1.Clinical efficacy of three surgical approaches for repairing adult giant atrial septal defects

Objective To compare the clinical efficacies among three surgical approaches of small inci-sion in the lower segment of the sternum,small incision in the right axilla and thoracoscopic 3 incisions for re-pairing adult giant atrial septal defects(ASD).Methods The medical records of 112 patients with giant ASD undergoing surgical repair in this hospital from January 2018 to January 2024 were retrospectively analyzed.According to the different surgical approaches,the patients were divided into the group A(n=35,small inci-sion in the lower part of the sternum),group B(n=37,small incision in the right axilla)and group C(n=40,thoracoscopic 3 incisions).The general data,operation time,cardiopulmonary bypass time,blood transfusion volume,incision length,postoperative VAS score,postoperative mechanical ventilation time,ICU stay time,postoperative hospitalization duration,hospitalization cost and postoperative complications occurrence rates were compared among the various groups.Results There were no statistically significant differences in the operation time,cardiopulmonary bypass time,blood transfusion volume,postoperative mechanical ventilation time,ICU stay time and postoperative hospitalization duration among 3 groups(P>0.05).The hospitalization cost in the group C was higher than that in the groups A and group B,the postoperative VAS score in the group B was higher than that in the group A and C,the incision size in the group C was shorter than that in the group A and group B,and the differences were statistically significant(P<0.05).All patients were suc-cessfully repaired.The incidence rate of postoperative pulmonary infection in the group A and group C was lower than that in the group B,and the difference was statistically significant(P<0.05).Conclusion All three surgical approaches could safely and effectively repair adult giant ASD.Different surgical approaches should be selected based on the specific conditions of the patients.
2.One-stage bone grafting combined with non-contact plate technique for treatment of post-traumatic femoral osteomyelitis and bone defects.

Eight patients with femoral osteomyelitis were admitted in Lanzhou General Hospital of PLA between July 2012 and July 2015. During the operation the femur was fixed with non-contact locking plate after thorough debridement. Iliac morselized cancellous bones without cortical bone were filled in the bone defect after debridement. The locking plates were placed in the lateral subcutaneous interface superficial to the vastus lateralis muscle. Drainage tubes were routinely placed postoperatively. The surgery was completed successfully in all patients, and the average operation time was (130±10) min (120-150 min). Intravenous antibiotics were administrated for 2 weeks and followed by 4 week-oral antibiotics after the operation in all 8 cases. Drainage tubes remained in situ about 8 days. All patients were followed up for 12-48 months, with an average of (19±7) months. Postoperative X-ray examination showed bone union in 7 cases with an average healing time of (16±5) weeks. One patient failed with relapsed infection, and was later treated with Ilizarov apparatus after secondary debridement. No plate and screw fracture, loose and fixation failure were observed in all 8 cases.

3.THE ACTIVITY CHANGES OF ALKALINE PHOSPHATASE AND ITS ULTRASTRUCTUAL LOCALIZATION DURING LIVER REGENERATION IN RATS

Objective To study the changes of AKP activity and its cytochemistry position during liver regeneration following partial hepatectomy. Methods Using colorimetry to analyze AKP activity; using non-denatured electrophoresis technique to analyze the kinds and activity of AKP; using electron microscope cytochemistry to analyze its position. Results During liver regeneration, AKP activity had two peaks at 16*!h and 19*!h and followed by decline dramatically; Three hepatic-derived AKP isoenzymes were detected, and 180*!kD AKP appeared only in liver regeneration; AKP appeared in different part with the process of liver regeneraion.Conclusion AKP plays an important role in liver regeneration and it may involve in cell metabolism, material transportation, DNA synthesis and cell differentation.
