Objective:
The treatment rate for alcohol use disorder (AUD) in Korea is significantly lower than its prevalence rate. Because untreated AUD can have harmful consequences, it is important to identify the factors that contribute to individuals with AUD seeking mental health services.
Methods:
We collected nationally representative data from the National Mental Health Survey of Korea 2021 and analyzed responses from 643 individuals with AUD, of which 76.8% were male. Factors related to mental health service utilization among individuals with AUD were classified into three categories: sociodemographic (such as sex, age, marital status, education, and monthly household income), clinical (including symptom severity, psychiatric comorbidity, suicidality, and physical illness), and psychological characteristics (like perceived stigma, loneliness and social isolation, and resilience). We used multiple logistic regression analyses to examine each characteristic separately and combined in a single model to determine the most significant factors.
Results:
The three logistic regression models revealed that sex, psychiatric comorbidity, physical illness, and perceived stigma are significantly linked to the utilization of mental health services among individuals with AUD. Results from the comprehensive model indicated that only physical illness and perceived stigma have significant associations with mental health service utilization.
Conclusion
These findings can assist in developing targeted interventions for individuals with AUD.

6.Self-Medication Experiences and Pharmacy Services for Minor Ailments

Background:
This study aims to analyze the experiences of self-medication for minor symptoms, examine over-the-counter (OTC) drug usage patterns, and evaluate consumer satisfaction with pharmacy services to explore future improvements in self-medication practices.
Methods:
A 27-item questionnaire was used in this cross-sectional survey to examine self-medication experiences for minor ailments among adults living in Korea during March to April 2019 through an online channel.
Results:
Of the 354 respondents, 98.9% experienced minor symptoms such as cough, cold, headache, muscle pain, indigestion, lower back pain, sore throat, diarrhea, body aches, heartburn, constipation, and fever for the past year. 31.9% purchased OTC medication from pharmacies, 37.6% visited healthcare providers such as clinics, and 30.5% rested at home, to cope with them. The main advantage of self-medication wasidentified as being easier and more convenient than physician visits, while the primary concern was uncertainty about the accuracyof self-diagnosis. 91.8% purchased OTC drugs from a pharmacy in the past year, 53.8% trusted pharmacists’ product choices, and72.6% were satisfied with pharmacists’ medication guidance. In cases of chronic illness, the rate of visits to hospitals, clinics, orpharmacies was higher, while the proportion of those who “endure and bear with it” was lower (p = 0.012).
Conclusion
This study highlights the crucial role of pharmacists in improving the quality of self-medication and provides foundational data for enhancing pharmacy services.
7.Cancer-Specific Sequences in the Diagnosis and Treatment of NUT Carcinoma

Purpose:
NUT carcinoma (NC) is a solid tumor caused by the rearrangement of NUTM1 that usually develops in midline structures, such as the thorax. No standard treatment has been established despite high lethality. Thus, we investigated whether targeting the junction region of NUTM1 fusion breakpoints could serve as a potential treatment option for NC.
Materials and Methods:
We designed and evaluated a series of small interfering RNAs (siRNAs) targeting the junction region of BRD4-NUTM1 fusion (B4N), the most common form of NUTM1 fusion. Droplet digital polymerase chain reaction using the blood of patients was also tested to evaluate the treatment responses by the junction sequence of the B4N fusion transcripts.
Results:
As expected, the majority of NC fusion types were B4N (12 of 18, 67%). B4N fusion-specific siRNA treatment on NC cells showed specific inhibitory effects on the B4N fusion transcript and fusion protein without affecting the endogenous expression of the parent genes, resulting in decreased relative cell growth and attenuation of tumor size. In addition, the fusion transcript levels in platelet-rich-plasma samples of the NC patients with systemic metastasis showed a negative correlation with therapeutic effect, suggesting its potential as a measure of treatment responsiveness.
Conclusion
This study suggests that tumor-specific sequences could be used to treat patients with fusion genes as part of precision medicine for a rare but deadly disease.
8.Clinical Sensitivity of the (1–3)-β-D-glucan Test for Predicting Candidemia

The sensitivity of the (1–3)-β-D-glucan (BDG) diagnostic test for candidemia varies in different clinical settings, and its usefulness in early diagnosis of candidemia is suboptimal. We evaluated the sensitivity of the test for early candidemia prediction. All adult patients with culture-proven candidemia who underwent a serum Goldstream Fungus (1–3)-β-D-Glucan Test within seven days prior to candidemia onset at a tertiary referral hospital between January 2017 and May 2021 were included. Any-positive BDG results within seven days prior to candidemia onset were obtained in 38 out of 93 (40.9%) patients. The positive rate increased when the test was performed near the day of candidemia onset (P=0.04) but reached only 52% on the day of candidemia onset. We observed no significant differences between BDG-positive and -negative groups in terms of underlying disease, risk factors for candidemia, clinical presentation, origin of candidemia, and 30-day mortality. Candida albicans was significantly associated with positive BDG results than with all-negative BDG results (P=0.04). The Goldstream BDG test is unreliable for candidemia prediction because of its low sensitivity. Negative BDG results in patients with a high risk of invasive candidiasis should be interpreted with caution.
9.Relationship between Psychological and Physical Disability and Nutrient Intake in the Elderly -The 8th Korea National Health and Nutrition Examination Survey-

This study provides basic data when planning nutrition support for healthy aging by revealing the relationship between various symptoms in the elderly and nutrient intake. From the 8th National Health and Nutrition Examination Survey, data from senior citizens aged 65 years or older in 2021 were used, and answers on the depression prevalence, activity restrictions, complaints of discomfort when chewing, and nutrient intake were analyzed through SPSS multiple logistic regression analysis. In the case of depression, there was a significant effect on the intake of four of the 20 nutrients (water, protein, cholesterol, and vitamin E). When experiencing activity restrictions, the intake of eight of the 20 nutrients (phosphorus, magnesium, vitamin A, vitamin D, beta-carotene, retinol, thiamine, and niacin) was affected. When experiencing chewing discomfort, the intake of three of the 20 nutrients (fat, magnesium, and vitamin E) was affected. Elderly people who suffer from depression, activity restrictions, and discomfort with chewing are at risk of various nutrient deficiencies, and related studies need to be conducted.
10.Evidence integration on health damage for humidifier disinfectant exposure and legal presumption of causation

OBJECTIVES:
Inhalation exposure to humidifier disinfectants has resulted to various types of health damages in Korea. To determine the epidemiological correlation necessary for presuming the legal causation, we aimed to develop a method to synthesize the entire evidence.
METHODS:
Epidemiological and toxicological studies are systematically reviewed. Target health problems are selected by criteria such as frequent complaints of claimants. Relevant epidemiologic studies are reviewed and the risk of bias and confidence level of the total evidence are evaluated. Toxicological literature reviews are conducted on three lines of evidence including hazard information, animal studies, and mechanistic studies, considering the source-to-exposure-to-outcome continuum. The confidence level of the body of evidence is then translated into the toxicological evidence levels for the causality between humidifier disinfectant exposure and health effects. Finally, the levels of epidemiological and toxicological evidence are synthesized.
RESULTS:
Under the Special Act revised in 2020, if the history of exposure and the disease occurred/worsened after exposure were approved, and the epidemiological correlation between the exposure and disease was verified, the legal causation is presumed unless the company proves the evidence against it. The epidemiological correlation can be verified through epidemiological investigations, health monitoring, cohort investigations and/or toxicological studies. It is not simply as statistical association as understood in judicial precedents, but a general causation established by the evidence as a whole, i.e., through weight-of-the-evidence approach.
CONCLUSIONS
The weight-of-the-evidence approach differs from the conclusive single study approach and this systematic evidence integration can be used in presumption of causation.
