1.Performance optimization of rubber seals based on Mooney-Rivlin model

The sealing performance is directly related to the safety,reliability and mission success rate of aerospace equipment.The sealing performance of rubber seals and the optimized design of the compression force directly affect the performance and work efficiency of the equipment.Based on the hyperelastic Mooney-Rivlin model of nonlinear materials,the mechanical properties of rubber are simulated,and a finite element model of rubber seals is established.Through simulation analysis,the effects of different cross-section dimensions and compression amounts on the sealing performance are studied and compared,and key parameters such as the contact stress distribution,maximum contact stress,and compression force.The size of the fillet affects the final deformation and the contact state.By optimizing the geometric parameters,the compression force can be reduced while ensuring the sealing performance,providing a theoretical basis for the optimized design of rubber seals.
2.Intravascular large B-cell lymphoma of the nasal turbinate presenting with hemophagocytic lymphohistiocytosis: a case report and literature review

Intravascular large B cell lymphoma (IVLBCL) is a rare, aggressive subtype of diffuse large B cell lymphoma, with nasal turbinate involvement being uncommon. We report a 51-year-old woman with a 1-month history of fever of unknown origin. Laboratory findings showed cytopenia, hypertriglyceridemia, elevated ferritin, increased soluble CD25, and bone marrow hemophagocytosis. No infectious cause was identified. PET-CT revealed abnormal 18F-fluorodeoxyglucose (FDG) uptake in the nasal turbinates. Turbinate biopsy revealed tumor cells localized predominantly within vascular lumens, positive for CD20, BCL6, PAX5, and MUM1, with a Ki-67 index >60%, confirming a diagnosis of IVLBCL with hemophagocytic lymphohistiocytosis (HLH). The patient received one cycle of the DEP regimen (liposomal doxorubicin, etoposide, and methylprednisolone) for HLH, followed by five cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) and consolidation with auto-HSCT, achieving sustained complete remission. IVLBCL outcomes are heterogeneous; early diagnosis and prompt treatment improve survival, and R-CHOP plus auto-HSCT may be an effective strategy.
3.Progress in research of textual quality evaluation of health-related media reports

Objective:To summarize the progress in the research of textual quality evaluation of health-related media reports.Methods:A systematic literature retrieval about textual quality evaluation of health-related media reports based on PubMed, Web of Science, Embase, Wanfang database, and China National Knowledge Infrastructure was conducted. Information regarding the textual quality definition, evaluation dimensions, indicators and methods of included papers was extracted.Results:A total of 29 study papers were included in this analysis, in which 26 were about retrospective textual quality evaluation of health-related media reports, and 3 were about the model or tool development for textual quality evaluation of health-related media reports. The topics of news reports included: 16 studies on injury, 3 on general health, 3 on infectious disease, 3 on cancer screening and treatment, 3 on chronic non-communicable disease, and 1 on medication risk. The definition of textual quality of health-related media reports and the dimensions of the quality evaluation varied across the studies. The quality evaluation indicators of media reports can be divided into three categories: availability of surveillance information, availability of professional information, and adherence to principles of media reporting. Most studies conducted the quality evaluation manually, with only 2 studies employing semi-automated or automated evaluation methods.Conclusions:No unified definition, set of dimensions, indicators, or automated algorithms exist for evaluating the textual quality of health-related media reports, which limits assessing massive news data effectively. It is necessary to conduct methodological studies on the textual quality evaluation of health-related media reports based on journalism and communication theory, infodemiology, deep learning, natural language processing, text mining, as well as specific disease and injury prevention theory.

6.Effect of preoperative oral carbohydrate intake on postoperative nausea and vomiting in elderly patients undergoing gynecological laparoscopic surgery

Objective:To evaluate the effect of preoperative oral carbohydrate intake on postoperative nausea and vomiting (PONV) in elderly patients undergoing gynecological laparoscopic surgery.Methods:This study was a randomized controlled trial. One hundred elderly patients, aged≥65 yr, with a body mass index of 18-30 kg/m 2, with American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ, who underwent gynecological laparoscopic surgery at the Baoding First Central Hospital from March 2023 to June 2024, were selected. Stratification was conducted by age (65-74 yr/≥75 yr) using an online randomization tool, and the patients were randomly divided into conventional fasting group (group C) and oral carbohydrate group (group P), with 50 cases in each group. Patients took oral carbohydrate drinks of 4 ml/kg at 2 h before operation, with total amount ≤400 ml in group P, while patients received conventional preoperative fasting protocols in group C. The occurrence and severity of PONV within 48 h postoperatively were observed. The visual analogue scale scores for hunger and thirst, State-Trait Anxiety Inventory scale scores, length of stay in the postanesthesia care unit, duration of hospital stay, time to first flatus, time to first ambulation, and patient′s satisfaction were recorded in the two groups. Results:Compared with group C, the incidence and severity of PONV were significantly decreased within 48 h after surgery, the visual analogue scale scores for hunger and thirst and State-Trait Anxiety Inventory scale scores were significantly decreased after entering the operating room, the length of stay in the postanesthesia care unit, time to first flatus and time to first ambulation were significantly shortened, and the patient′s satisfaction was significantly increased in group P ( P<0.05). Conclusions:Oral carbohydrate intake before operation can effectively reduce the occurrence and severity of PONV and improve the quality of postoperative recovery in elderly patients undergoing gynecological laparoscopic surgery.

In the research of major depressive disorder (MDD), the protective effect of psychological resilience has received increasing attention. This protective effect persists throughout the onset, development, and prognosis of MDD. This paper systematically reviews and summarizes the definition, mechanisms, and research tools of psychological resilience, as well as the research progress regarding its role in MDD. Meanwhile, it elaborates in detail on the deficiencies in current research and, based on this, offers perspectives on future research directions. This review aims to enhance the understanding of psychological resilience, deepen the scientific comprehension of the complex interaction between psychological resilience and MDD, and provide references for research in related fields.
8.Role of different cell-derived exosomal miRNAs in progression,diagnosis,and prognosis of gastric cancer

BACKGROUND:Tumor microenvironment can participate in the occurrence and development of gastric cancer and promote chemotherapy resistance in various ways.Among them,the tumor microenvironment crosstalk mediated by exosomal miRNAs can induce matrix reprogramming,participate in tumor heterogeneity,and form a microenvironment conducive to tumor proliferation,migration,invasion,immune escape,and chemotherapy resistance.OBJECTIVE:To review the mechanism of action of exosomal miRNAs in the microenvironment of gastric cancer and its application in the diagnosis and prognosis assessment of gastric cancer in recent years.METHODS:"Exosomal miRNAs,gastric cancer,angiogenesis,apoptosis,proliferation,migration,autophagy,invasion,immune response,chemotherapy resistance,biomarker"for English search terms and"exosomal miRNAs,gastric cancer"for Chinese search terms were searched in PubMed and CNKI databases.The search period was from 2017 to 2024.After preliminary screening by reading the title and abstract,the articles with poor correlation and repeated content were excluded,and 77 articles were finally included for induction and discussion.RESULTS AND CONCLUSION:(1)Exosomes,as important carriers of intercellular information exchange,can carry a variety of information substances such as miRNA,and realize intercellular signal transmission through three ways:activation of cell surface receptors on target cells,fusion with the plasma membrane of recipient cells,and endocytosis.(2)Exosomal miRNAs play an important role in the progression of gastric cancer by regulating the proliferation,apoptosis,autophagy,angiogenesis,invasion and metastasis,immune response,and the formation of drug resistance of gastric cancer cells.(3)The interaction between miRNAs and target mRNA and its regulatory network are widely found in tumorigenesis and human cancer development.Different types of exosomal miRNAs have different effects on the regulation of apoptosis of gastric cancer cells,and the effects of different exosomal miRNAs on apoptosis related proteins and pathways of gastric cancer cells are screened.Rational use of its inducers or inhibitors can regulate the apoptosis level of gastric cancer cells.(4)Exosomal miRNAs of different cell origin play an important role in the establishment of tumor microenvironment,angiogenesis,immune response,and chemotherapy resistance by inducing M1-polarized macrophages to M2 type.(5)Exosomal miRNAs exist extensively and stably in blood and other body fluids,and their differential expression in patients with gastric cancer can be used as a basis for diagnosis,prognosis,and treatment of patients with gastric cancer.Currently,exosomal miRNAs widely studied as biomarkers include miR-379-5p,miR-590-5p,miR-29s,miR-21,etc.Among them,the sensitivity and specificity of miR-590-5p are 63.7%and 86%,respectively.The expression level of miR-590-5p is closely related to the overall survival rate and the depth of invasion of gastric cancer patients.(6)The design of exosomal miRNAs mimics or inhibitors and their targeted delivery to the tumor site using nano-delivery vectors(such as exosomes and liposomes)to restore the normal level of miRNAs may be a new strategy for the treatment of gastric cancer.(7)Although exosomal miRNAs have great application prospects in the diagnosis and treatment of gastric cancer patients,there are still some problems to be solved.For example,the potential targets and mechanisms of exosomal miRNAs have not been fully explored,and their effectiveness and safety need to be further confirmed.The extraction and purification of exosomes lack standardized large-scale preparation processes.
