1.Analysis on Construction of Whole-course Management Model for Panvascular Diseases

Panvascular diseases are systemic diseases with atherosclerosis as the pathological core, involving multiple vascular beds and target organs throughout the body. Due to their wide range and complexity, the traditional single-discipline prevention and treatment model struggles to meet the needs of systematic management, while clinical diagnosis often remains one-sided and insufficient, leading to delayed treatment. Literature reviews show that panvascular diseases involve a wide range of lesion sites, numerous influencing factors, and are prone to endangering life and health. It is urgent to construct a comprehensive and whole-course prevention and treatment management system, with vascular health as the goal and patients as the core. First, early screening and risk assessment should be conducted for high-risk groups. In terms of treatment decisions for patients, multi-disciplinary collaboration is needed to establish a scientific and standardized prevention and treatment path. Second, it is important to attach great importance to a people-centered approach, enhance patients' familiarity with the disease through cognitive intervention, and shift from passive treatment to active health care. Thirdly, it is needed to leverage the advantages of modern science and technology, promote the deep integration of artificial intelligence innovations and modern medicine, and help traditional diagnosis and treatment plans evolve towards precision, intelligence, and personalization. This will open up new paths for the modernization of the whole-course management of pan-vascular diseases. Fourth, efforts should be made to continue to carry forward and innovate the characteristics of traditional Chinese medicine, adhere to equal emphasis on modern and traditional medicine, promote complementary advantages and coordinated development of Chinese and Western medicine, and form a unique Chinese model for the whole-course management of panvascular diseases. Fifth, through the reintegration and redistribution of government, medical insurance, and medical resources, comprehensive talents in the broad vascular disciplines should be cultivated and an efficient hierarchical management model established, providing reference and guidance for the whole-course management of comprehensive diseases in the future.
2.A novel bionic stent for external septal retention in East Asian rhinoplasty

Objective:To investigate the application effects of a novel framework: the external septal retention bionic framework in East Asian rhinoplasty.Methods:A retrospective analysis was performed based on the clinical data of the patients who received treatments in Department of Plastic and Aesthetic ( Burn) Surgery, the Second Xiangya Hospital, Central South University from January 2022 to July 2023 using an external septal retention bionic framework with autologous costal cartilage rhinoplasty. The patients were all treated with a V-shaped incision at the nasal columella, which continued to both sides of the nose to form a bird-shaped incision. During the operation, the sixth or seventh costal cartilage was sculpted to form the nasal columella support grafts (sturt), external nasal septal graft, cap grafts, and shield grafts, in which the nasal column support graft and external nasal septal graft were implanted in a mortise-and-tenon joint structure to form a septum bionic retained external stent in order to lengthen the nasal columellar column and strengthen the septum. A capsular graft was used to shape the tip of the nose and the rectus abdominis fascia was covered to soften the tip of the nose, and the shield grafts were used to fill out the subnasal lobules. A bulb was sculpted, shaped and implanted in the dorsal space to elevate the nasal dorsum. To observe the postoperative results and complications, a postoperative satisfaction survey was completed using the rhinoplasty outcome evaluation (ROE) and the visual analogue scale (VAS) with regular follow-up. Comparison of preoperative and postoperative scale scores was performed using paired samples t-test. Nasal stent stability was assessed by measuring the patients’ preoperative and postoperative nasolabial angle and nasal tip protrusion. The comparison of preoperative and postoperative nasolabial angle was performed using the paired-sample t-test, and the comparison of nasal tip protrusion was performed using the Wilcoxon signed rank-sum test, P<0.05 considered a statistically significant difference. Results:A total of 33 patients were enrolled, including 4 males and 29 females, with average age of 23.8 years(15-42 years). The follow-up period is (14.52 ± 4.72) months (3 to 21 months). Thirty-three patients recovered well after surgery, and no short-term complications such as infection or bleeding occurred. The patients' nasal aesthetic problems such as saddle nose, deviated nasal columella, bilateral asymmetry of the nose, bilateral nasal base depression, and rounded shield of the nasal tip were improved after surgery. Satisfaction survey results: ROE and VAS scores were significantly higher than those before surgery, which were [(17.92±2.56) vs.(12.78±3.14)] and [(8.08±1.66)vs(5.15±1.99)], ( P<0.05), 87.9% (27/33)of the patients were satisfied with the aesthetic and functional outcomes of the nose. Comparisons of nasolabial angle [(87.28±2.58)° vs. (97.64±1.95)°] and tip protrusion [1.88 (1.78, 1.95) cm vs. 2.16(2.01, 2.32) cm] measurements between preoperative and postoperative period of the 33 patients, the differences were statistically significant ( P<0.05). Conclusion:Rhinoplasty for East Asians employs an external septal retention bionic framework to retain the nasal septum. This technique can avoid septal cartilage resorption caused by compression of grafts transplanted in the future, and cause less damage to the anatomy and physiology of the nose. The framework is relatively easy to construct, with a stable and strong stent, and good patient satisfaction. It can achieve a more desirable aesthetic effect of the nose.

4.Relationship of Plasma Homocysteine with Gene Polymorphisms of MTHFR and MTRR among Han Women in Xiangtan City

Objective To investigate the relationship of plasma homocysteine with the genotype distribution of MTHFR and MTRR among Chinese Han women in Xiangtan. Methods MTHFR C677T, A1298C and MTRR A66G geno?typing was analyzed to detect the distribution of gene polymorphisms among 1 701 women from Xiangtan city then the data were compared with the rest of the Han women in Zibo, Zhengzhou, Yantai, Zhenjiang, Songzi, Huizhou, Qionghai. Plasma Hcy levels from 110 patients were measured and analyzed the correlation with gene polymorphisms. Results The frequency of MTHFR C677T genotype and allele frequencies in Xiangtan is 12.6%which is higher than Huizhou (10.9%) and Qionghai (6.1%) but lower than Zibo (43.6%), Zhengzhou (36.8%), Yantai (32.2%), Zhenjiang (21.8%) with statistically significant dif?ference (P<0.05). There is no significant different in MTHFR C677T between Xiangtan and Songzi. The frequency of MTH?FR A1298C genotype and allele frequencies in Xiangtan is 4.8%which is lower than Qionghai(7.1%)but higher than Zibo (1.4%),Zhengzhou(2.4%), Yantai(1.8%), Zhenjiang(3.5%)and Songzi(2.6%)with statistically significant difference. The frenquency of MTRR A66G genotype and allele frequencies in Xiangtan is 6.8%which is higher than Zibo (4.8%) but lower than Qionghai (9.3%) with statistically signifcant difference. Plasma Hcy concentration correlate with MTHFR C677T, Hcy concentration in TT population is higher than that in CT and CC population(μmol/L:8.52±2.01 vs 5.94±1.47 vs 5.71± 0.18);Plasma Hcy concentration also correlate with MTHFR A1298C and Hcy concentration in CC population is higher than AA and AC population(μmol/L:9.83 ± 2.26 vs 6.35 ± 2.13 vs 5.55 ± 1.75);Plasma Hcy concentration does not correlate with MTRR A66G. Conclusion The gene polymorphism of MTHFR C677T, A1298C and MTRR A66G among the Han women in Xiangtan was statistically different from other selected regions of China. Mutation in MTHFR C 677T and A1298C were associated with elevated plasma levels of Hcy.
5.Filling effect of autologous skin fibroblasts: a study of two-photon fluorescence microscopy

Objective To investigate the survival profile of the intradermally injected mouse autologous skin fibroblasts and the changes of the collagen fibers by using green fluorescent protein labeling and two-photon fluorescence microscopy. Methods The cultured cells were transfected by EGFP lentivirus, and then the cells were injected into the corresponding mouse skin. Biopsy was taken from the animals after 1 and 2 months. The specimens made serial frozen sections, the survival profile of the injected cells and the changes of the collagen fibers were observed by two-photon fluorescence microscopy. The collagenic area and dermal thickness were measured with image analysis software, and statistical analysis was also carried out. Results Two-photon fluorescence microscopy showed clear images of the injected cells and collagen fibers. Both the area of collagen fibers and the dermal thickness were significantly increased in injected cells after 2 months (P<0.05), however, there were no difference between injected cells and control after 1 mouth (P>0.05). Conclusions Autologous cultured fibroblasts could survive in a long time after transplantating into the skin, and collagen could be newly produced, the depth of dermis increases, which provides a possibility to treat mini-defects of the tissue.
6.A MORPHOLOGICAL STUDY OF THE EFFECT OF ACUTE EXPERIMENTAL ISCHEMIA ON THE PYRAMIDAL CELLS OF CANINE CEREBRAL CORTEX

In the present study, 10 healthy adult dogs were used,8 for the experimental group and 2 for the normal group. We observed the histochemical and ultrastrac- tural changes of the enzymes in the pyramidal cells in the cerebral cortex under the condition of acute ischemia. The result demonstrated that the activity of LDH, AcP and AchE was increased, while the activity of SDH, MAO, ATP ase was decreased. The ultrastructural chnges showed that in the acute ischemia gromp, there were enlargement perinucleus spaces of neurone, brisement, disappearance and vasicularization of the mitochondrial crista. This indicated that acute ischemia had an obvious effect on the histochcmistry of enzymes and the ultra- structure of the neurone in the cerebral cortex.
