1.Analysis of the efficacy of etoposide (Vp16) -intensified allogeneic hematopoietic stem cell transplantation in treating relapsed/refractory acute myeloid leukemia

Objective:To evaluate the efficacy of an etoposide (Vp16) -intensified conditioning regimen in allogeneic hematopoietic stem cell transplantation (allo-HSCT) for the treatment of relapsed/refractory acute myeloid leukemia (AML).Method:A retrospective analysis was conducted on the clinical data of 27 recipients with relapsed/refractory AML who underwent allo-HSCT using a Vp16-intensified conditioning regimen at Aerospace Center Hospital from January 2019 to January 2022. Transplantation-related complications and treatment outcomes were observed. Kaplan-Meier survival analysis was used to assess the overall survival (OS) and disease-free survival (DFS) rates.Result:Among the 27 recipients, there were 14 males and 13 females, with a median age of 41 years (range: 12~55 years). Except for one recipient who experienced primary graft failure, the remaining 26 recipients achieved hematopoietic reconstitution. The median neutrophil and platelet engraftment times were 13 days (range: 9~20 days) and 13.5 days (range: 11~33 days), respectively. Regimen-related toxicity (RRT) was mainly gastrointestinal toxicity and oral mucositis, and no deaths were attributed to RRT. A total of 12 recipients (44.44%) developed acute graft-versus-host disease (aGVHD), of whom 3 cases (11.11%) had grade III~IV aGVHD. Chronic GVHD (cGVHD) occurred in 13 recipients (48.15%), including 8 cases (29.63%) of extensive cGVHD. The median follow-up time after transplantation was 17 months (range: 1~48 months). Fifteen recipients (55.56%) survived without disease, while 12 recipients (44.44%) died— 9 due to relapse and 3 due to transplant-related complications. The 1-year overall survival and DFS rates were 74.07% and 59.26%, respectively; the 2-year overall survival and DFS rates were 59.26% and 55.56%, respectively. The 2-year relapse rate and transplant-related mortality (TRM) were 33.33% and 11.11%, respectively.Conclusion:The Vp16-intensified conditioning regimen in allo-HSCT appears to be a viable treatment option for patients with relapsed/refractory AML, offering favorable efficacy and manageable safety.

Type 2 diabetes mellitus(T2DM)has emerged as a significant public health challenge and a con-siderable threat to human well-being due to its chronic nature.Consequently,it cannot be overlooked.In response to this pressing issue,scientific weight loss treatments have demonstrated their effectiveness as a coping strategy.Particularly,for patients in the early stages of the disease with preserved pancreatic islet function and obesity,weight loss treatment has been shown to significantly enhance their glucose tolerance and ameliorate metabolic disorders.Ongoing research has led to a growing body of evidence regarding the mechanisms and clinical data supporting the use of weight loss therapy in miti-gating type 2 diabetes.This paper aims to evaluate the impact of weight loss therapy on the major metabolic organs of the body,such as the liver,pancreas,skeletal muscle,and adipose tissue,and to delve into the physiological and molecular mechanisms involved in the remission of T2DM.

Functional dyspepsia(FD)is a common functional gastrointestinal disease in clinical practice,which has the characteristics of high incidence,difficult to cure,and recurrence.FD belongs to the categories of"ruffian"and"stomach pain"in TCM,and the disease is located in the stomach,which is closely related to the liver and spleen,and the syndrome of liver depression and spleen deficiency is the most common.This article summarized the literature related to the TCM treatment for FD with liver depression and spleen deficiency syndrome,and concluded the clinical application,efficacy characteristics and mechanism,so as to provide reference for clinical treatment and basic research.The analysis found that the clinical efficacy of TCM in the treatment of FD is significant,which can not only improve the digestive symptoms of patients,but also improve their anxiety and depression state and daily life quality,and has the characteristics of overall regulation,syndrome differentiation and treatment,and improvement of physical fitness.Its mechanism may involve multiple pathways and levels such as abnormal gastric motility,abnormal brain-intestinal interaction and immune inflammatory response.
4.Batokines in the cross-talk between brown adipose tissue and other organs: An update

Brown adipose tissue(BAT), a metabolically active organ, not only plays a pivotal role in thermogenesis, but also exerts endocrine effects that regulate both its own metabolic processes and the function of various other organ systems. These effects are primarily mediated by brown adipose tissue-derived factors, or batokines. Recent advances in the understanding of batokine types, target organs, and underlying mechanisms have highlighted their significant role in the crosstalk between BAT and other organs. This review focuses on the various batokines secreted by BAT and their involvement in regulating interactions with key target organs such as the liver, cardiovascular system, skeletal muscle, and nervous system. By examining the specific functions and molecular mechanisms of these factors, this review aims to offer novel insights and suggest potential research directions for the prevention and treatment of related metabolic diseases.
5.Value of expression profile of mitochondrial ribosomal large subunit protein 13/BCL2-related protein A1/TP53 apoptosis-inducing protein 1 in serum in predicting chemotherapy response of triple-negative breast cancer and its immune regulatory mechanism

Objective To investigate the relationships of the mRNA expression levels of mitochon-drial ribosomal large subunit protein 13(MRPL13),BCL2-associated protein A1(BCL2A1)and TP53 apoptosis-inducing protein 1(TP53AIP1)in the serum of breast cancer patients with their chemotherapy efficacy as well as immune function.Methods A total of 148 triple-negative breast cancer patients undergoing chemotherapy were selected as the study subjects.Based on chemotherapy efficacy,they were divided into responsive group(97 cases)and non-responsive group(51 cases).Sixty healthy individuals undergoing physical examinations during the same period were selected as the control group.The mRNA expression levels of MRPL13,BCL2A1 and TP53AIP1 in the serum of patients were detected.The concentrations of immunoglobulin M(IgM),immunoglobulin G(IgG)and im-munoglobulin A(IgA)in the serum of patients were measured,and the proportions of CD4+and CD8+cells in whole blood were determined.The mRNA expression levels of MRPL13,BCL2A1 and TP53AIP1 were compared between the breast cancer group and the control group.The mRNA ex-pression levels of MRPL13,BCL2A1 and TP53AIP1 were compared between the responsive and non-responsive groups.The differences in immune function indicators before and after chemotherapy be-tween the responsive and non-responsive groups,were compared.The correlations of the mRNA ex-pression levels of MRPL13,BCL2A1 and TP53AIP1 with immune indicators were analyzed.Factors influencing chemotherapy efficacy were screened.Results The mRNA expression levels of MRPL13 and BCL2A1 in the breast cancer group were higher than those in the control group,while the mRNA expression level of TP53AIP1 was lower,with statistically significant differences(P<0.05).In the responsive group,the mRNA expression levels of MRPL13 and BCL2A1 were lower than those in the non-responsive group,while the mRNA expression level of TP53AIP1 was higher,with statistically significant differences(P<0.05).After chemotherapy,the IgM concentration in the responsive group was higher than that in the non-responsive group,with a statistically significant difference(P<0.05).The proportion of CD8+cells in the responsive group was lower than that in the non-responsive group,with a statistically significant difference(P<0.05).The mRNA expres-sion level of MRPL13 was positively correlated with IgM,IgG and CD8+(r=0.672,0.716,0.824,P<0.05).The mRNA expression level of BCL2A1 was positively correlated with IgA and CD4+(r=0.654,0.862,P<0.05).The mRNA expression level of TP53AIP1 was negatively cor-related with CD4+and CD8+(r=-0.846,-0.792,P<0.05).High expressions of MRPL13 and BCL2A1 were independent risk factors for chemotherapy efficacy in breast cancer patients(P<0.05),while high expression of TP53AIP1 was protective factor(P<0.05).Conclusion The mRNA expression levels of MRPL13,BCL2A1 and TP53AIP1 in the serum of triple-negative breast cancer patients undergoing chemotherapy are correlated with the levels of IgM,IgG and IgA,the proportions of CD4+and CD8+T cells,and chemotherapy efficacy.High expression of MRPL13 and BCL2A1 are independent risk factors for poor chemotherapy efficacy,while high expression of TP53AIP1 is a protective factor.
6.Incidence and related factors of functional bowel disease in submarine forces

Objective To investigate the incidence of functional bowel disease(FBD)and its relationship with depression,anxiety and sleep in navy submarine forces.Methods A questionnaire survey on the incidence of FBD was conducted on 364 naval soldiers who were enrolled according to Rome Ⅳ classification and diagnostic criteria.The risk factors of FBD were analyzed.The incidence of specific diseases of FBD was compared among soldiers with different jobs.The depression,anxiety,and sleep quality were investigated in FBD patients.Results In the 364 participants,132(36.3%)were diagnosed with FBD according to Roman Ⅳcriteria.Military rank and marital status might be risk factors for FBD.There was no significant difference in the overall incidence of FBD among the participants with various jobs(P>0.05).The incidence of functional constipation in the submariners was significantly higher than that in the land soldiers,while the incidence of functional diarrhea in the land soldiers was higher than that in the submariners(all P<0.05).The incidences of depression,anxiety and sleep disorder in the participants with FBD were higher than those in the participants without FBD.Conclusion There is a high incidence of FBD in submariners,which may be related to their psychological states.
7.Development and dissemination of precision medicine approaches in gastric cancer management.

Gastric cancer is a high-incidence malignancy that poses a serious threat to public health in China, ranking among the top three cancers in both incidence and mortality. The majority of patients are diagnosed at an advanced stage, resulting in limited treatment options and poor prognosis. To address key challenges in gastric cancer diagnosis and treatment, a research team led by Professor Jiafu Ji at Peking University Cancer Hospital has focused on the project "Development and Dissemination of Precision Medicine Approaches in Gastric Cancer Management". Through a series of high-quality multicenter clinical studies, the team established a set of new international standards in perioperative treatment, individua-lized drug selection, intelligent noninvasive diagnostics, and novel immunotherapy strategies. These advances have significantly improved treatment efficacy and reduced surgical trauma, achieving key technological breakthroughs in diagnosis, therapy, and mechanistic understanding, and systematically enhancing outcomes for gastric cancer patients. The project ' s findings had a broad international impact, including hosting China ' s first International Gastric Cancer Congress. Through nationwide dissemination, they have promoted the development of precision diagnosis and treatment of gastric cancer as a discipline, and led the formulation of the National Health Commission's guidelines for gastric cancer diagnosis and treatment. In recognition of its achievements, the project was awarded the First Prize of the 2024 Chinese Medical Science and Technology Award.

8.Characteristics of the first-visit cases of herpes zoster in Zhoushan City

Objective:
To analyze the characteristics of first-visit cases of herpes zoster in Zhoushan City, Zhejiang Province from 2021 to 2023, so as to provide the reference for improving herpes zoster prevention and control measures.
Methods:
Data on the incidence and vaccination of first-visit herpes zoster cases at all levels of public medical institutions in Zhoushan City from 2021 to 2023 were collected through the Zhoushan Comprehensive Health Information Platform and Zhoushan Immunization Program Information Management System. The incidence and outpatient proportion were calculated. The population distribution, seasonal distribution, and clinical consultation status of first-visit herpes zoster cases were described.
Results:
From 2021 to 2023, a total of 15 156 first-visit herpes zoster cases were reported in Zhoushan City, with an average annual incidence of 5.36‰. The incidences for each year were 5.78‰, 5.29‰ and 5.02‰, respectively, and the outpatient proportions were 0.15%, 0.14% and 0.11%, respectively, showed decreasing trends (both P<0.05). The number of doses of recombinant herpes zoster vaccine or live attenuated herpes zoster vaccine administered were 130, 312, and 633, respectively. The main consultation department was dermatology, with 11 004 cases (72.60%). The primary clinical diagnosis was visceral herpes zoster, with 5 901 cases (38.94%). A total of 1 936 cases (12.77%) had at least one underlying medical condition. The mean age of onset was (56.08±16.23) years, and the incidence showed an upward trend with increasing age (P<0.05). There were 7 386 male cases and 7 770 female cases, with a male-to-female ratio of 0.95∶1. The incidence among males aged ≥50 years was lower than that among females (6.53‰ vs. 8.69‰, P<0.05). The onset of the disease exhibited a significant seasonal pattern, with a peak period from June 21st to August 21st, covering 75% of the cases
Conclusions
From 2021 to 2023, the incidence and outpatient proportion of herpes zoster in Zhoushan City decreased. Summer was the peak season for onset, and women and the elderly were the key populations. It is necessary to strengthen the collaborative diagnostic and treatment capabilities of key departments such as dermatology and enhance the enthusiasm for vaccination among key populations.
9.Association of sedentary types with anxiety and depressive symptom among college freshmen

Objective:
To analyze the association of sedentary types with symptom of depressive and anxiety among college freshmen, so as to provide a reference for improving the mental health of college students.
Methods:
From October to November 2022, all college freshmen at three colleges and universities in Anhui Province were selected by a cluster sampling method. The Generalized Anxiety Disorder-7 (GAD-7), the Patient Health Questionnaire-9 (PHQ-9), and the Youth Leisure-Time Sedentary Behavior Questionnaire (YLSBQ) were used for the investigation. A binary Logistic regression analysis was conducted to investigate the relationship of different types of sedentary behavior with anxiety and depressive symptom.
Results:
The detection rates of anxiety and depressive symptom among college freshmen were 32.8% and 49.9%, respectively. The results of the binary Logistic regression model analysis showed that after controlling for gender, family location, parental education level, self rated family economic status and number of intimate partners, high level overall, video based, and social based sedentary time were associated with an increased risk of anxiety ( OR =1.26, 1.56, 1.27) and depressive symptom ( OR =1.42, 1.94, 1.29) among college freshmen; the association between moderate level sedentary time and depressive symptom was statistically significant ( OR =0.83) (all P <0.05). The overall trends of the association between sedentary behavior with symptom of anxiety and depressive were similar in both boys and girls.
Conclusions
Sedentary behavior is associated with an increased risk of anxiety and depressive symptom in college students. Reducing video based and social based sedentary behaviors is beneficial for mental health promotion in college students.
10.Development trajectory and influencing factors of intrinsic capacity in community-dwelling older adults

Objective:To investigate the current status and trajectory of intrinsic capacity in community-dwelling older adults and to analyze its influencing factors.Methods:A longitudinal study was conducted from September 2022 to March 2024. A total of 360 older adults were selected by convenience sampling from the Desheng community, Xicheng District of Beijing City and Liucun Community of Xingtai City, Hebei Province. The Intrinsic Capacity Assessment Scale and the Influencing Factors Questionnaire were administered at three-time points: baseline (T1), 6 months (T2), and 12 months (T3). Binary Logistic regression analysis revealed that age, grip strength, the number of geriatric syndromes, and types of social activities were significant influencing factors of intrinsic capacity trajectories in community-dwelling older adults ( P<0.05) . Results:A total of 360 participants were assessed at T1, with 58 and 30 lost to follow-up at T2 and T3, development respectively. Ultimately, 272 older adults completed the follow-up. The growth mixture modeling identified two trajectory types: a high-score stable group (92.3%, 251/272) and a low-score declining group (7.7%, 21/272). Binary Logistic regression analysis showed that age, grip strength, number of geriatric syndromes, and types of social activities were influencing factors of intrinsic capacity trajectories in community-dwelling older adults ( P<0.05) . Conclusions:Intrinsic capacity development trajectories in community-dwelling older adults are heterogeneous. Healthcare professionals should focus on older adults with advanced age, lower grip strength, more geriatric syndromes, and fewer types of social activities to assess and manage their intrinsic capacity and formulate targeted interventions at an early stage.
