1.A "crotch-shaped" autologous vascular graft in reconstruction of the defect of main artery in popliteal fossa to rebuild blood supply of lower extremities: a report of 10 cases

Objective:To explore the effect of autologous vascular graft bridging of the popliteal artery defect to reconstruct the blood supply of leg on survival of lower extremity.Methods:From May 2021 to May 2024, 10 patients with traumatic popliteal artery injury with defect were treated in the Department of Hand and Foot Surgery, Nanhua Hospital Affiliated to University of South China. A retrospective analysis was conducted on clinical data of the patients. Injury sites were: 6 in left lower extremity and 4 in right lower extremity. Causes of injury were: 4 of traffic accident, 2 of heavy object compression, 2 of electric saw, and 2 of falling from heights. Sixe patients had large vessel injury combined with fracture and (or) knee dislocation, and 4 had simple large vessel injury. Six patients had open vascular injury and 4 had closed vascular injury. There were 6 popliteal artery defect and 4 femoral artery defect, with the defects ranging from 5.0 to 10.0 cm in length. Based on the condition of the arterial defect, autologous great saphenous vein (9 patient) and small saphenous vein (1 patient) were taken and prepared to different forms of vascular bridging graft, such as "single crotch-shape" (6 patient), "double crotch-shape" (3 patient) and "triple crotch-shaped" (1 patient) vascular bridging grafts, to have the defects of popliteal artery reconstructed. Scheduled postoperative follow-up was conducted at outpatient clinic and via telephone interviews. Detailed outcomes including the peripheral blood flow, presence of necrosis or ulcer as well as patient satisfaction were recorded.Results:All of the blood vessel anastomoses in 10 patients were successful and the distal blood supply of the affected legs was restored. All of the affective legs survived. Postoperative follow-ups at 3 to 12 months showed good blood flow in the legs without necrosis nor ulcer as well as a high patient satisfaction.Conclusion:The "crotch-shaped" autologous blood vessel graft can timely and effectively reconstruct defected main artery in popliteal fossa, reduce ischemia time of the affected leg, promote extremity survival and restore its function.

3.Mesenchymal stem cells from different sources in treatment of inflammatory bowel disease

BACKGROUND:The incidence of inflammatory bowel disease has been steadily rising,accompanied by a lack of definitive therapeutic strategies.Recent research endeavors have illuminated the promising potential of mesenchymal stem cells in mitigating the symptoms of inflammatory bowel disease,offering a glimmer of hope for afflicted patients.OBJECTIVE:To review the mechanisms of action of mesenchymal stem cells derived from various sources in the management of inflammatory bowel disease,aiming to provide insights for future research endeavors.METHODS:Utilizing the keywords"mesenchymal stem cells,MSCs,exosomes,extracellular vesicles,EVs,inflammatory bowel disease,IBD,ulcerative colitis,UC,Crohn's disease,CD"in English and their Chinese equivalents in CNKI and PubMed databases,a total of 89 eligible articles were selected for this review.RESULTS AND CONCLUSION:Currently,six types of mesenchymal stem cells are being explored for inflammatory bowel disease therapy:bone marrow-derived mesenchymal stem cells,adipose tissue-derived mesenchymal stem cells,perinatal tissue-derived mesenchymal stem cells,induced pluripotent stem cell-derived mesenchymal stem cells,embryonic stem cell-derived mesenchymal stem cells,and gingival mesenchymal stem cells.Five administration routes have been adopted,with intravenous and intraperitoneal injections being the most prevalent,followed by local,mesenteric,and intra rectal injections.Their therapeutic mechanisms encompass differentiation,regeneration,anti-inflammatory effects,immune modulation,neuroprotection,antioxidant stress response,homing,modulation of gut microbiota,autophagy,ferroptosis,and endoplasmic reticulum stress.While sharing functional similarities,mesenchymal stem cells from different sources exhibit unique characteristics that confer them with distinct advantages and therapeutic potentials.Nevertheless,research into the specific properties of these mesenchymal stem cells remains limited,necessitating deeper exploration of their nuanced differences to optimize their therapeutic efficacy in inflammatory bowel disease.Additionally,the clinical safety of mesenchymal stem cells-based therapies warrants further observation and evaluation.

5.Efficacy of 450 nm blue laser vaporization prostatectomy in the treatment of benign prostatic hyperplasia

Objective To evaluate the safety and efficacy of 450 nm blue laser vaporization prostatectomy in the treatment of benign prostatic hyperplasia(BPH).Methods A retrospective analysis was conducted on 40 BPH patients treated with 450 nm blue laser vaporization prostatectomy at our hospital during Mar.and Nov.2023.The preoperative and postoperative maximum urinary flow rate(Qmax),post-void residual volume(PVR),international prostate symptom score(IPSS),and quality of life(QoL)score were compared.The operation time,postoperative bladder irrigation time,and postoperative hemoglobin decrease were recorded.Results All operations were successful.One month after surgery,the Qmax[(7.9±2.1)mL/s vs.(16.8±2.5)mL/s]was significantly higher,while PVR[(110.0±42.1)mL vs.(14.6±11.4)mL],IPSS[(25.0±3.1)vs.(11.8±4.0)],and QoL[(5.1±0.9)vs.(2.5±0.6)]were significantly lower,with statistical significance(P<0.05).The operation time was(52.5±15.5)min,the bladder irrigation time was(22.3±4.9)h,the postoperative hemoglobin drop was(6.5±3.8)g/L.Urinary retention occurred in 2 patients after removal of the catheter.Gross hematuria occurred in 1 patient after discharge.Conclusion Vaporization prostatectomy with 450 nm blue laser can significantly improve clinical symptoms and perioperative safety,with high vaporization efficiency and good hemostatic effects.It is worthy of clinical promotion and application.

Liquid chromatography-tandem mass spectrometry (LC-MS/MS) technology has the characteristics of high specificity and high throughput, making it rapidly applied and developed in the field of clinical testing. Its application in the monitoring of therapeutic drugs can effectively improve the quantitative accuracy and sensitivity, and formulate a personalized and optimal dosing plan for patients. However, this technology still faces some challenges, and automation, quality control, and quantitative traceability will be the future development direction.
7.Hyperaldosteronism caused by drospirenone and ethinylestradiol tablets: a case report

A young female patient with acne and elevated testosterone level underwent plasma steroid hormones testing and found a significant increase in aldosterone. We excluded testing interference and verified the absence of hypertension, hypokalemia, and adrenal occupancy, as well as primary and secondary hyperaldosteronism. During follow-up, a temporal correlation was found between aldosterone levels and the use of drospirenone and ethinylestradiol tablets. It was observed that the combination of drospirenone and ethinylestradiol could lead to the increase of aldosterone level and the concentration ratio of aldosterone to direct renin through different mechanisms. Drospirenone exerts an antagonistic effect on mineralocorticoid receptor to prevent the development of hypertension or hypokalemia. In clinical practice, it is necessary to pay attention to the effect of this drug on screening markers for primary aldosteronism. In the laboratory examination, when female patients with no symptoms of hypertension and hypokalemia but with elevated aldosterone levels are encountered, it can be verified whether they have a history of use of compound estrogen-progestin such as drospirenone and ethinylestradiol tablets, and appropriate tips are provided in the report.

The polyfoliate perforator flap is a new type of flap that was developed on the basis of the traditional polyfoliate myocutaneous flap, polyfoliate fascial flap and perforator flap. It overturns the traditional idea that the deep fascial vascular network is the fundamental for a survival of the flap, and enables the flaps to achieve the best profile and function of the recipient areas with minimal damage to the donor area. In order to improve the understanding of the polyfoliate perforator flap and further standardise its clinical application, this paper forms a consensus on the definition, classification, indications, operative points and precautions of the polyfoliate perforator flap, so as to provide references in diagnosis and treatment process and practical application for the surgeons.

Machine learning technology is more and more widely used in the medical field. Biliary tumor is a tumor originating from the biliary system with a very high degree of malignancy. Machine learning technology has been applied in the field of biliary malignancy to assist physicians in disease diagnosis and provide valuable prognostic prediction and early screening recommendations. This paper summarizes the research progress of machine learning in the field of biliary tract tumors in recent years, and provides references for the auxiliary diagnosis, early screening prediction and postoperative survival prediction of biliary tract tumors.
