1.Neoadjuvant therapy for hepatocellular carcinoma:recent advances

Hepatocellular carcinoma(HCC)is the fourth leading cause of cancer death in the world,and its prognosis is poor.Most HCC patients are diagnosed at an advance stage and are not candidates for surgery.Even if the early-stage tumor is resected,HCC patients tend to relapse within 5 years.In the era of targeted therapy and immunotherapy,neoadjuvant therapy has gained increasing attention and recommendation.For HCC patients with high recurrence risk,neoadjuvant therapy refers to preoperative local or systemic therapy,which can reduce tumor burden,remove tiny lesions,widen surgical margin,and reduce the recurrence risk.However,if the neoadjuvant therapy is not optimal,the timing of surgery will be delayed,resulting in disease progression or even loss of the chance for surgery.This article reviews the research progress of neoadjuvant therapy for HCC.
2.Clinical Value of AI-Assisted Transvaginal Color Ultrasound Combined with Serum Markers in Diagnosing Heterotopic Pregnancy after IVF-ET

Objective To investigate the early diagnostic value of AI-assisted transvaginal ultrasound combined with serum biomarkers for intrauterine and ectopic combined pregnancy(heterotopic pregnancy,HP)after IVF-ET.Methods This retrospective case-control study included 86 patients diagnosed with HP after IVF-ET at Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine between January 2022 and December 2024.Control groups consisted of 86 patients with normal intrauterine pregnancies and 86 patients with simple ectopic pregnancy.All patients were diagnosed through clinical diagnosis,imaging examinations,and surgical confirmation.An AI ultrasound image analysis model and serum markers(hCG,PAPP-A,APA,progesterone,PlGF)were used to jointly assess HP diagnostic efficiency.Diagnostic performance was compared by calculating sensitivity,specificity,positive predictive value,and negative predictive value.Results The AI ultrasound image analysis model's AUC was 0.835,significantly superior to traditional ultrasound(AUC=0.696).Serum hCG and PAPP-A levels were(36,210±9,820)IU/L and(0.81±0.25)μg/mL,respectively.The joint diagnostic AUC was 0.909,with a sensitivity of 94.2%,specificity of 88.4%,positive predictive value of 91.6%,and negative predictive value of 92.8%.Conclusion AI-assisted ultrasound combined with serological markers significantly improves the early diagnostic accuracy of HP,outperforming single methods and providing a reliable clinical tool.
3.Therapeutic potential of exosome derived from remote ischemic conditioning in alleviating neurologic dysfunction after cardiac arrest in a rat model

Objective:To investigate the therapeutic effect of exosomes derived from remote ischemic conditioning on neurological dysfunction after cardiopulmonary resuscitation in a rat model of cardiac arrest and the relationship with glycocalyx protection.Methods:Exosomes were isolated from the blood of healthy adult male Sprague-Dawley rats using ultracentrifugation after undergoing remote ischemic conditioning for use as intervention drugs. Nanoparticle tracking analysis technology was used for exosome detection. Thirty-six adult male Sprague-Dawley rats were randomly assigned to 3 groups ( n=12 each) :Sham group, Control group and Exosome group. Cardiac arrest was induced by asphyxia for 7 min in the Control and Exosome groups. Placebo or exosomes (1×10 10 Particles) were infused intravenously at 5 min after the rats had returned of spontaneous circulation. Neuropsychological deficit score (NDS), open field test, Y maze and Morris water maze were used to assess neurological outcomes. The levels of plasma Hyaluronic acid (HA) and syndecan-1 (Sdc-1) were detected by Elisa. The expression levels of matrix metalloproteinase-2/9 (MMP-2/9) in hippocampal CA1 region were detected by Western blot. Results:After undergoing remote ischemic conditioning, the plasma levels of exosomes were elevated in rats compared to normal rats. Compared with the control group, the behavioral experiment of rats in the exosomes group were significantly improved, as evidenced by an increase in horizontal locomotor distance (5.86±2.89 vs. 17.53±5.51, P< 0.05), an increase in the correct rate of spontaneous alternation (13.29±15.07 vs. 42.63±10.25, P< 0.05), and a shortening of avoidance latency (25.83±8.54 vs. 13.49±4.55, P< 0.05). Plasma HA and Sdc-1 levels were significantly lower 24 h after resuscitation (HA: 26.34±9.83 vs. 14.84±6.26, P< 0.05; Sdc-1: 0.05±0.03 vs. 0.02±0.02, P<0.05), along with significantly lower MMP-2/9 levels in hippocampal tissue. Conclusions:Exosomes extracted from the plasma of rats undergoing remote ischemic conditioning can improve neurological dysfunction after cardiac arrest in rats, and the mechanism may be related to the inhibition of metalloproteinases and the reduction of endothelial glycocalyx degradation.
4.Efficacy of Lenvatinib Combined with Anti–PD-1 Antibodies Plus Transcatheter Arterial Chemoembolization for Hepatocellular Carcinoma with Portal Vein Tumor Thrombus: A Retrospective, Multicenter Study

Purpose:
The prognosis of patients with hepatocellular carcinoma (HCC) and portal vein tumor thrombus (PVTT) is extremely poor, and systemic therapy is currently the mainstream treatment. This study aimed to assess the efficacy and safety of lenvatinib combined with anti–programmed cell death-1 antibodies and transcatheter arterial chemoembolization (triple therapy) in patients with HCC and PVTT.
Materials and Methods:
This retrospective multicenter study included patients with HCC and PVTT who received triple therapy, were aged between 18 and 75 years, classified as Child-Pugh class A or B, and had at least one measurable lesion. The overall survival (OS), progression-free survival (PFS), objective response rates, and disease control rates were analyzed to assess efficacy. Treatment-related adverse events were analyzed to assess safety profiles.
Results:
During a median follow-up of 11.23 months (range, 3.07 to 34.37 months), the median OS was greater than 24 months, and median PFS was 12.53 months. The 2-year OS rate was 54.9%. The objective response rate and disease control rate were 69.8% (74/106) and 84.0% (89/106), respectively; 20.8% (22/106) of the patients experienced grade 3/4 treatment-related adverse events and no treatment-related deaths occurred. The conversion rate to liver resection was 31.1% (33/106), with manageable postoperative complications. The median OS was not reached in the surgery group, but was 19.08 months in the non-surgery group. The median PFS in the surgery and non-surgery groups were 20.50 and 9.00 months, respectively.
Conclusion
Triple therapy showed promising survival benefits and high response rates in patients with HCC and PVTT, with manageable adverse effects.
5.A novel chemotherapy strategy for advanced hepatocellular carcinoma: a multicenter retrospective study.

BACKGROUND:
Chemotherapy is a common treatment for advanced hepatocellular carcinoma, but the effect is not satisfactory. The study aimed to retrospectively evaluate the effects of adding all-trans-retinoic acid (ATRA) to infusional fluorouracil, leucovorin, and oxaliplatin (FOLFOX4) for advanced hepatocellular carcinoma (HCC).
METHODS:
We extracted the data of patients with advanced HCC who underwent systemic chemotherapy using FOLFOX4 or ATRA plus FOLFOX4 at the Eastern Hepatobiliary Surgery Hospital, First Hospital of Jilin University, and Zhejiang Sian International Hospital and retrospectively compared for overall survival. The Cox proportional hazards model was used to calculate the hazard ratios for overall survival and disease progression after controlling for age, sex, and disease stage.
RESULTS:
From July 2013 to July 2018, 111 patients with HCC were included in this study. The median survival duration was 14.8 months in the ATRA plus FOLFOX4 group and 8.2 months in the FOLFOX4 only group ( P < 0.001). The ATRA plus FOLFOX4 group had a significantly longer median time to progression compared with the FOLFOX4 group (3.6 months vs. 1.8 months, P < 0.001). Hazard ratios for overall survival and disease progression were 0.465 (95% confidence interval: 0.298-0.726; P = 0.001) and 0.474 (0.314-0.717; P < 0.001) after adjusting for potential confounders, respectively.
CONCLUSION
ATRA plus FOLFOX4 significantly improves the overall survival and time to disease progression in patients with advanced HCC.

6.Acceptance of advanced breast cancer patients to the advance care planning and its influencing factors

Objective:To explore the acceptance of advanced breast cancer patients to the advance care planning, and analyze its potential influencing factors.Methods:From October 2019 to January 2021, convenience sampling was used to select 75 advanced breast cancer patients in the Oncology Department of the Second Affiliated Hospital of Xi'an Jiaotong University as the research object. The patients were investigated with the General Information Questionnaire, Patient's Advance Care Planning Acceptance Questionnaire (ACPQ), Eysenck Personality Questionnaire-Revised Short Scale for Chinese (EPQ-RSC) and the Social Support Rating Scale (SSRS). Univariate analysis, Pearson correlation analysis and multiple linear regression analysis were used for data analysis.Results:A total of 75 questionnaires were distributed and 72 valid questionnaires were returned. The total ACPQ score of advanced breast cancer patients was (70.60±4.46). The results of univariate analysis showed that there were statistically significant differences in the ACPQ scores of patients with different education levels, residence, marital status, bereavement experience, hospitalization experience, and knowledge of the time of disease diagnosis ( P<0.05). Multiple linear regression analysis showed that the social support was an influencing factor of patients' acceptance of the advance care planning, and the difference was statistically significant ( P<0.01) . Conclusions:The acceptance of advanced breast cancer patients to the advance care planning is mainly related to the patient's social support. Medical and nursing staff should strengthen the understanding and utilization of patients' social support, and promote patients' participation in advance care planning.
7.Correlation Analysis of ADPRT rs1136410 Polymorphism with the Occurrence of Non-small Cell Lung Cancer in Han Nationa- lity from Northern Jiangsu

OBJECTIVE: To investigate the correlation of ADPRT rs1136410 polymorphism with the occurrence of non-small cell lung cancer (NSCLC) in Han nationality from northern Jiangsu. METHODS: A total of 283 patients with primary NSCLC of Han nationality in Northern Jiangsu were selected from the Affiliated Hospital of Xuzhou Medical University during Nov. 2015-Dec. 2018 as NSCLC group. A total of 210 healthy subjects underwent physical examination were included in control group. PCR-RFLP was utilized to determine the genotypes at ADPRT rs1136410 locus. Logistic regression model was used to evaluate the effect of polymorphism and its interaction with smoking on the occurrence of NSCLC. RESULTS: There was no statistical significance in age and gender between 2 groups (P>0.05). The proportion of smoker in NSCLC group was significantly higher than control group (P<0.05). TT, TC and CC genotypes were detected at rs1136410 locus of ADPRT gene. The frequency of TT, TC and CC genotype were 41.9%,44.8% and 13.3%, and those of allele T and C were 64.3% and 35.7% in control group. The frequency of TT, TC and CC genotype were 21.6%, 50.2% and 28.2%, and those of allele T and C were 46.6% and 53.4% in NSCLC group, respectively. The frequencies of genotypes in 2 groups were in accordance with Hardy-Weinberg equilibrium (P>0.05), while there was significant difference in genotype and allele frequencies between 2 groups (P<0.05). Compared with TT genotype, the risk of NSCLC in individuals carrying TC and CC genotypes raised by 1.179, 3.122 folds [ORTC=2.179, 95%CI (1.435, 3.309), P<0.05; ORCC=4.122,95%CI(2.401,7.075),P<0.05]. Compared with individuals carrying TT genotype, the risk of NSCLC occurrence in non-smokers carrying TC and CC genotypes increased by 0.371, 1.328 fold [ORTC=1.371,95%CI (0.927,3.428),P<0.05; ORCC=2.328,95%CI (1.249,4.622),P<0.05]; and the risk of NSCLC occurrence in smokers carrying TC and CC genotypes increased by 0.928, 2.182 folds [ORTC=1.928,95%CI (1.257,2.957), P<0.05;ORCC=3.182,95%CI (1.760,5.754), P<0.05]. CONCLUSIONS: The rs1136410 locus mutant genotype of ADPRT gene is the risk factor of NSCLC in Han nationality from Northern Jiangsu, and smoking raises this risk of NSCLC occurrence in individuals with mutation genotypes of ADPRT rs1136410.
8.Correlation Analysis of XRCC1 rs25487 Polymorphism with the Occurrence of Lung Cancer

OBJECTIVE:To investigate the correlation of XRCC1 rs25487 polymorphism with the occurrence of lung cancer. METHODS:A total of 208 patients with primary lung cancer of Han nationality in Northern Jiangsu selected from the Affiliated Hospital of Xuzhou Medical University during Sept. 2015-Jul. 2016 were included in lung cancer group. A total of 214 healthy volunteers of the hospital underwent physical examination were included in control group. PCR-RFLP was used to detect the genotypes at XRCC1 rs25487 locus,and Logistic regression model was used to evaluate the correlation of genotypes with the occurrence of lung cancer. RESULTS:There was no statistical significance in the distribution of age and gender between 2 groups (P>0.05). The proportion of smoker in lung cancer group was significantly higher than control group,with statistical significance(P<0.05). AA,AG and GG genotypes were detected at rs25487 locus of XRCC1 gene. The frequency of AA,AG and GG genotype were 43.5%,41.1%and 15.4% in control group and 28.8%,48.6% and 22.6% in lung cancer group,respectively. The frequencies of genotypes in 2 groups were in line with Hardy-Weinberg equilibrium(P>0.05),but there was statistical significance in genotype distribution between 2 groups(P<0.05). Compared with AA genotype,the risk of lung cancer in individuals carrying AG genotype increased by 2.265 fold [OR=2.265,95%CI(1.299,3.950),P=0.040;after corrected with gender,age and smoking history OR=2.309,95%CI(1.274, 4.185),P=0.006],with statistical significance. The risk of lung cancer in individuals carrying GG genotype increased by 1.310 fold [OR=1.310,95%CI(0.771,2.228),P=0.318;after corrected OR=1.429,95%CI(0.811,2.518),P=0.217],without statistical significance. CONCLUSIONS:rs25487 locus mutant heterozy-gosity of XRCC1 gene is risk factor of lung cancer in Han nationality from Northern Jiangsu,and smoking can increase the risk of lung cancer.

10.Practice of independent designed experiment in occupational health and occupational medicine experimental teaching

Most of the current experiments are obsolete and verified,the students are not interested in and the experiments are out of touch with the actual work.These are the common problems in the experiment teaching of occupational health and occupational medicine.By taking the students who majored in preventive medicine as the subjects,this study aimed to explore the effects of independent designed experiment teaching mode,which was based on dividing the groups before class,choosing the projects by students themselves,deciding the design of the program via the discussion of teacher and students and doing the experimental reports and sharing the experience.The results showed that,in the premise of the preliminary master of the theory and basic skills of occupational health and occupational medicine,carrying out independent designed experiments in the last three weeks of semester,to a certain extent,could arouse students' interest in learning,cultivate their abilities of independent thinking,practice,problem analysis and solution,and team cooperation.However,restricted by lack of teachers,inadequate equipment and high cost and other factors,this teaching mode is only suitable for small class.
