1.Animal Model of Chronic Obstructive Pulmonary Disease and Intervention Effect of Traditional Chinese Medicine: A Review

Chronic obstructive pulmonary disease (COPD), as one of the three major causes of death, is a complex systemic disease with high prevalence, high mortality, high disability, frequent acute exacerbations, and a variety of pulmonary complications. The pathogenesis is complex. Western medicine has no effective specificity scheme for a complete cure. However, multiple-component and multiple-target characteristics of traditional Chinese medicine (TCM) demonstrate significant advantages in COPD treatment through multi-link, multi-pathway, and multi-mechanism intervention. Therefore, exploring the essence of COPD pathogenesis and discovering effective TCM treatment drugs through the application of TCM principles and prescriptions is a key focus of modern research. Animal models are of paramount importance in medical research. It is the first consideration to select appropriate animals, adopt reasonable modeling methods to replicate stable animal models that closely resemble the clinical manifestations and pathophysiological characteristics of COPD, and use appropriate evaluation methods to determine the success of COPD animal models in experimental research. The core of experimental research lies in observing the intervention effect of TCM on COPD animal models, exploring the specific pathways and regulatory mechanisms of TCM on COPD disease, and finding TCM monomers, single herbs, and TCM formulas with definite curative effects. At present, animal model research on COPD mainly involves model establishment, model evaluation, efficacy observation, mechanism exploration, and other aspects. In recent years, there has been no systematic organization, update, and reflection on the relevant research on TCM intervention in COPD animal models. This study reviewed the selection of animals for the COPD model, methods for establishing COPD animal models, model evaluation methods, and the intervention effects of TCM on COPD animal models. It aims to grasp the current research status and identify existing problems for further improvement, in order to provide evidence and support for scientific research and clinical treatment of COPD.

3.Refined management practices of in vitro diagnostic reagent catalogs in multi-campus medical institutions

The current situation of the in vitro diagnostic(IVD)reagent management was described,and a standard dictio-nary library of IVD reagents was constructed.An IVD reagent catalog information management system with the functions of management and price comparison was introduced in terms of its development process and application effect.Some mainte-nance and management measures for IVD reagent standard dictionary library were put forward including the establishment of an IVD reagent catalog management group and regular data maintenance and updating.References were provided for solving the problems due to the inconsistency of reagent catalog management and medical devices and materials without medical insurance codes in multi-campus medical institutions.[Chinese Medical Equipment Journal,2025,46(4):88-92]
4.Application effect of the stepped early activity program combined with lower limb joint rehabilitation devices in patients with mechanical ventilation

Objective To observe the effects of a stepped early activity program combined with lower limb joint rehabilitation devices in mechanically ventilated patients in the intensive care unit(ICU).Methods Sixty mechanically ventilated patients admitted to the ICU of Tianjin First Central Hospital from October 2022 to June 2023 were selected as study subjects and randomly divided into an intervention group(n=30)and a control group(n=30)using a random number table.The control group received routine rehabilitation nursing combined with lower limb joint rehabilitation devices,while the intervention group was additionally treated with the stepped early activity program.The duration of mechanical ventilation,length of ICU stay,incidence of delirium,Medical Research Council(MRC)muscle strength scores,phase angle(PA),and skeletal muscle mass index(SMI)were compared between the two groups.Results The intervention group showed significantly shorter durations of mechanical ventilation and the length of ICU stay compared to the control group[mechanical ventilation time(days):9.20±4.51 vs.11.73±4.59,the length of ICU stay(days):10.73±5.37 vs.14.00±6.03,both P<0.05].Post-intervention MRC muscle strength scores,PA,and SMI significantly increased in both groups,with greater improvements observed in the intervention group[MRC muscle strength score:54.17±2.10 vs.50.17±3.51;PA(°):5.80±0.60 vs.5.49±0.54;SMI(kg/m2):6.87±0.46 vs.6.62±0.45,all P<0.05].No statistically significant difference was found in delirium incidence between the two groups[26.7%(8/30)vs.33.3%(10/30),P>0.05].Conclusion The combination of a stepped early activity program and lower limb joint rehabilitation devices effectively shortens mechanical ventilation time and the length of ICU stay,restores muscle strength,and promotes recovery in mechanically ventilated ICU patients,demonstrating significant clinical value.
5.Chemical constituents from Asteris Radix et Rhizoma and their anti-inflammatory activities

AIM To study the chemical constituents from Asteris Radix et Rhizoma and their anti-inflammatory activities.METHODS The extract from Asteris Radix et Rhizoma was isolated and purified by column chromatography and high performance liquid chromatography,then the structures of obtained compounds were identified by physicochemical properties and spectral data.Their anti-inflammatory activities were evaluated by RAW264.7 model.RESULTS Thirteen compounds were isolated and identified as(Z)-9,10,11-trihydroxy-12-octadecenoic acid(1),tianshic acid(2),6,6-dimethyl-2-methlenebicyclo[3.1.1]hept-3-O-(6-O-apiofuranosyl)-β-D-glucopyranoside(3),ent-16β,17-dihydroxy-kauran-19-oic acid(4),ent-17-hydroxy-19-kauranoic acid(5),7β,17-dihydroxy-16α-ent-kauran-19-oic acid 19-O-β-D-glucopyranoside ester(6),paniculoside Ⅳ(7),thomimarine A(8),cyclo-(S-Pro-R-Leu)(9),4,5-di-O-caffeoylquinic acid 1-methyl ether(10),methyl 3,5-di-O-caffeoyl quinate(11),5-acetyl-3β-hydroxy-2β-(1-hydroxyisopropyl)-2,3-dihydrobenzofurane(12),4-ally-2,6-dimethoxyphenyl glucoside(13).Compounds 1 and 3-12 had inhibition on the release of NO in RAW264.7 cells,and 4-6,8,10-12 were better than the positive control.CONCLUSION Compounds 1,6,8-9 are isolated from Compositae family for the first time,and 2-5,7,10 and 11-13 are first isolated from this plant.Compounds 1,3-12 have anti-inflammatory activities.
6.Treating Elderly Insomnia According to the Theory"Treating Senile Diseases from the Perspective of Fu-Organs"

[Objective]To summarize Professor ZHANG Guangji's thoughts and clinical experience in the treatment of senile insomnia based on the theory of"treating senile diseases from the perspective of Fu-organs".[Methods]By sorting out the discussions of previous medical scholars on the theory of"treating senile diseases from the perspective of Fu-organs"and Professor ZHANG's clinical interpretation,this paper,from three aspects,including the meaning of"treating senile diseases from the perspective of Fu-organs",the theoretical basis and specific application of treating senile insomnia from the perspective of Fu-organs,combining with typical medical records observed in the clinic,explores the feasibility and practical ideas of treating senile insomnia from the perspective of Fu-organs.[Results]Professor ZHANG believes that the meaning of Fu-organs includes not only the six hollow organs but also the source of engendering transformation,as well as the blood vessels and Xuanfu(sweat pore).The theoretical basis for treating senile insomnia by"seeking treatment from the Fu-organs"lies in the fact that the smoothness of the Fu-organs is of great significance for the sleep health of the elderly,and the obstruction of the Fu-organs plays an important role in the occurrence of senile insomnia.Specifically,the obstruction of the stomach,blood vessels and sweat pore impedes the pathway where Yin and Yang converge,resulting in their inability to interact.This disruption affects both the onset and maintenance of sleep.Therefore,in treatment,it is essential to focus on maintaining the smooth circulation of the gastrointestinal tract,blood vessels and sweat pore,enabling the free flow of Yin and Yang.A medical case was presented here.An elderly patient with insomnia was diagnosed with the syndromes of liver Qi stagnation transforming into fire,blood stasis obstructing the collaterals,stomach Qi disharmony and obstruction of the Xuanfu(minute orifices).By adopting the treatment principles of soothing the liver and regulating the stomach,promoting blood circulation to remove stasis and opening the Xuanfu,and calming the mind,good therapeutic effects were achieved.Specifically,Zhishi Xiaopi Pills were used to smooth the Qi circulation in the stomach-related Fu-organs,Xuefu Zhuyu Decoction was employed to promote blood circulation in the blood-related Fu-organs,and Danzhi Xiaoyao Powder combined with acupuncture was utilized to open the Xuanfu.[Conclusion]The theory of"treating senile diseases from the perspective of Fu-organs"is feasible,and Professor ZHANG Guangji's clinical experience is unique,providing a certain perspective for the prevention and treatment of senile insomnia.
7.Poly gala fallax Hemsl.improves diabetic kidney disease in rats via Nrf2/SLC7A11/GPX4 signaling pathway

Aim To observe the interventional effects of Polygala fallax Hemsl.(PFH)on diabetic kidney disease(DKD)rats and the regulatory mechanism of ferroptosis.Methods Thirty-six SD rats were ran-domly divided into the control group,DKD group,fer-rostatin-1(Fer-1)group,PFH-L group,PFH-M group,and PFH-H group,with six rats in each group.Model-ing was induced by a one-time intraperitoneal injection of 35 mg·kg-1 streptozocin(STZ)in combination with high-sugar and high-fat feed.Ferrostatin-1(25μmol·kg-1)was injected intraperitoneally in Fer-1 group.The PFH-L,PFH-M,and PFH-H groups were gavaged with 50,100 and 200 mg·kg-1 of alcoholic extracts of PFH respectively,and the control and DKD groups were gavaged with an equal volume of distilled water once a day for eight weeks.At the end of drug administration,blood glucose,24h-UP,BUN and Scr levels were measured in each group of rats.HE stai-ning and Masson staining were used to observe renal histopathological changes.ELISA was employed to de-termine the levels of total iron,MDA and GSH activity.IHC was used to observe the expression of Nrf2,SLC7A11,and GPX4 in renal tissues.Western blot was used to detect the protein expression of COL1A1,α-SMA,TGF-,FTH-1,TFR-1,Nrf2,SLC7A1 1,GPX4,in renal tissues.RT-qPCR was used to detect the ex-pression levels of Nrf2,SLC7A11,GPX4 mRNA in re-nal tissues.Results Compared with the control group,blood glucose,24h-UP,BUN,Scr increased(P<0.05);glomerular volume increased,tubular dilata-tion and collagen fiber deposition were obvious;total i-ron,MDA content increased and GSH activity de-creased(P<0.05);there was increased protein ex-pression of COL1A1,α-SMA,TGF-β,TFR-1 and de-creased protein expression of FTH-1(P<0.01);there was decreased mRNA and protein expression of Nrf2,SLC7A11,GPX4 in DKD group rats(P<0.01).Compared with the DKD group,blood glucose,24h-UP,BUN and Scr decreased(P<0.05);renal tissue le-sions were significantly reduced;total iron and MDA content decreased,and GSH activity increased(P<0.05);COL1A1,α-SMA,TGF-β,and TFR-1 protein expression decreased and FTH-1 protein expression in-creased(P<0.05,P<0.01);Nrf2,SLC7A11,GPX4 mRNA and protein expression increased in Fer-1 and PFH dose groups(P<0.05,P<0.01).Conclusions PFH attenuates renal histopathological injury in DKD rats,and the mechanism may be related to the regula-tion of the Nrf2/SLC7A11/GPX4 signaling pathway.
8.To explore the curative effect of tangerine peel compound on chicken Eimeria tenella disease

The study explored the therapeutic effect and mechanism of the compound composed of tangerine peel,plum,agrimony and hawthorn on Eimeria tenella in chicks.In the experiment,144 chicks were divided into six groups:the blank group,the model group,the diclazuril group,and the high-dose,medium-dose and low-dose traditional Chinese medicine compound groups.The 14-day-old chickens were infected,the body weight and mortality were recorded,and the samples were dis-sected at 21 days of age.The anticoccidial effect of Chenpi compound at different doses was evalua-ted by calculating anticoccidial index,feed conversion rate and detecting serum cytokine levels and related mRNA expression levels.The results showed that the anticoccidial index of the middle dose group was 172.86,and the anticoccidial index of the low dose group was 158.98.In the middle and low dose groups,the levels of IL-1β,IL-6 and MDA in serum decreased significantly,while the levels of IL-4,IL10,T-AOC,SOD,CAT and GSH-Px increased significantly,and the mRNA ex-pression levels of related inflammation and antioxidant pathways changed.Further studies found that the medium-dose compound can regulate the ChTLR15/ChMyD88/ChNF-κB/ChNLRP3/Caspase-1 inflammatory signaling pathway,activate the Nrf2/Keap1/HO-1 antioxidant signaling pathway,and enhance the body's anti-inflammatory and antioxidant capacity;at the same time,it can increase the expression of intestinal tight junction ZO-1 and Occludin,repair the damaged in-testinal barrier,and play an anticoccidial role.The results showed that the middle dose of tangerine peel compound had a good effect in the treatment of coccidiosis,and its mechanism involved the regulation of anti-inflammatory and antioxidant pathways.
9.Progress in role and intervention of M2 microglia in ischemic stroke

Stroke is one of the main diseases that threaten human health,including ischemic stroke and hemorrhagic stroke,with the former being the main cause.The important pathogenesis of ischemic stroke includes neuroinflammation,oxidative stress,and excitatory toxic damage,and neuroinflammation plays an impor-tant role in the pathogenesis and rehabilitation process of ische-mic stroke.Microglia are inherent immune cells in the central nervous system,which monitor the site of injury and respond to the immune response as soon as a stroke occurs.The activated microglia are mainly polarized into pro-inflammatory M1 type and anti-inflammatory M2 type.The latter improves neurological dys-function by inhibiting neuroinflammation,promoting neuronal re-generation and myelin repair,and maintaining the integrity of the blood-brain barrier.It suggests that it may be a potential target for treating ischemic stroke by combating acute phase injury and promoting chronic phase rehabilitation.Precise regulation of M1/M2 activation has important therapeutic value in cerebral protection in ischemic stroke.This article focuses on the role of M2 microglia in ischemic stroke and the mechanism of various drugs or acupuncture and moxibustion therapy regulating the transformation of microglia into M2 type,in order to provide theo-retical basis for clinical treatment of stroke and new drug devel-opment.
10.Practice of Pharmaceutical Outpatient Service for Medication Therapy Management of A Case of Patient with Latent Autoimmune Diabetes in Adults

Objective To summarize a case of latent autoimmune diabetes in adults(LADA)who received medication therapy management(MTM)of pharmaceutical outpatient service,and to provide reference for the treatment of similar diseases.Methods The physician recommended a 61-year-old male patient with LADA to the pharmacy clinic on March 27,2023.The pharmacists provided MTM services for the patient through pharmacy inquiry,medication evaluation,medication reconciliation,medication education,and pharmacy follow-up,return visit or pharmacy follow-up for the patient once a week,and established the patient MTM archives.Results Through fifteen MTM services for over three months,including eight visits to the pharmaceuti-cal outpatient clinic and follow-up seven times.On July 10,2023,the atherosclerotic cardiovascular disease(ASCVD)monitoring indicators,such as fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),blood pressure,heart rate,low density lipo-protein cholesterol(LDL-C),and homocysteine(HCY),reduced from 7.17 mmol·L-1,6.8%,140-150/90-100 mmHg,80-90 times·min-1,2.15 mmol·L-1,16.30 μmol·L-1 to 6.06 mmol·L-1,6.1%,120-130/70-80 mmHg,60-70 times·min-1,1.63 mmol·L-1,8.67 μmol·L-1 after MTM services.The ASCVD monitoring indicators were effectively controlled.After thirty-two weeks,the re-examination of the patient's fasting C-P level remained stable,there was no significant decline in pancreas islet function,and cardiac ultrasound showed that cardiac function had a certain improvement.Conclusion By providing MTM serv-ices and reconciliation the medication regimen for the patient with LADA,the ASCVD monitoring indicators such as blood glu-cose,blood pressure,blood lipids,heart rate,and homocysteine were controlled to meet the standards,effectively protecting the pancreatic β-cell and target organs functions of the patient,and significantly improving his health management level.
