1.Cultivation mode of rehabilitation therapy talents in local colleges and universities under the background of New Medicine

ObjectiveTo explore the training path of rehabilitation therapy professionals in local colleges and universities under the background of New Medicine. MethodsTaking Jining Medical University as an example, combining the interdisciplinary integration, intelligent driving and people-oriented New Medicine attributes of rehabilitation therapy, this study focused on the current practical problems in rehabilitation education, accurately aligned with the requirements of new medicine construction, reshaped the curriculum system with new concepts, empowered practical innovation with new technologies, and cultivated humanistic literacy with new connotations. By deeply integrating digital construction with educational practice, it carried out the construction and practice of the New Medicine talent training model in rehabilitation therapy, helping students form a stereoscopic knowledge structure, possess the ability to apply intelligent rehabilitation technology and solve complex rehabilitation problems, and become high-quality rehabilitation therapy talents with both morality and ability. ResultsThe overall construction of the rehabilitation therapy major established a model of Dual fusion foundation, Three element empowerment and Three dimensional soul casting, including reform and innovation in curriculum system, practical mode and moral education mechanism. It has formed a health oriented "vertical and horizontal integration-discipline fusion" dual-fusion stereoscopic curriculum system, an intelligent led "theory and practice-science and education-industry and education" three element integration practical mode, and a confucian medical culture "curriculum-practice-evaluation" three-dimensional integrated moral education mechanism. Through practical testing, this model achieved significant results and was widely recognized, including a significant improvement in the quality of talent cultivation, prominent achievements in resource construction, and prominent achievements in health services, providing strong support for the construction of regional rehabilitation medical systems. ConclusionThe reform of the rehabilitation treatment talent training model at Jining Medical University under the background of New Medicine provides a replicable and promotable practical paradigm for the training of rehabilitation talents in similar universities through reshaping the curriculum system, strengthening practical innovation, and cultivating humanistic literacy.

4.Exploration of legal regulations of investigator-initiated trials

The investigator-initiated trials (IIT), as a widely existing form of clinical research both domestically and internationally, have attracted the attention of governments around the world due to the potential legal risks and issues they may cause, and the trend of legal supervision has gradually strengthened. The legal regulation of IIT in China is still in its early stages, with numerous legal issues that need to be clarified and sorted out. Based on the domestic and international legal reviews of IIT, this paper systematically sorted out the current situation of legal supervision of IIT in China and examined the existing issues, including weak legislative rules, incomplete regulatory systems, imperfect protection rules for research participants and informed consent systems, inadequate regulation of conflicts of interest management, and lax legal supervision of ethical review. Furthermore, this paper proposed suggestions for legal regulation of IIT from three perspectives, including strengthening legislation and emphasizing regulation, improving the mechanism for protecting research participants’ rights and interests, and balancing the legalization of IIT and the scientific development of clinical research, with a view to providing references for legal regulatory paths of IIT in China.
6.Differences in acute mountain sickness between first-time and multiple plateau entrants and its impact on psychological stress

Objective To investigate the differences in acute mountain sickness between individuals en-tering the plateau for the first time and those with multiple plateau experiences,and to examine the impact on psychological stress.Methods A total of 2 337 young plateau-acclimatized individuals from an acclimatization rotation area during November 2023 to January 2024 were selected as the study subjects.Those entering the plateau for the first time were classified into Group A(n=425),while those with previous plateau experience(at least one prior visit)were classified into Group B(n=1 912).Differences were analyzed between the two groups in terms of general characteristics,as well as acute mountain sickness(AMS)scores,Self-Rating De-pression Scale(SDS)scores,Symptom Checklist-90(SCL-90)scores,and various vital signs at the following time points:before entering the plateau(T0),on the 1st day(T1),4th day(T2),7th day(T3),and 10th day(T4)after entering the plateau.Results Group A was younger than Group B,with a statistically significant difference(P<0.05).At T1,T2,and T3,AMS scores in Group A were significantly higher than those in Group B(P<0.05).At T4,there was no statistically significant difference in AMS scores between the two groups(P>0.05).At T1,T2,and T3,blood oxygen saturation was lower and heart rate higher in Group A than in Group B,with statistically significant differences(P<0.05).After high-altitude acclimatization,no statistically significant differences in blood oxygen saturation or heart rate were observed between the groups at T4(P>0.05).At T1,there were no statistically significant differences in systolic or diastolic blood pres-sure between the groups(P>0.05).At T2,both systolic and diastolic blood pressure were higher in Group A than in Group B(P<0.05).At T3 and T4,after high-altitude acclimatization,systolic blood pressure de-creased in both groups but remained higher in Group A than in Group B(P<0.05).On the SDS scale,men-tal-emotional and somatic symptom scores were significantly higher in Group A than in Group B(P<0.001).On the SCL-90,all nine symptom factor scores were higher in Group A than in Group B(P<0.05).Conclusion Indi-viduals entering high-altitude areas for the first time experience more severe acute mountain sickness and more pronounced psychological stress.Therefore,enhanced monitoring,psychological counseling,and tailored pre-vention and treatment strategies should be implemented.

Lumbar disc (LD) herniation and aging are prevalent conditions that can result in substantial morbidity. This study aimed to clarify the mechanisms connecting the LD aging and herniation, particularly focusing on cellular senescence and molecular alterations in the nucleus pulposus (NP). We performed a detailed analysis of NP samples from a diverse cohort, including individuals of varying ages and those with diagnosed LD herniation. Our methodology combined histological assessments with single-nucleus RNA sequencing to identify phenotypic and molecular changes related to NP aging and herniation. We discovered that cellular senescence and a decrease in nucleus pulposus progenitor cells (NPPCs) are central to both processes. Additionally, we found an age-related increase in NFAT1 expression that promotes NPPC senescence and contributes to both aging and herniation of LD. This research offers fresh insights into LD aging and its associated pathologies, potentially guiding the development of new therapeutic strategies to target the root causes of LD herniation and aging.

9.Efficacy of short-term low-dose benzbromarone versus low-dose febuxostat in the treatment of Chinese male gout patients with renal uric acid underexcretion and its influence on liver function

Objective:To compare the efficacy of low-dose febuxostat and low-dose benzbromarone in lowering serum uric acid and their impact on liver function in male patients with renal underexcretion gout.Methods:This prospective cohort study enrolled 303 patients with renal underexcretion gout and normal baseline liver function. Participants were assigned to either the low-dose febuxostat group(20 mg qd) or the low-dose benzbromarone group(25 mg qd). A linear mixed-effects model was used to compare the uric acid target achievement rate(<360 μmol/L) and changes in liver enzyme levels between the two groups.Results:At week 4, the proportion of patients achieving the serum uric acid target(<360 μmol/L) was significantly higher in the benzbromarone group than that in the febuxostat group(64.2% vs 42.3%, P<0.001), with a trend toward greater efficacy at weeks 8 and 12. The incidence of alanine aminotransferase(ALT) or aspartate aminotransferase(AST) elevation above the upper limit was significantly higher in the febuxostat group compared to the benzbromarone group(35.2% vs 13.85%, P<0.001). After adjusting for baseline liver enzyme levels in the mixed-effects model, mean ALT and AST levels remained significantly higher in the febuxostat group than those in the benzbromarone group at weeks 4, 8, and 12( P<0.05). In the febuxostat group, ALT and AST levels significantly increased over time during weeks 0-4 and 4-8 ( P<0.001), peaking at week 8 followed by a decreasing trend. By week 12, the levels were not significantly different from baseline ( P>0.05). Whereas there was no significant difference at each follow-up time point in the benzbromarone group( P>0.05). Conclusions:In male patients with renal underexcretion gout, low-dose benzbromarone demonstrated superior urate-lowering efficacy and better hepatic safety compared to low-dose febuxostat.
10.Clinical Study of Wuwei Wentong Chubi Capsules in Treating Rheumatoid Arthritis with Cold and Dampness Obstruction Syndrome Based on Real World Data

Objective To evaluate the clinical efficacy and safety of Wuwei Wentong Chubi Capsules in patients with rheumatoid arthritis of cold and dampness obstruction syndrome under real world based on clinical medical record information system.Methods Medical history,symptoms and medication information of hospitalized patients with rheumatoid arthritis of cold and dampness obstruction syndrome in the Department of Rheumatology,First Affiliated Hospital of Anhui University of Chinese Medicine were collected from January 2020 to March 2024.According to whether to use Wuwei Wentong Chubi Capsules,patients were divided into treatment group(304 cases)and control group(378 cases).A database was established after improving the information and indicators of two groups of patients.TCM syndrome scores,disease activity scores and laboratory indicators of both groups were compared.Results Compared with before treatment,TCM syndrome score(severe joint pain,morning stiffness,swelling,worsening pain when encountering cold,poor joint flexion and extension,eating less and eating more,lack of breath and lazy speech,weak mouth and thirst,heavy head and body fatigue,loose stools and fatigue)in the treatment group significantly decreased after treatment(P<0.05),the disease activity score,erythrocyte sedimentation rate(ESR),high-sensitivity C-reactive protein(hs-CRP),rheumatoid factor(RF),complement(C3,C4),immunoglobulin(Ig)G,IgM,anti cyclic citrullinated peptide antibody(anti CCP antibody),thyroid stimulating hormone(TSH),anti thyroglobulin antibody(TGAb),and anti thyroid peroxidase antibody(TPOAb)were significantly reduced(P<0.05),and free triiodothyronine(FT3)and free tetraiodothyronine(FT4)increased(P<0.05);after treatment,TCM syndrome score(severe joint pain,morning stiffness,swelling,fatigue and weakness)in the control group significantly decreased,and the disease activity score,ESR,hs-CRP,RF,the levels of anti CCP antibodies,TGAb,and TPOAb were significantly reduced(P<0.05);compared with the control group after treatment,TCM syndrome scores in the treatment group showed more significant improvement(P<0.05)in terms of joint morning stiffness,swelling,poor flexion and extension,exacerbation of cold exposure,eating less and eating more,lack of breath and speech,weak and dry mouth,heavy head and body fatigue and fatigue.Disease activity score,ESR,hs-CRP,RF,the levels of anti CCP antibodies,C3,IgG,TSH and TPOAb decreased more significantly,while FT3 and FT4 increased more significantly(P<0.05);both groups had normal blood routine and liver and kidney function before and after treatment,with no significant difference(P>0.05).According to the analysis of association rules,after using Wuwei Wentong Chubi Capsules,there was a significant improvement in ESR,hs-CRP,RF,C3,IgG,TSH,FT4 and TPOAb indicators,with a degree of improvement greater than 1,indicating a strong association.Conclusion Wuwei Wentong Chubi Capsules can effectively improve the cold and dampness obstruction syndrome and spleen deficiency dampness excess syndrome in patients with rheumatoid arthritis cold dampness obstruction syndrome,reduce disease activity index and immune inflammatory indicators,and have a certain regulatory effect on thyroid immune dysfunction and the clinical application has good safety.
