Objective:To investigate the efficacy of nasolacrimal duct stent placement combined with endonasal endoscopic dacryocystorhinostomy (EES-DCR) in the treatment of chronic dacryocystitis and analyze the recurrence rate of this condition. Methods:A retrospective analysis was performed on 312 patients with chronic dacryocystitis admitted to Department of Otolaryngology Head and Neck Surgery, Xi'an People's Hospital (Xi'an Fourth Hospital) from April 2021 to April 2023. All patients were divided into the non-implantation group (EES-DCR only) and the implantation group (nasolacrimal duct stent placement combined with EES-DCR) based on the surgical procedure used. Baseline data were excluded using propensity score matching method, and 156 patients were included in each group. Clinical efficacy, incidence rates of complications, and epiphora status (epiphora degree, tear film breakup time, Schirmer test strip wetting length) before surgery and at 1 month after surgery, as well as recurrence rate within 1 year of treatment were compared between the two groups. Results:The total efficacy in the implantation group was significantly higher than that in the non-implantation group [98.08% (153/156) vs. 93.59% (146/156), χ2 = 3.93, P = 0.047). The overall incidence rate in the implantation group was significantly lower than that in the non-implantation group [5.13% (8/156) vs. 12.18% (19/156), χ2 = 4.90, P = 0.027). At 1 month after surgery, both the degree of epiphora and the wetting length of Schirmer test strip significantly decreased in both groups, while the tear film breakup time significantly increased (both P < 0.05) compared with before surgery. The degree of epiphora and the wetting length of Schirmer test strip in the implantation group were (1.25 ± 0.26) points and (12.37 ± 2.24) mm, respectively, which were significantly lower than those in the non-implantation group [(1.44 ± 0.29) points, (13.32 ± 2.60) mm, t = 4.81, P < 0.001; t = 3.45, P < 0.001]. The tear film breakup time in the implantation group was (12.43 ± 2.31) seconds, which was significantly longer than that in the non-implantation group [(11.38 ± 2.15) seconds, t = 4.15, P < 0.001]. The total recurrence rate in the implantation group was 5.13% (8/156), which was significantly lower than that in the non-implantation group [11.54% (18/156), χ2 = 4.19, P = 0.041]. Conclusions:Nasolacrimal duct stent placement combined with EES-DCR effectively treats chronic dacryocystitis, reduces the degree of epiphora, and lowers the recurrence rate of the disease. This combined therapy has few complications and is safe and effective.

Abstract: Objective To confirm three severe scrub typhus patients in Mianyang of Sichuan Province using laboratory diagnosis and analyze their clinical characteristics to provide theoretical evidence for the prevention and treatment of scrub typhus in this area. Methods From June 2021 to August 2022, blood specimens and clinical data were collected from three severe scrub typhus patients in Mianyang of Sichuan Province. The epidemiological history was surveyed and the collected clinical data were analyzed. Acute-phase whole blood, serum samples at admission, and convalescent-phase serum samples two weeks later were collected, and the IgG titers of anti-Orientia tsutsugamushi were detected by indirect immunofluorescence assay (IFA) assays. Total DNA was extracted from anticoagulation whole blood using DNA extraction kit, and semi-nested polymerase chain reaction (PCR) was used to amplify the special gene of O. tsutsugamushi. Phylogenetic trees were constructed using the maximum likelihood (ML) method to determine the genotype of the strains. Results The clinical symptoms of these three patients were extremely similar to other infectious febrile diseases, with two cases showing obvious eschars on physical examination and all three cases causing pneumonia. Epidemiological investigation revealed that all had a history of wild animal exposure or fieldwork. Specific IgG antibodies to O. tsutsugamushi tests showed that convalescent serum antibodies increased by four times or more compared to the acute-phase serum. The TSA56 gene sequence fragment was obtained by PCR amplification, and BLASTn comparison on the NCBI website showed a close phylogenetic relationship with the TA763 genotype and Karp genotype of Orientia tsutsugamushi. The results indicated that these three cases were indeed infections of O. tsutsugamushi. Conclusions Laboratory diagnosis confirmed the clinical diagnosis of severe scrub typhus in three patients, providing specific evidence of etiology. This is the first report of etiology-confirmed cases of scrub typhus in Mianyang City, Sichuan Province. Analyzing their clinical features as well as the genotypes is of guiding significance for early diagnosis, differential diagnosis, and adjuvant therapy of scrub typhus in this region.

BACKGROUND:Polychlorinated biphenyls inhibit the differentiation of P19 cels into cardiomyocytes. In the meanwhile, microRNAs play an important role in regulating cel differentiation.
OBJECTIVE:To explore the effect of microRNA-32(miR-32) on the polychlorinated biphenyls-inhibited differentiation of P19 cels into cardiomyocytes.
METHODS:P19 cels were co-cultured with 2.5 μmol/L polychlorinated biphenyls and 1% dimethyl sulfoxide. Afterwards, α-actinin, desmin, cTnI and GATA4 were identified by western blot assay.Wddwxrof polychlorinated biphenyls on the expression of miR-32 was evaluated by real-time PCR. Mouse eukaryotic vector expressing miR-32 was constructed by gene recombination technology, andtransfected into P19 celsfolowed byco-culturedwith2.5 μmol/Lpolychlorinated biphenylsand1% dimethyl sulfoxide. Then, expressions of differentiation-related proteins were detected by western blot assay.
RESULTS AND CONCLUSION:Polychlorinated biphenylsinhibited the differentiation of P19 cels into cardio myocytes anddecreased the miR-32 expression. Cel lines overexpressing miR-32 was successfuly established in mice.Furthermore, we found thatoverexpression of miR-32 weakens the inhibition of polychlorinated biphenyls to the differentiation of P19 cels into cardiomyocytes.

OBJECTIVETo explore the functional role of protein kinase D1 (PKD1) in the activation of nuclear factor-κB (NF-κB) signal pathway and NF-κB transcription mediated by Aspergillus fumigatus.
METHODSA549 cells and HEK293 cells were transfected with green fluorescence protein (GFP) or GFP-PKD1 followed by treatment with 1×10(5) CFU/ml Aspergillus fumigatus conidia for different time lengths. The phosphorylation levels of PKD1, IκB and p65 (pS276) in the transfected cells were measured by Western blotting. A549 cells were transfected with GFP-PKD1 or siRNA-PKD1, and the phosphorylation of IκB and p65 (pS276) was examined. Finally, NF-κB-luc and renilla luciferase reporter pRL-SV40 were cotransfected into GFP- or GFP-PKD1-transfected A549 cells before exposure of the cells to Aspergillus fumigatus conidia for 24 h, and NF-κB transcriptional activity in the cells was determined using dual-luciferase reporter assay.
RESULTSOverexpression of PKD1 significantly increased Aspergillus fumigatus conidia-stimulated phosphorylation of PKD1, IκB and p65 (pS276), whereas PKD1 knockdown by siRNA-PKD1 suppressed IκB and p65 (pS276) phosphorylation. Dual luciferase assay demonstrated that PKD1 overexpression markedly enhanced Aspergillus fumigatus-induced NF-κB transcription in A549 cells.
CONCLUSIONPKD1 may contribute to the activation of NF-κB signal pathway and NF-κB transcription induced by Aspergillus fumigatus.
