Mercury possesses high toxicity and metal-dissolving properties, enabling it to form amalgam alloys with precious metals such as gold, silver, tin, and lead. The purification of target metals can be achieved through the volatilization process of heated amalgam. This report presents a case of toxic encephalopathy caused by acute mercury poisoning. The Department of Occupational Medicine and Toxicology at Beijing Chaoyang Hospital, affiliated with Capital Medical University, admitted and successfully treated a clinical case of toxic encephalopathy resulting from acute exposure to metallic mercury vapor. The nursing protocol and clinical outcomes are detailed herein. This paper conducts a retrospective analysis of clinical data to provide support for the prevention and control of mercury poisoning.

Periodontitis is a common chronic inflammatory disease that causes the periodontal bone destruction and may ultimately result in tooth loss.With the progression of periodontitis,the osteoimmunology microenvironment in periodontitis is damaged and leads to the formation of pathological alveolar bone resorption.CD301b+macrophages are specific to the osteoimmunology microenvironment,and are emerging as vital booster for conducting bone regeneration.However,the key upstream targets of CD301b+macrophages and their potential mechanism in periodontitis remain elusive.In this study,we concentrated on the role of Tim4,a latent upstream regulator of CD301b+macrophages.We first demonstrated that the transcription level of Timd4(gene name of Tim4)in CD301b+macrophages was significantly upregulated compared to CD301b-macrophages via high-throughput RNA sequencing.Moreover,several Tim4-related functions such as apoptotic cell clearance,phagocytosis and engulfment were positively regulated by CD301b+macrophages.The single-cell RNA sequencing analysis subsequently discovered that Cd301b and Timd4 were specifically co-expressed in macrophages.The following flow cytometric analysis indicated that Tim4 positive expression rates in total macrophages shared highly synchronized dynamic changes with the proportions of CD301b+macrophages as periodontitis progressed.Furthermore,the deficiency of Tim4 in mice decreased CD301b+macrophages and eventually magnified alveolar bone resorption in periodontitis.Additionally,Tim4 controlled the p38 MAPK signaling pathway to ultimately mediate CD301b+macrophages phenotype.In a word,Tim4 might regulate CD301b+macrophages through p38 MAPK signaling pathway in periodontitis,which provided new insights into periodontitis immunoregulation as well as help to develop innovative therapeutic targets and treatment strategies for periodontitis.
4.CD301b+ macrophage: the new booster for activating bone regeneration in periodontitis treatment.

Periodontal bone regeneration is a major challenge in the treatment of periodontitis. Currently the main obstacle is the difficulty of restoring the regenerative vitality of periodontal osteoblast lineages suppressed by inflammation, via conventional treatment. CD301b+ macrophages were recently identified as a subpopulation that is characteristic of a regenerative environment, but their role in periodontal bone repair has not been reported. The current study indicates that CD301b+ macrophages may be a constituent component of periodontal bone repair, and that they are devoted to bone formation in the resolving phase of periodontitis. Transcriptome sequencing suggested that CD301b+ macrophages could positively regulate osteogenesis-related processes. In vitro, CD301b+ macrophages could be induced by interleukin 4 (IL-4) unless proinflammatory cytokines such as interleukin 1β (IL-1β) and tumor necrosis factor α (TNF-α) were present. Mechanistically, CD301b+ macrophages promoted osteoblast differentiation via insulin-like growth factor 1 (IGF-1)/thymoma viral proto-oncogene 1 (Akt)/mammalian target of rapamycin (mTOR) signaling. An osteogenic inducible nano-capsule (OINC) consisting of a gold nanocage loaded with IL-4 as the "core" and mouse neutrophil membrane as the "shell" was designed. When injected into periodontal tissue, OINCs first absorbed proinflammatory cytokines in inflamed periodontal tissue, then released IL-4 controlled by far-red irradiation. These events collectively promoted CD301b+ macrophage enrichment, which further boosted periodontal bone regeneration. The current study highlights the osteoinductive role of CD301b+ macrophages, and suggests a CD301b+ macrophage-targeted induction strategy based on biomimetic nano-capsules for improved therapeutic efficacy, which may also provide a potential therapeutic target and strategy for other inflammatory bone diseases.

5.Cost-effectiveness analysis of different screening modes for thalassemia in Hunan Province

Objective:To analyze the costs and effectiveness of five common screening modes and genetic screening for thalassemia in China in order to find the optimal way and provide evidence for the implementation of thalassemia prevention and control projects in Hunan Province.Methods:From June 2020 to April 2021, 12 971 couples from 14 cities and autonomous prefectures in Hunan Province were selected as the study population. The diagnosis of thalassemia was based on the results of genetic testing. Results of routine blood test and hemoglobin electrophoresis were collected and analyzed. The efficacy of five screening modes, at the cut-off value of <80 fl or 82 fl for the mean corpuscular volume (MCV), was analyzed by positive predictive value, negative predictive value, Jorden index and cost-effectiveness ratio. Sensitivity analysis was used to assess the feasibility of genetic screening at different costs after fixing the costs of routine blood and hemoglobin electrophoresis. The five thalassemia screening models are as follows: Mode 1: The woman had a blood routine test first. If the result was positive, the spouse required a blood routine test. If both results were positive, a thalassemia gene test should be offered to the couple. Mode 2: Both husband and wife were screened by blood routine and hemoglobin electrophoresis. If one or both of them were positive, both would be tested for thalassemia gene. Mode 3: The couple received blood routine tests initially. If either was positive, both should receive hemoglobin electrophoresis testing. If either was positive, both parties will conduct thalassemia gene testing. Mode 4: The woman was screened by blood routine and hemoglobin electrophoresis. If any one of them was positive, the woman would be tested for thalassemia gene. If the gene test result was positive, the spouse should receive thalassemia gene. Mode 5: Both spouses conducted a blood routine test. If either was positive, both would conduct hemoglobin electrophoresis test. If both were positive, both spouses should receive thalassemia gene testing. Gene testing mode: The woman would be tested for thalassemia, and her spouse would have thalassemia test too if her result was positive.Results:When using MCV<80 fl as the cut-off for diagnosing thalassemia, the Youden indices of the five prenatal screening modes in Hunan Province were 0.551, 0.639, 0.898, 0.555 and 0.356, while when using MCV<82 fl as the cut-off, the Youden indices were 0.549, 0.629, 0.851, 0.548 and 0.356. When the MCV cut-off value was <80 fl, the missed diagnosis rates of the five screening modes were 44.44%, 0.00, 0.00, 18.52% and 62.96%, and the cost-effectiveness ratios were 21 709, 250 939, 76 870, 138 463 and 92 860 yuan (RMB)/couple, respectively. When the price of genetic testing was lower than 55 yuan (RMB), the cost-effectiveness ratio of genetic screening was lower than that of Mode 3.Conclusions:MCV<80 fl can be considered as the positive criteria in blood routine screening for thalassemia in Hunan Province, and the cost-effectiveness ratio of Mode 3 (the couple received blood routine tests initially. If either was positive, both should receive hemoglobin electrophoresis testing. If either was positive, both parties will conduct thalassemia gene testing) is the best. Genetic screening has certain advantages with the decreasing price.
6.A multicentre retrospective analysis of surgical effects of the 1310 Hypertensive intracerebral hemorrhage

Objective To explore the relationship between different hemorrhage position,hemorrhage volume,surgical time and outcome of treatment with surgical methods of HICH.Methods A total of 1310 patients were admitted from six hospitals from January 2004 to January 2008,the 1310 patients were divided into six groups according to different operation:craniotomy through bone flap (group A),craniotomy through small bone window (group B),stereotactic drilling drainage (group C1 and group C2),neuron-endoscopy operation (group D) and external ventricular drainage (group E),considering hemorrhage position,hemorrhage volume,surgical time and result of surgical methods were reviewed and analyzed.Results ①Craniotomy through bone flap should be selected with the case of superficial or deep hematoma volume (> 80 mL),median line structure distinct motion,metaphase or advanced stage of hernia of brain.②Craniotomy through small bone window and neuron-endoscopy should be selected with the case of moderate hematoma volume (50-80 mL) ③Drilling drainage should be selected with the case of small hematoma volume in superficial or deep hematoma volume (20-50 mL) ④Extemal drainage should be selected in dealing with ventricular hemorrhage.Small bone window or neuron-endoscopy should be selected in ventricular casting mould.⑤The appropriate operation time for patients with hematoma volume less than 80 mL should be 6-12 hours and large hematoma should be immediately operated to save lives.The operation time should depend on patients detail condition.Conclusions Craniotomy through bone flap was suitable for large hematoma and hernia of brain; Stereotactic drilling drainage should be selected in patients with hematoma volume less than 80mL; and the operation results in dealing with HICH would be improved via suitable operation time and surgical methods and adividual according to Hemorrhage position and Hemorrhage volume.
7.Relationships between aggressive behavior and attachment for adult prisoners

Objective To explore the relationship between aggressive behavior and attachment styles for adult prisoners.Methods A sample of 413 criminals filled out measures for Aggression Questionnaire and Adult Attachment Scale.Results There existed significant differences of anxiety,anger,hostility,physical attacks and the total score of AQ among different criminals type (F =2.80,5.80,5.15,16.82,10.69,P < 0.001 or P < 0.05).And there existed significant differences of anxiety,anger,hostility,physical attacks,verbal attacks and the total score of AQ among different adult attachment style (F =18.14,34,17,7,83,22,75,P < 0.01).The close dimension of AAS was significantly negatively related with anger,hostility,physical attacks and the total score of AQ (r =-0.15--0.20,P < 0.01).And the anxiety dimension of AAS was significantly positively related with anger,hostility,physical attacks,verbal attacks and the total score of AQ(r =0.30-0.52,P < 0.01).Regression analysis found that anxiety dimension and close dimension of AAS were fairly effective variables in the prediction of the criminal attacks.Conclusion There are close relationships between aggressive behavior and adult attachment for criminals.The anxiety dimension and close dimension make a certain prediction to the criminal attacks.
8.Study on the causes of deafness in a consanguineous marriage family using DNA microarray.

OBJECTIVE:
To determine the possible gene mutation and its different sites that leads to hearing loss in a consanguineous marriage pedigree.
METHOD:
Blood samples from a Changchun pedigree were obtained with informed consent. Their genomic DNA were extracted from peripheral blood and PCR was performed. Nine of hot spot mutations in four most common deafness pathologic gene were detected with the DNA microarray, including GJB2, GJB3, PDS and mtDNA 12S RNA gene. At the same time, the results were confirmed with the traditional methods of sequencing.
RESULT:
GJB2 gene of 235 delC and 299-300 delAT compound heterozygous mutation was found in propositus. His father was 299-300 delAT homozygous mutation and mother was 235 delC homozygous mutation. In the relatives on the paternal side, the affected patients all were 299-300 delAT homozygous mutation and normal hearing member was 299-300 delAT heterozygous carrier. This GJB2 mutation come from grandparents of consanguineous marriage.
CONCLUSION
GJB2 gene mutation played on an important role in this deafness family. It is essential approach for genetic diagnosis of non - syndromic sensorineural hearing loss.

9.Effects of ropivacaine combined with fentanyl for epidural labor analgesia

Objective To evaluate effects of ropivacaine combined with different concentrations of fentanyl for epidural labor analgesia. Methods In this multicenter double-blinded randomized study 128 parturients at full term and 2-3 cm of cervical dilatation who requested epidural analgesia were randomly allocated to one of 4 groups: group F0 received epidural ropivacaine alone (n = 33); group F1 received epidural ropivacaine with fentanyl 1 ?g?ml-1 (n = 30) ; group F2 epidural ropivacaine + fentanyl 2?g?ml-1(n = 33) and group F3 epidural ropivacaine + fentanyl 3 ?g?ml-1(n = 32). Epidural catheter was placed at L2,3 and advanced 4 cm into the epidural space in cephalad direction. A bolus of 15 ml of ropivacaine alone or with fentanyl was given after correct epidural placement was confirmed. EC50 of epidural ropivacaine was determined by up-and-down sequential experiment. The initial concentration of epidural ropivacaine was 0. 12% . If effective the next parturient received ropivacaine of lower concentration; if ineffective the ropivacaine concentration was increased. Each time the concentration of epidural ropivacaine increased/decreased by 0.01% . The analgesia was assessed using VAS score (0-10 0 = no pain, 10 = worst pain) . If VAS score was less than 3 within 30 min of ropivacaine administration, analgesia was defined as effective. EC50 of ropivacaine was calculated according to Dixon and Massey. Results Four of the 128 parturients enrolled were excluded because of uncertain results of interrupted observation. The EC50 of epidural ropivacaine for labor analgesia and the 95% confidence interval (95% CI) of EC50 were 0.110% (95% CI 0.109 0%-0.111 6%) in group F0; 0.089% (95% CI 0.087 7%-0.091 1%) in group F1; 0.073% (95% CI 0. 071 7%-0.0744%) in group F2 and 0.060% (95% CI 0.056 0%-0.634%) in group F3 respectively. 10.Investigation on Water Source Pollution by Cryptosporidium parvum and Giardia lamblia in the Small Towns and Villages in Shenzhen,China

Objective To investigate the water source pollution by parasitic protozoa in small towns and villages in Shenzhen,China,in order to present the data to establish the national hygienic standard for controlling parasitic protozoa pollution.Methods The samples of water source were collected from 8 village level water plants in Shenzhen.Giardia lamblia and Cryptosporidium were detected,according to the EPA methods,by filtering,rinsing,magnetic isolation and staining.Results Giardia lamblia was found in water sources of 6 water plants and Cryptosporidium was found in one water plant.Conclusion At present,some water sources of village level water plants where the reservoirs were used as the water source in Shenzhen have been polluted by Giardia lamblia and Cryptosporidium,that indicates the potentiality of the water-born-diseases in that areas.
