1.Clinical efficacy and influencing factors of Chaihu anxin capsules in the treatment of mild-to-moderate depression

OBJECTIVE To evaluate the clinical efficacy of Chaihu anxin capsules in the treatment of mild-to-moderate depression, analyze the influencing factors of its efficacy, construct a nomogram prediction model.METHODS Patients with mild-to-moderate depression treated at The First Hospital of Hebei Medical University from May 1, 2023 to March 10, 2025 were retrospectively included and categorized into Chaihu anxin capsules group and milnacipran group according to treatment regimen. Propensity score matching (PSM) was used to balance baseline characteristics, and treatment efficacy was compared after 8 weeks, including 24 Hamilton Depression Scale (HAMD-24) scores and changes, Hamilton Anxiety Scale (HAMA) scores and changes, and treatment response rate after 8 weeks of treatment. Patients receiving Chaihu anxin capsules were further classified as responders or non-responders based on the occurrence of treatment response. Univariate and multivariate Logistic regression analyses were conducted to identify the influencing factors of treatment response, and a nomogram model was constructed to predict the probability of treatment response, its predictive performance was evaluated using receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis.RESULTS A total of 297 patients were included before PSM, including 196 patients in the Chaihu anxin capsules group and 101 patients in the milnacipran group. After PSM, each group included 44 patients. After PSM, the HAMD-24 scores and HAMA scores at 8 weeks after treatment in the Chaihu anxin capsules group, as well as the HAMD-24 scores in the milnacipran group, were significantly lower than their respective baseline values ( P <0.05); the change in HAMA score in the Chaihu anxin capsules group was significantly greater than that in the milnacipran group ( P =0.04). Multivariable Logistic regression identified age, total bilirubin, and baseline HAMD-24 score as independent predictors of treatment response ( P <0.05). The area under the ROC curve of the constructed nomogram prediction model was 0.72 (95% confidence interval: 0.637-0.799), and the corrected Brier score was 0.143 (95% confidence interval: 0.112-0.176); compared with the “all intervention” and “all non intervention” strategies, applying this nomogram prediction model could achieve higher net benefits within the threshold probability range of 8% to 37%.CONCLUSIONS The efficacy of Chaihu anxin capsules in improving mild-to-moderate depression is comparable to that of milnacipran, and it has certain advantages in improving anxiety symptoms; age, total bilirubin, and baseline HAMD-24 score are independent influencing factors for the treatment response of Chaihu anxin capsules. The nomogram prediction model constructed based on these factors has good discrimination, calibration, and clinical applicability.
2.Comparison of the performance of two methods to determine set-up errors for DIBH patients with left sided breast cancer in radiotherapy

Objective:To establish the basic procedures of the application of optical surface monitoring system (OSMS) in the deep inspiration breath hold (DIBH) radiotherapy for patients with left sided breast cancer and compare the performance of OSMS and cone-beam CT (CBCT) in the determination of the set-up errors of DIBH radiotherapy for patients with left sided breast cancer.Methods:Twenty patients with left sided breast cancer received DIBH radiotherapy. Through the registration of CBCT images with the planning CT images, and the registration of OSMS radiography images with the outer contour of the body surface, translational set-up errors and rotational errors were determined along the lateral-medial ( Rx), superior-inferior ( Ry) and anterior-posterior ( Rz) directions. Pearson correlation analysis was performed to evaluate the correlation of the set-up errors determined by two methods, and Bland- Altman plot analysis was used to assess the coincidence of these two methods. Results:Two methods were positively correlated. The Rz volume was 0.84, 0.74 and 0.84 in the x, y and z directions, and 0.65, 0.41 and 0.54 in the Rx, Ry and Rz directions, respectively (all P<0.01). The 95% CI of agreement were within preset 5 mm tolerance (-0.37-0.42cm, -0.39-0.41cm, -0.29-0.49cm ) in x, y and z directions for two methods. The 95% CI of agreement were within preset 3 ° tolerance -2.9°-1.4°, -2.6°-1.4°, -2.4°-2.5°in Rx, Ry and Rz directions for two methods. The system errors of 20 patients with left sided breast cancer receiving DIBH radiotherapy were <0.18cm and the random errors were <0.24cm. Conclusions:OSMS is equivalent to CBCT in the determination and stimulation of set-up errors for patients with left sided breast cancer receiving DIBH radiotherapy. The combination of CBCT and OSMS is a safe and reliable method.
3.Metallothionein involvement in myocardial protection of basic fibroblast growth factor

AIM: To observe whether metallothionein plays a role in cardiac protective effect of basic fibroblast growth factor on anoxic/reperfusion (A/R) injury in cultured cardiomyocytes and study the possible mechanism of cardiac protection by bFGF.METHODS: The present study made the model of myocyte A/R injury after having a 24 h incubation by bFGF( 10-10、10-9、10-s mol/L) and bFGF( 10-9 mol/L) + PD098059 respectively. We measured the levels of MT and MDA in myocytes, and the changes of LDH and protein in cultured medium. We also counted the number of viable cell in groups. RESULTS: The contents of myocardial MT were significantly increased after treatment by bFGF. The levels of MT in I0-l0 mol/L、10-9 mol/L and 10-8 mol/L bFGF treated groups increased 54 %、 62%、 76% respectively, compared with the A/R group, and the number of viable cell were also greatly increased, LDH and protein leakage in cultured medium and MDA contents in myocyte were dramatically decreased in bFGF treated groups. All the protection were completely disappeared with the inhibition of MT production with PD098059, theinhibitor of mitogen- activated protein kinase(MAPK). CONCLUSION: MT involves in the protection of bFGF in cultured cardiomyocytes. It might be related with activation of MAPKase.

6.The effect of homocysteine on airway smooth muscle cells and fibroblasts

AIM: To study the effect of homocysteine (HCY) on proliferation of airway smooth muscle cells and fibroblasts and the effect of HCY on collagen prodution of airway fibroblasts. METHODS: [3H]-TdR incorpora- tion was measured in cultured airway smooth muscle cells. The [3H]-TdR and [3H]-proline incorporation were mea- sured in cultured airway fibroblasts. RESULTS: HCY induced proliferation of airway smooth muscle cells and fibroblasts in a concentration - dependent manner. HCY also induced collagen production of airway fibroblasts in a concentration - dependent manner. The inhibitors of protein kinase C, H7 and polymyxin B, inhibited HCY - induced proliferation of airway smooth muscle cells. CONCLUSIONS: HCY induced proliferation of airway smooth muscle cells and fibroblasts, HCY also induced collagen production of airway fibroblasts. The HCY - induced proliferation of airway smooth muscle cells may be related to the pathway of PKC signal transduction.
