1.Construction and application of an exercise intervention scheme for patients with colorectal cancer

Objective To construct an exercise intervention scheme for postoperative colorectal cancer patients and evaluate its application.Methods The research team summarized the evidence on exercise recommendations for patients with colorectal cancer(CRC)and constructed an exercise intervention scheme through group discussion,expert consultation,semi-structured interviews,and onsite consultation.Patients with CRC admitted to the Department of Colorectal Surgery of a tertiary A hospital in Jilin province were selected as the study subjects by a convenience sampling method from October 2023 to March 2024.The test group and the control group included 35 patients in each group.The test group received basic nursing and exercise intervention,the control group received basic nursing and knowledge-based education.The 2 groups were compared in terms of physical activity,number of 30 s chair-stand test,and physical activity compliance score at 1 week post-intervention and 1 month post-intervention.Results There were 4 cases dropped in the test group and 3 cases dropped in the control group.The results of repeated measures ANOVA showed that there were between-group and time interaction effects for walking time,sedentary time,and the number of chair stands in 30 s in the 2 groups(P<0.05).The experimental group's walking time,sedentary time,the number of chair stands in 30 s,moderate physical activity time and the physical activity compliance scores were better than those of the control group after the intervention,and the differences were statistically significant(P<0.05).Conclusion This exercise intervention based on a multi-theory model significantly increased the moderate physical activity time and walking time,decreased the sedentary time and improved compliance with physical activity recommendations,which may improve lower limb muscle strength in postoperative colorectal cancer patients.
2.Clinical value of preoperative intestinal ultrasound in predicting early postoperative recurrence in patients with Crohn's disease

Objective:To investigate the value of preoperative intestinal ultrasound parameters in predicting early postoperative recurrence(EPR)in patients with Crohn's disease(CD).Methods:Ninety-five patients with CD who underwent I-stage intestinal resection at the Sixth Affiliated Hospital, Sun Yat-sen University from March 2015 to December 2020 were retrospectively enrolled. The patients were divided into EPR group ( n=50) and non-EPR (NEPR) group ( n=45) based on recurrence within one year postoperatively. Differences in preoperative intestinal ultrasound parameters including bowel wall thickness,bowel wall stratification, color Doppler grading, mesenteric fat hypertrophy (MFH) , mesenteric lymphadenopathy, abscess/fistula, abdominal effusion, and clinical factors such as preoperative C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were compared between the two groups. The predictive values of ultrasound parameters with statistically significant differences between the two groups were analyzed. Univariate and multivariate Logistic regression analyses were used to identify independent predictive factors associated with EPR in patients with CD. Results:During the 1-year follow-up,EPR occurred in 52.6%(50/95)patients with CD. Among clinical factors,preoperative CRP and ESR levels showed statistically significant differences between the EPR and NEPR groups(all P<0.05). For ultrasound parameters,the incidences of mesenteric fat hypertrophy(MFH)and abscess/fistula were significantly higher in the EPR group than the NEPR group(all P<0.05). MFH demonstrated a significantly higher AUC value for predicting EPR compared to abscess/fistula(0.797 vs.0.617, P=0.002). Univariate Logistic analysis showed that CRP,ESR,MFH and abscess/fistula were candidate variables for diagnosing EPR(all P<0.05). Multivariate Logistic regression analysis indicated that MFH( OR=13.800, P<0.001)and the laboratory measure CRP( OR=1.015, P=0.030)were effective predictive factors for EPR. Conclusions:Preoperative intestinal ultrasound parameter MFH may serve as a valuable predictor for assessing EPR risk in patients with CD.
3.Comparison of peri-diaphragmatic hydrodissection versus artificial ascites/pleural effusion in assisting percutaneous ultrasound-guided microwave ablation of right subphrenic colorectal liver metastasis

Objective:To evaluate the efficacy and safety of peri-diaphragmatic hydrodissection in assisting microwave ablation(MWA)of right subphrenic colorectal liver metastasis(CRLM).Methods:A retrospective analysis was conducted on 101 patients with right subphrenic CRLMs(206 lesions)who underwent percutaneous ultrasound-guided MWA at the Sixth Affiliated Hospital,Sun Yat-sen University from June 2020 to June 2023. Patients were divided into contrast-enhanced ultrasound(CEUS)with hydrodissection group( n=63)and artificial ascites/pleural effusion group( n=38,there were 17 patients with artificial pleural effusion,21 patients with artificial ascites). In the CEUS with hydrodissection group,the peri-diaphragmatic water dissection was established during the post-vascular phase of CEUS. Lesion detection rates,visualization scores,local tumor progression(LTP),and complications were compared between the two groups. Results:In both the CEUS with hydrodissection group and the artificial ascites/pleural effusion group,the lesion detection rates(96.9% and 98.7%)and visualization scores[(3.7 ± 1.3)points and(4.6 ± 0.7)points]improved with water dissection assistance compared to conventional ultrasound[detection rates of 57.5% and 55.7%,visualization scores of(1.9 ± 1.0)points and(2.6 ± 1.5)points]and CEUS alone[detection rates of 78.0% and 78.5%,visualization scores of(3.1 ± 1.5)points and(3.7 ± 1.6)points](all P<0.001). The CEUS with hydrodissection group achieved comparable success rates(96.8% vs. 97.4%, P>0.99)but required less saline[60(30,90)ml vs. 500(500,1 000)ml, P<0.001]than artificial ascites/pleural effusion group. No significant differences were observed in LTP rates,complications,or pain scores(all P>0.05). Conclusions:CEUS combined with peri-diaphragmatic hydrodissection improves detection rates and visualization of right subphrenic CRLM while reducing fluid volume compared to artificial hydrothorax/ascites,serving as a safe and effective adjunct for percutaneous MWA.
4.Development and Initial Validation of the Multi-Dimensional Attention Rating Scale in Highly Educated Adults.

OBJECTIVES:
To report the development, validation, and findings of the Multi-dimensional Attention Rating Scale (MARS), a self-report tool crafted to evaluate six-dimension attention levels.
METHODS:
The MARS was developed based on Classical Test Theory (CTT). Totally 202 highly educated healthy adult participants were recruited for reliability and validity tests. Reliability was measured using Cronbach's alpha and test-retest reliability. Structural validity was explored using principal component analysis. Criterion validity was analyzed by correlating MARS scores with the Toronto Hospital Alertness Test (THAT), the Attentional Control Scale (ACS), and the Attention Network Test (ANT).
RESULTS:
The MARS comprises 12 items spanning six distinct dimensions of attention: focused attention, sustained attention, shifting attention, selective attention, divided attention, and response inhibition.As assessed by six experts, the content validation index (CVI) was 0.95, the Cronbach's alpha for the MARS was 0.78, and the test-retest reliability was 0.81. Four factors were identified (cumulative variance contribution rate 68.79%). The total score of MARS was correlated positively with THAT (r = 0.60, P < 0.01) and ACS (r = 0.78, P < 0.01) and negatively with ANT's reaction time for alerting (r = -0.31, P = 0.049).
CONCLUSIONS
The MARS can reliably and validly assess six-dimension attention levels in real-world settings and is expected to be a new tool for assessing multi-dimensional attention impairments in different mental disorders.

5.Allogeneic hematopoietic stem cell transplantation for pediatric acute leukemia harboring the PICALM-MLLT10 fusion in two cases.

A retrospective analysis was conducted on the clinical course of two children with PICALM-MLLT10-positive acute leukemia treated at Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, between July 2021 and July 2023. The patients were diagnosed with acute T-lymphoblastic leukemia with central nervous system involvement and high-risk acute myeloid leukemia, respectively. Both achieved bone marrow complete remission after conventional chemotherapy combined with venetoclax. Following conversion to molecular negativity, they underwent sequential allogeneic hematopoietic stem cell transplantation. At the latest follow-up, both patients were alive and in good clinical condition. These observations suggest that proceeding to hematopoietic stem cell transplantation after venetoclax-based chemotherapy may improve the long-term survival of children with PICALM-MLLT10-positive leukemia.

6.Metabolic Characteristics of 18F-FDG in Different Types of Myeloid Leukemia Cells and Tumor-Bearing Nude Mice.

OBJECTIVE:
To investigate the metabolic characteristics of 18F-fluorodeoxyglucose (18F-FDG) in myeloid leukemia by in vitro culture of myeloid leukemia cells and construction of tumor-bearing nude mouse model.
METHODS:
U937, THP-1, HL60 and K562 cells were cultured in vitro. The cells in logarithmic growth phase (l×10 5 cells/well) were added with 18F-FDG, and the uptake rate of 18F-FDG was measured at 15, 30, 60 and 120 min after addation, respectively. The four kinds of cells were inoculated subcutaneously into the hind limbs of nude mice to establish a tumor-bearing nude mouse model. When the tumor size was about 500 mm3, 18F-FDG was injected through the tail vein of the mice, and positron emission tomography/computed tomography was performed at 60 min after injection. The morphology of tumor-bearing cells was observed by hematoxylin-eosin (HE) staining in serial pathological sections.
RESULTS:
After co-incubation with 18F-FDG, the 18F-FDG uptake rates of U937 cells were significantly higher than THP-1, HL60 and K562 cells at 4 time points (all P <0.05), and THP-1 cells were higher than K562 cells (all P <0.05). The uptake rate of 18F-FDG by leukemia cells was rapid in the first 60 min, then tended to be stable. Pathological analysis showed that subcutaneous inoculation of U937, THP-1, HL60 and K562 cells could successfully establish tumor-bearing nude mouse models of myeloid leukemia. The 18F-FDG uptake value in U937 tumor-bearing nude mice was significantly higher than THP-1, HL60 and K562 tumor-bearing nude mice (all P <0.01). The 18F-FDG uptake values in THP-1 and HL60 tumor-bearing nude mice were significantly higher than that in K562 tumor-bearing nude mice (both P <0.01).
CONCLUSION
The tumor-bearing nude mouse model of myeloid leukemia can be successfully constructed by subcutaneous inoculation. The 18F-FDG uptake rate of acute myeloid leukemia (AML) cells is higher in cells cultured in vitro and tumor-bearing nude mouse model. 18F-FDG may have better clinical application value for AML.

7.Structural challenges and development pathways of the disease control supervisor system:A SWOT-CLPV integrated analysis

Objective:This study applies an integrated SWOT-CLPV framework combined with stakeholder analysis to systematically assess the strengths,weaknesses,opportunities,and threats of China's disease control inspector system,while identifying its control factors,leverage points,key problems,and vulnerabilities.Methods:Drawing on literature review,policy document analysis,and expert interviews with seven public health professionals,we extracted and categorized SWOT elements.A CLPV interaction analysis was conducted alongside stakeholder mapping to evaluate internal dynamics and systemic risks.Results:The inspector system demonstrates strengths in policy innovation and medical-public health integration,with external opportunities stemming from rising public health awareness and digital health advancements.However,the system faces weak endogenous momentum,limited leverage,and prominent control constraints and problem-prone areas,especially among grassroots institutions and inspectors themselves.Cross-sectoral coordination barriers and uneven local implementation contribute to significant institutional vulnerabilities.Conclusion:To enhance implementation and resilience,the system requires capacity building for key actors,improved governance structures,incentive and evaluation reforms,and strengthened coordination mechanisms to support the sustained and adaptive development of public health supervision.
8.Efficacy analysis of a novel inguinal tourniquet for compression hemostasis

Objective To assess the efficacy of a novel inguinal tourniquet in healthy individuals and to investigate the relationship between localized inguinal compression and femoral artery blood flow occlusion.Methods A self-controlled study was conducted.From November 9 to November 30,2024,11 volunteers were recruited at the Third Medical Center of Chinese PLA General Hospital.Three compression methods--finger pressure,a novel groin tourniquet,and a SAM junction tourniquet(SJT)—were applied bilaterally to the inguinal region until distal blood flow signals disappeared.Each compression method was tested in 22 trials with a 5-minute interval between operations.Differences in hemostatic efficacy between bilateral inguinal regions and across compression methods were compared.Subsequently,the novel tourniquet was incrementally pressurized in 120 mmHg multiples using an integrated pressure device to analyze trends in popliteal artery blood flow velocity.Observational indicators included the internal pressure of the tourniquet pressurization device,peak systolic velocity(PSV)of popliteal artery,inguinal surface pressure magnitude,inguinal surface pressure distribution,and pain scores(assessed using a single-dimensional numerical rating scale).Results No statistically significant difference was observed in the minimum pressure required to occlude femoral artery blood flow bilaterally(P>0.05).The success rates of femoral artery blood flow occlusion at the inguinal region were 100%for the novel inguinal tourniquet,SJT,and finger pressure.The novel inguinal tourniquet induced the highest pain scores,ranging from 5 to 8.A significant reduction in PSV of popliteal artery was noted when the intra-tourniquet pressure reached 360 mmHg and 480 mmHg(P<0.05),with a 95%hemostasis efficacy observed within the range of 360-600 mmHg.No significant association was observed between the recovery of popliteal artery blood flow after limb movement and inguinal pressure distribution(P>0.05).The PSV of popliteal artery exhibited the strongest negative correlation with the average pressure within the inguinal compression area(r=-0.79,P<0.001),with a linear regression fitting line of y=69.69-0.13x(P<0.001,R2=0.58).Conclusions The novel inguinal tourniquet effectively occludes femoral artery blood flow within a pressure range of 360-600 mmHg,accompanied by moderate-to-severe pain.Its hemostatic mechanism mainly relies on increasing the mean pressure within the inguinal compression area.
9.Diagnostic value of ultrasonic shear wave elastography for clinically significant prostate cancer

Objective:To explore the diagnostic value of shear wave elastography(SWE)for clinically significant prostate cancer(csPCa).Methods:We retrospectively analyzed the clinical data of 359 cases with suspected prostate cancer(PCa)in Baoji Central Hospital from June 2017 to July 2023.All the patients underwent the following examinations in the order of serum prostate-spe-cific antigen(PSA)testing,transrectal ultrasonography(TRUS),measurement of the stiffness of the entire prostate gland by SWE,and TRUS-guided prostate puncture biopsy.The stiffness of the entire prostate gland was defined as the average of Young's modulus at both sides of the base,middle,and apex of the prostate,including the maximum Young's modulus(Emax),mean Young's modulus(Emean),and minimum Young's modulus(Emin).We analyzed the correlation of the parameters of the stiffness of the entire prostate gland with the pathological results,focusing on their diagnostic performance for csPCa.Results:Of the 359 cases,189 were diag-nosed by pathological puncture biopsy as BPH,26 as non-csPCa,and 144 as csPCa.The PSA level,Emax,Emean and Emin were significantly higher in the csPCa than those in the BPH and non-csPCa groups(all P<0.01),but showed no statistically significant difference between the BPH and non-csPCa groups(all P>0.05).The area under the receiver operating characteristic curve(AUC),optimal cut-off value,sensitivity,specificity,positive predictive value(PPV),negative predictive value(NPV)and accura-cy of Emax in the diagnosis of csPCa were 0.852,143.92 kPa,72.22%,84.65%,75.91%,81.98%and 79.67%;those of Emean were 0.868,82.42 kPa,67.36%,91.16%,83.62%,80.66%and 81.62%;and those of Emin were 0.682,32.73 kPa,47.22%,89.30%,73.91%,71.54%and 72.14%,respectively.In the non-csPCa group,Emax,Emean and Emin were found be-low the optimal cut-off value in 73.08%(19/26),92.31%(24/26)and 88.46%(23/26),respectively.Conclusion:The stiff-ness of the entire prostate gland measured by SWE contributes to the diagnosis of csPCa,reduces unnecessary detection of non-csPCa,and provides some reference for its active surveillance.
