1.Association between dysbiosis of the gut and lung microbiome and asthma in children

Objective To analyze the types and distribution of microbiome in intestinal and lung tissues of children with asthma, and to explore the correlation between microbiota changes and asthma. Methods From 2021 to 2023, a total of 28,939 children with asthma who visited Ezhou Central Hospital, Maternal and Child Health Hospital or Ezhou Egang Hospital were selected as the study subjects, and 2,000 healthy children who underwent outpatient physical examinations at these three hospitals during the same period were selected as the control group. The distribution and characteristics of intestinal and pulmonary microbiome in the two groups were analyzed by 16SrDNA sequencing. Logistic regression analysis model was used to analyze the correlation between microbiota distribution and asthma occurrence. Results In the intestinal tissues of children with asthma compared to healthy children, the abundance of Bacteroidetes at the phylum level decreased, while the abundance of Firmicutes and Proteobacteria increased significantly (P<0.05), and the abundance of Prevotalle and Clostridium at the genus level increased significantly. In lung tissues of asthmatic children compared to health children, the abundance of Firmicutes at the phylum level decreased while the abundance of Proteobacteria increased significantly (P<0.05), and the abundance of Neisseria, Prevotella and Actinomyces at the genus level increased significantly. Binary logistic regression results showed that the abundances of Lactobacillus (OR=0.842, 95% CI: 0.533-0.947), Bacteroides fragilis (OR=0.649, 95%CI: 0.377-0.890), Bifidobacterium (OR=0.901, 95% CI: 0.633-0.994), and Parabacteroides distasonis (OR=0.547, 95% CI: 0.192-0.708) in the intestinal tissues were all protective factors for the asthma in children. In the lung tissue, the abundance of Neisseria (OR=2.140, 95% CI: 1.749-3.305) was a risk factor for the asthma in children, and Prevotella (OR=0.691, 95% CI: 0.491-0.926) was a protective factor for the asthma in children (P<0.05). Conclusion The dysbiosis of intestinal and pulmonary microbiome is closely related to the occurrence of asthma in children, and the detection of microbiota is of great significance for the diagnosis of childhood asthma.
2.Early warning and prognostic assessment of urosepsis using white blood cells, procalcitonin, and T helper cell subsets type 1 and 2 cytokine profiles

Objective:To investigate the early warning value and prognostic assessment of patients with urosepsis using white blood cells (WBC), procalcitonin (PCT), combined with T helper cell subsets type 1 (Th1) and T helper cell subsets type 2 (Th2) cytokine profiles.Methods:A retrospective selection was made of 97 patients with urosepsis admitted to Inner Mongolia University for Nationalities Affiliated Hospital from January 2019 to June 2023 as the case group. During the same period, 60 patients with simple urinary tract infections were selected as the control group, and 60 patients with simple urinary tract infections transitioning to urosepsis were collected as the transition group. The clinical data and laboratory indexes of the three groups were collected, and the differences of WBC, PCT and Th1/Th2 cytokines profiles were compared. Point-biserial correlation was used for analysis. The subject work curve (ROC) was drawn. The area under the curve (AUC) was used to evaluate the early warning value of WBC, PCT and Th1/Th2 cytokines profiles in patients with urosepsis. The patients in the case group were followed up for 28 d. According to the survival outcome, they were divided into death group and survival group. The differences of WBC, PCT and Th1/Th2 cytokines profiles between the two groups were compared. Point-biserial correlation was used to assess their relationship with prognosis, and ROC curves were plotted to evaluate the predictive value of these indicators for urosepsis prognosis.Results:The levels of WBC, PCT, interleukin-6 (IL-6), IL-10, tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ) in different groups of patients were compared. The case group had higher levels than the transformation group and the control group: (12.02 ± 4.11) × 10 9/L vs. (9.88 ± 3.11) and (6.25 ± 2.21) × 10 9/L, (38.52 ± 12.9) ng/L vs. (25.54 ± 10.12) and (12.53 ± 5.01) ng/L, (60.36 ± 20.62) ng/L vs. (45.21 ± 6.95) and (30.64 ± 10.65) ng/L, (25.62 ± 6.85) ng/L. vs. (22.41 ± 3.62) and (20.41 ± 5.95) ng/L, (5.81 ± 0.85) ng/L vs. (5.21 ± 0.15) and (4.71 ± 0.86) ng/L, (5.01 ± 0.62) ng/L vs. (4.51 ± 0.21) and (3.81 ± 0.54) ng/L, there were statistical differences ( P<0.05). Point-biserial correlation confirmed a positive correlation between these indicators and urosepsis ( r>0, P<0.05). ROC curves showed that WBC, PCT, IL-6, IL-10, IFN-γ, and TNF - α had good predictive value for urosepsis, and several combinations had high predictive value, with AUC values of 0.771, 0.871, 0.823, 0.707, 0.860, 0.753, and 0.939, respectively. WBC, PCT, IL-6 and IFN-γ had good predictive value for the transition from simple urinary tract infection to urosepsis, while IL-10 and TNF-α had lower predictive value. The combined predictive value of the above indicators was the highest, with AUC values of 0.824, 0.861, 0.871, 0.869, 0.623, 0.644 and 0.965, respectively, all of which had certain predictive value. Out of 97 patients, 34 died, with a mortality rate of 35.05% (34/97). The levels of WBC, PCT, IL-6, IL-10, TNF - α, and IFN - γ in the deceased group were significantly higher than those in the survival group: (18.00 ± 6.39) × 10 9/L vs. (13.91 ± 4.51) × 10 9/L, (4.96 ± 2.17) ng/L vs. (3.41 ± 0.62) ng/L, (167.52 ± 10.52) ng/L vs. (158.61 ± 14.50) ng/L, (54.50 ± 9.51) ng/L vs. (48.42 ± 12.62) ng/L, (5.84 ± 1.21) ng/L vs. (5.10 ± 1.48) ng/L, (4.33 ± 1.82) ng/L vs. (3.54 ± 1.12) ng/L, there were statistical differences ( P<0.05). Point-biserial correlation analysis revealed a positive correlation between WBC, PCT, IL-6, IL-10, TNF-α, IFN-γ and prognosis ( r>0, P<0.05). The ROC curve was drawn to show that WBC, IL-6, IL-10, TNF-α and IFN-γ had a relatively low predictive value for the prognosis of patients. PCT and the combination of various indicators had a good predictive value for the prognosis of patients, and the combined predictive value was the highest. The AUCs were 0.675, 0.687, 0.637, 0.637, 0.637, 0.727 and 0.880, respectively. Conclusions:WBC, PCT, IL-6, IL-10, TNF-α and IFN-γ are associated with urosepsis and its prognosis, and the combined predictive value is the highest.

Objective To summarize the changing prevalence of carbapenem resistance in Enterobacterales based on the data of CHINET Antimicrobial Resistance Surveillance Program from 2015 to 2021 for improving antimicrobial treatment in clinical practice.Methods Antimicrobial susceptibility testing was performed using a commercial automated susceptibility testing system according to the unified CHINET protocol.The results were interpreted according to the breakpoints of the Clinical & Laboratory Standards Institute(CLSI)M100 31st ed in 2021.Results Over the seven-year period(2015-2021),the overall prevalence of carbapenem-resistant Enterobacterales(CRE)was 9.43%(62 342/661 235).The prevalence of CRE strains in Klebsiella pneumoniae,Citrobacter freundii,and Enterobacter cloacae was 22.38%,9.73%,and 8.47%,respectively.The prevalence of CRE strains in Escherichia coli was 1.99%.A few CRE strains were also identified in Salmonella and Shigella.The CRE strains were mainly isolated from respiratory specimens(44.23±2.80)%,followed by blood(20.88±3.40)%and urine(18.40±3.45)%.Intensive care units(ICUs)were the major source of the CRE strains(27.43±5.20)%.CRE strains were resistant to all the β-lactam antibiotics tested and most non-β-lactam antimicrobial agents.The CRE strains were relatively susceptible to tigecycline and polymyxins with low resistance rates.Conclusions The prevalence of CRE strains was increasing from 2015 to 2021.CRE strains were highly resistant to most of the antibacterial drugs used in clinical practice.Clinicians should prescribe antimicrobial agents rationally.Hospitals should strengthen antibiotic stewardship in key clinical settings such as ICUs,and take effective infection control measures to curb CRE outbreak and epidemic in hospitals.

6.A novel TNKS/USP25 inhibitor blocks the Wnt pathway to overcome multi-drug resistance in TNKS-overexpressing colorectal cancer.

Modulating Tankyrases (TNKS), interactions with USP25 to promote TNKS degradation, rather than inhibiting their enzymatic activities, is emerging as an alternative/specific approach to inhibit the Wnt/β-catenin pathway. Here, we identified UAT-B, a novel neoantimycin analog isolated from Streptomyces conglobatus, as a small-molecule inhibitor of TNKS-USP25 protein-protein interaction (PPI) to overcome multi-drug resistance in colorectal cancer (CRC). The disruption of TNKS-USP25 complex formation by UAT-B led to a significant decrease in TNKS levels, triggering cell apoptosis through modulation of the Wnt/β-catenin pathway. Importantly, UAT-B successfully inhibited the CRC cells growth that harbored high TNKS levels, as demonstrated in various in vitro and in vivo studies utilizing cell line-based and patient-derived xenografts, as well as APCmin/+ spontaneous CRC models. Collectively, these findings suggest that targeting the TNKS-USP25 PPI using a small-molecule inhibitor represents a compelling therapeutic strategy for CRC treatment, and UAT-B emerges as a promising candidate for further preclinical and clinical investigations.
7.Analysis of influencing factors of hyperlipidemia in areas of Ningxia based on structural equation model

Objective To explore the influencing factors of hyperlipidemia in 18-80 years old in Ningxia by structural equation model, and to analyze the direct and indirect effects of influencing factors of hyperlipidemia, so as to provide a basis for the formulation of prevention and treatment measures. Methods A total of 925 patients with hyperlipidemia from a chronic disease survey in 4 counties of Ningxia in April 2017 were selected as the case group (n=925), and residents without hyperlipidemia matched by sex and age were selected as the control group (n=925). A self-designed questionnaire was used to investigate the two groups of subjects. SPSS 22.0 software was used to conduct single factor T or Z test or χ2 test for the possible influencing factors of hyperlipidemia, and Amos22.0 was used to construct structural equation model. Results The structural equation model showed that physiological condition had the greatest effect on hyperlipidemia, and the standardized regression coefficient was -0.351. The second was the monitoring of three key blood indicators (three-high indicators), and the total effect value was 0.082, while personal condition and dietary status had no direct influence on the prevalence of hyperlipidemia. Conclusion Physiological status is the most important factor affecting the prevalence of hyperlipidemia in 18 ~ 80 years old in Ningxia, followed by the monitoring of the three-high indicators. In the future, residents should be encouraged to strengthen health management, especially people with overweight, high uric acid, high blood glucose and hypertension, to control the level of blood lipids and reduce the incidence of hyperlipidemia.
8.Correlation between allostatic load and clinical characteristics of first-episode schizophrenia and its influence upon the onset

Objective:To explore the possible correlation between allostatic load (AL) and clinical symptoms in patients with first-episode schizophrenia (FES) and AL influence upon the onset.Methods:Patients with FES (schizophrenia group, n=93) who received treatment at Beijing Huilongguan Hospital from October 2017 to July 2019 and the healthy controls (control group, n=111) recruited during the same period were enrolled in the research. Systolic blood pressure, diastolic blood pressure, resting heart rate, body mass index, waist-hip ratio, high-density lipoprotein cholesterol, total cholesterol, glycosylated hemoglobin, high-sensitivity C-reactive protein, 12-hour urine adrenaline, 12-hour urine norepinephrine, 12-hour urine cortisol, and serum dehydroepiandrosterone of all subjects were collected to calculate the AL index. The positive and negative symptom scale (PANSS) was used to assess the psychopathological symptoms of patients. Covariance analysis was used to compare the differences of AL index and the thirteen biological components between FES patients and controls. Pearson correlation analysis was used to explore the correlation between AL index and clinical symptoms, and the Logistic regression analysis was used to explore the risk factors of schizophrenia. Results:AL index among schizophrenic patients was higher than the controls (5.87±1.90 vs. 3.50±2.21, F=65.97, P<0.001). AL index was possitively correlated with PANSS positive symptoms ( r=0.25, P=0.019) and was a risk factor of schizophrenia ( OR=1.76, 95% CI: 1.47-2.11, P<0.01). Conclusions:The AL is possitively correlated with positive symptoms in the FES, and the higher AL may be one of important inducements to the schizophrenia onset.
9.Correlation between allostatic load and clinical characteristics of first-episode schizophrenia and its influence upon the onset

Objective:To explore the possible correlation between allostatic load (AL) and clinical symptoms in patients with first-episode schizophrenia (FES) and AL influence upon the onset.Methods:Patients with FES (schizophrenia group, n=93) who received treatment at Beijing Huilongguan Hospital from October 2017 to July 2019 and the healthy controls (control group, n=111) recruited during the same period were enrolled in the research. Systolic blood pressure, diastolic blood pressure, resting heart rate, body mass index, waist-hip ratio, high-density lipoprotein cholesterol, total cholesterol, glycosylated hemoglobin, high-sensitivity C-reactive protein, 12-hour urine adrenaline, 12-hour urine norepinephrine, 12-hour urine cortisol, and serum dehydroepiandrosterone of all subjects were collected to calculate the AL index. The positive and negative symptom scale (PANSS) was used to assess the psychopathological symptoms of patients. Covariance analysis was used to compare the differences of AL index and the thirteen biological components between FES patients and controls. Pearson correlation analysis was used to explore the correlation between AL index and clinical symptoms, and the Logistic regression analysis was used to explore the risk factors of schizophrenia. Results:AL index among schizophrenic patients was higher than the controls (5.87±1.90 vs. 3.50±2.21, F=65.97, P<0.001). AL index was possitively correlated with PANSS positive symptoms ( r=0.25, P=0.019) and was a risk factor of schizophrenia ( OR=1.76, 95% CI: 1.47-2.11, P<0.01). Conclusions:The AL is possitively correlated with positive symptoms in the FES, and the higher AL may be one of important inducements to the schizophrenia onset.
10.Analysis of the understanding and application of the knowledge of premature infants in pediatricians participating in the Second Chinese Preterm Infant Academic Conference

Objective:To investigate the understanding and application of the knowledge of premature infants in pediatricians participating in the Second Chinese Preterm Infant Academic Conference through questionnaires, and to carry out targeted continuing education for pediatricians in order to better guide the clinical diagnosis and treatment of grassroots pediatricians.Methods:The research was conducted in the form of a questionnaire survey.The survey objects were pediatricians participating in the " Second Chinese Preterm Infant Academic Conference" , and the results of the questionnaire were summarized and analyzed.The questionnaires were designed based on personal education background, employment time, current status of neonatal specialty in the hospital where they were, and knowledge of premature infant nutrition.Results:A total of 554 valid questionnaires were received.Among the pediatricians surveyed, 37.2% had a master's degree or higher; 344(62.1%) pediatricians came from Grade Ⅲ class A hospitals.According to the geographical distribution, age, education, working years, hospital level, hospital nature, annual delivery of obstetrics in hospitals, the number of beds in neonatal specialty wards, and the number of beds in NICU, the respondents were grouped, and the current status of premature births in China and the situation of using the growth curve(Fenton2013) of premature infants in work were compared.It was found that the proportion of those who used the Fenton curve of the 2013 version in Grade Ⅲ class A hospitals was higher than that in Grade Ⅲ class B hospitals and Grade Ⅱ hospitals(χ 2=47.663, P<0.05). The ratio was higher among women and children's health centers and general hospitals(χ 2=8.072, P<0.05). Conclusion:The academic annual conference for the purpose of continuing education, set up a pre-conference questionnaire for the participants during the preparation period, and set the content of the annual conference according to the needs of the target population, which can better solve the clinical problems of primary doctors and promote the improvement of primary doctors' diagnosis and treatment.
