphonate, calcium and vitamin D. She showed clinical improvement without developing any additional vertebral fracture. When young women during pregnancy or just after the delivery complain of persistent back pain, PAO should be considered in the differential diagnosis, and early recognition and treatment are needed for PAO.

10.Long-term Treatment Outcomes for Autoimmune Hepatitis in Korea.

Immunosuppressive therapy can improve clinical, biochemical and histological features and considerably prolong survival in patients with autoimmune hepatitis. Although ethnicity may affect disease severity and presentation, the long-term outcome of immunosuppression in Korean populations is unknown. This study was aimed to assess the efficacy of immunosuppressive therapy and determine the prognosis of autoimmune hepatitis in Korean populations. We reviewed the medical records of 86 patients diagnosed as having autoimmune hepatitis at the Samsung Medical Center between 1994 and 2008. Seventy-two (83.7%) patients reached remission after a median treatment duration of 3.5 months (range 1 to 44 months). Attempts to withdraw medications were made in 24 cases after the median treatment duration of 36 months (median 6 to 125 months). Thirteen of 24 (54.1%) patients relapsed after treatment withdrawal. Of the 86 patients, 6 (7.2%) experienced disease progression and the overall 5-and 10-yr progression-free survival rates were 91.2% and 85.5%, respectively. In conclusion, immunosuppressive therapy for autoimmune hepatitis results in a favorable rate of remission and excellent progression-free survival, but the relapse rate after treatment withdrawal is high. This suggests that long-term immunosuppressive therapy may be particularly important for treatment of Korean patients.
