1.Role of exosomal miR-320c in gingerol-mediated defense against Staphylococcus aureus infection

ObjectiveTo investigate the inhibitory effect of gingerol, an active component of ginger, on Staphylococcus aureus (S. aureus) infection, and to preliminarily explore its mechanism related to extracellular vesicles (EVs). MethodsS.aureus infection models were established in human umbilical vein endothelial cell (HUVECs), Vero E6 cells, and C57BL/6 mice. Experimental groups included control, infection, and gingerol-treated groups. Bacterial load and VE-cadherin protein expression were detected using immunofluorescence and Western blot. EVs were isolated by size exclusion chromatography and characterized by transmission electron microscopy and nanoparticle tracking analysis. miRNA sequencing was performed on EVs. ResultsGingerol treatment significantly reduced the bacterial load in both in vitro and in vivo infection models and upregulated VE-cadherin expression. miRNA sequencing of EVs revealed that S. aureus infection upregulated the expression of hsa-miR-320c, while gingerol treatment reversed this abnormal expression. Bioinformatic analysis further predicted that the target genes of hsa-miR-320c were significantly enriched in cell junction-related pathways. ConclusionGingerol exhibits clear antibacterial and host-protective effects, by regulating hsa-miR-320c in EVs to maintain endothelial barrier integrity.

Farrerol is a dihydroflavone compound extracted from the dried leaves of Rhododendron dauricum belonging to the Ericaceae family. This article systematically reviews relevant research on farrerol both domestically and internationally, summarizes its pharmacological activities and mechanisms of action, and finds that it exerts cardiovascular and cerebrovascular protective effects by maintaining the homeostasis of vascular endothelial function and inhibiting vascular intimal hyperplasia; exerts renal protective effects by activating the nuclear factor erythroid 2-related factor 2 (Nrf2) signaling pathway to reduce renal damage; exerts liver protective effects by inhibiting the phosphatidylinositol 3-kinase/protein kinase B signaling pathway, activating the Nrf2 signaling pathway, improving insulin resistance, and reducing lipid deposition in the liver; exerts neuroprotective effects by activating the Nrf2/Keap1 signaling pathway, inhibiting the Toll-like receptor 4 and cyclic GMP-AMP synthase/stimulator of interferon genes signaling pathways to reduce neuroinflammatory damage; exerts bone protective effects by promoting tendon formation and inhibiting osteoclast differentiation; exerts antitumor effects by inducing tumor cell apoptosis and weakening tumor cell invasion and metastasis; and exerts antibacterial effects by downregulating α -toxin expression, interfering with β -lactamase/penicillin-binding protein 2a (PBP2a), and inhibiting PBP2a oligomerization. Currently, research on the pharmacological effects of farrerol is still mainly focused on animal and cell experiments. Further mechanistic studies and clinical trials are needed to provide theoretical basis for its new drug development and clinical application.

4.Prevention and Treatment Ideas of Epileptogenesis in Children under the Perspective of Traditional Chinese Medicine and Western Medicine

Epileptogenesis is a dynamic process of gradual progression from normal developing brain to pathological epileptic brain, which is the latent and budding stage of epilepsy. Combining the understanding of epileptogenesis in children from Western medicine and traditional Chinese medicine (TCM), we proposed that the viewpoints of constitutional transformation, phlegm pathogen inducing epilepsy, and brain collateral damage, which correspond to key pathological mechanisms, namely gene polymorphism, immunoinflammation, and microvascular dysfunction of the blood-brain barrier, respectively. Based on these insights, strategies for prevention and treatment of epileptogenesis in children, as well as potential research directions are explored.
6.Construction and application of the "Huaxi Hongyi" large medical model

Objective To construct large medical model named by "Huaxi HongYi"and explore its application effectiveness in assisting medical record generation. Methods By the way of a full-chain medical large model construction paradigm of "data annotation - model training - scenario incubation", through strategies such as multimodal data fusion, domain adaptation training, and localization of hardware adaptation, "Huaxi HongYi" with 72 billion parameters was constructed. Combined with technologies such as speech recognition, knowledge graphs, and reinforcement learning, an application system for assisting in the generation of medical records was developed. Results Taking the assisted generation of discharge records as an example, in the pilot department, after using the application system, the average completion times of writing a medical records shortened (21 min vs. 5 min) with efficiency increased by 3.2 time, the accuracy rate of the model output reached 92.4%. Conclusion It is feasible for medical institutions to build independently controllable medical large models and incubate various applications based on these models, providing a reference pathway for artificial intelligence development in similar institutions.
7.Prevalence of brucellosis patients combined with Mycobacterium tuberculosis infection and diagnostic efficiency of different tuberculosis immunological methods

Objective:To investigate the prevalence of brucellosis patients combined with Mycobacterium tuberculosis( M. tuberculosis) and diagnostic efficiency of different tuberculosis immunological methods in screening brucellosis patients complicated with tuberculosis. Methods:A retrospective study was conducted to collect medical records of hospitalized patients diagnosed with brucellosis in the Department of Infectious Diseases 920th Hospital of Joint Logistics Support Force from January 1, 2020 to December 31, 2023, who had tuberculosis interferon gamma release assay (IGRA) examination records. The incidence of M. tuberculosis co-infection in brucellosis patients with different characteristics, as well as the diagnostic efficiency of different tuberculosis immunological methods were compared. Results:A total of 233 patients with brucellosis were included, and 63 patients with combined with M. tuberculosis infection were detected, with a combined M. tuberculosis infection rate of 27.0%. According to different characteristics, the incidence of M. tuberculosis co-infection in brucellosis patients aged > 45 years (31.9%, 52/163) was significantly higher than that in patients aged ≤45 years (15.7%, 11/70), and the difference was statistically significant (χ 2 = 6.51, P = 0.011). Among different tuberculosis immunological methods, the positive rates of IGRA, tuberculosis antibody and tuberculin skin test (TST) were 21.0% (49/233), 5.3% (12/226), and 4.9% (9/185), respectively, with statistically significant differences among the three groups (χ 2 = 38.92, P < 0.001). Conclusions:The proportion of brucellosis patients combined with M. tuberculosis infection is relatively high, and middle-aged and elderly brucellosis patients are the key population for complicated infections. When screening brucellosis patients combined with M. tuberculosis infection, the diagnostic efficiency of IGRA is significantly better than that of tuberculosis antibody and TST testing.
8.Circ_0000437 promotes proliferation, invasion, migration and epithelial-mesenchymal transition of breast cancer cells by targeting the let-7b-5p/CTPS1 axis.

OBJECTIVES:
To investigate the role of circular RNA circ_0000437 in regulating biological behaviors of breast cancer cells and the molecular mechanism.
METHODS:
Breast cancer MCF-7 and MDA-MB-231 cells were transfected with sh-circ_0000437, mimics, inhibitor, si-CTPS1, or their respective negative controls. qRT-PCR was used to detect the expression levels of circ_0000437, let-7b-5p, CTPS1, Notch1, Hes1, and Numb in breast cancer cell lines and tissues. RNase R digestion was used to confirm the circular structure of circ_0000437 and its subcellular localization in the breast cancer cells was determined by cellular distribution analysis. The changes in proliferation, invasion and migration of the transfected cells were assessed using CCK-8 assay, Transwell assay and scratch assay. Dual-luciferase reporter gene and RNA immunoprecipitation assays were employed to validate binding interactions among circ_0000437, let-7b-5p, and CTPS1. The cellular expressions of CTPS1, E-cadherin, N-cadherin, and vimentin proteins were detected with Western blotting. A tumor-bearing mouse model was used to verify the oncogenic mechanism of circ_0000437 and CTPS1.
RESULTS:
Circ_0000437 and CTPS1 were upregulated while let-7b-5p was downregulated in breast cancer tissues and cell lines. Circ_0000437 or CTPS1 knockdown obviously suppressed breast cancer cell proliferation, invasion, migration and epithelial-mesenchymal transition (EMT). Overexpression of let-7b-5p produced similar inhibitory effects, whereas inhibition of let-7b-5p significantly enhanced malignant behaviors of the cells. In the tumor-bearing mouse models, circ_0000437 knockdown significantly suppressed tumor growth, but co-transfection of the cells with pcDNA-CTPS1 accelerated tumor growth. Binding sites were identified between circ_0000437 and let-7b-5p and between let-7b-5p and CTPS1, and circ_0000437, let-7b-5p, and CTPS1 showed functional interactions in breast cancer cells.
CONCLUSIONS
Circ_0000437 is upregulated in breast cancer tissues and cells, and its high expression promotes proliferation, invasion, migration and EMT of breast cancer cells through the let-7b-5p/CTPS1 axis.

9.Cleansing skin wound by irrigation with tap water replacing normal saline

Wound cleansing is an essential step in skin wound management. It can prevent local infection and optimize healing micro-environment by removing necrotic tissue and foreign matter, reducing microbial load, breaking bacterial biofilm formation and so on. Many randomized controlled trials and meta-analysis abroad have concluded that there is no significant difference in the incidence of wound infection and healing rate between the wounds irrigated with tap water and with sterile normal saline for skin wound cleansing. Considering the current requirements of medical fee policies in China, we recommend the use of tap water instead of saline or other wound cleansing solutions for cleansing skin wounds.
10.Supplementing rehabilitation training with low-frequency transcranial magnetic stimulation improves the abnormal spine posture of persons with Parkinson′s disease

Objective:To observe any effect of supplementing rehabilitation training with low-frequency repetitive transcranial magnetic stimulation (rTMS) on the abnormal spine posture of Parkinson′s disease (PD) patients.Methods:A total 40 PD patients with Pisa syndrome (PS) were randomly divided into an observation group and a control group, each of 20. Both groups received conventional drug therapy and rehabilitation training (including myodynamia training and balance function training), while the observation group was additionally provided with low-frequency rTMS of the primary motor cortical area (M1) on the convex side of the scoliosis. Before the treatment and after 2 and 3 weeks, the scoliosis angle was measured, and motor functioning and balance were evaluated using the timed up and go test (TUGT) and the Berg balance scale (BBS). The subjects′ mental state was quantified using the exercise self-efficacy (ESE) scale, and the modified Barthel Index (MBI) was used to quantify their ability in the activities of daily life.Results:After the treatment, significant improvement was observed in the average scoliosis angles, TUGT, BBS, ESE and MBI scores of both groups compared to the pre-treatment levels. In the control group, all of the indicators had returned to their pre-treatment levels 3 weeks after treatment, but in the observation group they remained significantly improved.Conclusions:Low-frequency rTMS combined with rehabilitation training can significantly reduce the scoliosis angle of PD patients, improve their motor functioning and balance, increase their exercise confidence and improve their ability in the activities of daily living over the long term. The combination is worth applying and promoting in clinical practice.
