1.Intelligent segmentation and staging system for esophageal cancer based on DAEUnet and ConvNeXt networks

Objective To construct an intelligent segmentation and T-stage diagnostic model for esophageal cancer based on the DAEUnet and ConvNeXt networks using transfer learning.Methods Dicom raw data from 126 patients diagnosed with esophageal cancer between January 2018 and April 2022 were collected,including 100 cases from Department of Thoracic Surgery at the First Affiliated Hospital of Army Medical University and 26 cases from the Department of Thoracic Surgery at Shanxi Cancer Hospital.After data augmentation,a total of 60 275 images were obtained.The DAEUnet esophageal cancer intelligent segmentation network was built,and on this basis,3 classification networks,ConvNeXt,Swin Transformer,and ResNet were constructed for T-stage diagnosis of esophageal cancer.Results The Dice similarity coefficient(DSC)for esophageal cancer intelligent segmentation using the DAEUnet network was 0.82,and the DSC value of the esophagus,aorta,normal esophagus,mediastinal lymph nodes,and heart was 72.4%,87.5%,79.3%,60.5% and 96.8%,respectively.Among the 3 T-stage diagnosis models for esophageal cancer,the ConvNeXt model performed the best,with a precision value for T1~T4 stages of 0.65,0.727,0.889 and 0.92,respectively,and an AUC value of 0.892,which were superior to the ResNet and Swin Transformer networks.Conclusion The proposed DAEUnet and ConvNeXt-based intelligent segmentation and T-stage diagnosis model for esophageal cancer improves T-stage accuracy and treatment efficiency.
2.Exploration and practice of patient satisfaction evaluation management in multi-campus public hospitals

Patient satisfaction is one of the core indicators to measure the service quality of medical institutions. To this end, a multi-campus public hospital in Shanghai constructed a management system of patient satisfaction evaluation. Since 2021, its call center has conducted a full coverage satisfaction assessment for discharged patients from its three campuses and collected dissatisfaction information feedback. The hospital organized relevant clinical departments and functional departments to fully communicate with the dissatisfied patients according to the feedback information, followed by a joint rectification. The hospital regularly conducts in-depth analysis of all complaints for timely discovery of common problems in different campuses for continuous improvement. This practice can provide reference for multi-campus hospitals to promote homogeneous management, to improve management efficiency, service quality and patient satisfaction.
3.Effect of nectin-1 expression after being inhibited by lentivirus on epileptogenesis and mossy fiber sprouting

Objective:To explore the effect of adhesion molecule nectin-1 expression on epileptic seizure and mossy fiber sprouting.Methods:(1) Adult male SD rats were randomly divided into control group ( n=6), empty vector group ( n=6) and lentivirus interfered group ( n=24), and rats in the lentivirus interfered group were further divided into 4 subgroups ( n=6) according to the time points of lentivirus injection (3, 7, 14 and 30 d after injection). The protein expression of nectin-1 in the hippocampus of rats was detected by Western blotting. (2) Another 12 male SD rats were randomly selected and divided into lentivirus epilepsy group ( n=6) and empty vector epilepsy group ( n=6). Lentivirus and empty lentivirus vector were injected into the hippocampus of the above two groups respectively, and then, pilocarpine epilepsy models were kindled; the behavior changes of these rats were recorded, and changes of mossy fiber sprouting in the hippocampus were observed by Timm staining. Results:(1) There were significant differences in hippocampal nectin-1 protein expressions in each group ( F=76.120, P=0.000); the nectin-1 expression in the 7, 14, and 30 d subgroups was significantly decreased as compared with that in the control group and empty vector group ( P<0.05). (2) The results of behavior changes showed that the times required for the lentivirus epilepsy group to be kindled successfully ([34.33±2.38] min) were significantly longer than those for the empty vector epilepsy group ([24.50±2.06] min, t=7.650, P=0.000). In terms of seizure level, the seizure level of rats in the lentivirus epilepsy group was significantly lower at each time point within one h modeling than that of rats in the empty vector epilepsy group ( P<0.05). After kindling, the time of spontaneous seizure appeared in the lentivirus epilepsy group was significantly longer than that in the empty vector epilepsy group, and the frequencies of spontaneous seizure in the lentivirus epilepsy group were significantly decreased as compared with those in the empty vector epilepsy group ( P<0.05). Timm staining scores in the lentivirus epilepsy group (3.500±0.224) were significantly lower than those in the empty vector epilepsy group (4.667±0.211, t=9.289, P=0.000). Conclusion:Inhibition of nectin-1 expression in hippocampal area of epileptic rats can delay the occurrence of epilepsy and reduce the frequencies of spontaneous seizures of epileptic rats, whose mechanism may be related to the reduction of nectin-1 in the formation of mossy fiber sprouting in abnormal neural circuits of hippocampal area.
4.The Isolation and Identification of Infectious Bronchitis Virus PTFY Strain in Muscovy Ducks.

In July 2009, some farms of breeding Muscovy ducks on the peak of egg laying suffered the decrease of hatching rate and the quality of the eggs showing low mortality and no evident respiratory symptoms. The swelling and congestive ovary was visible after autopsy. This study was brought out for the diagnosis of these cases. The virus was isolated and identified by the methods of virus culture in chicken embryo, physical and chemical properties test, hemagglutinin test, NDV (Newcastle diseases Virus) interference test, electron microscope observation, pathogenicity test and the gene sequence analysis. The results indicated the virus showed the characters of inducing dwarf embryo after inocubation, the sensibility to lipid solvent and the hemagglutination capacity after pancreatic enzyme treatment, the typical morphology of coronavirus, the interference to NDV replication and the homology among 84.7% - 99% of the particial N gene sequences to the reference IBV (Avian infectious bronchitis virus) strains. The strain was identified as IBV isolate and this study confirmed the pathogenicity of IBV to Muscovy ducks.

5.Correlation between the expression of nuclear factor-kappa B,monocyte chemoattractant protein-1 and CD68 in lupus nephritis

Objective To observe the expression of nuclear factor-kappa B(NF-κ3)and monocyte chemnattractant protein-1(MCP-1),and the infiltration of mononuclear macrophage CD68 in lupus nephritis (LN).The association between NF-κB,MCP-1 and MΦ with kidney pathology and clinical manifestations is explored.MethodsNF-κB,MCP-1 and MΦ in renal biopsy specimens from 49 cases of LN were detected using immunohistochemical techeniques.Forty-nine cases of renal tissues were examined for NF-κB by in situ hybridization.The relationship between NF-κB,MCP-1 and MΦ with kidney pathology and clinical manifestations were analyzed.Results ①Compared with the control group ,the expression of NF-κB,MCP-1 and MΦ in LN was significantly higher(P<0.01).The expression of MCP-1 positively correlated with MΦinfiltration and NF-κB(P<0.01)in glomeruli and renal tubule and renal interstitium of LN.The expression of NF-κB,MCP-1 and the infiltration of MΦ in LN Ⅳ was significandy higher than non LN Ⅳ and the control group(P<0.01,P<0.05).There was no significant difference between renal NF-κB positive group and negative groups in the degree of the immunohistochemical and in situ hybridization examination(P>0.05).②The histological activity index,urine protein volume(24 h)and serum creatinine in LN were significantly higher than the control group(P<0.01),and the expression of NF-κB in LN was correlated with histological activity index,urine protein volume(24 h)and serum creatinine(P<0.01,P<0.01,P<0.05).MCP-1 and CD68 expression in LN were correlated with histological activity index and urine protein volume(24 h),but not correlated with serum creatinine(P>0.05).Conclusion NF-κB induced MΦ by activating MCP-1 may be one cause of kidney injury of LN.NF-κB signal pathway may act as a new therapeutic target for MΦinfiltration and proliferation inhibition in kidney.
6.Preparation and purity determination of sinapine from seeds of Brassica alba

Objective:Preparated from seeds of Brassica alba, Sinapine was determinated by RP-HPLC. Methods: Seeds of Brassica alba were shattered, defatted by ether, extracted by alcohol, and crystallized by potassium thiocyanate, then ethanol and water were used to recrystallize for several times. The determination was performed by reversed-phase high performance liquid chromatography(RP-HPLC), using ZORBAX SB-Aq column 4.6 mm?250 mm, 5 ?m. The mobile phase consisted of acetonitrile-0.08 mol/L KH2P04 (20:80) at a flow rate of 1.0 ml/min, and the detection wavelength was 326 nm. Results: Pale yellow needle-like sinapine crystal was obtained, and the yield rate was 0.82%. The purity of sinapine thiocyanate crystal was over 95%. Conclusion: Preparation sinapine by this solvent extraction method was simple, high-purity, high yield, and could be used for industrial production.
7.Optimizing extraction process of volatile oil from Semen Sinapis Albae and analyzing its chemical components

the hydrolysis temperature.7 Components had been identified from 10 separated peaks,these accounted for 99.999% of the total volatile oil.The main components were Allyl isothiocyannte(89.411%),3-Butenyl isothiocyanate(7.364%) and 3-Butenenitrile(1.275%).Conclusion:The optimum extraction conditions were:hydrolysis temperature 50,pH value of buffer solution 5.0,concentration of ascorbic acid 0.5mmol/L.The yield rate of the volatile oil of this process can reach 0.164%,and this process was stable and feasible;the main components of the volatile oil were isothiocyanates,and the volatile oil also contained a small amount of aldehydes,ethers and nitriles.
