ObjectiveTo observe the clinical efficacy of the Yiqi Yangyin Huoxue prescription (YYHP) in the treatment of cathartic colon (CC) and its effects on fecal short-chain fatty acids (SCFAs), and to explore the correlations among CC severity indicators and between these indicators and patient history. MethodsAccording to the inclusion and exclusion criteria, 98 patients meeting the diagnostic criteria of both traditional Chinese and Western medicine for CC with the syndrome of Qi-Yin deficiency complicated by blood stasis were randomly assigned to an observation group and a control group. The observation group received YYHP granules, while the control group received lactulose. Both medications were administered twice daily, one sachet each time, half an hour after breakfast and dinner, with a treatment course of 8 weeks. The primary constipation symptom score, Patient Assessment of Constipation Quality of Life (PAC-QOL) score, and TCM syndrome score were assessed before and after treatment and at the 8th week after the end of treatment. The overall clinical effective rate, as well as the efficacy attenuation index and degree, were evaluated. Fecal SCFA levels were measured using gas chromatography-mass spectrometry (GC-MS). Spearman correlation analysis was performed to explore the correlations among CC severity indicators and between these indicators and patient history. ResultsThe overall clinical effective rate in the observation group (95.83%) was higher than that in the control group (78.72%) (P<0.05). After treatment, the total scores for primary constipation symptoms, PAC-QOL, and TCM syndromes decreased in both groups (P<0.05), with more significant reductions in the observation group (P<0.05). The severity of all primary constipation symptoms was alleviated in both groups (P<0.05). In terms of "excessive straining and difficult defecation", "anal heaviness, incomplete evacuation, and bloating sensation", "abdominal distension", and "defecation frequency", the observation group showed better efficacy than the control group (P<0.05). Scores of the four PAC-QOL dimensions and the scores and severity of primary and secondary TCM symptoms were reduced in both groups (P<0.05), with more significant reductions in the observation group (P<0.05). After treatment, acetic acid, propionic acid, butyric acid, and total SCFAs in the observation group increased significantly (P<0.05). The efficacy attenuation index and degree in the observation group were lower than those in the control group (P<0.05). No severe adverse reactions occurred in either group, and there was no statistically significant difference in the incidence of adverse reactions between the two groups. Positive correlations of varying degrees were observed among the total scores of primary constipation symptoms, PAC-QOL, and TCM syndromes, as well as between these scores and the history of stimulant laxative use, disease duration, and age. ConclusionYYHP can effectively alleviate the primary constipation symptoms in CC patients, improve quality of life, and ameliorate TCM syndromes, with good safety. It also has the advantage of a lower rebound degree after drug withdrawal, and its mechanism may be related to increasing fecal SCFA levels. Long-term abuse of stimulant laxatives may aggravate the severity of CC and prolong the disease course.

Chronic obstructive pulmonary disease (COPD) is a respiratory disease characterized by airflow limitation, and its high morbidity and disability rates pose a severe challenge to global public health. Traditional Chinese medicine (TCM) has demonstrated definite efficacy and distinctive characteristics in the treatment of COPD. Based on the understanding of ancient and modern physicians regarding the etiology, pathogenesis, and stage-based syndrome differentiation of COPD, this article systematically elaborates on the etiology, pathogenesis, and clinical principles of syndrome differentiation and treatment of COPD. COPD is characterized by deficiency at root and excess in manifestation, with the coexistence of deficiency and excess. Qi deficiency is the fundamental pathogenesis underlying the occurrence and development of COPD and runs throughout the entire disease course. The disease begins with deficiency of lung qi and gradually affects the spleen and kidney, leading to the internal generation of dampness, phlegm, and blood stasis. Phlegm and blood stasis are not only pathological products but also deep-rooted causes of recurrent disease. Together with qi deficiency, they are mutually causal and form a vicious cycle. During the acute exacerbation stage of COPD, phlegm and blood stasis constitute the predominant underlying pathology and are often induced by invasion of pathogenic factors due to deficiency of healthy Qi. In addition, innovative understandings of the etiology and pathogenesis of COPD have emerged from the perspectives of collateral disease, latent pathogens, smoke toxins, and dysfunction of the Shaoyang pivot. During the stable stage, deficiency of Qi in the lung, spleen, and kidney predominates and is often accompanied by phlegm and blood stasis. In the clinical differentiation and treatment of COPD, treatment should be based on different disease stages and syndrome characteristics. During the acute exacerbation stage, eliminating pathogenic factors is the primary principle, with treatment determined according to syndrome differentiation. For wind-cold attacking the lung, the treatment is to ventilate the lung, disperse cold, and relieve cough. For phlegm-dampness accumulating in the lung, the treatment is to dry dampness, resolve phlegm, and direct qi downward. For phlegm-heat obstructing the lung, the treatment is to clear the lung, resolve phlegm, and relieve asthma. For phlegm and blood stasis obstructing the lung, the treatment is to eliminate phlegm, remove blood stasis, and relieve asthma. During the stable stage, reinforcing healthy Qi is the key principle. For lung-spleen qi deficiency, the treatment is to tonify the lung and strengthen the spleen, resolve phlegm, and direct Qi downward to relieve asthma. For deficiency of both lung and kidney, the treatment is to tonify the lung and benefit the kidney, resolve phlegm, promote Qi reception, and relieve asthma. When accompanied by phlegm and blood stasis, the treatment should focus on tonifying deficiency, resolving phlegm, and removing blood stasis. This article aims to systematically summarize the TCM etiology, pathogenesis, syndrome differentiation and treatment principles of COPD, as well as the current shortcomings in related research, in order to provide a reference for the clinical treatment of COPD with TCM, give full play to the advantages of TCM in the prevention and treatment of COPD, and optimize the whole-course management of COPD.

ObjectiveTo investigate the improvement effect of Qibai Pingfei capsules on pulmonary vascular remodeling in rats at different stages of chronic obstructive pulmonary disease (COPD) and to analyze its possible mechanism of action. MethodsMale Sprague-Dawley (SD) rats were randomly divided into a normal group, an early COPD model group, an advanced COPD model group, an early-intervention high-dose group, a late-intervention high-dose group, an early-intervention low-dose group, a late-intervention low-dose group, an early-intervention pyrrolidine dithiocarbamate (PDTC) group, and a late-intervention PDTC group, with 15 rats in each group. A rat model of early COPD was constructed by using cigarette smoke combined with airway infusion using lipopolysaccharide(LPS), and a rat model of advanced COPD was constructed by using airway infusion with LPS, cigarette smoke, and hypoxia. All groups except the normal group were given LPS airway drops on days 1 and 14 of the experiment, smoked for 1 h per day, and administered the drug once a day for 40 weeks from day 15 onward. In the high- and low-dose groups, rats were given 1 g·kg-1 and 250 mg·kg-1 Qibai Pingfei capsules, respectively by gavage, and in PDTC groups, rats were given 100 mg·kg-1 of PDTC by intraperitoneal injection. The advanced COPD model group underwent 6 h of hypoxia per day in weeks 5-6. Lung function and mean pulmonary artery pressure were tested in rats. Morphologic changes in lung tissues were detected by hematoxylin-eosin(HE)staining. Collagen deposition in lung tissues was examined by Masson staining, and the levels of inflammatory factors including interleukin-1β(IL-1β), interleukin-6(IL-6), and tumor necrosis factor-α(TNF-α)in lung tissues were detected by enzyme-linked immunosorbent assay (ELISA). The number of inflammatory cells in the alveolar lavage fluid of rats in each group was detected by Giemsa staining, and the protein expression of Toll-like receptor 4(TLR4), myeloid differentiation factor 88(MyD88), nuclear factor-κB(NF-κB), TNF-α, vascular endothelial-cadherin(VE-cadherin), α-smooth muscle actin(α-SMA), and platelet endothelial cell adhesion molecule-1(CD31) was detected by Western blot in the lung tissues of rats. ResultsCompared with the normal group, the model group showed significantly decreased forced expiratory volume in 0.3 s (FEV0.3), forced vital capacity (FVC), and FEV0.3/FVC ratio related to lung function (P<0.05), thickening of pulmonary vasculature, increased collagen deposition in the lungs, and enhanced mean pulmonary arterial pressure and expression levels of IL-6, IL-1β, and TNF-α (P<0.05). Additionally, the model group also exhibited increased numbers of macrophages, lymphocytes, and neutrophils (P<0.05), significantly higher protein expression of TLR4, MyD88, NF-κB, TNF-α, and α-SMA (P<0.05), and significantly lower protein expression of VE-cadherin and CD31 (P<0.05). Lung function was significantly improved in the Qibai Pingfei capsules groups compared with the model group (P<0.05), with mean pulmonary arterial pressure reduced and pulmonary vascular thickening and collagen deposition in the lungs ameliorated. The Qibai Pingfei capsules groups also showed reduced expression levels of IL-6, IL-1β, and TNF-α (P<0.05) and decreased numbers of macrophages, lymphocytes, and neutrophils (P<0.05), as well as reduced protein expression of TLR4, MyD88, NF-κB, TNF-α, and α-SMA (P<0.05) and elevated protein expression of VE-cadherin and CD31 (P<0.05) in rat lung tissues. ConclusionQibai Pingfei capsules inhibits inflammatory response and endothelial-to-mesenchymal transition probably by regulating the TLR4/NF-κB signaling pathway, thus improving pulmonary vascular remodeling in COPD model rats and showing therapeutic effects in the early stage of COPD.

Journal of Environmental and Occupational Medicine 2025;42(3):319-324
Circadian rhythm refers to the 24-hour periodic changes in behavior, physiology, and molecular processes in the human body. Disruptions to the circadian rhythm not only affect mental health but are also associated with various metabolic disorders, including the regulation of bone and muscle metabolism. Research has shown that work-related musculoskeletal disorders (WMSDs) are influenced not only by workload but also by circadian rhythm factors, such as shift work. This review examined the relationships between circadian rhythm-related antecedents, outcomes, and WMSDs, exploring their shared metabolic markers and mechanisms. It provided a systematic overview of the intrinsic connection between circadian rhythm disruptions and WMSDs. While current studies highlight the impact of circadian rhythm disturbances on musculoskeletal disorders, further investigation is required to address the confounding factors involved. Future research should integrate chronobiology with both subjective and objective data to explore the pathway from environmental factors to intermediate phenotypes to diseases, ultimately providing a more comprehensive understanding of the network mechanisms underlying WMSDs.

In order to investigate the effects of LAL on the lipolysis and lipid synthesis of dairy adi-pocytes,the protein expressions of lipid synthesis-related molecules,acetyl-CoA carboxylase-1(ACC1),phosphorylated transcription factor-α(CEBPα),diacylglyceryl acyltransferase 2(DGAT2),sterol regulatory element binding protein 1(SREBP1),and peroxisome proliferator-ac-tivated receptor-γ(PPARγ),and lipolysis-related molecules, lipid droplet coated protein-1(PLIN1),triglyceride lipase(ATGL),hormone-sensitive lipase(HSL),p-HSL,and the target pro-tein lysosomal acid lipase(LAL),were detected in adipose tissues of healthy dairy cows and ketosis cows. The lipid synthesis and lipolysis-related protein expressions in adipocytes were detected by Western blot technology. The primary bovine adipocytes were cultured in vitro with overexpressed LAL,and the lipolysis model of adipocytes was constructed by adding isoproterenol(ISO).The re-sults showed that the expression of LAL in adipose tissue of ketosis cows was significantly lower than that of healthy cows(P<0.01).Compared with healthy cows, the protein expression levels of lipid synthesis-related proteins ACC1,CEBPα,DGAT2,SREBP1 and PPARγin adipose tissue of clinical ketosis cows were significantly decreased, while the protein expression and phosphorylation levels of lipolysis-related proteins,PLIN1,ATGL,and p-HSL were significantly increased. The a-bove results confirmed that the lipid synthesis of adipose tissue of ketosis cows was inhibited, and the lipolysis was enhanced. In vitro results showed that ISO could downregulate the protein ex-pression levels of lipid synthesis related molecules, and upregulate the protein expression and phosphorylation levels of lipid lysis related molecules in bovine adipocytes. The content of basal lipid synthesis and ISO-induced lipid synthesis proteins in bovine adipocytes of LAL overexpres-sion group was significantly increased, while the content of basal lipid lysis and ISO-induced lipid lysis proteins was significantly decreased. In conclusion,in vivo and in vitro studies have shown that LAL can inhibit the lipid lysis of bovine adipocytes and promote the lipid synthesis of bovine adipocytes.

Objective:To study the relationship between hemoglobin glycation index (HGI) and blood lipid indices such as low-density lipoprotein cholesterol (LDL-C), non-high-density lipoprotein cholesterol (non-HDL-C), and plasma atherogenic index (AIP).Methods:This cross-sectional study included 16 049 participants from the Beijing Apple Garden community between December 2011 and August 2012. The subjects were divided into three groups based on the HGI quartile: low ( n=5 388), medium ( n=5 249), and high ( n=5 412). The differences in blood lipid indicators between different HGI groups were compared and multivariate logistic regression model was established to analyze the association between HGI and dyslipidemia. And multivariate logistic regression model was established to analyze the relationship between HGI and blood lipid indicators in different glucose metabolism populations. Results:There were 16 049 participants in all (mean age: 56 years), including 10 452 women (65.1%). They were classified into normal glucose tolerance (9 093 cases), prediabetes (4 524 cases), and diabetes (2 432 cases) based on glucose tolerance status. In the general population, with the increase of HGI, LDL-C, non-HDL-C, and AIP gradually increased (all P values for trends were <0.05), and the proportion of abnormalities increased significantly ( χ2=101.40, 42.91, 39.80; all P<0.001). A multivariate logistic regression model was established, which suggested a significant correlation between HGI and LDL-C, non-HDL-C, and AIP (all P<0.05), after adjusting for factors such as age, sex, fasting blood glucose, hypertension, body mass index, smoking, and alcohol consumption. In the overall population, normal glucose tolerance group, and diabetes group, HGI had the highest correlation with non-HDL-C ( OR values of 1.325, 1.678, and 1.274, respectively); in the prediabetes group, HGI had a higher correlation with LDL-C ( OR value: 1.510); and in different glucose metabolism groups, AIP and HGI were both correlated ( OR: 1.208-1.250), but not superior to non-HDL-C and LDL-C. Conclusion:HGI was closely related to LDL-C, non HDL-C, and AIP in the entire population and people with different glucose metabolism, suggesting that HGI may be a predictor of atherosclerotic cardiovascular disease.
