ObjectiveTo investigate the protective effect and mechanism of Wendan Ningxin granules (WNG) on susceptibility to atrial fibrillation (AF) in mice with inflammatory injury. Methods100 C57BL/6 mice were divided into a blank control group, a model group, a low-dose WNG group (2.34 g·kg-1·d-1), a high-dose WNG group (4.68 g·kg-1·d-1), and an amiodarone positive control group (0.091 g·kg-1·d-1), with 20 mice in each group. Except for the blank control group, mice in other groups received intraperitoneal injections of lipopolysaccharide (LPS) to establish an inflammatory injury model. Treatment groups received continuous intragastric administration of their respective interventions for four weeks. During the fourth week, the treatment groups received LPS injections concurrently with their treatments. The blank control and model groups received distilled water (10 mL·kg-1·d-1) by gavage, with a gavage volume of 10 mL·kg-1 for all groups, once daily. Hematoxylin-eosin (HE) staining and Sirius red staining were used to observe atrial tissue morphology and fibrosis degree. Immunohistochemistry was used to assess the expression of α-smooth muscle actin (α-SMA) in mouse atrial tissue. Electrophysiological detection was performed using a multi-channel electrophysiology mapping system to measure AF inducibility, AF duration, and atrial effective refractory period (AERP). High-resolution optical mapping was used to measure action potential duration (APD) dispersion, conduction heterogeneity index, and calcium transient (CaT) dispersion. Real-time quantitative polymerase chain reaction (Real-time PCR) was used to detect mRNA expression of proteins related to diastolic calcium leakage in mouse atria: Ca2+/calmodulin-dependent protein kinase Ⅱ(CaMKⅡ), ryanodine receptor 2(RyR2), sarco/endoplasmic reticulum Ca²⁺-ATPase (SERCA), and sodium-calcium exchanger (NCX). Western blot analysis was performed to detect the expression of CaMKII, RyR2, SERCA, and NCX proteins in myocardial tissue from each group. Enzyme-linked immunosorbent assay (ELISA) was used to measure serum levels of inflammatory factors interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α). ResultsPathological staining results showed that compared with the blank control group, the model group exhibited disrupted atrial tissue structure, inflammatory cell infiltration, atrial fibrosis, and diffuse infiltration of numerous brown α-SMA positive cells in the atrial interstitium (P<0.01). AF could be induced by electrical stimulation with a longer duration. AERP was shortened, while APD dispersion, conduction heterogeneity index, and CaT dispersion were increased (P<0.01). The expression of proteins associated with diastolic calcium leakage, including CaMKⅡ, RyR2, and NCX1, showed elevated mRNA and protein levels, whereas SERCA2a mRNA and protein expression decreased (P<0.05). Serum levels of inflammatory factors IL-1β and TNF-α were elevated (P<0.01). Compared with the model group, intervention with WNG alleviated cardiac structural damage, reduced inflammatory cell infiltration, improved atrial fibrosis, and reduced the diffuse infiltration of α-SMA positive cells (P<0.01). AF inducibility and AF duration upon electrical stimulation were significantly reduced (P<0.05), AERP was prolonged (P<0.05), mRNA and protein expression of CaMKⅡ, RyR2, and NCX1-proteins associated with diastolic calcium leakage-were reduced, whilst mRNA and protein expression of SERCA2a increased (P<0.05), and serum levels of IL-1β and TNF-α were decreased (P<0.01). ConclusionBoth low‑ and high‑dose WNG can effectively reduce susceptibility to inflammation-related AF. The mechanism by which WNG reduce AF susceptibility may be related to regulating proteins involved in sarcoplasmic reticulum diastolic calcium leak, thereby improving cardiac electrical remodeling, and alleviating inflammation-induced myocardial fibrosis, thus improving cardiac structural remodeling.

Objective:To investigate the efficacy of recombinant human interferon α-2b spray, administered under medication guidance, in the clinical treatment of herpangina. Methods:A total of 76 children with herpangina who were treated at The First People's Hospital of Yongkang between October 2020 and October 2022 were included in this study. Using the random number table method, these patients were randomly assigned to an observation group ( n = 42) and a control group ( n = 34). The control group received conventional treatment, whereas the observation group was administered recombinant human interferon α-2b spray under medication guidance. A comparative analysis was conducted between the two groups, evaluating clinical efficacy, inflammatory factor levels, treatment compliance, and the negative conversion rate of throat swab virology. Results:After treatment, the overall response rate of the observation group [95.24% (40/42)] and treatment compliance [97.62% (41/42)] were significantly higher than those of the control group [76.47% (26/34), 79.41% (27/34), χ2 = 4.27, 4.82, P = 0.040, 0.030]. The levels of C-reactive protein [(6.28 ± 1.64) mg/L], white blood cell count [(6.11 ± 1.10) × 10 9], and serum amyloid A [(3.47 ± 0.89) mg/L] in the observation group were significantly lower than those in the control group [(7.51 ± 1.16) mg/L, (7.51 ± 1.16) × 10 9, (7.82 ± 1.30) mg/L, t = 3.69, 6.46, 17.27, all P < 0.001]. The positive conversion rate of throat swab virology in the observation group [4.76% (2/42)] was lower than that in the control group [26.47% (9/34), χ2 = 5.51, P = 0.190]. Conclusion:The use of recombinant human interferon α-2b spray under medication guidance in the treatment of herpangina can improve treatment compliance, rapidly alleviate clinical symptoms, and significantly improve the prognosis.

Objective:To investigate the etiological characteristics of hand, foot and mouth disease (HFMD) and the VP1 gene evolution of coxsackievirus A6 (CVA6) in Qingdao from 2020 to 2022.Methods:Total enterovirus (EV), enterovirus 71 (EV71), coxsackievirus A16 (CVA16), CVA6 and coxsackievirus A10 (CVA10) were detected by fluorescent RT-PCR in throat swab samples from HFMD patients in Qingdao from 2020 to 2022. The full-length sequence amplification of VP1 gene and gene sequence determination of CVA6 isolates were performed, and the phylogenetic and molecular characteristics were analyzed by MEGA-X software. Results:The overall positive rate of enteroviruses in 1 645 samples collected during 2020-2021 was 80.73%(1 328/1 645). The proportions of EV71, CVA16, CVA6, CVA10 and other enteroviruses were 0.08%(1/1 328), 19.65%(261/1 328), 68.15%(905/1 328), 0.98%(13/1 328) and 11.14%(148/1 328), respectively. Children under 5 years old accounted for 75.68% (1 245/1 645) of all cases and 100% (2/2) of severe cases. A phylogenetic tree was constructed based on the VP1 region sequences of 45 CVA6 strains showing all strains belonging to the D3a subtype. There were 10 non-synonymous mutations of amino acids in the VP1 protein by gene evolution analysis.Conclusions:CVA6 D3a subtype is the main epidemic strain causing HFMD in Qingdao from 2020 to 2022, especially in children under 5 years old.

Objective:To investigate the molecular features of hand, foot and mouth disease (HFMD) caused by coxsackievirus A10 (CVA10) in Qingdao and analyze the clinical features of mild and severe cases. Methods:A total of 6 677 cases of HFMD routinely monitored by Qingdao Women and Children Hospital from 2014 to 2021 were enrolled. Throat swab samples were collected. Clinical data of these cases were retrospectively analyzed. Virus nucleic acid was extracted from the samples and the serotypes of enteroviruses were identified. The VP1 genes of CVA10 strains were amplified and sequenced. A phylogenetic tree based on the VP1 gene sequences was constructed using MEGA7. 0 software. SPSS23.0 software was used for statistical analysis. Results:There were 285 cases positive for CVA10, including 183 males and 102 females, and children under five years old accounted for 89.8%. Most of CVA10 infection occurred between the months of April to September. The count of white blood cells, the percentage of neutrophils, the concentration of hemoglobin, and the levels of aspartate aminotransferase and alanine transaminase were significantly higher in severe patients than in mild patients. Besides, chest radiography and brain CT revealed more abnormalities in severe patients, and the duration of ECG monitoring was longer in them. Compared with mild cases, severe cases developed rash early than fever with rash mostly on buttocks ( P<0.05). Phylogenetic analysis showed that most of the CVA10 strains circulating in Qingdao between 2014 and 2021 belonged to clade Ⅰ, and there were two variations A23V and I283V in the amino acid sequence of clade Ⅰ. Conclusions:This study showed that children of all ages were susceptible to CVA10, especially those under five years old. CVA10 showed complex and diverse epidemic trends in different regions and years.

Objective: To analyze the current situation and problems of primary health care personnel since the new round of China's health care reform, and to provide a reference for stabilizing the primary health care personnel. Methods: This paper uses descriptive statistical analysis to calculate Gini coefficient and Theil index in order to ana-lyze the equity in headcount, structure and distribution of primary health care personnel among all health care institu-tions nationwide. Results: Between 2008 and 2013, the number of primary health care personnel significantly in-creased and their quality was improved. Moreover, although the Gini coefficient and Theil index of primary health care personnel continuously decreased in China's eastern, central and western provinces the difference inside the east-ern China is the main factor affecting the overall difference. Conclusions:Not only the problems of insufficient num-ber and loss of personnel still coexist in the primary health care personnel in China, but also there are other issues such as the structural imbalance of professional titles is especially prominent, the urban-rural and regional gaps are still large, etc. The main reasons for these issues are related to the irrational incentive mechanism, the inefficiency of the synergistic effects of administrative staffing policy and some reform measures, etc. This paper suggests further re-forming the income distribution system and improving primary governance capability in order to strengthen the policies to synergically attract and stabilize primary healthcare personnel.
