OBJECTIVE To investigate the application and influencing factors of sodium-dependent glucose transporters 2 inhibitors(SGLT-2i) in patients with heart failure with preserved ejection fraction(HFpEF) in the real world. METHODS Data from 358 patients with HFpEF who were hospitalized at the First Affiliated Hospital of Chongqing Medical University from May 2023 to May 2024 were retrospectively collected. The patients were divided into the SGLT-2i group and the non-SGLT-2i group based on whether they were prescribed SGLT-2i upon discharge. Baseline characteristics, comorbidities, and differences in drug treatment were compared between the two groups. Based on univariate analysis, multivariate Logistic regression analysis was performed to identify independent influencing factors of SGLT-2i use in patients with HFpEF, followed by further stratified analysis. RESULTS Among 358 HFpEF patients, the overall utilization rate of SGLT-2i was 33.5%. Combined with type 2 diabetes [OR=9.063,95%CI(4.924-16.679) ] , atrial fibrillation [OR=3.135,95%CI(1.590-6.178) ] , coronary artery heart disease [OR=1.888,95%CI(1.072-3.327) ] and the use of loop diuretics [OR=3.822, 95%CI (1.588-9.200) ] were all independent influencing factors for the use of SGLT-2i in patients with HFpEF ( P <0.05). The results of the stratified descriptive analysis were consistent with those of the multivariate analysis, showing a higher utilization rate of SGLT-2i among patients with concomitant T2DM,atrial fibrillation, coronary artery heart disease, and those receiving loop diuretics ( P <0.05); whereas the utilization rate of SGLT-2i was comparable across patients with different levels of renal function ( P >0.05). CONCLUSIONS In the real-world clinical practice, the utilization of SGLT-2i in patients with HFpEF remains suboptimal, and treatment coverage still needs to be improved. Their use of SGLT-2i is primarily influenced by the presence of type 2 diabetes, atrial fibrillation, coronary artery heart disease, and the use of loop diuretics.

One of the basic questions in the aging field is whether there is a fundamental difference between the aging of lower invertebrates and mammals. A major difference between the lower invertebrates and mammals is the abundancy of noncoding RNAs, most of which are not conserved. We have previously identified a noncoding RNA Terc-53 that is derived from the RNA component of telomerase Terc. To study its physiological functions, we generated two transgenic mouse models overexpressing the RNA in wild-type and early-aging Terc-/- backgrounds. Terc-53 mice showed age-related cognition decline and shortened life span, even though no developmental defects or physiological abnormality at an early age was observed, indicating its involvement in normal aging of mammals. Subsequent mechanistic study identified hyaluronan-mediated motility receptor (Hmmr) as the main effector of Terc-53. Terc-53 mediates the degradation of Hmmr, leading to an increase of inflammation in the affected tissues, accelerating organismal aging. adeno-associated virus delivered supplementation of Hmmr in the hippocampus reversed the cognition decline in Terc-53 transgenic mice. Neither Terc-53 nor Hmmr has homologs in C. elegans. Neither do arthropods express hyaluronan. These findings demonstrate the complexity of aging in mammals and open new paths for exploring noncoding RNA and Hmmr as means of treating age-related physical debilities and improving healthspan.

Objective:The purpose of this study was to analyze the epidemiological characteristics of hand, foot, and mouth disease (HFMD) among individuals aged 6 years and above in Guangzhou from 2010 to 2023, with the intention that a scientific basis be provided for effective prevention and control measures in older age groups. Methods:Data on HFMD incidence among individuals aged 6 years and above in Guangzhou from 2010 to 2023 were collected and analyzed. Descriptive epidemiological methods were used to analyze the distributions and pathogen components of HFMD cases in Guangzhou residents aged 6 years and above. Spatial autocorrelation analysis and Getis-Ord Gi* analysis were conducted to identify the spatial clustering patterns of HFMD at the street town level. Results:From 2010 to 2023, the gender ratio of HFMD cases in persons aged 6 and above was 1.49∶1 in Guangzhou. The average annual incidence rate of HFMD among individuals aged 6 years and above in Guangzhou was 25.75 per 100 000. Furthermore, the reported incidence rate indicated an increased tendency. The annual incidence showed a bimodal distribution, with the main peak occurring from May to July and the secondary peak from September to October. HFMD incidence rates vary by townships, with hotspots clustered in urban and urban-rural regions. The dominant pathogen shifts from year to year. Enterovirus 71 (EV71) was the prevalent strain in 2010, followed by other enteroviruses and Coxsackievirus (CV)-A16 from 2011 to 2016. Since 2017, CV-A6 has steadily become the major pathogen. Conclusions:The incidence of HFMD cases among individuals aged 6 years and above in Guangzhou increased generally from 2010 to 2023, with hotspots localized in urban and urban-rural areas. The pathogen composition altered dramatically, with the proportion of EV71 dropping overall, while CV-A6 eventually became the dominating strain. Therefore, it is imperative to focus on the prevention and control of HFMD in this age group, especially by strengthening measures in areas with high prevalence.
5.Current status and progress of transarterial interventional treatment for liver metastases from neuroendocrine tumors
Journal of Interventional Radiology 2025;34(5):456-460
Clinically, neuroendocrine liver metastasis(NELM)is not uncommonly seen, which seriously affects the quality of life and prognosis of patients. For unresectable NELM,hepatic intra-arterial therapy is an effective option for controlling tumor progression and relieving symptoms. This therapy mainly includes transcatheter arterial embolization(TAE),transcatheter arterial chemoembolization(TACE),and selective internal radiotherapy(SIRT),and there was no significant difference in the long-term efficacy among the three therapies.Usually,TAE is recommended for patients with low-grade NELM.Conventional TACE may be considered for NELM patients who have poorly differentiated tumor or high hepatic tumor burden(HTB),and drug-eluting beads TACE is not recommended for NELM patients because it has significant hepatobiliary toxicity.SIRT can be adopted for NELM patients having portal vein thrombosis or having a history of biliary bowel surgery. In comprehensive treatment strategies for unresectable NELM,the combination use of hepatic intra-arterial interventional therapy and systemic therapy has already shown its promising prospects.Through reviewing the relevant literature, this paper summarizes the current situation and research progress of NELM.

ObjectiveTo observe the effect of Youguiwan on the leptin/Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) signaling pathway in the lung tissue of the rat model of chronic obstructive pulmonary disease (COPD) due to kidney-Yang deficiency. MethodForty rats were modeled for COPD with the syndrome of kidney-Yang deficiency by intratracheal instillation of lipopolysaccharide on day 1 and day 14 and continuous fumigation for 6 weeks, during which hydrocortisone was injected intramuscularly at an interval of 3 days. The modeled rats were randomized into model, high- (11.7 g·kg-1), medium- (5.85 g·kg-1), and low-dose (2.93 g·kg-1) Youguiwan, and aminophylline (0.054 g·kg-1) group. In addition, 8 SD rats were set as the blank group. After the completion of modeling, the rats in each group were administrated with the corresponding drug by gavage for 28 consecutive days. After the last administration, samples were collected. A lung function analyzer was used to evaluate the lung function of rats. Enzyme-linked immunosorbent assay was employed to measure the levels of interleukin-17A (IL-17A), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in the bronchoalveolar lavage fluid (BALF). Hematoxylin-eosin staining was employed to observe the pathological changes in the lung tissue, and Masson staining was employed to observe the deposition of blue collagen fibers around bronchi in the lung tissue and calculate the inflammation score. The immunofluorescence assay was employed to measure the protein content of collagen type Ⅰ (ColⅠ) and α-smooth muscle actin (α-SMA) in the bronchi. The protein and mRNA levels of leptin, IL-17A, JAK2, and STAT3 in the lung tissue were determined by Western blot and real-time fluorescence quantitative polymerase chain reaction, respectively. ResultCompared with the blank group, the model group showed decreased lung function (P<0.01), elevated levels of IL-6, IL-17A, and TNF-α in the BALF (P<0.01), and increased lung inflammation score, deposition of subcutaneous collagen fibers in the airway, and ColⅠ and α-SMA proteins (P<0.01). Furthermore, the modeling up-regulated the proteins and mRNA levels of leptin, IL-17A, JAK2, and STAT3 in the lung tissue (P<0.01) and enhanced the phosphorylation of JAK2 and STAT3 (P<0.01). Compared with the model group, high- and medium-dose Youguiwan improved the lung function, decreased the inflammation score, reduced collagen fiber deposition and ColⅠ and α-SMA proteins, lowered the levels of IL-6, IL-17A, and TNF-α in the BALF, down-regulated the mRNA and protein levels of leptin, JAK2, STAT3, and IL-17A, and weakened the phosphorylation of JAK2 and STAT3 (P<0.05, P<0.01). The aminophylline group had higher IL-17A and TNF-α levels than the high-dose Youguiwan group, lower IL-17A level than the medium and low-dose Youguiwan groups, and lower TNF-α level than the low-dose Youguiwan group. Compared with the aminophylline group, the high- and medium-dose Youguiwan groups showed reduced deposition of collagen fibers and protein levels of ColⅠ and α-SMA around the bronchi in the lung tissue (P<0.05, P<0.01), decreased inflammation score, and down-regulated protein and mRNA levels of leptin, JAK2, STAT3, and IL-17A in the lung tissue. ConclusionYouguiwan can prevent airway remodeling by inhibiting IL-17A to reduce inflammation and collagen deposition in COPD rats, which may be related to the inhibition of the leptin/JAK2/STAT3 signaling pathway.
