Emerging evidence suggests that dysbiosis of the gut microbiota is associated with the pathogenesis of Parkinson's disease (PD), a prevalent neurodegenerative disorder. The microbiota-gut-brain axis plays a crucial role in the development and progression of PD, and numerous studies have demonstrated the potential therapeutic benefits of modulations in the intestinal microbiota. This review provides insights into the characterization of the gut microbiota in patients with PD and highlights associations with clinical symptoms and underlying mechanisms. The discussion underscores the increased influence of the gut microbiota in the pathogenesis of PD. While the relationship is not fully elucidated, existing research demonstrates a strong correlation between changes in the composition of gut microbiota and disease development, and further investigation is warranted to explain the specific underlying mechanisms.

2.Relationship between preoperative gut microbiota and postoperative ventilator-associated pneumonia in patients undergoing coronary artery bypass grafting

Objective:To evaluate the relationship between preoperative gut microbiota and post-operative ventilator-associated pneumonia (VAP) in patients undergoing coronary artery bypass grafting.Methods:This was a secondary analysis of a previous research project study. Patients who received invasive mechanical ventilation treatment after elective off-pump coronary artery bypass grafting at the Second Hospital of Hebei Medical University from April to September 2023 were selected and divided into VAP group and non-VAP group based on whether VAP occurred after surgery. Fecal samples were collected from patients before surgery, and 16S rRNA gene sequencing was used to analyze the characteristics of preoperative gut microbiota in the two groups. The differences in the diversity of gut microbiota between the two groups were compared. The linear discriminant analysis was used to identify the gut microbiota with significant differences between groups (differential bacteria), and logistic regression analysis was conducted to assess the association between differential bacteria and VAP. Receiver operating characteristic curves were drawn to analyze the predictive value of the differential bacteria for VAP.Results:A total of 79 patients were finally included, with 25 in VAP group and 54 in non-VAP group. The Beta diversity analysis showed statistically significant differences between VAP group and non-VAP group (pseudo- F=2.00, P=0.002). The linear discriminant analysis indicated that Bifidobacterium, Blautia and Megamonas were enriched in non-VAP group, while Klebsiella was enriched in VAP group. Multivariate logistic regression analysis showed that the relative abundance of Bifidobacterium was a protective factor for postoperative VAP ( OR=0.32, 95% confidence interval [ CI] 0.15-0.71, P=0.005), and the relative abundance of Klebsiella was a risk factor for postoperative VAP ( OR=2.49, 95% CI 1.143-5.43, P=0.022). The area under the receiver operating characteristic curve of the relative abundance of Bifidobacterium for predicting VAP was 0.80 (95% CI 0.69-0.90, P<0.001) and of the relative abundance of Klebsiella was 0.70 (95% CI 0.57-0.83, P=0.005). Conclusions:Bifidobacterium is a protective factor, while Klebsiella is a risk factor for postoperative VAP in patients undergoing coronary artery bypass grafting, and the relative abundance of both bacteria has a certain predictive value for VAP.

4.Study on the current status and influencing factors of unplanned readmission to ICU for neurosurgical patients

Objective To investigate the current status of unplanned readmission to the ICU in neurosurgical patients and to study its influencing factors,aiming to inform the construction of targeted nursing interventions for medical staff.Methods From January 2020 to September 2022,the relevant data of patients transferred out from the ICU of a tertiary-level hospital in Beijing were retrospectively analyzed.After using propensity score matching,a sample with balanced covariates between groups was obtained,and single-factor and Logistic regression analyses were used to study the influencing factors of unplanned readmission to the ICU in neurosurgical patients.Results The data of a total of 4 789 patients were included,and 159 patients(3.32%)had unplanned readmission to the ICU.Respiratory failure and pulmonary infection were the main respiratory complications causing unplanned readmission to the ICU in neurosurgical patients.Logistic regression analysis showed that the duration of mechanical ventilation,the length of the first stay in the ICU,tracheal intubations ≥2 times,presence of cerebrospinal fluid drainage tube or lumbar drainage tube,existence of mild to moderate consciousness disorder at discharge from ICU,low white blood cell count,and low hemoglobin count were independent influencing factors for unplanned readmission to ICUs in neurosurgical patients(P<0.05).Conclusion Medical staff should pay attention to the high-risk group of unplanned return to the ICU,assess the risk of transfer in a timely manner,carry out targeted nursing measures and strengthen the quality of nursing care to reduce the incidence of unplanned retum to the ICU in neurosurgical patients.
5.Analysis of the first frozen-thawed embryo transfer clinical outcomes in young patients applying natural cycle and hormone replacement therapy

Objective:To analyze the clinical outcomes of the first frozen-thawed embryo transfer (FET) in patients <35 years old applying natural cycle (NC) and hormone replacement therapy (HRT).Methods:A retrospective cohort study was conducted to analyze 4 814 young infertility patients who underwent the first FET in Reproductive Medicine Center of the First Affiliated Hospital of Zhengzhou University from January 2016 to June 2021. According to different endometrial preparation protocols, they were divided into 2 groups: NC group and HRT group, who were matched the baseline data using 1∶1 propensity score matching (PSM). After the matching, the two groups of their baseline data, pregnancy outcomes and perinatal outcomes were compared, and then we adjusted the confounding factors which affect live birth rate by univariate and multivariate logistic regression analysis. Based on antral follicle count (AFC), number of embryos transferred and number of high-quality embryos transferred, the effect of NC and HRT on the live birth rate were further analyzed.Results:Before PSM, 2 131 patients in NC group and 2 683 patients in HRT group were included. The differences in female age, male age, body mass index (BMI), basal follicle-stimulating hormone (bFSH), anti-Müllerian hormone (AMH), AFC, endometrial thickness on conversion day, and number of embryos transferred were all statistically significant between the two groups (all P<0.05). And the differences in number of high-quality embryos transferred and type of embryos transferred between the two groups were not statistically significant (all P>0.05). After PSM, 1 441 patients in each of NC group and HRT group were included, and there were no significant differences in their baseline characteristics such as female age, male age and BMI between the two groups (all P<0.05). The live birth rate [50.66% (730/1 441)] and the clinical pregnancy rate [60.31% (869/1 441)] in NC group were significantly higher than those in HRT group [44.69% (644/1 441), P=0.001; 54.27% (782/1 441), P=0.001], and the incidence of very low birth weight in NC group was significantly lower than that in HRT group, and there were no statistical significances in other indicators between the two groups (all P>0.05). After adjusting confounders including bFSH, AMH, AFC, endometrial thickness on conversion day, number of embryos transferred and high-quality embryos transferred using multivariate logistic regression analysis, the results showed that NC was an independent protective factor for live birth rate in the first FET cycle ( aOR=1.280, 95% CI: 1.103-1.486, P=0.001). Stratified analysis showed that those with AFC<11, AFC 11-12, 2 embryos transferred and 2 high-quality embryos tranferred in NC group had significantly higher live birth rate [49.03% (151/308), 49.09% (349/711), 56.38% (442/784), 57.85% (350/605)] than those in HRT group [36.36% (120/330), P=0.001; 43.14% (286/663), P=0.027; 48.97% (379/774), P=0.003; 48.68% (294/604), P=0.001]. Conclusion:NC-FET had higher live birth rate and clinical pregnancy rate than HRT-FET in young patients.

8.Comprehensive AIDS prevention ability of young students who is men have sex with men and associated factors

Objective:
To understand the characteristics of KAP (knowledge, attitude and practice) related to the prevention of AIDS among young male MSM (man sex with man) in China, and to evaluate the comprehensive ability of AIDS prevention and associated factors to provide a scientific basis for the development of MSM AIDS prevention and control.
Methods:
From November to December 2019, sampling was recruited by using snowball sampling methods centered on Beijing and Tianjin. A cross-sectional survey of 222 young students MSM was conducted through an online questionnaire with information of general demographic characteristics, sexual behavior and HIV testing, psychological status and comprehensive ability to prevent AIDS.
Results:
Average score on comprehensive AIDS prevention knowledge was (87.64±6.41),with knowledge,attitude,and behavior score of (11.32±1.67)(54.87±3.36) and (52.18±4.30) respectively.The pass rate of knowledge, attitude, practice related to AIDS prevention were respectively 95.5%, 56.3%, and 27.0%. The separation rate of knowledge from belief was 42.8%, the separation rate of knowledge from behavior was 71.2%, and the separation rate of faith from information was 48.2%. Whether have received AIDS prevention education was related to knowledge qualification rate (P<0.01).
Conclusion
Current AIDS education is associated with the knowledge level of AIDS prevention among young MSM groups, but it is not enough to improve their attitudes and behaviors. The comprehensive ability of MSM in preventing AIDS among young students is still not optimistic. Intervention measures need to be further strengthened to improve the comprehensive ability of young students MSM group to prevent AIDS.
9.Association between ego-centric social network and HIV status of MSM young students

Objective:
To identify the characteristics of social network and the association between ego-centric network and HIV status among young MSM Chinese students.
Methods:
The cross-sectional study was conducted in Chongqing, Tianjin, Harbin and Xi an city from April to December 2017 and from March to May 2018. A mixed recruitment method of snowball sampling and RDS approach was used to recruit participants who reported information on social network and received HIV test. The Multiple Regression Analysis method was used to for the analysis of association between ego-centric social network and HIV status of men who have sex with men (MSM) among young students.
Results:
The sample included 547 participants who nominated 1 088 social partners in total with average age of 13 to 60 years old. The MSM with different sexual orientation from their social members (aOR=0.38), embedded in a large network (aOR=0.63), with a high individual betweenness centrality (aOR=0.27) were at lower risk of HIV-positive status; while MSM who differed greatly in education level with their social members(aOR=1.60), existed in sexual networks(aOR=1.41), existed in the “risky networks” (aOR=1.88) , with high network density (aOR=1.91) and a high individual degree (aOR=4.10) had higher risk of HIV-positive status(P<0.05).
Conclusion
MSM with great difference in education level from social members, existed in sexual networks, with a large network density and a high degree were exposed to higher risk of HIV-positive status.
10.Effect of peer intervention by popular opinion leaders among young students who have sex with men

Objective:
The proposal of this study is to determine the feasibility of using online social software to guide popular opinion leaders(POLs) among young students MSM (men who have sex with men, MSM) carrying out comprehensive AIDS prevention in young students MSM.
Methods:
From November to December 2019 in Tianjin, 22 POLs who had previously experienced AIDS prevention education and training foucus on life skills were recruited. Though snowball sampling method, 100 POLs partners (friends, sexual partners, classmates, etc) were invited into the study. From January to March 2020, regular posters will be sent through the WeChat platform to guide POL to intervene their partners/sexual partners. The questionnaire surveys were conducted before and after the intervention.
Results:
After the intervention, both the proportion of young students MSM had sex and with multiple sex partners decreased(χ2=12.12, 5.94, P<0.05). The comprehensive ability of AIDS prevention has improved significantly, the "National Eight Items" correct rate has risen from 90.0% to 99.0%(χ2=5.82, 7.56, P<0.05), and the "Youth Eight Items" correct rate has increased from 86.0% to 98.0%(P<0.01). Knowledge score increased from(11.08±1.89) to (11.84±1.27); Attitude score increased from (54.29±4.01) to (56.06±3.75), the behavior score increased from (52.12±4.78) to (55.08±4.25), skill score increased from (51.96±4.75) to (54.67±5.04), The comprehensive prevention ability score (percent) increased from (87.30±6.95) to (91.86±5.42)(t=-4.21, -3.51, -5.32, -5.16, -6.45, P<0.01).
Conclusion
Using Wechat platform to guide POL carrying out comprehensive AIDS prevention is effective for improving the comprehensive AIDS prevention ability of young students MSM.
