1.Research progress on the correlation of solar radiation with pterygium

As a common ophthalmic disease, pterygium exhibits a considerably high incidence in global area, particularly in regions with harsh natural environment. While it is unlikely to cause blindness, pterygium frequently disrupts the normal homeostasis of the tear film, leading to discomfort such as dry eyes and foreign body sensation, thereby significantly impairing patients' quality of life. Furthermore, the high surgical intervention rate for pterygium consumes substantial medical resources. Therefore, investigating the underlying mechanisms of pterygium development is essential for its prevention and control. Extensive research has demonstrated the correlation between solar radiation and pterygium occurrence, confirming that ultraviolet rays within solar radiation contribute to the onset and progression of pterygium through multiple pathways, including direct DNA damage to ocular surface cells, oxidative stress-induced injury, involvement in inflammatory regulation, and extracellular matrix remodeling. This paper reviews both domestic and international epidemiological trends of pterygium, as well as the multifaceted mechanisms by which solar radiation influences pterygium development. The aim is to enhance understanding of the relationship between solar exposure and pterygium occurrence, while emphasizing and guiding public awareness of solar radiation protection in clinical practice.
2.The effectiveness and safety of endoscopic mucosal resection with precutting for rectal neuroendocrine neoplasm smaller than 1 cm in diameter

Objective To investigate the effectiveness and safety of endoscopic mucosal resection with precutting(EMR-P)for the treatment of rectal neuroendocrine neoplasm(RNEN)smaller than 1 cm in diameter.Methods Clinical data of 177 patients with RNEN smaller than 1 cm in diameter from December 2016 to December 2021 were retrospectively analyzed.According to different treatment protocols,177 patients with RNEN were divided into endoscopic mucosal resection(EMR)group(n = 46),EMR-P group(n = 40)and endoscopic submucosal dissection(ESD)group(n = 91).The en bloc resection rate,complete resection rate,operation time,postoperative hospitalization time and incidence of operative complications among the three groups were compared.Results The complete resection rate in the EMR-P group(95.0%)and ESD group(97.8%)were significantly higher than that in the EMR group(87.0%)(P<0.05);The operation time in the EMR-P group(9.86±2.23)min was longer than that in the EMR group(4.12±0.88)min,EMR-P group and EMR group were shorter than that in the ESD group(19.55±3.67)min,the difference was statistically significant(P<0.05);Postoperative hospitalization time in the EMR group was(2.45±0.29)d and EMR-P group was(2.43±0.23)d,which were shorter than that in the ESD group(3.30±0.32)d,and the difference was statistically significant(P<0.05).There were no significant difference in the rates of en bloc resection and operative complications among the three groups(P>0.05).Conclusion EMR-P for the treatment of RNEN<1 cm in diameter has the advantages,such as simple operation,short operation time and hospitalization time,high histological complete resection rate and low complication rate,which is worthy of clinical application.
3.Evaluation of Animal Model of Autism Spectrum Disorders Based on Characteristics of Clinical Symptoms of Traditional Chinese and Western Medicine

ObjectiveBased on the new method of animal model evaluation, this paper summarized and analyzed the characteristics of traditional Chinese medicine(TCM) and Western medicine syndromes in existing autism spectrum disorder(ASD) animal models, and put forward suggestions for improvement. MethodRelevant literature on ASD animal models in China National Knowledge Infrastructure(CNKI) and PubMed were searched. According to the diagnostic standards of traditional Chinese and western medicine, core symptoms and accompanying symptoms were assigned values, and the clinical consistency of the models was comprehensively evaluated. ResultMost ASD model experimental animals were rodents, and the modeling methods included genetic and non-genetic. Domestic research focused on biochemical induction, while foreign research used genetic models more commonly. Among all models, valproic acid induction had the highest clinical consistency, followed by the neuroligin 4(NLGN4) and contactin associated protein like 2(CNTNAP2) gene knockout models. Most modeling methods could meet the characteristics of surface validity and structural validity, but did not clearly distinguish TCM syndromes. Currently, there is no model that has a high degree of clinical agreement between TCM and western medicine at the same time. ConclusionThe existing ASD animal models are mostly constructed under the guidance of western medicine, which lacks the characteristics of TCM syndromes. And the selection of evaluation indicators of western medicine is relatively single, without specifying the types of TCM syndromes. It is recommended to apply TCM intervention factors during the process of model preparation, to improve the evaluation indicators of traditional Chinese and western medicine, and to emphasize the study of on non-human primates, so as to lay a solid foundation for future experiments.
4.Relationship of self-control and perception of social support with mental health status of university students during the normalization of COVID-19 prevention and control

Objective To verify the mental health status and methods to regulate emotion as well as identify the associated factors among Northwest University students during the lockdown period of the COVID-19 outbreak in Xi'an.Methods A questionnaire survey using the random sampling was conducted among the college students in Xi'an,using Self-Control Scale(SCS),Perceived Social Support Scale(PSSS),the Depression-Anxiety-Stress Scale Lite(DASS-21),and Self-Rating Scale of Sleep(SRSS).Results A total of 512 valid questionnaires for college students were collected.The statistical results showed that during the lockdown period after the COVID-19 epidemic outbreak in Xi'an,there were gender and discipline differences in self-control and understanding of social support among college students.That is,compared with boys,girls were more likely to receive social support(P=0.031),friend support(P=0.008),and other support(P=0.029).Compared to liberal arts majors,college students majoring in science and engineering were more likely to engage in self-control,especially in healthy habits(P=0.001),moderate entertainment(P=0.002),and impulse control(P=0.001).However,there was no significant difference in the influence of students'age or whether they came from rural or urban areas on their self-control and understanding of social support abilities.The statistical results also found that the age of college students was significantly correlated with their level of stress and sleep status.Their self-control ability and ability to comprehend social support also had correlation with their mental health status and sleep status during the lockdown period of the epidemic.Conclusion College students were prone to have negative emotions and sleep disorders during the lockdown period of sudden epidemic,which were more common in senior grades,male college students,and liberal arts majors.Active self-control and seeking social support can effectively moderate negative emotions and sleep disturbances.

Background:
Peste des petits ruminants (PPR) is a contagious and fatal disease of sheep and goats. PPR virus (PPRV) infection induces endoplasmic reticulum (ER) stress-mediated unfolded protein response (UPR). The activation of UPR signaling pathways and their impact on apoptosis and virus replication remains controversial.
Objectives:
To investigate the role of PPRV-induced ER stress and the IRE1-XBP1 and IRE1-JNK pathways and their impact on apoptosis and virus replication.
Methods:
The cell viability and virus replication were assessed by 3-(4,5-Dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide assay, immunofluorescence assay, and Western blot. The expression of ER stress biomarker GRP78, IRE1, and its downstream molecules, PPRV-N protein, and apoptosis-related proteins was detected by Western blot and quantitative reverse transcription-polymerase chain reaction, respectively. 4-Phenylbutyric acid (4-PBA) and STF-083010 were respectively used to inhibit ER stress and IRE1 signaling pathway.
Results:
The expression of GRP78, IRE1α, p-IRE1α, XBP1s, JNK, p-JNK, caspase-3, caspase-9, Bax and PPRV-N were significantly up-regulated in PPRV-infected cells, the expression of Bcl-2 was significantly down-regulated. Due to 4-PBA treatment, the expression of GRP78, p-IRE1α, XBP1s, p-JNK, caspase-3, caspase-9, Bax, and PPRV-N were significantly downregulated, the expression of Bcl-2 was significantly up-regulated. Moreover, in PPRV-infected cells, the expression of p-IRE1α, p-JNK, Bax, and PPRV-N was significantly decreased, and the expression of Bcl-2 was increased in the presence of STF-083010.
Conclusions
PPRV infection induces ER stress and IRE1 activation, resulting in apoptosis and enhancement of virus replication through IRE1-XBP1s and IRE1-JNK pathways.
6.The value of serum related cytokines in predicting intestinal mucosal injury in severe acute pancreatitis patients

Objective:To analyze the value of serum related cytokines in predicting intestinal mucosal injury in patients with severe acute pancreatitis (SAP) and its correlation with intestinal mucosal injury.Methods:A total of 92 patients with SAP admitted to the First Hospital of Qinhuangdao from January 2020 to December 2021 were included in the study. According to the presence or absence of intestinal mucosal barrier dysfunction, the patients were divided into intestinal mucosal barrier dysfunction group (33 cases) and non-intestinal mucosal barrier dysfunction group (59 cases). Another 100 healthy subjects were selected as the control group. Clinical data of the subjects were collected. Serum levels of procalcitonin (PCT), D-lactic acid (D-L), endotoxin, diamine oxidase (DAO), citrulline and intestinal fatty acid binding protein (I-FABP) of the three groups were compared, and the correlation between the above indexes was analyzed by Pearson correlation analysis. Receiver operating characteristic (ROC) curve was used to analyze the value of each indicator in predicting intestinal mucosal barrier dysfunction in SAP patients.Results:The levels of serum PCT, D-L, endotoxin, DAO and I-FABP in intestinal mucosal barrier dysfunction group, non-intestinal mucosal barrier dysfunction group and control group showed a downward trend, while the level of serum citrulline showed an upward trend, with statistically significant difference (all P<0.05). Pearson correlation analysis showed that serum citrulline was negatively correlated with serum PCT, D-L, and endotoxin levels ( r=-0.740, -0.629, -0.310, all P<0.05); There was a positive correlation between serum DAO and serum PCT, D-L and endotoxin levels ( r=0.482, 0.779, 0.338, all P<0.05); There was a positive correlation between serum I-FABP and serum PCT, D-L and endotoxin levels ( r=0.613, 0.421, 0.341, all P<0.05). The ROC curve results showed that the area under the curve (AUC) of serum PCT, D-L, endotoxin, DAO, citrulline, and I-FABP predicting intestinal mucosal injury in SAP patients were 0.816, 0.789, 0.732, 0.801, 0.812, and 0.857, respectively. The AUC of the combination of the above indicators predicting intestinal mucosal barrier dysfunction in SAP patients was 0.909, significantly higher than that predicted by each index alone (all P<0.05). Conclusions:The occurrence of intestinal mucosal barrier dysfunction in SAP patients may be related to the increase of serum PCT, D-L, endotoxin, DAO, I-FABP levels and the decrease of citrulline levels. It may be considered to predict the risk of intestinal mucosal injury by detecting the levels of various indicators in patients′ serum.

Pancreatic adenocarcinoma (PAAD) is one of the most lethal malignancies. Although gemcitabine (GEM) is a standard treatment for PAAD, resistance limits its application and therapy. Secoemestrin C (Sec C) is a natural compound from the endophytic fungus Emericella, and its anticancer activity has not been investigated since it was isolated. Our research is the first to indicate that Sec C is a broad-spectrum anticancer agent and could exhibit potently similar anticancer activity both in GEM-resistant and GEM-sensitive PAAD cells. Interestingly, Sec C exerted a rapid growth-inhibiting effect (80% death at 6 h), which might be beneficial for patients who need rapid tumor shrinkage before surgery. Liquid chromatography/mass spectrometry and N-acetyl-l-cysteine (NAC) reverse assays show that Sec C sulfates cysteines to disrupt disulfide-bonds formation in endoplasmic reticulum (ER) proteins to cause protein misfolding, leading to ER stress and disorder of lipid biosynthesis. Microarray data and subsequent assays show that ER stress-mediated ER-associated degradation (ERAD) ubiquitinates and downregulates YAP to enhance ER stress via destruction complex (YAP-Axin-GSK-βTrCP), which also elucidates a unique degrading style for YAP. Potent anticancer activity in GEM-resistant cells and low toxicity make Sec C a promising anti-PAAD candidate.

DECIDE-Diet trial was taken as a case to introduce the methods of blinding and blinding assessment for feeding trials, report the details of blinding, conduct a blinding survey and calculate Jame's BI and Bang's BI. Jame's BI was 0.683 (95% CI: 0.593~0.772). The Bang's BI for the intervention group was 0.340 (95% CI: 0.199~0.481), and for the control group was 0.086 (95% CI: -0.060~0.231). The blinding of the DECIDE)-Diet was generally successful, but the intervention group may infer their group to a certain extent. Feeding trials should report the details of blinding and consider blinding assessment.
9.Correlation between self-reported hearing impairment and physical function in the elderly in China

Objective:To explore the status quo of self-reported hearing impairment and physical function among the elderly in China, and to analyze the correlation between the two.Methods:This study was a cross-sectional study. From January to May 2022, the elderly in 8 provinces or municipalities in East China, South China, Central China, North China, and Southwest China were selected as research objects by convenience sampling. The General Information Questionnaire, the Mini-Mental State Examination (MMSE) and the Short Physical Performance Battery (SPPB) were used for the investigation. Multi-category Logistic regression analysis was used to explore the correlation between physical function impairment and self-reported hearing impairment in the elderly. A total of 5 996 questionnaires were collected, of which 5 822 were valid questionnaires, and the valid recovery rate was 97.1%.Results:Among the 5 822 elderly people, 869 (14.9%, 869/5 822) elderly people self-reported hearing impairment. The total score of SPPB in 5 822 elderly people was (5.89±4.26) , of which 2 899 (49.8%) elderly had low physical function, 1 454 (25.0%) elderly had moderate physical function, and 1 469 (25.2%) elderly had high physical function. Multi-category Logistic regression analysis showed that the elderly with self-reported hearing impairment had a high risk of developing low physical function ( OR=2.214, P<0.05) and moderate physical function ( OR=1.432, P<0.05) . In addition, cognitive impairment, female gender, advanced age, low body mass index (BMI) , low monthly household income, and having three or more chronic diseases were also common risk factors for low and moderate physical function ( P<0.05) . Low education level, unmarried status, living in rural areas and irregular exercise were specific risk factors for low physical function ( P<0.05) . Conclusions:The elderly have poor physical function, and the elderly with self-reported hearing impairment had a high risk of developing low physical function. Attention should be paid to the hearing screening of the elderly. For the elderly with self-reported hearing impairment, they should be encouraged to communicate with others to reduce social loneliness, and participate in sports as much as possible under the premise of ensuring safety, so as to maintain and improve their physical function.
10.The mid-term clinical effect observation of Dynesys applied to the treatment of lumbar degenerative diseases with fatty infiltration of multifidus muscle

Objective:To investigate the clinical effect of Dynamic neutralization system applied to the treatment of lumbar degenerative diseases with fatty infiltration of multifidus muscle.Methods:From Jan 2015 to Dec 2017, a total of 53 patients of lumbar degenerative diseases with multifidus fatty infiltration treated by Dynesys in our hospital were analyzed, included 21 males and 32 females, aged 66.2±7.4 (range 48-81) years. There were lumbar spinal stenosis in 37 casesand lumbar disc herniationin 16 cases; the index level included L 2-S 1 in 3 cases, L 3-S 1 in 13 cases, L 2-L 5 in 5 cases, L 4-S 1 in 17 cases, and L 3-L 5 in 15 cases. The pedicle screws were inserted at the point of intersection of the outer edge of superior articular process and the midline of transverse process. After discectomy of herniated disc and hyperplastic ligamentum flavum, the distance between the upper and lower pedicle screws was measured and then the spacer of the corresponding length was cut out. Finally, the spacer was placed and fixed between the upper and lower pedicle screws by the elastic rope. The degree of multifidus fat infiltration, lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), sacral slop (SS), range of motion (ROM), intervertebral height (IH), Japanese Orthopaedic Association (JOA) score, Oswestry disability index (ODI), the MOS 36-item short-form health survey (SF-36) and visual analog scale (VAS) were evaluated postoperatively. Results:The operation was performed successfully in all the patients. The operation duration was 173.5±64.7 (range 125-240) min. Intraoperative blood loss was 469.5±118.2 (range 380-620) ml. The patients were followed up for 47.9±6.7 (range 38-62) months averagely. At the last follow-up, the degree of fatty infiltration of the multifidus muscle showed no further progress by MR scan. There was no significant difference in ROM and IH at different time points preoperativelyand postoperatively. The LL recovered from 37.6°±8.8° to 43.2°±9.1°, the PT decreased from 24.7°±9.3° to 20.5°±5.1°, and the SS increased from 22.1°±7.7°to 26.3°±8.0°. The JOA score increased from preoperative 6.4±1.2 to 20.6±2.8, ODI decreased from preoperative 50.6%±11.3% to 13.0%±3.4%, SF-36 increased from preoperative 81.5±3.6 to 95.5±4.2, and the VAS decreased from preoperative 4.2±1.0 to 1.1±0.6. One patient experienced loosening and displacement on the left side pedicle screw of the L2 vertebral body 3.5 years after operation, and herclinical symptom improved significantly after conservative treatment.Conclusion:Dynesysis is safe and effective for the treatment of lumbar degenerative diseases with fatty infiltration of multifidus muscle, and it can restore the complete structure and function of tension band at lower back and prevent the progress of multifidus muscle fat infiltration combined with postoperative rehabilitation training.
