1.Ligustrazine relieves spinal cord inflammation in rats with lumbar disc herniation by regulating PI3K/Akt/NF-κB signaling pathway

Objective:To investigate the effects of tetramethylpyrazine(TMP)on spinal cord inflammation and phosphatidylinositol 3-kinase/serine-threonine kinase/nuclear factor-κB(PI3K/Akt/NF-κB)signaling pathway in rats with lumbar disc herniation(LDH).Methods:The rats were divided into Control group,lumbar disc herniation group(LDH group),TMP low-dose,high-dose group(TMP-L,TMP-H group),TMP high-dose+PI3K inhibitor group(TMP-H+LY294002 group)by random number table method.LDH rat model was established by autologous nucleus pulposus transplantation and treated with corresponding drugs.After administration,the pain threshold of rats in each group was measured by paw tactile tester and plantar thermal stimulation tester.Hematoxylin-eosin(HE)staining was used to observe the histomorphological changes of dorsal root ganglion.Enzyme linked immunosorbent assay(ELISA)was used to measure the levels of tumor necrosis factor α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)in the spinal dorsal horn.Immunohistochemistry was used to detect the expression of ionized calcium binding adapter molecule 1(Iba-1)and c-Fos in the spinal dorsal horn.Western blot was used to detect the expression of PI3K/Akt/NF-κB signaling pathway related proteins,and the ratios of p-PI3K/PI3K,p-Akt/Akt and p-NF-κB p65/NF-κB p65 were calculated.Results:At 24h before intervention,the paw withdrawal mechanical threshold(PWMT)and paw withdrawal thermal latency(PWTL)in the LDH group were lower than those in the Control group(P<0.05).At 24h after intervention,compared with Control group,the LDH group had severe nucleus deviation,blurred nucleoli,large amount of inflammatory cell infiltration and obvious cell swelling,decreased PWMT,PWTL,p-PI3K/PI3K and p-Akt/Akt,and increased levels of TNF-α,IL-1β,IL-6 and Iba-1,c-Fos and p-NF-κB p65/NF-κB p65(P<0.05).Com-pared with the LDH group,the neurons had less nuclear deviation,clearer nucleoli,less cell swelling and inflammatory cell infiltration in the TMP-L group and TMP-H group,and the PWMT,PWTL,p-PI3K/PI3K and p-Akt/Akt were in-creased in turn,while the levels of TNF-α,IL-1β,IL-6 and Iba-1,c-Fos,p-NF-κB p65/NF-κB p65 were decreased in turn(P<0.05).Compared with the TMP-H group,the TMP-H+LY294002 group had more severe pathological dam-age of neurons,obvious nuclear deviation,blurred nucleoli,cell swelling and inflammatory cell infiltration,decreased PWMT,PWTL,p-PI3K/PI3K and p-Akt/Akt,and increased levels of TNF-α,IL-1β,IL-6 and Iba-1,c-Fos and p-NF-κB p65/NF-κB p65(P<0.05).Conclusion:TMP can inhibit the activation of microglia and the expression of Iba-1 and c-Fos,and improve spinal cord inflammation in LDH rats,and its mechanism may be related to the regulation of PI3K/Akt/NF-κB signaling pathway.
2.A retrospective analysis of 107 cases of hematologic disease complicated with candidemia

Objective To investigate the clinical features,etiological features and prognosis of patients with hematologic diseases complicated with candidemia for improving clinical diagnosis and treatment.Methods A retrospective analysis was conducted for 107 hematological patients complicated with candidemia who were treated in the First Affiliated Hospital of Zhengzhou University,Henan Cancer Hospital,Henan Provincial People's Hospital,or Zhengzhou People's Hospital from June 2022 to May 2024.The clinical data and pathogenic bacteria were analyzed by univariate and multivariate analyses.Results The Candida pathogen of the 107 cases of candidemia were mostly Candida tropicalis(73.8%),followed by Candida parapsilosis,Candida glabrata,and Candida albicans.Antifungal susceptibility testing showed that 43.9%,47.7%,and 48.6%of the Candida strains were resistant voriconazole,fluconazole and itraconazole,respectively.Logistic regression analysis found that disease not in remission(OR=7.795,95%CI:2.274-26.723),septic shock(OR=10.376,95%CI:1.129-95.388),multiple organ dysfunction syndrome(MODS)(OR=9.107,95%CI:1.789-46.361),and inappropriate antifungal treatment(OR=3.422,95%CI:1.153-10.153)were risk factors for 30-day mortality in hematological patients with candidemia.Conclusions Candidemia in patients with hematological diseases is associated with high mortality rate,the major pathogen of which is Candida tropicalis.The Candida isolates showed high resistance rates to azoles.Disease not in remission,septic shock,MODS,and inappropriate antifungal treatment are the risk factors for mortality.

This consensus was developed by the Orthodontic Society of the Chinese Stomatological Association to provide a systematic, scientific, and practical guideline for informed consent in orthodontic care. Orthodontic treatment is typically lengthy, highly individualized, and involves multiple factors such as growth and development, occlusal function, and facial esthetics. Rapid technological advances and diverse risk profiles make the traditional reliance on orthodontist experience or institutional templates insufficient to ensure patients′ full understanding and autonomous decision-making. To address this, the expert panel conducted extensive reviews of domestic and international guidelines, analyzed representative dispute cases, and performed multicenter patient-clinician surveys. Using a multi-round Delphi method, the group established a standardized informed consent framework covering the initial consultation, treatment, and retention phases. The consensus emphasizes that informed consent is not only a fundamental legal and ethical requirement but also a key step in building trust, improving patient compliance, and enhancing treatment satisfaction. Orthodontists should clearly and comprehensively explain treatment plans, potential risks, uncertainties, and associated costs, while respecting the autonomy of patients or guardians, and maintain continuous communication and dynamic evaluation throughout the treatment process. 7.Improvement effect and mechanism of triptolide on sciatica rats by regulating cGAS/STING signaling pathway

OBJECTIVE To investigate the improvement effect and mechanism of triptolide (TP) on sciatica rats. METHODS Sciatica rat model was prepared and then randomly divided into model group (normal saline), indomethacin group (positive control, 7.5 mg/kg), TP low-dose and high-dose groups (TP-L group and TP-H group, 50, 100 μg/kg TP), and high-dose TP+ stimulator of interferon gene (STING) activator group (TP-H+DMXAA group, 100 μg/kg TP+25 mg/kg DMXAA), with 12 rats in each group. Another 12 unligated rats were selected as sham operation group (normal saline). After 14 days of intraperitoneal administration, the paw mechanical withdrawal threshold (PWT) and paw withdrawal thermal latency (PWL) were detected; the pathological changes, morphology of sciatic nerve and the number of microglia in sciatic nerve were observed. The levels of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α), mRNA and protein expression levels of cyclic guanosine monophosphate- adenosine monophosphate synthase (cGAS) and STING in sciatic nerve were detected. RESULTS Compared with sham operation group, PWT and PWL of rats in model group were obviously reduced and shortened, the number of Nissl bodies was obviously decreased, while the number of microglia, sciatic neuropathology score, the levels of IL-1β and TNF-α, mRNA and protein expressions of cGAS and STING were obviously increased (P<0.05), and sciatic nerve injury was serious. Compared with model group, the changes of various indexes in indomethacin group, TP-L group and TP-H group were opposite to the above (P<0.05), and sciatic nerve injury was reduced. STING activator DMXAA weakened the inhibitory effect of TP on the activity of microglia and inflammatory response in sciatica rats (P<0.05). CONCLUSIONS TP may reduce the activity of microglia and inflammatory response by down-regulating the cGAS/STING signaling pathway, thus alleviating sciatica in rats.
8.Construction of prognostic model for endometrial carcinoma based on bioinformatics

Objective Differential genes related to prognosis of endometrial carcinoma(EC)were screened and prognostic models were constructed.Methods Gene Expression data of EC and normal control samples were downloaded from The Cancer Genome Atlas(TCGA)database and Gene Expression Omnibus(GEO)dataset GSE63678 to screen out common differential genes.LASSO regression analysis was used to screen out the genes with prognostic significance and construct prognostic characteristics.EC patients with complete information were obtained from the TCGA database and randomly divided into the training group and the validation group in a ratio of 1:1.In the training group,survival curves were constructed based on prognostic characteristics.Functional annotation and pathway enrichment analysis were conducted using gene ontology(GO)analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)analysis.Combined with prognostic features and clinical risk factors,a calibration curve and C-index were used to evaluate the performance of the histogram.Finally,use a verification group for validation.Results A total of 4800 and 257 differentially expressed genes were screened from TCGA and GEO databases respectively,of which 73 up-regulated genes and 52 down-regulated genes were co-expressed.6 prognostic genes(ORMDL2,BNC2,TTK,MAMLD1,KCTD7 and DCLK2)were screened out by LASSO regression analysis.The survival curve showed that the overall survival of patients in the high-risk group was significantly lower than that in the low-risk group(P<0.01).GO analysis and KEGG results exhibited that prognostic signature was associated with cell cycle.The nomogram showed powerful predictive ability in the training and validation groups.Conclusion We constructed a predictive model based on prognostic genes,which can accurately predict the prognosis of patients with EC and provide new theoretical support for clinical diagnosis and treatment.
9.Application of various genetic techniques for the diagnosis of Prader-Willi syndrome.

OBJECTIVE:
To discuss the advantages and technical limitations of various molecular genetic techniques in the diagnosis of two infants featuring all-round developmental retardation.
METHODS:
The two patients were initially screened by using chromosomal microarray analysis (CMA). For patient 1, his parents were also subjected to CMA analysis, and the data was analyzed by using ChAS and UPD-tool software. For patient 2, methylation-specific PCR (MS-PCR) was carried out.
RESULTS:
Patient 1 was diagnosed with maternal uniparental disomy (UPD) type Prader-Willi syndrome (PWS) by CMA and UPD-tool family analysis. His chromosomes 15 were of maternal UPD with homology/heterology. Patient 2 was diagnosed with deletion type PWS by combined CMA and MS-PCR.
CONCLUSION
Correct selection of laboratory methods based on the advantages and limitations of various molecular techniques can help with diagnosis of genomic imprinting disorders and enable better treatment and prognosis through early intervention.
10.Influence of alprostadil on pancreatic blood flow and gastrointestinal function related indicators of patients with severe acute pancreatitis

ABSTRAT:Objective To observe the influence of alprostadil on the pancreatic blood flow and gastrointestinal function related indicators of patients with severe acute pancreatitis.Methods A total of 72 patients with severe acute pancreatitis in our hospital were randomly divided into two groups.The control group was treated with conventional treatment and the observation group were treated with alprostadil on the basis of control group,the pancreatic blood flow and gastrointestinal function related indicators of two groups before and after the treatment were detected and compared.Results The pancreatic blood flow and gastrointestinal function related indicators of two groups before treatment showed no significant difference (P > 0.05),while those indicators of two groups after treatment were all significantly improved,and the observation group was significantly better than the control group (P < 0.05).Conclusion The alprostadil can obviously improve the pancreatic blood flow and gastrointestinal function related indicators of patients with severe acute pancreatitis.
