1.Single-Port Robotic Bilateral Cortical-Sparing Adrenalectomy in MEN 2A Patients With Bilateral Pheochromocytomas: A Case Report

Multiple endocrine neoplasia type 2A (MEN 2A) is a hereditary disorder characterized by medullary thyroid carcinoma (MTC), pheochromocytomas, and primary hyperparathyroidism. We present the case of a 28-year-old male diagnosed with MEN 2A, exhibiting bilateral MTC and bilateral pheochromocytomas. The patient underwent a total thyroidectomy with bilateral central lymph node dissection, followed by bilateral corticalsparing adrenalectomy (CSA) utilizing single-port (SP) robotic posterior retroperitoneoscopic approach. The patient’s blood pressure remained consistently stable at an average of 125/85 mmHg after surgery, and he was discharged without complications on postoperative day 5 with good postoperative recovery and patient satisfaction. Adrenal function was successfully preserved, allowing for gradual tapering of hormone replacement therapy. This case underscores the efficacy of SP robotic CSA in managing bilateral pheochromocytomas within the context of MEN 2A, highlighting its role in tumor resection while maintaining adrenal function. The minimally invasive nature of the SP robotic approach and CSA offered advantages including reduced operative time, diminished postoperative discomfort, and enhanced cosmetic outcomes. Nonetheless, careful patient selection and diligent long-term follow-up are imperative to monitor adrenal function and detect potential tumor recurrence.
2.Positive rate of canine pancreatic lipase immunoreactivity (SNAP cPL) in relationship with severity of clinical signs and concurrent disorders: a retrospective study

Acute pancreatitis (AP) is a common disorder characterized by the inflammation of the exocrine pancreas in dogs, and the severity of the clinical signs of pancreatitis varies greatly. Therefore, the diagnosis of pancreatitis is challenging. The canine pancreatic lipase immunoreactivity test (SNAP cPL; IDEXX Laboratories Inc., USA) is rapid and relatively accurate in diagnosing pancreatitis. The purpose of this study was to evaluate the positivity rate of the SNAP cPL test in dogs regarding the severity of gastrointestinal (GI) signs and concurrent diseases. Electronic medical records of dogs were reviewed. For retrospective classification, 80 dogs with GI signs who underwent the SNAP cPL test were enrolled. Additionally, concurrent diseases on the day of the SNAP cPL test were reviewed. A positive SNAP cPL test result was observed in 26 of the 80 dogs (32.5%) with GI signs. The severity of GI signs using a scoring system and the positivity rate of SNAP cPL were statistically related. Furthermore, dogs with concurrent chronic kidney disease had a significantly higher positivity rate of SNAP cPL than those without chronic kidney disease. This information might help clinicians to decide whether a SNAP cPL test is needed.

Public health and safety professionals, including firefighters, police officers, and emergency medical personnel, serve as critical pillars of public safety and societal well-being. These professions require navigating environments characterized by ‘brittleness’, ‘anxiety’, ‘nonlinearity’, and ‘incomprehensibility’—conditions that lead to chronic physical and psychological stress. This stress significantly elevates the risk of mental health disorders such as post-traumatic stress disorder, anxiety disorders, mood disorders, and sleep disturbances, while also increasing the likelihood of human errors driven by cognitive lapses. Such challenges extend beyond individual health, undermining organizational efficiency and ultimately jeopardizing public safety and societal welfare. Existing mental health management systems predominantly depend on reactive interventions, which are insufficient to meet the dynamic and unpredictable demands of high-risk occupational environments. As a proactive alternative, this paper introduces the concept of ‘Mind Vital Signs,’ an innovative framework that expands the traditional concept of vital signs into the mental health domain. Mind Vital Signs integrates physiological indicators—including heart rate variability, physical activity, respiratory rates, and sleep patterns—with psychological and behavioral data such as ecological momentary assessments and life logs. By employing real-time monitoring and advanced analytics, this multidimensional system facilitates early detection of mental health risks and supports targeted and timely preventive interventions. The implementation of Mind Vital Signs not only bolsters individual resilience and organizational stability but also enhances operational efficiency and strengthens public safety and societal well-being. Future research should prioritize clinical validation and policy development to ensure the effective integration and scalability of Mind Vital Signs in high-risk occupational settings.
