Objective: As the demand for community mental health services continues to grow, the need for well-equipped and organized services has become apparent. This study aimed to optimize the roles and infrastructure of mental health services, by establishing, among other initiatives, standardized operating models. Methods: The study was conducted in multiple phases from May 12, 2021, to December 29, 2021. Stakeholders within South Korea and metropolitan mental health welfare centers were targeted, but addiction management support centers, including officials, patients, and their families, were integrated as well. A literature review and survey, focus group interviews, a Delphi survey, and expert consultation contributed to comprehensive revisions and improvements of the mental health service model. Results: The proposed model for community mental health welfare centers emphasizes the expansion of personnel and infrastructure, with a focus on severe mental illnesses and suicide prevention. The model for metropolitan mental health welfare centers delineates essential tasks in areas such as project planning and establishment, community research, and education about severe mental illnesses. The establishment of a 24-hour emergency intervention center was a crucial feature. In the integrated addiction support center model, the need to promote addiction management is defined as an essential task and the establishment of national governance for addiction policies is recommended. Conclusion This study proposed standard operating models for three types of mental health service centers. To meet the increasing need for community care, robust mental health service delivery systems are of primary importance.

Purpose: The purpose of this study is to compare scapular alignment, muscle strength around the scapula and neck according to the classification of forward head posture (FHP), round shoulder posture (RSP) and forward head with round shoulder posture (FHRSP). Methods: Scapular alignment, muscle strength around the scapula, and neck strength were measured according to the FHP and RSP alignment classification for male college students. Scapular alignment was confirmed by measuring acromial depression, scapular lateral displacement, and scapular acromion distance. Muscle strength was measured during isometric contraction of upper trapezius (UT), middle trapezius (MT), lower trapezius (LT), and neck muscles. Results: Acromial depression was significantly greater in the FHP (7.20±1.02 cm) than in the RSP group (5.60±1.26 cm) and FHRSP (5.26±1.75 cm) (p＜0.05). The strength of the UT was significantly greater in the FHRSP (1.12±0.12 N/BW) than in the FHP (0.87±0.19 N/BW), and the strength of the LT was significantly greater in the control group (0.20±0.04 N/BW) than in the FHP (0.15±0.03 N/BW) and RSP (0.15±0.04 N/BW) (p＜0.05). The LT:UT ratio was significantly greater in the control group (0.20±0.05) than in the FHRSP (0.15±0.02) (p＜0.05). Conclusion UT strength was higher in RSP subjects with scapular elevation accompanied by FHP than in those with FHP alone. And LT strength and the LT:UT ratio were higher in subjects withno abnormalities in postural alignment. We suggest that these results can serve as a reference for evaluation and intervention according to postural alignment in clinical practice.

Background: Small cell lung cancer (SCLC) is called ‘smoker’s disease’ because it is strongly associated with smoking and most cases occur in smokers. However, it can also occur in never smokers. We investigated the clinical features of never smokers with SCLC and compared their treatment outcomes with those of smokers with SCLC. Methods: We retrospectively reviewed the clinical data of patients who had proven SCLC and had received chemotherapy at a single cancer center between July 2002 and April 2021. Results: Of 1,643 patients, 1,416 (86.2%) were enrolled in this study. A total of 162 (11.4%) and 1,254 (88.6%) patients were never smokers and smokers, respectively. There were more female never smokers than smokers (n=130; 80.2% vs. 79, 6.3%, p=0.000), and the incidence of ischemic heart disease was lower among never smokers than among smokers (4/1,416, [2.5%] vs. 83/1,416 [6.6%], p=0.036). Never smokers showed less symptoms at diagnosis than smokers (80.9% vs. 87.2%, p=0.037); however, they showed more toxicity after first-line treatment (61.7% vs. 47.8%, p=0.001). The objective response rate (ORR) was significantly higher in never smokers (74.1% vs. 59.6%, p=0.000). In the multivariate analysis, never smoking and second-line treatment were associated with a better ORR. However, progression-free survival and overall survival were not significantly different between never smokers and smokers. Conclusion In conclusion, never smokers accounted for 11.4% of patients with SCLC. They had distinguishing clinical characteristics and showed better chemotherapeutic responses than smokers.

Purpose: Smoking may have a protective role in developing ulcerative colitis (UC) but have the opposite effect on Crohn’s disease (CD). This study aimed to determine the risk of developing inflammatory bowel disease (IBD) according to smoking status and onset age of smoking. Materials and Methods: We collected data on the smoking experiences of participants aged 20–39 years who underwent biannual examinations provided by the Korean National Health Screening Program from 2009 to 2012. IBD diagnosis was identified using the National Health Insurance Service. The risk of IBD according to smoking status and onset age of smoking was analyzed after adjusting for major clinical variables. Results: During a median 10.59-year follow-up, the risk of UC in ex-smokers was significantly higher than that in non-smokers, and the earlier ex-smokers started smoking, the higher risk of UC [ex-smokers whose onset age of smoking was <20 years, adjusted hazard ratio (aHR) 1.928, 95% confidence interval (CI)=1.649–2.255; 20–24 years, aHR 1.728, 95% CI=1.541–1.939; 25–29 years, aHR 1.676, 95% CI=1.489–1.887; ≥30 years, aHR 1.226, 95% CI=1.010–1.486]. The risk of UC was significantly lower in current smokers whose onset age of smoking was 25–29 years than in non-smokers (aHR 0.825, 95% CI=0.709–0.959). The risk of CD did not differ according to smoking status and onset age of smoking. Conclusion Ex-smokers who started smoking at a young age have a high risk of UC, even after adjusting for the smoking amount.

The Short Inflammatory Bowel Disease Questionnaire (SIBDQ) score is strongly correlated with HRQoL in IBD patients. This study aimed to assess the factors influencing HRQoL in IBD patients. Methods: In this prospective study, all patients with ulcerative colitis (UC) and Crohn’s disease (CD) completed the SIBDQ at enrollment; some patients also completed a second SIBDQ at follow-up. Multiple linear regression analysis was used to determine associations between SIBDQ scores and clinical factors. Results: A total of 1,020 patients participated (UC, 67%; CD, 33%). The median SIBDQ score was 52 (interquartile range, 44 to 59). In UC patients, the stool frequency (β=–2.333, p<0.001), Physician Global Assessment score (β=–3.950, p<0.001), fecal calprotectin level (β=–4.014, p<0.001), and corticosteroid use (β=–4.809, p=0.006) were negatively correlated with the SIBDQ score. In CD patients, the number of diarrhea episodes per day (β=–1.467, p=0.024) and Crohn's Disease Activity Index score (β=–0.045, p<0.001) were negatively correlated with the SIBDQ score. A total of 202 patients completed the second SIBDQ within a mean of 3.4 years. The distributions of SIBDQ score changes were as follows: decrease >10%, 28%; –10%10%, 43%. In both the initial SIBDQ and follow-up SIBDQ, scores for items pertaining to systemic symptoms (tension and fatigue) were relatively low. Conclusions Bowel movement-related problems significantly affect the HRQoL of both UC and CD patients. IBD patients scored lower on SIBDQ items related to general well-being. After 3 years of follow-up at the IBD clinic, 43% of patients showed a significant improvement in HRQoL.

Despite advancements in antifungal therapies, the prevalence of dermatophytosis continues to increase, posing significant challenges for diagnosis and treatment. This review provides an overview of the prevalence, clinical features, and diagnostic methods for dermatophytosis in Korea. A comprehensive review of domestic reports and research on dermatophytosis in Korea was conducted using data from KoreaMed.This study covers the various clinical presentations, age distributions, seasonal trends, and types of dermatophytes identified in different body areas. Dermatophytosis in Korea have shown changing patterns in terms of prevalence and causative species. Trichophyton rubrum remains the most common causative organism, with varying presentations depending on the site of infection, such as tinea pedis, tinea corporis, tinea capitis, tinea faciei, and onychomycosis. Diagnostic practices largely depend on the potassium hydroxide smear test, which has limited sensitivity, particularly for onychomycosis. Recent trends indicate an increase in the incidence of dermatophytosis among older adults, which may be linked to underlying health conditions and lifestyle changes. Dermatophytosis in Korea exhibit diverse clinical manifestations influenced by factors such as patient age, body site, and seasonal variation. Improved diagnostic methods are needed to enhance the accuracy and guide appropriate treatment, especially with the emergence of antifungal-resistant strains. This review aims to provide a better understanding of the current state of dermatophytosis in Korea and contribute to more effective management and prevention strategies.
