1.Construction and effect analysis of an assessment model based on DPSIR for operational quality of medical equipment

Objective:To construct an assessment model for operational quality of medical equipment by adopting Driving Force-Pressure-State-Impact-Response(DPSIR)model,and analyze its application value in operational management for medical equipment.Methods:The DPSIR model was adopted to formulate 20 evaluation indicators from five dimensions:driving force,pressure,state,influence and response.The evaluation indicators were evaluated by using composite weight method.According to the evaluation results,the management process of medical equipment was improved to achieve the optimal allocation for medical equipment.A total of 442 medical equipment in clinical use at Nanjing Brain Hospital from January 2022 to December 2023 were selected,and they were randomly divided into a conventional management mode group(221 equipment)and an assessment model mode group for operational quality of medical equipment by using DPSIR(221 equipment,model management mode group).The management quality of equipment operation,management efficiency and safety of using equipment between the two management modes were compared.A self-made questionnaire was used to investigate the satisfaction scores of managers for equipment,clinicians and clinical nurses on the clinical services of the equipment under the two management modes.Results:The scores of the standardization degrees of equipment operation,cleaning and disinfection,maintenance and repairing fault of adopting the model management mode were respectively(93.25±4.69),(93.47±5.36),(91.48±4.59)and(93.44±4.98),all of which were higher than those of adopting the conventional management mode,and the differences were statistically significant(t=15.489,12.920,10.905,16.354,P<0.05).The average utilization rate,stability rate of quality,and effective rate of diagnosis and treatment of the equipment of adopting the model management mode were(92.36±3.65)%,(90.58±4.36)%and(93.47±4.69)%,all of which were higher than those of adopting the conventional management mode,and the differences were statistically significant(t=12.376,10.726,11.721,P<0.05).The satisfaction scores of managers for equipment,clinicians and nurses for adopting the model management mode were higher than those of adopting the conventional management mode,and the differences were statistically significant(t=12.376,10.726,11.721,P<0.05).The incidence rates of corresponding infections,pressure-related injuries and risks caused by operation problems of equipment in adopting the model management mode were significantly lower than those in adopting the conventional management mode,and the differences were statistically significant(x2=12.280,15.000,14.141,P<0.05).Conclusion:The assessment model based on DPSIR for operational quality of medical equipment can enhance the management efficiency for medical equipment,and improve the satisfaction of users in using equipment,and raise the quality and level of management for equipment.
2.Plasma Metabolomic Analysis of Colorectal Cancer Patients with Spleen-Qi Deficiency and Damp-heat Stasis-toxin Syndrome Based on UPLC-Q-Exactive-Orbitrap-MS

ObjectiveTo observe and analyze the plasma metabolite differences among colorectal cancer patients with spleen-qi deficiency, damp-heat stasis-toxin syndrome(SRYD), non-spleen-qi deficiency, damp-heat stasis-toxin syndrome(non-SRYD), and normal human beings(Normal), aiming to identify unique metabolites specific to SRYD colorectal cancer patients and their potential biomarkers. MethodsBased on the diagnostic criteria of SRYD and non-SRYD colorectal cancer, 30 patients were included, including 10 patients with SRYD colorectal cancer and 20 patients with non-SRYD colorectal cancer, while 10 individuals were recruited for the Normal group. Metabolome sequencing of plasma from the three groups was performed by ultra-performance liquid chromatography-quadrupole-electrostatic field orbitrap mass spectrometry(UPLC-Q-Exactive-Orbitrap-MS). Multivariate statistical analysis were performed by principal component analysis(PCA) and partial least squares-discriminant analysis(PLS-DA), and the intergroup differential metabolites were identified based on variable importance in the projection(VIP) value>1 and t-test P<0.05. And pathway enrichment analysis based on Kyoto Encyclopedia of Genes and Genomes(KEGG) was performed to explore the metabolites and metabolic pathways specific to SRYD colorectal cancer patients. ResultsMetabolome sequencing results showed some differences in metabolic profiles between the groups. A total of 111 plasma differential metabolites were found in the SRYD group and the Normal group, of which 31 were up-regulated and 80 were down-regulated, mainly including stearoyl lysophosphatidylcholine, indole-3-acrylic acid, and dehydroepiandrosterone sulfate(P<0.05). The non-SRYD group exhibited 97 differentially expressed metabolites compared to the Normal group, with 36 up-regulated and 61 down-regulated, mainly including stearoyl lysophosphatidylcholine, sphingosine, and palmitoyl lysophosphatidylcholine(P<0.05). And the SRYD group exhibited 19 differentially expressed metabolites compared to the non-SRYD group, of which 5 were up-regulated and 14 were down-regulated, mainly including dihydrosphingosine, palmitic acid, and linoleoylethanolamide(P<0.05). The significant differential metabolites were subjected to KEGG analysis to obtain significantly enriched metabolic pathways in each group, and the results showed that 11 metabolic pathways such as primary bile acid synthesis, cholesterol metabolism and bile secretion were differential signaling pathways specific to SRYD colorectal cancer. Further retrieval of the above key signaling pathways showed that bile acids were up-regulated in both bile secretion and primary bile acid synthesis pathways, and there was a trend of up-regulation of glycochenodeoxycholic acid, taurochenodeoxycholic acid, and chenodeoxycholic acid. ConclusionPrimary bile acid synthesis, cholesterol metabolism, and bile secretion-related pathways may be differential signaling pathways specific to SRYD colorectal cancer, and bile acid is a core molecule in the metabolic pathway, which can serve as potential biomarkers closely related to the development and progression of SRYD colorectal cancer.

4.Sodium tanshinone ⅡA sulfonate inhalation attenuates pulmonary hyper-tension associated with chronic obstructive pulmonary disease in mice

AIM:To investigate the effects of nebulized sodium tanshinone ⅡA sulfonate(STS)in a mouse model of pulmonary hypertension associated with chronic obstructive pulmonary disease(COPD-PH).METHODS:A to-tal of 32 healthy SPF-grade male C57BL/6 mice were randomly divided into 4 groups:control(CTL,n=8)group,COPD-PH(CS+LPS,n=8)group,STS-treated COPD-PH(CS+LPS+STS,n=8)group,and STS(n=8)group.The COPD-PH model was established through whole-body exposure to cigarette smoke(CS)combined with lipopolysaccharide(LPS)in-halation.Mice were subjected to cigarette smoke exposure in a chamber(9 cigarettes/h,2 h/session,2 sessions/d,6 d/week)for 60 d,except on days of LPS inhalation.On days 1 and 14,COPD-PH model mice received LPS(7.5 μg/mouse in 50 μL saline)via intranasal inhalation,while the CTL and STS groups received an equivalent volume of saline.STS was administered via nebulized inhalation(5 mg/kg,30 min per session,twice daily)immediately before CS exposure.At the end of the modeling period,lung function and right heart pressure were assessed.Bronchoalveolar lavage fluid(BALF)was collected for inflammatory cell counting.Levels of interleukin-6(IL-6)in BALF supernatants and plasma were measured using ELISA.Pathological changes in the airway and lung tissues were evaluated.RESULTS:(1)Com-pared to CTL mice,those exposed to CS and LPS exhibited lesions characteristic of COPD-PH,including emphysema,lung inflammation,decreased lung function,and increased right ventricular systolic pressure(RVSP)and right ventricu-lar hypertrophy index(RVHI)(P<0.05);(2)COPD-PH mice showed significantly elevated IL-6 levels in both BALF and plasma(P<0.05);(3)STS treatment alleviated emphysema and lung inflammation,improved lung function,prevent-ed increases in RVSP and the RV/(LV+S)ratio,and reduced IL-6 levels in both BALF and plasma(P<0.05).CON-CLUSION:The results indicate that nebulized inhalation of STS significantly slows the progression of COPD-PH,likely due to its ability to inhibit lung inflammation and reduce IL-6 expression in the lungs.

7.Establishment of reference interval for detection indicators of thyroid function in children aged 0-15 years in Nanning area of China

Objective To establish the reference intervals for test indicators of thyroid function,namely thyroid stimulating hormone(TSH),free thyroxine(FT4),and free triiodothyronine(FT3),in the children aged 0 to 15 years old in Nanning,China.Methods A total of 1 289 healthy children aged 0 to 15 years old who attended the Guangxi International Zhuang Medicine Hospital Affiliated with Guangxi University of Chinese Medicine from October 2018 to August 2023 were selected.The concentrations in serum TSH,FT4,and FT3 were measured by chemiluminescent microparticle immunoassay(CMIA).According to the Clinical and Laboratory Standards Institute(CLSI)EP28-A3c guideline,the nonparametric percentile method was used to establish the reference intervals for TSH,FT4,and FT3 in the children aged 0 to 15 years old in Nanning area.Results The established reference intervals were as follows:TSH(male):0 to<1 month:0.88-7.81 μIU/mL,1 month to 15 years:0.59-5.06 μIU/mL;TSH(female):0 to<1 month:0.93-8.42μIU/mL,1 month to 15 years:0.60-4.30 μIU/mL.FT4(male):0 to<1 month:0.99-1.92 pg/mL,1 month to 15 years:0.86-1.33 pg/mL;FT4(female):0 to<1 month:1.05-2.06 pg/mL,1 month to 15 years:0.85-1.37 pg/mL;FT3:0 to<1 month:2.16-4.24 pmol/L,1 month to<11 years:2.75-4.49 pmol/L,11 to 15 years:2.45-4.34 pmol/L.Significant differences were observed among different gender and age groups for TSH,FT4,and FT3 levels(P<0.05).Conclusion This study successfully established the refer-ence intervals of TSH,FT4,and FT3 in the children aged 0 to 15 years old in Nanning area,which were significantly different among various gender and age groups.
8.The induction and cryopreservation of erythroid progenitor cells derived from umbilical cord blood mononuclear cells.

OBJECTIVETo discover the techniques for ex vivo generation and cryopreservation of erythroid progenitor cells (EPCs)derived from umbilical cord blood (UCB)mononuclear cells (MNCs).
METHODSUCB was chosen as the source of EPCs. Erythrocytes were precipitated by hydroxyethyl starch (HES). MNCs were separated by Ficoll density gradient centrifugation. Erythroid progenitor cell were generated from MNC ex vivo in suspension culture supplemented with stem cell growth factor, insulin growth factor, erythropoietin, Fms- liketyrosinekinase ligand, transferrin and dexamethasone. Cell maturation was evaluated by morphologic analysis and CD71/CD235a expression profiling. In vitro induced cells were cryopreserved using different cryopreservation media. The cell survival rate, phenotype and proliferation curves were detected after cell thawing.
RESULTSWith the extension of culture time, the total number of cells increased significantly accompanied with the elevation of CD71 and CD235 positive populations. After 14- day inducing, the cells reached to approximately 110 times of the starting number with the cell viability as (88.92±0.95)%. The percentages of cell surface markers were (86.77±9.11)% for CD71 and (64.47±16.67)% for CD71/CD235, respectively. With the extension of inducing time, wright- Giemsa staining showed that the middle erythroblasts appeared mostly at day 10, and the late erythroblasts were seen at day 14. The red pellets were present at day 14, which indicated the more production of hemoglobin. Colony forming assay showed that erythroid colonies at induction day 7 were higher than that for non-induced cells (326.00±97.96vs 61.60±20.03 per 2 000 cells). With the extension of culture time, the number of erythroid colonies decreased. Induced EPCs were preserved with different cryopreservation solutions, in which 10% DMSO were better than 5% DMSO. Additionally, 10% DMSO + 2% HSA showed no different with 10% DMSO + 5% HSA. Combined 50% plasma with 2% HSA was more effective.
CONCLUSIONSThis non- serum culture media could effectively induced and expanded EPCs, and 10% DMSO + 2% HSA + 50% plasma appeared to be a desirable cryopreservation solution for EPCs from UCB.
