1.Investigation of hyperuricemia in aircrew and analysis of the correlation with other biochemical indexes

Objective:To provide references for the prevention and treatment of hyperuricemia (HUA) in aircrews by investigating the prevalence of HUA in aircrews and analyzing the correlation between HUA and other biochemical indicators.Methods:The physical examination data of male aircrews who participated in physical examination in Beidaihe Rehabilitation and Recuperation Center of PLA from 2020 to 2022 were retrospectively analyzed. According to the year, they were divided into 2020, 2021 and 2022 groups. According to the age, they were divided into 20-29, 30-39, 40-49 and 50-59 years groups. The prevalence of HUA among different years and age groups were compared and analyzed. The subjects were divided into HUA group and non-HUA group according to whether they had HUA. The differences of physical examination indexes were compared between 2 groups, and the potential effects of other biochemical indexes on HUA were analyzed by multivariate Logistic regression.Results:A total of 2 365 aircrews were enrolled. Eight hundred and sixty-six aircrews were diagnosed as HUA, accounting for 36.62%. The prevalence rate increased year by year, and the difference was statistically significant ( χ2=6.70, P=0.010). There was no significant difference in prevalence rate among different age groups ( P>0.05). There were significant differences in creatinine ( t=9.40, P<0.001), serum uric acid ( t=59.66, P<0.001), urea nitrogen ( t=4.60, P<0.001), direct bilirubin ( Z=3.40, P=0.001), alanine aminotransferase ( Z=7.59, P<0.001), low density lipoprotein cholesterol ( t=5.89, P<0.001), triglyceride ( Z=8.02, P<0.001) and total cholesterol ( t=6.79, P<0.001) between HUA group and non-HUA group. Multivariate Logistic regression analysis showed that increased alanine aminotransferase ( OR=1.014, P<0.001), urea nitrogen ( OR=1.091, P<0.001), creatinine ( OR=1.038, P<0.001), and triglyceride ( OR=1.164, P=0.001) level were the risk factors for HUA. Conclusions:The prevalence of HUA in aircrew is increasing year by year. Alanine aminotransferase, urea nitrogen, creatinine and triglyceride are the risk factors for HUA. It is suggested that attention should be paid to the increasing trend of HUA in aircrews year by year, to the impact of abnormal liver and kidney function on metabolism and blood lipid levels, and early intervention measures should be taken for related risk factors.
2.Pharmacodynamic Substances in Promoting Osteogenic Differentiation of Epimedii Folium and Epimedii Wushanensis Folium Based on Chemical Fingerprint-cell Metabolomics Correlation Analysis

ObjectiveTo determine the pharmacodynamic substance basis of Epimedii Folium(EF) and Epimedii Wushanensis Folium(EWF) in promoting osteogenic differentiation, and to establish a method to analyze the material basis of Chinese materia medica based on the correlation between chemical fingerprint and cellular metabolomics. MethodThe chemical fingerprints of 15 batches of EF with 4 species and 3 batches of EWF were analyzed by ultra performance liquid chromatography-quadrupole-electrostatic field orbitrap high-resolution mass spectrometry(UPLC-Q-Exactive Orbitrap-MS), and partial least squares-discriminant analysis(PLS-DA) was used to analyze the peak areas of chemical fingerprints of samples. The effects of different samples on proliferative activity of MC3T3-E1 osteoblast precursors, as well as the activity of alkaline phosphatase(ALP) in osteoblasts were detected by cell counting kit-8(CCK-8) and enzyme-linked immunosorbent assay(ELISA). At the same time, UPLC-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS) was used to analyze the effects of different samples on the metabolomics of MC3T3-E1 cells, then metabolic peak table of osteogenic differentiation cells was constructed, and pharmacodynamic index mean Y0 was introduced into the peak table. PLS was used to calculate mean Y0 of each group, and the mean Y0 was added to the peak table of chemical fingerprint to construct the correlation between chemical fingerprint and cell metabolome, the pharmacodynamic components of EF and EWF that promote bone differentiation were screened according to variable importance in the projection(VIP) value>1. The pharmacodynamic effects of EF and EWF were evaluated according to the mean Y0 of each group. ResultThe chemical fingerprints of EF with different origins and EWF were completely separated. Compared with the blank group, the activity of MC3T3-E1 cells in EF and EWF groups was significantly increased, the activity of ALP in the Epimedium brevicornu(Gansu province), E. koreanum and E. pubescens groups was significantly increased(P<0.05). The results of cell metabolomics showed that the blank group and the model group had an obvious trend of separation. EF with different origins and EWF had different distance from the model group, indicating that EF with different origins and EWF had different effect on promoting osteogenic differentiation. Chemical fingerprint-cell metabolomics integration analysis screened 9 components closely related to the efficacy of EF and EWF, including diphylloside B, epimedin C, icariin, baohuoside Ⅰ, yinyanghuo B, β-anhydroicaritin, magnoflorine, cryptochlorogenic acid and quercetin. E. koreanum had the strongest effect on promoting osteogenic differentiation. ConclusionThis study determined that the material basis of EF and EWF promoting osteogenic differentiation were mostly flavonoids, alkaloids and organic acids, which provided ideas and methods for the screening of pharmacodynamic components and the prediction of therapeutic effect of Chinese materia medica.

6.Effect of varicocele on semen quality of officers and soldiers stationed along the coast

Objective:To investigate the grade of varicocele and its impact on semen quality of officers and soldiers stationed in coastal areas.Methods:The research subjects were 146 patients with left-sided varicocele and 32 healthy men selected from the male officers and soldiers who had visited the Beidaihe Sanatorium of PLA Joint Logistics Support Force. They were given physical and ultrasonic examination of spermatic vein and were divided into different groups by ultrasonic grading according to the inner diameter of their spermatic vein. Their semen quality was routinely analyzed.Results:There was no significant difference in semen volume or sperm deformity rate among the groups( P > 0.05). As varicocele aggravated,the sperm survival rate,sperm density,and the proportion of Grade A + B sperm all decreased significantly in the groups of clinical type II and III compared with those in the groups of normal type,subclinical type,and clinical type I( P < 0.05). Conclusion:The incidence of varicocele in officers and soldiers of coastal garrisons is significantly higher than that in the general population. The varicocele grade is closely related to the semen quality. Varicocele of clinical type I and below has less impact on the semen quality.

Objective:To explore the ultrasonographic characteristics of asymptomatic hyperuricemia(HUA)and gout knee joint lesions in naval officers and sailors,so as to improve the HUA prevention and treatment in them.Methods:A total of 168 HUA cases were selected from the naval officers and sailors admitted to the Beidaihe Rehabilitation and Recuperation Center of Joint Logistic Support Force from January 2016 to December 2021,of whom 22 cases were diagnosed as gout(gout group),and the other 146 cases were asymptomatic hyperuricemia(HUA group). Another 58 cases with normal blood uric acid who were admitted to the center during the same period were selected as the control group. All three groups underwent high-frequency and color doppler musculoskeletal ultrasonography on knee joints,ankle joints,first metatarsophalangeal joints,and the surrounding soft tissues. The correlations between serum uric acid level and articular cartilage lesions in the three groups were analyzed and compared.Results:Compared with the HUA group,the gout group had significantly higher serum uric acid level and body mass index(BMI)( P<0.05),and a significant higher detection rate of sonographic features,such as synovial hyperplasia,synovitis,synovial sac effusion,double track sign,bone erosion,and gout stone( P<0.05);besides,the mean cartilage thickness and maximum gout stone diameter in the gout group were significantly increased( P<0.05). Logistic regression analysis showed that the double track sign and gout stone in the HUA group were the influencing factors of the incidence of HUA in naval officers and sailors( P<0.05 or P<0.01). In the gout group,six typical sonographic features of synovial hyperplasia,synovitis,synovial sac effusion,double track sign,bone erosion,and gout stone,as well as mean cartilage thickness and maximum gout stone diameter,were the risk factors of gout in naval officers and sailors( P<0.05 or P<0.01). Conclusion:Musculoskeletal ultrasonography can detect the uric acid salt crystallization and the changes of joint structure in early stage of HUA,thus providing basis for early diagnosis of HUA and gout.

10.A study on the relationship between biomarkers and left ventricular myocardial native T 1 value in patients with lone atrial fibrillation

Objective:To investigate the relationship between serum procollagen Ⅲ amino terminal peptide (PIIINP), collagen I carboxyl terminal cross-linking peptide (CTXI), high-sensitivity C-reactive protein (hs-CRP) and cardiac magnetic resonance (CMR) T 1 mapping value in patients with lone atrial fibrillation (AF). Methods:Fifty-five patients with lone AF in Beijing Anzhen Hospital from July 2017 to June 2018 were prospectively enrolled. Another 20 healthy volunteers were examined at the same time to provide normal reference range. All patients completed PIIINP, CTXI, hs-CRP and CMR examination within one week. CMR examination including cine, pre-contrast T 1 mapping, and late gadolinium enhancement sequences. We used t test, Mann-Whitney U test or chi square test to compare the difference of the above indexes between AF patients and the control group. Spearman correlation analysis was used to determine the associations between left ventricular native T 1 value and blood biomarker in AF patients. Results:All the patients were paroxysmal AF with an average age of (48±10) years, of which 46 (83.6%) were male. The PIIINP, CTXI, hs-CRP, left ventricular native T 1 value of AF patients were 5.83 (3.52, 12.40) ng/ml, 4.63 (3.31, 6.82) μg/ml, 3.41 (1.72, 6.61) mg/L and (1 261±23) ms, respectively, which all significant higher than those in healthy subjects (all P<0.05). The native T 1 value of left ventricular myocardium was positively correlated with PIIINP ( r=0.492, P<0.05) and hs-CRP ( r=0.516, P<0.05), but not with CTXI ( r=0.021, P>0.05). Conclusions:The PIIINP, CTXI and hs-CRP increased in patients with lone AF, PIIINP and hs-CRP were correlated with elevated native T 1 value of left ventricular myocardium.
