1.Research on the Mechanism of Traditional Chinese Medicine Scientific and Technological Achievements Transformation Based on Complex Adaptive System Theory

The improvement of the mechanism for the transformation of Traditional Chinese Medicine(TCM)scientific and technological achievements,as well as increasing the rate of achievement transformation,is an urgent need for the industrialization of scientific and technological achievements.Due to the uniqueness of theories,complexity of compound prescriptions,and inheritance of experience in TCM,the rate of scientific and technological achievements transformation is low.Based on the theory of Complex Adaptive System,it elaborates on the four characteristics of the TCM achievement transformation process,namely,aggregation,diversity,nonlinearity,and flow,as well as the three mechanisms,namely,the mechanisms of identification,building blocks and internal model.It also puts forward relevant suggestions such as promoting the deep aggregation of cross-border subjects,strengthening the construction of a multi-dimensional identification system,and constructing a highly adaptable internal model mechanism.
2.Hepatotoxicity of Brucea javanica bitter alcohol in nude mice based on liver drug metabolizing enzyme CYP450 expression

AIM:Hepatotoxicity of Brucea javani-ca picryl with broad-spectrum anticancer effect in nude mice based on hepatic drug metabolizing en-zyme CYP450 activity.METHODS:Fifty-six nude mice were randomly divided into blank group,Bru-cea javanica low-dose group(2 mg/kg),Brucea ja-vanica high-dose group(4 mg/kg),and cisplatin group(2 mg/kg),with 14 mice in each group.The blank group was injected with the same amount of normal saline every 3 days for 6 weeks.Calculate the mortality rate of nude mice in each group,ob-serve the general growth state of nude mice,re-cord the weight change of nude mice before and af-ter administration,weigh and record the liver weight after taking materials,and calculate the liv-er coefficient(liver weight/weight mass×100%),ob-serve and record the liver color and morphology.Hematoxylin-eosin(HE)staining was used to ob-serve the pathological changes of liver tissue.De-tection of alanine aminotransferase(ALT),aspar-tate aminotransferase(AST),lactate dehydrogenase(LDH),alkaline phosphatase(AKP)and albumin(ALB)levels in serum of nude mice by ELISA.Real-time PCR and Western blot were used to detect the mRNA and protein expression levels of CYP2E1,CYP3A11,CYP2C19,CYP1A2,CYP2D6 and CYP2C9,which were key enzymes of drug metabolism in nude mice liver.RESULTS:Compared with the blank group,the mortality rate of nude mice in the low-dose Brucea javanica bitter alcohol group was 0,the growth state was good,the diet,movement,and mental state were normal,the weight change and liver coefficient ratio were consistent,the liver color was ruddy,the liver lobule morphology was complete under the microscope,the structure was clear,the liver cells were arranged regularly,and there was no inflammatory cell infiltration.There was no significant difference in the content of ALT,AST,LDH,AKP,and ALB.There was no significant difference in the mRNA and protein expression of CYP2E1,CYP3A11,CYP2C19,CYP1A2,CYP2D6,and CYP2C9(all P>0.05).Compared with the blank group,the mortality rate of nude mice in the high-dose group of Brucea javanica bitter alcohol was 14.3%,the growth state was slightly poor,the diet,movement,and mental state were reduced,the weight growth was slow,the liver coefficient ratio was increased,the liver color was reddish brown,some liver lobule boundaries were unclear,a small number of liver cells were loosely arranged,the contents of ALT,AST,LDH,AKP,and ALB were signif-icantly increased,the mRNA levels of CYP2E1,CYP3A11,CYP2C19,CYP1A2,CYP2D6,and CYP2C9 were significantly reduced,and the protein expres-sions of CYP2E1,CYP3A11,CYP1A2,and CYP2D6 were significantly reduced(all P<0.05 or P<0.01),but there was no statistical difference in the mRNA and protein expression of CYP2C19,and the pro-tein expression of CYP2C9(P>0.05).Compared with the blank group,the mortality rate of nude mice in the cisplatin group was 35.7%,the growth state was poor,the diet,action,and mental state were low,the weight gain was less,the liver coefficient ratio was significantly increased,the liver color was dark red,the liver sinusoids and central veins were congested,the hepatocytes were disordered,the nuclei were consolidated and contracted,and the arrangement was loose,the contents of ALT,AST,LDH,AKP,and ALB were significantly increased,and the mRNA and protein expressions of CYP2E1,CYP3A11,CYP2C19,CYP1A2,CYP2D6,and CYP2C9 were significantly reduced(all P<0.05 or P<0.01).CONCLUSION:The dose of Brucea javanica bitter alcohol is correlated with hepatotoxicity to nude mice.High doses of Brucea javanica bitter alcohol have hepatotoxicity to nude mice,which may be re-lated to reducing serum levels of ALT,AST,LDH,AKP,and ALB,inhibiting the expression of multiple subtypes of enzymes in the key enzyme CYP450 of liver drug metabolism,and then reducing the me-tabolism of toxic substances.
3.The role of principles of surgical oncology in the development of minimally invasive surgery for head and neck cancers

The classic principles of oncological surgery were accepted based the procedures from radical surgery for breast cancer described by Halsted in 1894, which can be summarized as tumor en bolc resection, clean surgical margin, regional lymph nodes dissection, and tumor-free technique. The classic principles of oncological surgery are the cornerstone of cancer surgical treatment and have significantly promoted the development of oncological surgery. The minimally invasive surgery is the trend of contemporary oncological surgery for the characteristics of minimal invasion, better tissue identification, precise manipulation, and fast postoperative recovery. Head and neck cancers are usually related to the upper aerodigestive tract, as well as important nerves and blood vessels in head and neck. The anatomical structures of the relative organs are intricate and delicate, and their functions are crucial. Therefore, there is an urgent need for minimally invasive surgical techniques. However, the head and neck cancers which are larger or located in some special sites such as the skull base have to be removed by piecemeal resection in minimally invasive surgery. The piecemeal resection of tumor increases the risks of tumor positive surgical margins and tumor implantation metastasis. The goal of radical surgery for cancers is to completely remove the tumor and obtain a really clean surgical margin. If head and neck cancer is indication for radical surgery, appropriate operative manipulation (en bloc or piecemeal resection) is able to achieve the goal of radical surgery. Giving attention to both the advantages of the principles of oncological surgery and minimally invasive surgical techniques benefits more patients with head and neck cancers from better survival rate and minor invasive morbidities.

Artificial intelligence (AI) technology enhances the function of acupuncture clinical decision support system (CDSS) by promoting the accuracy of its diagnosis, assisting the formulation of personalized therapeutic regimen, and realizing the scientific and precise evaluation of its therapeutic effect. This paper deeply analyzes the unique advantages of AI-based acupuncture CDSS, including the intelligence and high efficiency. Besides, it points out the challenges of data security, the lack of model interpretation and the complexity of interdisciplinary cooperation in the development of acupuncture CDSS. With the continuous development and improvement of AI technology, acupuncture CDSS is expected to play a more important role in the fields of personalized medicine, telemedicine and disease prevention, and to further advance the efficiency and effect of acupuncture treatment, drive the modernization of acupuncture, and enhance its position and influence in the global healthcare system.

8.Finite element analysis of revision prostheses for tibial bone defects with different lengths of tibial stems

Objective:To evaluate the mechanical performance of customized metal prosthesis with tibia stems of varying lengths for tibial bone defects reconstruction.Methods:Morphologically matched postoperative finite element models of bone defect revision were developed, with three gradients (15 mm, 30 mm, and 45 mm) according to the degree of bone defect and were reconstructed with 3D printed tantalum metal prosthesis using three tibial stem lengths (80 mm, 120 mm, and 150 mm), respectively. Conventional static and dynamic (walking gait) loading was performed to analyze the peak tibial stress distribution and accumulated sliding distance at the prosthetic interface, and to assess the effects of the three tibial stems of different lengths on the stability of the customized tibial defect restorations and the internal tibial stress state.Results:The peak accumulated sliding distance of the dynamically loaded morphologically matched restorations ranged from 17.94 to 21.31 mm with static loading, which were 68% to 84.3% higher than those of 10.26 to 11.69 mm with static loading. The peak tibial stresses in the dynamically loaded model were greater than those in the statically loaded model, with an increase of 28%-49.2%, including 132.94-143.88 MPa in the statically loaded model and 170.41-200.14 MPa in the dynamically loaded model. The overall accumulated sliding distance of the tibia prosthetic model gradually decreased from the tibial osteotomy surface, and the accumulated peak sliding distances ranged from 10.26 to 11.69 mm for static loading, and from 17.94 to 21.31 mm for dynamic loading. The bone tissue stresses in the anterolateral and medial-posterior tibia increased gradually from top to bottom, and the maximum stress value in each section was in the posterior medial tibia (the maximum value was 200.14 MPa). The highest bone tissue stress in the lateral tibia was affected by the tibial stem length, which resulted in a different location, and it was the area most affected by stress shielding (maximum value of 170.65 MPa).Conclusion:For stability assessment of morphologically matched tantalum customized prosthesis, physiological gait dynamic loading studies are more reliable than static loading; the choice of tibial stem length depends on a combination of accumulated peak sliding distances and tibial bone stress distribution factors.
9.Effects of Point Ligation Therapy Combined with Acupoint Catgut Embedment on Facial Nerve Electromyo-graphy and Serum Neurotrophic Factors for Patients with Refractory Facial Paralysis

ObjectiveTo observe the clinical effectiveness of point ligation therapy combined with catgut embedment in acupoint for patients with refractory facial paralysis, and to explore the possible mechanisms from the perspective of neurotrophic factors. MethodsTotally 168 patients with intractable facial paralysis were randomly divided into point ligation plus catgut embedment group and electroacupuncture group, with 84 cases in each group. Both groups took methylcobalamin tablets orally, on the basis of which the point ligation plus catgut embedment group gave point ligation therapy at Quanliao (SI 18), Dicang (ST 4), Jiache (ST 6), and catgut embedment in the local acupoints on the affected side combined with the distal acupoints, and the point ligation therapy combined with catgut embedment in acupoint was performed once every 30 days for three treatments; the electroacupuncture group took the same acupoints and gave electroacupuncture, with sparse and dense waves, once every other day. Both groups were treated for 90 days. Before treatment and after 30, 60, 90 and 120 days (follow-up), patients of both groups were observed for House-Brackmann Facial Nerve Grading (HB) and modified Portmann score; facial nerve electromyography was performed before treatment and after 90 days of treatment to record the wave amplitude and latency of facial nerve motor conduction, and serum brain-derived nerve growth factor (BDNF), nerve growth factor (NGF), fibroblast growth factor (FGF2) levels were determined. The clinical effectiveness of both groups after 90 days of treatment was compared, and the adverse events occurred in the course of treatment in all patients were recorded. ResultsAt 30 days, 60 days, 90 days and follow-up visits, the HB grading of patients in both groups improved compared with that before treatment (P<0.05); the distribution of the number of patients with HB grading in the point ligation plus catgut embedment group was superior to that in the electroacupuncture group after 60 days and 90 days of treatment and at follow-up visits (P<0.05). The maximal amplitude of the facial nerve electromyography in both groups after 90 days of treatment was large, and the latency was shorter when compared with those before treatment (P<0.05), and the maximum wave amplitude of the point ligation plus catgut embedment group was higher than that of the electro-acupuncture group, and the latency period was shorter than that of the electro-acupuncture group (P<0.05). The modified Portmann scores of the both groups of patients after 30, 60 and 90 days of treatment and follow-up were higher than those before treatment, and the scores of the point ligation plus catgut embedment group were higher than those of the electroacupuncture group (P<0.05). Serum BDNF, NGF and FGF2 levels in both groups after 90 days of treatment were higher than before, and the point ligation plus catgut embedment group was significantly higher than the electroacupuncture group (P<0.05). The total clinical effective rate of the point ligation plus catgut embedment group (92.68%) was higher than that of the electroacupuncture group (81.01%, P<0.05). Adverse reactions occurred in both groups, mainly including local haematoma or subcutaneous bleeding, allergic reaction, severe pain, and needle fainting, all of which disappeared after symptomatic treatment. ConclusionThe point ligation therapy combined with catgut embedment in acupoint for patients with refractory facial paralysis are effective. The therapy improve the facial nerve function, and its mechanism of action may be related to increasing the level of serum neurotrophic factor and thus promoting facial nerve repairment.
10.Allocation of school health staff and clinics in primary and secondary schools in Guangdong Province

Objective:
To understand the allocation of health staff and clinics in primary and secondary schools in Guangdong Province, so as to provide a reference for school health construction.
Methods:
In December 2020, a total of 17 205 primary and secondary schools in 21 cities in Guangdong Province were investigated by combining document verification and on site surveys. The allocation of health staff and clinics in different cities and types of schools was analyzed using SPSS 25.0 software.
Results:
The proportion of schools in Guangdong Province that met the national standard for school health staff ratio was 37.7%. Among different types of schools, the lowest ratio was found in nine year schools (33.1%), while the highest was in twelve year schools (61.3%). Among the 17 205 schools in the province, 12.6% had clinics with an area of >40 m 2, and 6.9% with occupational licenses for medical institutions. The proportion of full time health staff was 39.1%. Among all school health staffs, the proportions of permanent staff, temporary staff, and appointed staff were 47.5%, 29.0% and 23.6%, respectively. The rate of school health staff with professional qualifications was 44.9%, while 48.3% had graduated from medical related majors, and 20.9% held a bachelor s degree or higher as their first degree. There were differences among various types of schools, with primary schools having the lowest health staff configuration and professional level (24.0% and 35.7%, respectively).
Conclusions
There are still issues of insufficient staff and low professional level in health staff and establishment of clinics in primary and secondary schools in Guangdong Province, and the allocation is uneven across different types of schools. The allocation of clinics in the province is still in urgent need of improvement.
