1.Microneedle delivery platform integrated with Staphylococcus epidermidis-derived extracellular vesicles-based nanoantibiotics for efficient bacterial infection atopic dermatitis treatment.

Due to the difficulty of overcoming the abnormal epidermal barriers and addressing S. aureus infections without disrupting indigenous skin microbiota, effective treatment of bacterial infection atopic dermatitis (AD) remains a significant clinical challenge. Skin microbiota-derived extracellular vesicles (EVs) shows protentional for skin disease treatment, but the lack of antimicrobial activity and limited skin penetration hamper their application in bacterial infection AD treatment. Here, we developed novel nanoantibiotics by loading Lev into S. epidermidis-derived EVs (Lev@SE-EVs), with supreme antimicrobial activity, regulating epidermal immune responses and enhanced epidermal barrier functionality. The nanoantibiotics were further integrated into hyaluronic acid-based microneedle (MN) for efficient transdermal delivery of therapeutic agents and effectively treating bacterial infection in AD. Upon insertion into the skin, the rapidly released Lev@SE-EVs from MN are uptake by S. aureus in a selective manner, fibroblasts, and surrounding immune cells to exert therapeutic effects in the infected dermal layer, resulting in mitigated skin inflammation, reduced S. aureus burden and increased dermis repair. Notably, Lev@SE-EVs induce IL-17A+ CD8+ T-cell accumulation in the skin in an unrelated inflammation manner, which may represent heterologous protection. This EVs-integrated MN assisted Lev@SE-EVs to alleviate skin inflammation, repair skin, and provide an effective and safe therapeutic approach for bacterial infection AD treatment.
2.Molecular epidemiological characteristics of GII.3P12 norovirus outbreaks in China, 2022-2023

Objective:To understand the molecular evolutionary characteristics of the recombinant strain GII.3[P12] of norovirus acute gastroenteritis outbreaks and aggregated outbreaks in China from 2022 to 2023.Methods:Epidemiological information, case information, clinical samples, as well as detection and genotyping information of norovirus outbreaks and aggregated outbreaks from 2022 to 2023 were collected; positive samples of the GII.3[P12] recombinant strain were subjected to nucleic acid extraction, whole-genome amplification, and sequence analysis; and homology simulation method were used to construct a three-dimensional structure and predict antigenic epitopes.Results:From January 2022 to December 2023, a total of 1 136 norovirus outbreaks and aggregated outbreaks were reported in China′s norovirus outbreak surveillance network, and genotyping result were successfully obtained for 942 outbreaks, with GII dominating, accounting for 76.0% (716/942), and the proportion of GI and mixed genotypes being 15.8% (149/942) and 8.2% (77/942). Norovirus outbreaks caused by GII were dominated by GII.3[P12] (22.5%, 161/716), while other major genotypes included GII.17[P17] (18.7%, 134/716), GII.4_Sydney 2012[P16] (11.6%, 83/716) and GII.6[P7] ( 11.3%, 81/716). 2022-2023 Outbreaks caused by GII.3[P12] were concentrated in February-March (54.0%, 87/161), with the main outbreaks occurring in nursery and primary schools (87.5%), the mode of transmission was mainly human-to-human (68.9%), and the main susceptible population was children aged 3-7 years (93.3%). In this study, the genome sequences of 25 GII.3[P12] recombinant strains were obtained, and according to the phylogenetic analysis, it was shown that the GII.3[P12] recombinant strains in China in 2022-2023 belonged to the Cluster IV cluster of sublineage b (7 strains) and sublineage c (18 strains). A total of 11 linear and 8 conformational epitopes were predicted by epitope prediction analysis, and the predicted linear and conformational epitopes had overlapping positions, and each conformational epitope was part of the predicted linear epitope with conserved potential antigen-binding and receptor-binding sites.Conclusions:The recombinant strain GII.3[P12] is one of the epidemic strains that will cause outbreaks and clusters of norovirus in China in 2022-2023, and its genome did not undergo significant mutation.
3.Therapeutic effect of fecal microbiota transplantation for patients with refractory gout

Objective To explore the effects of fecal microbiota transplantation on blood uric acid(UA)metabolism,adverse reactions,and gout control in patients with refractory gout.Methods A total of 102 patients with refractory gout in the First Affiliated Hospital of Medical College of Shantou University from June 2020 to June 2023 were randomly divided into two groups,with 51 cases in each group.Control group received benzbromarone and febuxostat treatment,while the observation group re-ceived fecal microbiota transplantation.The gout control status and occurrence of adverse reactions were observed in both groups;intestinal flora,UA,levels of C-reactive protein(CRP),interleukin-18(IL-18),interleukin-6(IL-6),and scores of joint pain,swelling and limited mobility were compared between the two groups before and after treatment.Results Five patients in each group were lost to follow-up.The total gout control rate in the observation group was 89.13%,which was significantly higher than 69.57% in the control group(P<0.05).At 4 and 12 weeks after treatment,the levels of Escherichia coli,UA,CRP,IL-18 and IL-6 as well as the scores of joint swelling,pain and limited mobility in the observation group were significantly lower than those in the control group,while the lev-els of Bifidobacterium,Lactobacillus and Bifidobacterium to Escherichia coli(B/E)ratio were signifi-cantly higher than those in the control group(P<0.01).The total incidence of adverse reactions was 10.87% in the observation group and 6.52% in the control group,with no significant between-group difference(P>0.05).Conclusion Fecal microbiota transplantation has a definite therapeutic effect for patients with refractory gout,which can significantly reduce UA level and control clinical symptoms,and its mechanism may be related to the correction of intestinal flora imbalance.
4.Latent profiles analysis and influencing factors of Health-Promoting Lifestyle in patients with conservative treatment of Lumbar Disc Herniation

Objective To explore the latent profile of health-promoting lifestyle in patients with lumbar disc herniation treated conservatively and analyze its influencing factors,so as to provide reference for the develop-ment of targeted interventions.Methods Patients with conservative treatment of Lumbar Disc Herniation at the orthopedic outpatient clinic of a tertiary hospital in Guangzhou from March to December 2024 were selected as study subjects.General Information Questionnaire,Health-Promoting Lifestyle Profile Revised-Ⅱ,Visual Analogue Scale,Oswestry Disability Index and Social Support Rating Scale were used for the investigation.Latent profile analysis was used to explore the potential categories of health-promoting lifestyle in patients with lumbar disc her-niation and the influencing factors were identified by univariate analysis and disordered multiple logistic regression analysis.Results A total of 422 lumbar disc herniation patients treated conservatively were included and the score of Health-Promoting Lifestyle Profile Revised-Ⅱ Scale was(102.44±13.19)points.The health-promoting lifestyle of patients with lumbar disc herniation can be divided into three potential profile:low health promotion group-low responsibility and less exercise type(18.7%),balanced health promotion group(51.7%)and high health promotion group-spiritual social leading type(29.6%).The results of logistic regression analysis showed that course of disease,oswestry disability index,social support level and take regular physical exercise are the influenc-ing factors of health-promoting lifestyle in patients with conservative treatment of lumbar disc herniation(P<0.05).Conclusions There is heterogeneity in the health-promoting lifestyle in patients with conservative treat-ment of lumbar disc herniation.Nursing staff can carry out targeted interventions according to the latent profile type of health-promoting lifestyle in lumbar disc herniation patients treated conservatively to improve their health-promoting lifestyle.

Age-related hearing loss(ARHL)is a common chronic disease that poses a serious threat to the physical and mental health of the elderly in an aging society.It is a sensorineural hearing loss characterized by the loss of auditory hair cells,stria vascularis lesions,apoptosis of spiral ganglia,and degeneration of the audi-tory central nervous system,reducing the quality of life of the patients.This article reviews the research progress in the relationship of ARHL with Alzheimer's disease,depression,and frailty,as well as the daily intervention in ARHL.This review aims to improve people's awareness and attention to the health hazards of ARHL and to delay the occurrence and development of ARHL by implementing daily intervention measures to form a healthy lifestyle.
8.Effect of Dendrobii Caulis Mixture on Cell Inflammatory Response and Apoptosis in Diabetic Rats with Nonalcoholic Fatty Liver Disease Based on NF-κB/NLRP3/IL-1β Signaling Pathway

ObjectiveTo investigate the effect of Dendrobii Caulis mixture on cell inflammatory response and apoptosis in diabetic rat with non-alcoholic fatty liver disease(NAFLD) and its possible mechanism. MethodForty male SD rats were randomly divided into blank group and model group of type 2 diabetes mellitus(T2DM) with NAFLD according to body weight. The model was established by high-sugar and high-fat diet combined with intraperitoneal injection of streptozotocin(STZ), which was randomly divided into the model group, Dendrobii Caulis mixture group(16.67 g·kg-1·d-1) and metformin group(100 mg·kg-1·d-1) according to blood glucose and body weight, and 10 rats in each group. Rats in each group were administered by continuous gavage for 4 weeks, the blank and model groups were given saline by gavage at 10 mL·kg-1·d-1. Fasting blood glucose(FBG), serum insulin(INS), glycosylated serum protein(GSP), triglyceride(TG), total cholesterol(TC), high density lipoprotein(HDL-C), low density lipoprotein(LDL-C), alanine aminotransferase(ALT) and aspartate aminotransferase(AST) were detected in each group of rats. The liver tissues of rats in each group were stained with hematoxylin-eosin(HE) to observe the pathological changes, and the positive expressions of nuclear transcription factor(NF)-κB, NOD-like receptor heat protein structural domain-related protein 3(NLRP3), interleukin(IL)-1β and tumor necrosis factor(TNF)-α were observed by immunohistochemistry. Western blot and fluorescence quantitative polymerase chain reaction(Real-time PCR) were used to detect the protein and mRNA expression of B-cell lymphoma-2(Bcl-2), Bcl-2 associated X protein(Bax), Caspase-3 and NF-κB p65, NLRP3, IL-1β, TNF-α in liver tissue of rats in each group. ResultCompared with the blank group, FBG, GSP, TC, TG, LDL-C, AST and ALT levels were increased, INS and HDL-C levels were decreased, Bax, Caspase-3, NLRP3, IL-1β, TNF-α protein and mRNA expression were increased in model group, the ratio of p-NF-κB/NF-κB protein increased, the expression of Bcl-2 protein and mRNA decreased, and the positive immunohistochemical expression of NF-κB, NLRP3, IL-1β and TNF-α increased, and the differences were statistically significant(P<0.01). The morphological structure of the liver was disrupted, and obvious fat vacuoles were seen. Compared with the model group, FBG, GSP, TC, TG, LDL-C, AST and ALT levels of Dendrobii Caulis mixture group and metformin group were decreased, INS and HDL-C levels were increased, and protein and mRNA expressions of Bax, Caspase-3, NLRP3, IL-1β and TNF-α were decreased, the protein ratio of p-NF-κB/NF-κB decreased, the expression of Bcl-2 protein and mRNA increased, and the positive immunohistochemical expressions of NF-κB, NLRP3, IL-1β and TNF-α decreased, and the differences were statistically significant(P<0.01). The liver morphology and structure were relatively complete, and the fat vacuoles were reduced. ConclusionDendrobii Caulis mixture can inhibit cell apoptosis, reduce inflammatory response and alleviate liver injury in rats with T2DM and NAFLD, the mechanism may be related to inhibiting the activation of NF-κB pathway, blocking the activation of NLRP3 inflammatory vesicles, reducing IL-1β secretion, attenuating Caspase-3 activity and reducing the ratio of Bcl-2/Bax.
9.Safety and efficacy of ciprofol and propofol for gynecologic surgery with general anesthesia: a meta-analysis

Objective:To evaluate the safety and efficacy of ciprofol and propofol for gynecological surgeries with general anesthesia through a meta-analysis.Methods:Electronic databases including PubMed, Embase, Cochrane, Web of Science, China National Knowledge Infrastructure, Wanfang Data, China Biomedical Literature Database, and China Science and Technology Journal Database were searched for randomized controlled trials comparing the safety and efficacy of ciprofol and propofol in gynecological surgeries with general anesthesia from inception to May 2023. Meta-analysis was performed using Revman 5.4 software.Results:Six randomized controlled trials were included, involving 741 patients, of which 371 received ciprofol and 370 received propofol. Compared with propofol group, the emergence time was significantly prolonged, the difference in mean arterial blood pressure, systolic blood pressure and diastolic blood pressure before and after anesthesia induction was decreased, and the incidence of injection pain, respiratory depression, body movement and hypotension was decreased in ciprofol group ( P<0.05). There were no significant differences between the two groups in terms of time of successful anesthesia induction, difference in BIS values and heart rate before and after anesthesia induction, and incidence of tachycardia, bradycardia and hypertension ( P>0.05). Conclusions:Ciprofol is comparable to propofol in terms of efficacy and has better safety than propofol when used in gynecologic surgeries with general anesthesia.
10.Clinical characteristics of elderly-onset gouty arthritis and risk factors for tophi

Objective:To analyze the clinical characteristics of elderly-onset gouty arthritis and risk factors of tophi.Methods:A total of 1 239 gout patients were retrospective selected in the outpatient department of the Gout Clinical Medical Center of the Affiliated Hospital of Qingdao University from 2016 to 2022. According to age of onset, they were divided into the young and middle-aged group(aged<60) consisted of 826 cases, and the elderly group(aged≥60) consisted of 413 cases. Compare the clinical characteristics of elderly with Young and Middle-aged patients.Results:The systolic blood pressure, fasting blood glucose, creatinine, regular exercise, comorbidities, and tophi in the elderly group was higher than that in the middle-aged and young group. The proportion of diastolic blood pressure, serum triglycerides, eGFR, serum uric acid, alcohol consumption rate, and family history of gout was lower than that of young and middle-aged group( P<0.05); In the elderly-onset group, the initial site of arthritis was commonly observed in the first metatarsophalangeal joint. The proportion of the first attack with the upper limb joint was higher in old age group than in young and middle age group( P<0.05). Renal underexcretion type was the main subtype in the elderly group, and the proportion of overproduction type was higher than that of the young and middle-aged group( P<0.05). The logistic regression analysis showed that age, urea nitrogen, disease duration≥10 years and family history of gout were risk factors for tophi in elderly patients( P<0.05). Conclusion:The elderly-onset gout has unique clinical characteristics, characterized by a higher prevalence of tophi, a higher rate of complications. An initial site of arthritis commonly observed in the first metatarsophalangeal joint and the predominant type of uric acid excretion is renal excretion impairment. Early diagnosis and treatment, control of blood uric acid levels, smoking cessation and alcohol, regular exercise should be applied to prevent or delay the formation of tophi.
