OBJECTIVE To observe the effects of laminar flow cleaning system on disinfection of bacterial colo-nies in the air of bronchoscopy room and on protection of the health care workers.METHODS The specimens were collected for laboratory test from the Second Affiliated Hospital of Wenzhou Medical University between Jul.2024 and Sep.2024 and were divided into four groups according to the status of personnel activity and the disinfection system:the static control group, the static study group, the dynamic control group and the dynamic study group. The bacterial colonies were collected from the air of bronchoscopy room under the static and dynamic state by planktonic bacteria method, and the bacterial colonies were collected from the body surfaces of health care workers with the use of cotton swab.RESULTS The bacterial colony counts were(19.75±9.89)CFU/0.3 m3 in the static study group at 0 h,(7.95±4.08)CFU/0.3 m3 at 1 h(P=0.012);the bacterial colony counts were(87.50±30.07)CFU/0.3 m3 in the dynamic study group at 0h,(51.05±18.94)CFU/0.3 m3 at 4h(P<0.001);the bacterial colony counts declined in the study groups under both static and dynamic state. The bacteria colony counts of the two groups were(F=0.312,P=0.582)at 0 h under the dynamic state,(F=15.229,P=0.001)at 4 h;the bacterial colony counts were lower in the study group than in the control group at the rest of time points except for 0 h.There was no significant difference in the average bacterial colony counts on body surfaces of health care workers between the two groups at 0h;the bacterial colony counts of the control group were(1.23±1.19)CFU/cm2at 4 h,the study group(0.19±0.24)CFU/cm2(P=0.007);the difference value was(-0.77±0.99)CFU/cm2 in the control group,(-0.09±0.16)CFU/cm2(P=0.032)in the study group;the bacterial colony counts on body surfaces of the health care workers were increased less remarkably in the study group than in the control group.CONCLUSION The laminar flow cleaning system can remarkably reduce the bacterial colony counts in the air of bronchoscopy room, effectively control the increase of bacterial colonies on the body surfaces of health care workers, and offer better occupational protection of health care workers.

Objective:To evaluate the effect of accelerated rehabilitation surgery (ERAS) under diagnosis-intervention packet (DIP) in patients with early cancer of digestive tract undergoing endoscopic submucosal dissection (ESD).Methods:The 64 patients with early cancer of digestive tract treated with ESD in the Gastroenterology Department of the Second People′s Hospital of Jiaozuo were selected by randomized controlled trial and convenient sampling method. According to random number table method, they were divided into routine group and observation group, 32 patients in each group. All patients in the 2 groups paid their medical expenses by DIP method, the routine group was treated with traditional perioperative nursing, and the observation group was treated with ERAS perioperative management mode. The postoperative complication rate, length of hospital stay, DIP allocation ratio, and patient satisfaction with nursing were compared between the two groups. Results:There were 16 men and women in the routine group, 14 men and 18 women in the observation group. After intervention, the incidence of postoperative complications was 21.88% (7/32) in the routine group and 3.12% (1/32) in the observation group, and the difference between the two groups was statistically significant ( χ2=5.14, P<0.05). The length of stay was (10.93 ± 2.87) d in the routine group and (9.01 ± 1.53) d in the observation group, and the difference between the two groups was statistically significant ( t=4.13, P<0.05). The average hospitalization expenses per case was (20 108.23 ± 6 495.49) yuan in the routine group and (18 589.03 ± 4 439.46) yuan in the observation group, and the difference between the two groups was statistically significant ( t=20.57, P<0.05). The DIP allocation ratio of the observation group was 87.98% (303 419.26/344 872.99), and that of the routine group was 69.33% (244 864.99/353 187.65), and the difference between the two groups was statistically significant ( χ2=4.81, P<0.05). The satisfaction of the observation group was 96.88% (31/32) and the routine group was 78.13% (25/32), and the difference between the two groups was statistically significant ( χ2=5.14, P<0.05). Conclusions:The accelerated rehabilitation surgical nursing can effectively reduce the postoperative complications, the average length of stay, the average hospitalization expenses per case under DIP in patients with early cancer of digestive tract treated by ESD, improve the DIP allocation ratio of ESD diseases and the patient′s nursing satisfaction, which reflects the value of nursing work and can be applied to the nursing management of other surgical diseases.

The variable domain of heavy-chain antibody (VHH) has been developed widely in drug therapy, diagnosis, and research. Escherichia coli is the most popular expression system for VHH production, whereas low bioactivity occurs sometimes. Mammalian cells are one of the most ideal hosts for VHH expression at present. To improve the yield of VHH in Expi293F cells, we optimized the signal peptide (SP) and codons of VHH. Firstly, the fusion protein VHH1-Fc was used to screen SPs. The SP IFN-α2 showed the highest secretion as quantified by enzyme-linked immunosorbent assay (ELISA). Subsequently, codon optimization by improving GC3 and GC content doubled the yield of VHH1 and kept its binding activity to Senecavirus A (SVA). Finally, the mean yields of other 5 VHHs that fused with SP IFN-α2 and codon-optimized were over 191.6 mg/L, and these VHHs had high recovery and high purity in the culture supernatant. This study confirms that SP IFN-α2 and codon optimization could produce VHHs in Expi293F cells efficiently, which provides a reference for the large-scale production of VHHs.

Objective To explore the clinical effect of using small needle knife in autologous fat grafting for the treatment of atrophic scar. Methods Twelve patients with atrophic scar were included in this study during March 2014 and December 2016 in the Hangzhou First People's Hospital,Zhejiang University School of Medicine. We used small needle knife during fat grafting to treat the atrophic scar.Observer Scar Assessment Scale (POSAS) was used to assess the clinical effect of the patients. Results Aesthetic and functional improvements were observed.Resolution of pain and increases in scar elasticity were objectively assessable.Improvement of both clinical evaluation and patient perception was obtained. Conclusions Use of small needle knife during fat grafting is a good alternative for the treatment of atrophic scars.
