1.Mechanism of active ingredient compatibility of Dimocarpus longan Lour. leaves in improving glucose and lipid metabolism disorders in type 2 diabetic mellitus rats

OBJECTIVE To explore the mechanism of the active ingredient compatibility(quercetin, quercitrin and kaempferol at a mass ratio of 2∶9∶3)of Dimocarpus longan Lour. leaves(abbreviated as CDL) on ameliorating glucose and lipid metabolism disorders in type 2 diabetes mellitus (T2DM) rats. METHODS SD rats were randomly divided into blank control group, model group, metformin hydrochloride group (100 mg/kg), and CDL high-, medium- and low-dose groups (280, 140, 75 mg/kg), with 10 rats in each group. Rats in the blank control group were fed with standard chow, while rats in the other groups were given high-sugar and high-fat diet combined with intraperitoneal injection of streptozotocin to establish the T2DM rat model. After successful modeling, rats in each administration group were given corresponding drug solution, and rats in the blank control group and model group were intragastrically administered with equal volume of pure water, once a day, for consecutive 4 weeks. Fasting blood glucose (FBG) was detected at fixed time every week. The curves of oral glucose tolerance test (OGTT) and intraperitoneal insulin tolerance test (IPITT) were plotted, and the area under curve (AUC) was calculated. The pancreatic islet function indexes [fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), insulin sensitivity index (ISI)],blood lipid indexes [total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C)] and hepatic glycogen content were determined. The pathological morphological changes of liver and pancreatic tissues were observed. The protein and mRNA expression levels of molecules related to phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway in liver tissues were detected. RESULTS Compared with the blank control group, the FBG, AUC of IPITT curve, AUC of OGTT curve, HOMA-IR, the levels of FINS, TC, TG and LDL-C, as well as the protein and mRNA expression of phosphatase and tensin homolog, forkhead box protein O1 and glycogen synthase kinase-3β in liver tissues were significantly increased in the model group ( P <0.05). ISI, the levels of HDL-C and hepatic glycogen content, along with the protein and mRNA expression of PI3K, insulin receptor substrate-1, Akt and protein expression of phosphorylated Akt in liver tissues were markedly decreased ( P <0.05). In model group rats, the arrangement of hepatocytes was irregular, the overall structure of pancreatic lobules was disordered, and a large number of inflammatory cell infiltration was observed. Compared with the model group, most of the above quantitative indexes were significantly reversed in the CDL high-dose group ( P <0.05), and the pathological lesions of liver and pancreas were obviously alleviated. CONCLUSIONS CDL can regulate glucose and lipid metabolism disorders, elevate insulin sensitivity and relieve insulin resistance in T2DM rats. Its mechanism may be related to the activation of the PI3K/Akt signaling pathway.

3.Expert consensus on sensitive indicators for assessment of the quality of nursing in operating theatre

Objective To develop the Expert Consensus on Sensitive Indicators for Assessment of the Quality of Nursing in Operating Theatre and provide a scientific and practical guidance for improving the quality of nursing in operating theatre.Methods The writing team established by the Operating Room Nursing Professional Committee of Guangdong Nursing Association conducted systematic literature retrieval and screening,and used the updated clinical Guidelines for Research and Evaluation Ⅱ in UK 2017.AGREE Ⅱ and the evidence evaluation system of the Australian JBI(Joanna Briggs Institute,JBI)Evidence-Based Health Care Center evidence level system(2016 Edition)comprehensively analyzed the evidence related to the sensitive indicators for evaluating the quality of operating room nursing and the suggestions of the writing group members.The first draft was formed based on the three-dimensional quality evaluation theoretical framework of"structure-process-result".Through the Delphi method,after two rounds of expert consultations and members'votes,the first draft was deeply revised and improved.Results Based on the three-dimensional quality evaluation theoretical framework of"structure-process-outcome"proposed by American scholar Donabedian,the expert consensus finally included five primary indicators:basic nursing quality,quality indicators of patient safety,quality indicators of hospital infection control,quality indicators of medication and safety management,and quality indicators of specialised nursing in operating theatre.The secondary indicators consisted of one structural indicator(management of commonly used instrument and equipment in operating theatre)and 17 process indicators(e.g.,infusion and blood transfusion management,body temperature management,etc.).The tertiary indicators included 26 process indicators and 11 outcome indicators(e.g.,incidence of adverse reactions of infusion during surgery,incidence of intra-operative hypothermia,etc.).Conclusion The evidence-and guideline-based Expert Consensus on Sensitive Indicators for Assessment of the Quality of Nursing in Operating Theatre based on eviclence and guidelines was established through rigorous evidence-based methods.It is operational and practical,and offers theoretical support and practical guidance for the managers of operating theatre to improve the quality of nursing.

5.Correlation between the diameter of the left common iliac vein and lower extremity deep venous thrombosis evaluated by multi spiral CT venography

Objective To analyze the correlation of the left common iliac vein(LCIV)diameter and the lower extremity deep venous thrombosis(DVT)by direct multi spiral computed tomography venography(MSCTV).Methods The MSCTV images of 218 patients with lower extremity DVT(DVT group)and the enhanced CT images of 168 patients in the control group were analyzed retrospectively.The diameter of the LCIV was measured and the compression degree was evaluated.Results In the DVT group,the diameter of the LCIV significantly reduced,with an average value of(5.150±2.352)mm.In the control group,the average diameter of the LCIV was(6.401±2.550)mm.The difference between the two groups was statistically significant(Z=-5.453,P<0.05).The degree of compression in the LCIV was 61%(50%-95%)in the DVT group and 39.6%(0%-73%)in the control group,showed a statistically significant difference(Z=-10.555,P<0.05).There were statistically significant differences in LCIV diameter between males and females within both DVT and control groups(P<0.05),and there were no statistically significant differences in LCIV compression degree between males and females within both DVT and control groups(P>0.05).Regression analysis showed a positive correlation between LCIV diameter and lower extremity DVT.Conclusion The lower extremity MSCTV enables accurate diagnosis of DVT by visualizing compression of the LCIV and quantifying its degree of compression.A reduced diameter of the LCIV is an independent risk factor for lower extremity DVT.
6.Effects of marital self-disclosure nursing on fear and quality of life of maintenance hemodialysis patients

Objective:To explore the effect of marital self-disclosure nursing on the fear and quality of life of maintenance hemodialysis patients.Methods:From January to December 2019, 106 maintenance hemodialysis patients who were treated at Jinhua Municipal Central Hospital in Zhejiang Province were selected as the research object. The patients were divided into the control group and the intervention group by random number table method, with 53 cases in each group. The control group conducted routine nursing, and the intervention group implemented marital self-disclosure nursing on this basis. The Fear of Progression Questionnaire-Short Form (FoP-Q-SF) and the Kidney Disease Questionnaire (KDQ) were used to compare the fear of disease progression and quality of life between the two groups before and after intervention.Results:After 12 weeks of intervention, the scores of physiological health dimension and social family dimension in the fear of disease progression in the intervention group were (15.2±1.8) and (13.0±2.1) respectively, which were lower than those of the control group, and the difference was statistically significant ( P<0.05) . After 12 weeks of intervention, the total KDQ score of the intervention group was (152.8±15.2) , and that of the control group was (138.9±15.7) , and the difference was statistically significant ( P<0.05) . Conclusions:The application of marital self-disclosure nursing in maintenance hemodialysis patients can reduce the fear and improve the quality of life of patients.
7.Effects of WeChat online support system on continued nursing care for patients receiving enteral bladder augmentation

Objective? To establish a WeChat online system supporting continued nursing care for patients receiving enteral bladder augmentation and to evaluate its effects on continued nursing care for patients with neurogenic bladder in spinal cord injury who received enteral bladder augmentation. Methods? Totally 60 patients with low-compliant neurogenic bladder in spinal cord injury who received enteral bladder augmentation and were discharged from Zhejiang Provincial People's Hospital between March 2016 and March 2018 were selected by convenient sampling, and divided into the observation group (n=30) and the control group (n=30) according to the random number table. Patients in the control group received conventional nursing care after discharge, while patients in the observation group received nursing care after discharge via the WeChat online support system. The follow-up duration was 3 months. Bladder self-management compliance, urinary catheter-associated complications, bladder function, quality of life and negative emotions at discharge and 3 months after discharge were compared between the two groups. Results? The score of bladder self-management compliance, anxiety score, depression score, and scores of physical function, role function, emotional function and social function in quality of life in the observation group were better than those in the control group 3 months after discharge (P<0.05); the rates of urinary tract infection and urethral injury of the observation group were 3.3% and 6.7%, while those of the control group were 26.7% and 33.3%, respectively; the incidence rate of urinary catheter-associated complications of the observation group was lower than that of the control group (P<0.05); and the parameters of bladder function such as bladder volume, bladder compliance, residual urine volume and maximum detrusor pressure of the observation group were better than those of the control group (P< 0.05). Conclusions? The WeChat online support system can improve patients' bladder self-management compliance, reduce the urinary catheter-associated complications, enable the recovery of bladder function, ameliorate their negative emotions, and improve their quality of life.
8.Effects of visual management health education model on incontinence-associated dermatitis in inpatients

Objective To explore the effects of visual management health education model on incontinence-associated dermatitis (IAD) in inpatients.Methods A total of 55 incontinence patients hospitalized in the People's Hospital of Zhejiang Province from February 2017 to February 2018 were selected as the observation group,and visual management health education was carried out.Another 58 incontinence patients hospitalized from January 2016 to December 2016 were selected as the control group,and routine health education was carried out.The incidence and duration of IAD,self-efficacy and nursing satisfaction were compared between the two groups after 1 months of nursing intervention.Results There were 1 cases of first degree dermatitis and 1 case of second degree dermatitis,with a total incidence of 3.64%.There were 6 cases of first degree dermatitis,2 cases of second degree dermatitis,and 1 cases of third degree dermatitis,with a total incidence of 15.52%.The difference was statistically significant (P < 0.01).The duration of IAD of the observation group and the control group was (13.5 ± 3.1) d and (7.6 ± 2.4) d respectively,and the difference between the two groups was statistically significant (P< 0.01).The results,coping,self management,general self-efficacy and total score of the patients in the observation group before and after visual management nursing intervention were significantly higher than those in the control group,and all the differences were statistically significant (P < 0.05).The satisfaction on technical operation and clinical nursing quality of the patients in the observation group was higher than that of the control group (P < 0.05).There was no significant difference in the ward environment and service attitude between the two groups (P > 0.05).Conclusions Visual management can reduce IAD and improve self-efficacy and nursing satisfaction of inpatients with incontinence.
10.Application effect of clinical nursing path of Traditional Chinese Medicine in facial paralysis patients

Objective To discuss the application effect of clinical nursing pathway of Traditional Chinese Medicine ( TCM) in facial paralysis patients. Methods A total of 80 cases of facial paralysis patients from February 2012 to August 2014 were randomly divided into the observation group ( n=40 ) and the control group (n=40) according to random number table method. The control group was adopted routine treatment and nursing care, while the observation group was given clinical nursing pathway of TCM treatment and nursing care. The average length of hospital stay, hospital expenses, nursing compliance and satisfaction were compared. Results The hospitalization time and cost of the observation group were (13 ± 2. 3) days and (7 382. 0 ± 123. 1) yuan, which were significantly better than those of the control group (t=10. 70, 39. 55, respectively;P<0. 05). In the observation group, 36 patients were totally complied, three patients were partially complied, and one patient was not complied, which were significantly better than those of the control group (Z=4. 96,P<0. 05). In the observation group, 37 patients were totally satisfied, three patients were basically satisfied, and no patient was not satisfied, which were significantly better than those of the control group (Z =4. 97,P <0. 05). Conclusions The application of TCM clinical nursing pathway in facial paralysis patients can reduce the length of hospital stay and hospital costs, and improve patients′care compliance and satisfaction.
