Objective:To compare the midium-to-long-term clinical outcomes of Ceramage? polymer ceramic composite resoin on-lay and CAD/CAM fabricated all-ceramic onlays in the restoration of endodontically treated posterior teeth.Methods:A total of 75 patients with 92 endodontically treated posterior teeth exhibiting class Ⅱ defects were included and randomly divided into two groups:the Ceramage? composite resin group(48 teeth)and the CAD/CAM fabricated all-ceramic group(44 teeth).The respective restora-tions were performed,and patients were followed up for 24 months.Clinical outcomes of the restorations and periodontal conditions were evaluated using the modified USPHS criteria,and patient satisfaction was assessed.Results:After 24 months of follow-up,the Ceramage? composite resin group showed higher rates of secondary caries,marginal discrepancy,and marginal discoloration,as well as higher plaque index,gingival index,and sulcus bleeding index compared to the CAD/CAM fabricated all-ceramic group.Howev-er,the Ceramage? group exhibited lower rates of restoration wear and fracture(P<0.05).Patient satisfaction was similar between the two groups(P>0.05).Conclusion:Both Ceramage? polymer ceramic composite resoin onlay and CAD/CAM fabricated all-ce-ramic onlays achieved favorable mid-to-long-term clinical outcomes.The choice of restoration method should comprehensively con-sider the patient's oral condition and restorative needs.

3.Best evidence summary of emergency service surge capacity

Objective:To search, evaluate and integrate the best evidence of the best evidence for emergency service surge capacity.Methods:According to the "6S" model of evidence resources, the related evidence on emergency service surge capacity in Guidelines International Network, National Guideline Clearinghouse, Canadian Medical Association CPG Infobase, Scottish Intercollegiate Guidelines Network, European Society for Emergency Medicine, the American College of Emergency Physicians, Emergency Nurses Association, Cochrane Library, PubMed, Embase, CINAHL, Web of Science, BMJ Best Practice, UpToDate,CKNI, Wanfang, and VIP database were searched by computer. The retrieval time limit was from the establishment of the database to Dec 31, 2023. Literature quality assessment and data extraction were performed by 2 researchers.Results:A total of 11 articles were included in this study, including 1 guideline, 7 expert consensuses and 3 systematic reviews, which summarized 43 pieces of evidence involving 7 categories, namely core elements, organizational management, space management, personnel allocation, material allocation, education and training, and support services.Conclusions:The best evidence summarized in this study can provide a reference for emergency service to improve surge capacity. In clinical application, emergency departments should focus on organizational, space, personnel and materials management, combined with the type of emergency events, to maximize their routine, emergency and crisis response capabilities, so as to respond to medical surges effectively.
4.Clinical eficacy of customized titanium mesh combined with concentrated growth factors for restoration of anterior alveolar ridge defects

Objective:To observe the clinical efficacy of customized titanium mesh combined with concentrated growth factors(CGF)for restoration of anterior alveolar ridge defects.Methods:105 patients with anterior alveolar ridge defects were included and underwent implant restoration.The patients were classified into 2 groups by random number table method,those in control group(n=53)received customized titanium mesh,in study group(n=52)received customized titanium mesh combined with CGF.After 1 year follow-up,the success rate of implantation,alveolar ridge height,alveolar ridge bone augmentation,alveolar bone mineral density,bone thickness,aesthetic results and complications were compared between the 2 groups.Results:In 1-year follow-up,no implant loosening and shed-ding occurred in the 2 groups,and the success rate of implantation was 100%.The height of the alveolar ridge,the horizontal bone aug-mentation,the alveolar bone mineral density and bone thickness of study group were greater than those of the control group(P<0.05).Study group scored higher on pink aesthetic/white aesthetic score(PES/WES)than control group(P<0.05).In study group,there were 2 cases of postoperative swelling and pain,2 cases of periodontal infection,1 case of gingival bleeding and 1 of allergy.In control group,there was 1 case of postoperative swelling,1 of pain,1 of gingival bleeding,1 of allergy and 2 of periodontal infection(P>0.05).Conclusion:Application of customized titanium mesh combined with CGF for restoration of anterior alveolar ridge defects can improve the height of the alveolar ridge and bone metabolism,ensure the aesthetic results.
5.Emergency Department nurses' cognition on palliative care: a qualitative study

Objective:To understand the cognition and attitudes of Emergency Department nurses towards palliative care, and to explore the factors that hinder or promote the development of palliative care in the Emergency Department, so as to provide basic data for emergency palliative care.Methods:From March to June 2021, semi-structured interviews were conducted with 11 nurses in the Emergency Department of Peking Union Medical College Hospital using the purposeful sampling. The interview data were organized and analyzed using Colaizzi's 7-step analysis method.Results:A total of three themes were induced. The themes were that Emergency Department patients and their families were often caught in decision-making dilemmas, and Emergency Department nurses had a high degree of acceptance of palliative care, but there were barriers to implementation in the Emergency Department, and the implementation methods of emergency palliative care needed to be further explored, and Emergency Department nurses were willing to participate in palliative care training.Conclusions:Emergency palliative care needs to consider the characteristics of the Emergency Department work environment and explore appropriate implementation methods. Training related to palliative care can be increased, the palliative care literacy of Emergency Department nurses can be improved, so as to provide effective support for life-limited patients and their families.

7.Study on mental health status of pregnant women and its influencing factors in the third trimester

Objective:To understand the mental health status of pregnant women in the third trimester of pregnancy, and explore the influencing factors.Methods:The general demographic information and pregnancy information of 575 pregnant women in the Chinese Pregnant Women Cohort Study were collected. The mental health status of pregnant women in the third trimester was investigated by using Edinburgh Postpartum Depression Scale, Self-Rating Anxiety Scale and University of California at Los Angeles. Multivariate unconditional logistic regression was used to analyze the influencing factors for mental health of pregnant women.Results:In the third trimester of pregnancy, the incidence of depression was 16.52%, the incidence of anxiety was 11.13%, and the incidence of feeling loneliness was 26.26%. Logistic regression analysis showed that compared with those with education level of junior high school below, those with education level of college or bachelor's degree ( OR=0.418, 95% CI: 0.184-0.950) and master's degree or above ( OR=0.116, 95% CI: 0.027-0.503) were less likely to feel loneliness. Pregnant women with higher annual family income (10 000 RMB yuan) were less likely to suffer from depression (≥20 vs. <10: OR=0.527, 95% CI: 0.279-0.998), anxiety (10-20 vs. <10: OR=0.363, 95% CI: 0.180-0.731; ≥20 vs. <10: OR=0.271, 95% CI: 0.132-0.554) and feeling loneliness (≥20 vs. <10: OR=0.477, 95% CI: 0.276-0.826). Conclusions:The education level and family income have impacts on the mental health status of pregnant women in the third trimester. Medical staff should give targeted psychological advice to pregnant women to improve their mental health status.
8.Comparative study on efficacy and safety of generic and original pemetrexed in patients with non-small cell lung cancer

Objective:To compare the efficacy and safety between the generic pemetrexed produced by Sichuan Huiyu Pharmaceutical Co., LTD and the original pemetrexed produced by Eli Lilly Nederland B.V. in the treatment for patients with non-small cell lung cancer (NSCLC).Methods:The subjects were patients who received generic and original pemetrexed from March 2019 to December 2019 in Cancer Hospital of Chinese Academy of Medical Sciences. Demographic characteristics (age, gender, past history, etc.), treatment-related information (NSCLC stage, treatment regimen, underlying diseases, etc.), occurrence of adverse events, and efficacy evaluation in patients were collected. Patients were divided into the generic group and the original group, and the general situation, clinical use of pemetrexed, and adverse events in patients in the 2 groups were compared. After propensity score matching for 7 variables such as gender, age, body weight, body surface area, Eastern Cooperative Oncology Group (ECOG) score, tumor stage, and standardized chemotherapy dose, the efficacy and safety in patients between the 2 groups after 2 cycles of treatment with generic and original pemetrexed were compared.Results:A total of 182 patients were enrolled in the study, including 85 patients in the generic group and 97 patients in the original group. The differences in age, gender, ECOG score, body weight and body surface area, and underlying chronic diseases between the 2 groups were not statistically significant (all P>0.05). The patients with advanced stage Ⅲ and Ⅳ cancer in the generic group were significantly more than those in the original group [92%(78/85) vs. 79% (77/97), P=0.032]. The proportion of palliative chemotherapy and maintenance chemotherapy in the generic group was higher than that in the original group ( P<0.001). The difference in median dosage between the generic group and the original group was statistically significant [900 (800, 1 000) mg/m 2vs. 800 (800, 900) mg/m 2, P=0.019]. The difference in chemotherapy cycles between the 2 groups was statistically significant [5 (4, 10) vs. 4 (2, 4), P<0.001]. The overall incidence of adverse events and the incidences of bone marrow toxicity and liver toxicity in the generic group were higher than those in the original group ( P=0.018, P=0.037, P=0.018). After propensity score matching, there were 38 patients in both groups, and the differences in the objective response rate, disease control rate and the incidence of adverse events between the 2 groups were not statistically significant [26% (10/38) vs. 32%(12/38), P=0.723; 89% (34/38) vs. 96% (36/38), P=0.674; 47% (18/38) vs. 24%(9/38), P=0.055]. Conclusion:After propensity score matching, the difference in the efficacy and safety between the generic and the original pemetrexed was not significant.

10.Depression symptoms and influencing factors in early pregnant women in China

Objective To analyze the prevalence of depression in early pregnancy, explore its possible influencing factors, and provide reference for mental health care during pregnancy. Methods A baseline survey of 9 193 early pregnant women recruited by the Chinese Pregnant Women Cohort Study (CPWCS) program was conducted. Edinburgh Postnatal Depression Scale was used to assess the depression symptoms in early pregnancy. The χ2 test and multivariable logistic regression were used to analyze the influencing factors. Results The rate of depression in early pregnant women in CPWCS was 46.50%. Multivariate analysis showed that unplanned pregnancy (OR=1.23, 95%CI: 1.12~1.35, P<0.001), passive smoking (OR=1.28, 95%CI: 1.18~1.39, P<0.001), drinking (OR=1.35, 95%CI: 1.12~1.63, P=0.002) and consumption of carbonated or sugary beverages (OR=0.80, 95%CI: 0.73~0.88, P<0.001) were risk factors for depression during early pregnancy, while enough sleep (OR=0.71, 95%CI: 0.57~0.88, P=0.002), moderate-high level of physical activity (OR=0.87, 95%CI: 0.78~0.98, P=0.023), healthy intake of vegetables and fruits(OR=0.60, 95%CI: 0.51~0.70, P<0.001), animal foods(OR=0.82, 95%CI: 0.74~0.91, P<0.001) and milk and dairy products(OR=0.81, 95%CI: 0.72~0.90, P<0.001)were protective factors. Conclusion The rate of depression was high and it was affected by many factors. It is necessary to pay more attention to depression symptoms during early pregnancy and carry out targeted psychological health care during pregnancy.
