1.Development of features analysis workflow for Klebsiella pneumoniae based on clinical metagenomics next generation sequencing data

Objective:To identify strain-specific features of Klebsiella pneumoniae by analyzing metagenomics next generation sequencing (mNGS) data, thereby expanding the downstream applications of mNGS. Methods:The sequences of K.pneumoniae strains were organized from both the self-built database of the long-term multi-center research cohort in China established by the Peking University People′s Hospital from 2009 to 2020 (with 2 345 sequences) and the public databases (with 19 648 sequences). The existing large-scale databases were compressed, and a set of strains representative of clonal groups were screened. A strain genome information library was constructed based on k-mer features, and the most matching representative sequences in the database were searched for the raw mNGS data. The search results of the self-built library and public library were merged and optimized to update the prediction of antimicrobial-resistance characteristics and avoid the impact of uneven data distribution on the results. A total of 314 clinical samples from patients with K.pneumoniae detected by mNGS in the Clinical Microbiology Laboratory of Peking University People′s Hospital from 2022 to 2024 were retrospectively collected, and 101 samples with positive clinical culture results were selected to validate the prediction results. The antimicrobial-resistance phenotypes were verified by clinical antimicrobial susceptibility test results. Whole-genome sequencing was performed on the culture strains of 14 samples randomly selected using random numbers to verify the genotypes. Single nucleotide polymorphism distance analysis was used to verify the occurrence of outbreak events. The χ2 test and Mann-Whitney U test were used for statistical analysis. Results:A representative strain sequence k-mer feature library containing self-built and public sub-libraries was constructed. The library construction required only about 1 hour with <3 GB storage, with a high compression ratio and low update cost. Using k-mer-based analysis, mNGS data achieved precise strain characterization within 4 minutes and and <5 GB memory occupation. There was a significant difference in the antimicrobial-resistance rates to more than half of the antibiotics between the self-built database (90.8%, 2 130/2 345) and the public database (22.7%, 4 457/19 648) ( χ2=4 634.1, P<0.001). After optimizing the search results, the mean category agreement, sensitivity, and specificity of the prediction for eight antibiotics reached 84.8% (323/381), 78.9% (131/166), and 91.2% (196/215), respectively. The target genotypes were successfully detected in 10 out of 12 samples, and two outbreak events (2 samples per event) were successfully identified. Conclusions:An independent analysis process adapted to the needs of identifying the features of K. pneumoniae strains in mNGS data was developed. This process requires minimal computational resources and processing time and can directly achieve the simultaneous analysis of the antimicrobial-resistance phenotypes of K. pneumoniae at the strain level and their corresponding genomic characteristic profiles based on the raw mNGS reads.
2.Clinicopathological analysis of 15 cases of liver metastatic solid pseudopapillary neoplasms

Purpose To investigate the clinicopathological features,immunophenotypic profile,differential diag-noses,and prognostic implications of liver metastatic solid pseudopapillary neoplasm(LMSPN).Methods A retro-spective analysis was conducted on the clinicopathological features,immunohistochemical profile,and clinical outcomes of of 15 cases of LMSPN cases,supplemented by a literature review.Results Of the 15 patients,12 were female and 3 were male,with a mean age of 43 years(range 25-67 years).Multiple hepatic lesions were observed in 9 cases,some of which were accompanied by abdominal or omental metastasis.The tumors exhibited a cystic-solid appearence on gross examination,ranging from 0.5 to 15 cm in diameter.Histologically,the tumors showed typical cystic-solid and pseudopapillary areas,with tumor cells arranged around small blood vessels forming characteristic pseudopapillary structures.Tumor cells exhibited relatively uniform morphology,however,some cases presented with tumor necrosis(5/15),cytologic/nuclear atypia(4/15),mitotic figures(5/15),lymphovascular invasion(6/15),perineural inva-sion(3/15),and lymph node metastasis(2/15).Immunohistochemically,tumor cells showed variable expression ofβ-catenin,LEF1,vimentin,CD10,α1-ACT,PR,E-cadherin,NSE,CD56,Syn and Ki67.Notably,the nuclear ex-pression level of Ki67 and PR were significantly associated with prognosis(P<0.05).β-catenin,LEF1,PR,and Ki67 were predominantly expressed in the nuclei,while markers such as CKpan,CgA,Hep Par-1,Arginase-1,CK7,and CK19 were negative or only weakly expressed.Follow-up data were available for 11 patients(range 10-157 months).Four patients died of widespread hepatic and abdominal metastases,while 7 remained alive.Conclusion The liver is the most most frequent site of distant metastasis for solid pseudopapillary neoplasms of the pancreas.High expression of Ki67 and PR is associated with unfavorable prognosis in LMSPN.

Objective:This study analyzed the epidemiological characteristics of the clustered cases of severe fever with thrombocytopenia syndrome (SFTS) in Jiangsu province, so as to provide a scientific basis for prevention and control efforts.Methods:In this study we searched for cases related to clustered outbreaks and conducted epidemiological investigations of the cases, their co-exposed individuals, and close contacts. The basic information of the cases and their disease progression were collected and documented. The source of infection and transmission routes were analyzed. Fisher’s exact probability test and independent samples t-test were used to compare the differences between index and secondary cases. Results:From 2018 and 2024, a total of nine cluster outbreaks were reported in Jiangsu province, involving 32 cases of SFTS with 12 fatalities. Seven clustered outbreaks occurred in rural communities, while two occurred in medical institutions. The primary cause of these outbreaks was direct contact with confirmed cases.Conclusions:In recent years, Jiangsu province has experienced multiple clustered outbreaks of SFTS. Targeted health education should be implemented in high-risk areas to increase awareness of personal protective measures. Medical institutions should standardize the management of confirmed cases, educate patients′ families about prevention and control measures, and strengthen training for healthcare workers to ensure effective infection control within hospitals.

7.Exploring Efficacy and Mechanism of Saffron Floral Bio-residues for Treatment of Hyperuricemia Combined with Gouty Arthritis Based on Compound Compatibility Environment

ObjectiveTo investigate the efficacy and mechanism of saffron floral bio-residues(SFB) in the treatment of hyperuricemia(HUA) combined with gouty arthritis(GA) in a compound compatibility setting. MethodScreening candidate control Chinese medicines for compound and SFB based on network target distance calculation and data analysis. After adaptive feeding of 80 SD rats for 7 days, 10 rats were randomly selected as the blank group, while the remaining 70 rats were intraperitoneally injected with 3% potassium oxonate and orally administered with 1% adenine for 14 consecutive days. On the 13th day, rats were injected with 2.5% sodium urate solution into the right ankle joint cavity to induce swelling of the joint capsule on the opposite side, inducing a HUA combined with GA model. At the same time, the modeling rats were randomly divided into 7 groups, including the model group, benzbromarone group(positive drug, 0.02 g·kg-1), Tongfengshu tablets group(9 g·kg-1), Tongfengshu granules group(9 g·kg-1), SFB granules group(3.6 g·kg-1), Plantaginis Semen granules group(3.6 g·kg-1), and new formula group(SFB replacing Plantaginis Semen in Tongfengshu granules, 9 g·kg-1), with 10 rats in each group. Each treatment group was orally administered with the corresponding drugs according to body weight, while the control and model groups were given equal volume of distilled water by gavage once a day for 14 consecutive days. After 14 days of synchronous administration and modeling, changes in gait, ankle joint swelling and mechanical pain threshold in rats were observed, and serum uric acid, creatinine, urea nitrogen and xanthine oxidase(XOD) were measured. Enzyme-linked immunosorbent assay(ELSIA) was used to detect the levels of tumor necrosis factor(TNF)-α, interleukin(IL)-1β and IL-6 in rat serum, hematoxylin-eosin(HE) staining was used to observe the pathological changes in the liver, kidney and ankle joints of rats, Western blot was used to detect the expression levels of uric acid transporter 1(URAT1), glucose transporter 9 (GLUT9), organic anion transporter 1(OAT1), adenosine triphosphate(ATP) binding cassette transporter G2(ABCG2), and liver XOD proteins. ResultThrough network pharmacology analysis, Plantaginis Semen was selected as a candidate control herb, and Tongfengshu tablets was used as a compound compatibility environment to explore the efficacy of SFB in reducing blood uric acid levels and treating GA. Animal experiments showed that compared with the blank group, the gait score and joint swelling degree of the model group were significantly increased, and the mechanical pain threshold was significantly decreased(P<0.01). Compared with the model group, the gait score, joint swelling degree and mechanical pain threshold of rats in each medication group were improved to varying degrees. Biochemical indicators showed that compared with the blank group, the serum uric acid, creatinine, urea nitrogen and XOD levels of the model group were significantly increased(P<0.01). Compared with the model group, the serum uric acid and XOD levels of rats in each treatment group were significantly decreased(P<0.01). ELISA results showed that compared with the blank group, the levels of serum TNF-α, IL-1β and IL-6 in the model group were significantly increased(P<0.01). Compared with the model group, the levels of TNF-α, IL-1β and IL-6 in the benzbromarone group, Tongfengshu tablets group, Tongfengshu granules group and new formula group were significantly reduced(P<0.05,P<0.01). Western blot results showed that compared with the blank group, the expression levels of URAT1 and GLUT9 proteins in renal tissue and OXD protein in liver tissue of the model group were significantly increased, while the expression levels of renal OAT1 and ABCG2 were significantly decreased(P<0.01). Compared with the model group, the expression levels of renal URAT1 and GLUT9 in the SFB granules group, Tongfengshu granules group and new formula group were significantly decreased, while the expression levels of renal OAT1 and ABCG2 were significantly increased, and the expression of XOD protein in liver tissue was significantly decreased(P<0.05, P<0.01). Pathological analysis showed that focal infiltration of neutrophils, cell necrosis and nuclear fragmentation were observed in the liver tissue of the model group, sodium urate deposition crystals and tubular dilation appeared in renal tissue, synovial hyperplasia and inflammatory cell infiltration appeared in ankle joint. Compared with the model group, the abnormal degrees of liver, kidney and ankle joint tissue of rats in each treatment group were alleviated. ConclusionThe new formula of SFB replacing Plantaginis Semen has the same effect in the treatment of HUA combined with GA. This study proposes a new strategy to investigate the efficacy of new resources of Chinese medicine in a compound compatibility environment, which can provide a new demonstration for the research and development of new resources of Chinese medicine.

9.A scope review of self-management intervention research in patients with hematopoietic stem cell transplantation

Objective To review the research of self-management intervention in patients with hematopoietic stem cell transplantation,clarify the intervention elements,so as to provide references for clinical practice and future related research.Methods According to the scoping review framework,a literature search was performed in the PubMed,Embase,CINAHL,Web of Science,Cochrane Library,CNKI,Wan fang Database,VIP,CBMdisc for all the studies on self-management intervention of patients with hematopoietic stem cell transplantation.The retrieval time was from the establishment of the database to December,2022.There were 2 researchers who extracted and analyzed the data of the included literature.Results A total of 12 articles were included,including 3 randomized controlled trials,8 quasi-experimental studies,and 1 mixed study.6 studies were conducted with interventions based on the theoretical basis or nursing model such as self-management oriented 5A nursing model,continuous nursing model,problem-based learning theory.The intervention methods of 12 studies included nurse-led self-management guidance,mobile applications,multidisciplinary collaboration,and graphic cards.The intervention time was mostly 1~3 months.The outcome indicators included feasibility evaluation indicators such as operability,ease of use,and user satisfaction of the application,and effect evaluation indicators such as self-management ability,quality of life,and medication compliance of patients.The results showed that self-management interventions could improve patient self-management ability,quality of life,medication compliance,and improve their psychological and nutritional status.Conclusion The research on self-management intervention in patients with hematopoietic stem cell transplantation is still in the development stage,and its positive effect has been preliminarily verified.Medical staff should strengthen the self-management evaluation of patients with hematopoietic stem cell transplantation,play the role of multidisciplinary team,use electronic health intervention methods,and formulate multi-level,scientific and effective self-management intervention programs.
10.Erythroderma caused by apatinib combined with toripalimab

A 66-year-old male patient with malignant melanoma received combined treatments with apatinib (oral 250 mg once daily), temozolomide (oral 500 mg on day 1 and 400 mg/d on days 2-4), toripalimab(240 mg intravenous infusion on days 1 and 15). After 5 cycles (28 days as a cycle), the patient developed multiple rashes, which were not alleviated after reducing the dose of apatinib to 250 mg orally once every 3 days in the 6th treatment cycle. Laboratory tests showed that serum albumin was 28.5 g/L, total serum protein was 55.5 g/L, eosinophil percentage was 0.096, and venous potassium was 3.17 mmol/L. Erythroderma caused by combination of apatinib and toripalimab was considered, and the 2 drugs were stopped. The patient received the treatments of methylprednisolone, loratadine, diphenhydramine, triamcinolone acetonide and econazole nitrate cream and mupirocin ointment (external coating), and skin care. At the same time, symptomatic treatments such as protein supplement, diuresis, potassium supplement, and stomach protection were given. After 10 days of treatments, the rash subsided, and desquamation and itching were improved.
