Objective: To analyze the etiologic distribution of difficult crossmatching and the characteristics of red blood cell antibodies, to summarize compatible transfusion strategies based on serological abnormalities, and to evaluate red blood cell transfusion efficacy and changes in bilirubin levels before and after transfusion. Methods: A retrospective analysis was conducted on difficult crossmatching cases handled by the Department of Blood Transfusion of our hospital from January 2024 to December 2025. Cases that met the criteria for difficult crossmatching determination and had complete data were included in the analysis of the causes and antibody distribution. Patients who completed red blood cell transfusion and had complete hemoglobin (Hb) data before and within 24 hours after transfusion were included in the exploratory subgroup analysis of transfusion efficacy; among them, those who also had total bilirubin and indirect bilirubin data before and after the corresponding transfusion event were included in the bilirubin analysis. According to the main serological abnormal manifestations before transfusion and the actual compatible transfusion management pathways, the cases included in the exploratory subgroup analysis were divided into three groups: the alloantibody-related difficult crossmatching group, the autoantibody-related or autoantibody combined with alloantibody-related difficult crossmatching group, and the ABO blood group identification abnormality-related difficult crossmatching group. The Hb levels before and after transfusion, the Hb increase after transfusion of each unit of red blood cells, and the changes in bilirubin levels before and after transfusion were compared among the groups. Results: A total of 322 difficult crossmatching cases met the predefined criteria, of which 287 were included in the analysis of causes and antibody distribution. There were 183 cases of alloantibody-related difficult crossmatching (63.76%), 54 cases related to inconsistency between ABO forward and reverse typing (18.82%), 44 cases related to autoantibodies (15.33%), and 6 cases related to interference from anti-CD38 monoclonal antibody drugs (2.09%). Among the 183 alloantibody-related cases, 79 cases had only a single alloantibody detected, among which antibodies of the Rh blood group system accounted for 49.37% (39/79). Among single alloantibodies, anti-E was detected most frequently, with 35 cases (44.30%), followed by anti-M with 19 cases (24.05%) and anti-Le
with 13 cases (16.46%). A total of 109 cases were included in the exploratory subgroup analysis of transfusion efficacy, including 76 cases in the alloantibodyrelated difficult crossmatching group, 17 cases in the autoantibody-related or autoantibody combined with alloantibody-related difficult crossmatching group, and 16 cases in the ABO blood group identification abnormality-related difficult crossmatching group. The sex distribution and history of previous transfusion differed between cases included in and not included in this analysis. The Hb levels after transfusion in all three groups were higher than those before transfusion, and the differences were statistically significant (P<0.05). The Hb increase per unit of red blood cells did not differ significantly among the three groups (P=0.086). Among 73 cases with paired total and indirect bilirubin data corresponding to the transfusion events, the comparison of changes in total bilirubin among the three groups showed no statistically significant difference (P=0.864), and the comparison of changes in indirect bilirubin among the three groups also showed no statistically significant difference (P=0.914). Conclusion: Alloantibody-related factors were the main causes of difficult crossmatching in this study. Among single alloantibodies, Rh blood group system antibodies, especially anti-E, were relatively common. In the included exploratory subgroup, after individualized compatible transfusion based on the causes of difficult crossmatching, antibody specificities, and crossmatching results, Hb levels were higher after transfusion than before transfusion in all three groups. Within the scope of this study, the magnitude of bilirubin changes did not differ significantly among the different types of difficult crossmatching.

Aim To explore the role of the ten-eleven translocation 2(TET2)/ubiquinol-cytochrome C reductase hinge protein(UQCRH)axis in the inhibition of vascular smooth muscle cell(VSMC)apoptosis by fibroblast growth fac-tor-2(FGF-2).Methods Cultured VSMCs were divided into control group,FGF-2 group, and FGF-2+fibroblast growth factor receptor(FGFR)pan-inhibitor LY2874455 group.VSMCs overexpressing TET2(OETET2)or UQCRH(OEUQCRH)were divided into control group,FGF-2 group, and OETET2+FGF-2 or OEUQCRH+FGF-2 group.Ho-echst33342 and PI staining were used to detect cell apoptosis,CCK-8 assay was employed to measure cell proliferation, and Western blot was used to examine the expression levels of apoptosis-related proteins pro-Caspase-3,cleaved Caspase-3,Bax,Bcl-2,as well as TET2 and UQCRH.The NCBI and methprimer websites were utilized for predicting and analyzing CpG island sites in the UQCRH gene promoter. Results FGF-2 could inhibit VSMC apoptosis, promote proliferation, downregulate apoptosis-related proteins cleaved Caspase-3,Bax,TET2,and UQCRH expression, and upregulate anti-ap-optotic protein Bcl-2 expression(compared with control group,P<0.05).However,it did not affect pro-Caspase-3 ex-pression(compared with control group,P>0.05).LY2874455 could counteract the effects of FGF-2(compared with FGF-2 group,P<0.05).Overexpression of TET2 or UQCRH could reverse the anti-apoptotic and pro-proliferative effects of FGF-2 on VSMC,with upregulation of apoptosis-related protein expression and downregulation of anti-apoptotic protein expression(compared with FGF-2 group,P<0.05).The UQCRH gene promoter region contained three CpG islands.Overexpression of TET2 could upregulate UQCRH expression in VSMC treated with FGF-2(compared with FGF-2 group,P<0.05).Conclusion FGF-2,by inhibiting TET2 expression and UQCRH expression, reduces VSMC apoptosis and promotes its proliferation.

Microvascular invasion (MVI) is an independent predictor of early recurrence and poor prognosis following hepatocellular carcinoma (HCC) resection and transplantation. As a novel non-invasive diagnostic tool, radiomics can extract the quantitative imaging features of tumors and peritumoral tissues with high throughput, providing more information on tumor heterogeneity than conventional and functional imaging of visual analysis and having a good application prospect in predicting the presence of MVI in HCC patients, thereby improving the accuracy of HCC diagnosis and prognosis. The value of the multimodal radiomics method based on various imaging methods in evaluating the possibility of MVI in HCC patients is elucidated here in combination with the latest research progress.

Objective To analyze the detection results of apolipoprotein B (ApoB) and serum uric acid (SUA) in patients with non-alcoholic fatty liver disease (NAFLD). Methods A total of150 NAFLD patients admitted to our hospital were selected as study group, and 150 healthy people with physical examination in the same period were as the control group. Low density lipoprotein cholesterol (LDL-C), triglyceride (TG), alanine aminotransferase (ALT), waist-hip ratio (WHR), body mass index (BMI), total cholesterol (TC), ApoB, creatinine (Cr), SUA levels were measured in the two groups. The correlation between SUA, ApoB and NAFLD was analyzed by multiple Logistic regression analysis. At the same time, smoking, coronary heart disease, diabetes and hypertension were compared between the two groups. Results The levels of LDL-C, TG, ALT, WHR, TC, BMI, SUA and ApoB in the study group were higher than those in the control group (P < 0. 05). The levels of Cr in the study group were significantly lower than those in the control group (P < 0. 05). The results of multiple Logistic regression analysis showed that the levels of SUA and ApoB were independently and positively correlated with NAFLD and were independent risk factors for NAFLD. The incidences of smoking, coronary heart disease, diabetes mellitus and hypertension were significantly higher than that of the control group (P < 0. 05). Conclusion The levels of SUA and ApoB are positively correlated with NAFLD. The higher the levels of SUA and ApoB are, the higher the incidence of cardiovascular disease is. Therefore, blood lipid intervention should be carried out in time to control the development of NAFLD.

Early small hepatocellular carcinoma (SHCC) can be cured by surgery and interventional operation to improve the survival rate of paitents,so the accurate diagnosis of SHCC is of great significance.Presently, the dynamic contrast-enhanced MR scan could only obtain single arterial phase imaging by single breath hold, and it had breathing motion artifact because of the long scanning time. The early arterial transient enhancement of SHCC was easy to be misdiagnosed. The CAIPIRINHA-Dixon-TWIST-VIBE (CDT-VIBE) sequence couldobtain high-quality multiple arterial phases images and hepatic arterial dominant (HAD) images in short-time scanning. And it could not only detect small focal lesions which were difficult to find by other imaging examination, but also find the start enhanced difference between lesions which were relevant to lesions property and blood supplement. The CDT-VIBE has a high clinical value in diagnosis and treatment of SHCC.The research progresses of CDT-VIBE in diagnosis of SHCC was reviewed in this article.
