1.Expert consensus on clinical application of parenteral direct thrombin inhibitors in perioperative period

OBJECTIVE To form an expert consensus on the clinical application of parenteral direct thrombin inhibitors (DTIs) in patients during the perioperative period. METHODS Led by Sichuan Academy of Medical Sciences & Sichuan Provincial People’s Hospital (the Affiliated Hospital of UESTC), a multidisciplinary working group was established. Through literature review and the Delphi method, clinical questions related to the rational perioperative use of parenteral DTIs were identified. A structured design was adopted using the “Population-Intervention-Comparison-Outcome” framework; systematic searches were conducted in CNKI, Medline, Embase and other databases. Relevant evidence from randomized controlled trials and cohort studies was included and synthesized. Evidence quality was assessed using the Grades of Recommendations Assessment,Development and Evaluation (GRADE) approach, and recommendations were formulated through multiple rounds of Delphi surveys and expert consensus meetings. RESULTS &CONCLUSIONS Seven recommendations (each with an expert consensus rate exceeding 90%) on the use of parenteral DTIs in perioperative patients were developed. These recommendations specify drug selection, dosing ranges, key monitoring points, and safety management strategies for parenteral DTIs in various scenarios, including the perioperative period of ventricular assist device implantation, the perioperative period of cardiac surgery, perioperative patients with lower-extremity atherosclerotic disease, the perioperative period of percutaneous coronary intervention in patients with acute coronary syndrome, the perioperative period of carotid artery stenting in patients with carotid stenosis, the perioperative period of patients with right heart thrombosis, and patients who develop related thrombosis and dysfunction after a central venous catheter insertion. In addition, warning and management pathways for perioperative bleeding and thrombotic events were proposed. This expert consensus, which is formulated based on the best available evidence, provides evidence-based guidance for standardized and individualized use of parenteral DTIs in perioperative period.
2.Impact of hyperglycemia on liver fibrosis and end-stage liver disease in patients with comorbidities of chronic hepatitis B and steatotic liver disease

ObjectiveTo analyze the clinicopathological features of patients with comorbidities of chronic hepatitis B (CHB) and steatotic liver disease (SLD), to investigate the impact of hyperglycemia on the risk of liver fibrosis and end-stage liver diseases (ESLD), and to provide a basis for the clinical management of such population. MethodsA total of 668 adult patients with CHB-SLD confirmed by liver biopsy in The Fifth Medical Center of Chinese PLA General Hospital from January 2011 to December 2019 were enrolled, and a retrospective cohort was established with the time of liver biopsy as the baseline and the onset of ESLD as the endpoint. All patients were followed up to March 31, 2024. Propensity score matching (PSM) was performed at a ratio of 1∶4 to balance baseline features between groups, resulting in 82 patients in the hyperglycemia group and 281 in the non-hyperglycemia group. The two groups were compared in terms of metabolic profiles, laboratory markers, and histopathological features. The Mann-Whitney U test was used for comparison of non-normally distributed quantitative data between two groups. The chi-square test or Fisher exact test was used for comparison of categorical data between two groups. A multivariate Logistic regression analysis was used to investigate the influencing factors for advanced fibrosis (AF), and the Kaplan-Meier survival analysis and the Cox proportional-hazards regression model were used to determine the influencing factors for the development of ESLD. ResultsCompared with the non-hyperglycemia group, the hyperglycemia group had a significantly higher number of factors for metabolic disorders, a significantly higher degree of hepatic steatosis, and a significantly higher detection rate of AF (all P<0.05). The multivariate Logistic regression analysis showed that hyperglycemia was a risk factor for AF (odds ratio = 1.753, 95% confidence interval [CI]: 1.017 — 3.023, P=0.043). The survival analysis showed that hyperglycemia increased the risk of ESLD (χ2=4.340, P=0.037). The multivariate Cox regression analysis confirmed that hyperglycemia was a significant metabolic risk factor for ESLD in patients with AF (adjusted hazard ratio=3.208, 95%CI: 1.201 — 8.568, P=0.020). ConclusionHyperglycemia can increase the risk of AF and ESLD in CHB-SLD patients. Clinical monitoring and active management should be strengthened for patients who have already developed AF and hyperglycemia.
3.Expression profiles of HBsAg and HBcAg in liver tissue and their correlation with serological markers in children with chronic hepatitis B

ObjectiveTo investigate the expression features of HBsAg and HBcAg in liver tissue and their correlation with HBV serum markers in children with chronic hepatitis B (CHB). MethodsA total of 257 patients who were consecutively admitted to The Fifth Medical Center of Chinese PLA General Hospital from January 2013 to December 2023 and underwent liver biopsy to achieve a confirmed diagnosis of CHB were enrolled in this study. The NIS-Elements system was used to capture the immunohistochemical images of HBsAg and HBcAg in liver tissues, and Image J software was used for quantitative analysis. The one-sample chi-square test was used for within-group comparison of continuous data, and the Pearson/Spearman/Kendall’s Tau-b correlation analysis was used to investigate the correlation between viral antigen expression and serological markers. ResultsAmong the 257 CHB patients, there were 162 children (76 children aged<5 years and 86 children aged 5 — 18 years) and 95 adults. There were significant differences in the expression pattern, area, and intensity of HBsAg and the area and intensity of HBcAg in liver tissue between different age groups and between the children with different HBeAg statuses (all P<0.05). In the children aged<5 years, HBsAg staining area was significantly negatively correlated with anti-HBs and HBeAg (both P<0.05)and was significantly positively correlated with ALT and AST (both P<0.05), and HBsAg staining intensity was significantly positively correlated with qHBsAg (P<0.05) and was significantly negatively correlated with anti-HBs (P<0.05). In the children group, HBsAg staining area was negatively correlated with anti-HBs and HBeAg (both P<0.05), and HBsAg staining intensity was positively correlated with qHBsAg (P<0.05) and was negatively correlated with anti-HBs (P<0.05). In the adult group, HBsAg staining area was positively correlated with ALT, AST, and liver inflammatory activity (all P<0.05), and HBsAg staining intensity was positively correlated with qHBsAg, HBeAg, and HBV DNA (all P<0.05) and was negatively correlated with liver inflammatory activity and fibrosis degree (both P<0.05). In the children aged<5 years, HBcAg staining area was positively correlated with qHBsAg and HBV DNA (both P<0.05), and HBcAg staining intensity was significantly positively correlated with HBV DNA (P<0.001). In the children aged 5 — 18 years, the area and intensity of HBcAg staining were positively correlated with qHBsAg, HBeAg, and HBV DNA (all P<0.05). In the children group, HBcAg staining area was positively correlated with qHBsAg, HBeAg, and HBV DNA (all P<0.05), and HBcAg staining intensity was positively correlated with qHBsAg and HBV DNA (both P<0.05). In the adult group, the area and intensity of HBcAg staining were positively correlated with qHBsAg, HBeAg, and HBV DNA (all P<0.001), and HBcAg staining area was positively correlated with the serum level of ALT (P=0.043). ConclusionThe expression levels of HBsAg and HBcAg in liver tissue of children with CHB are significantly correlated with serological markers, and in clinical practice, HBsAg and HBcAg combined with serological markers can help to assess the condition of the liver, determine the immune stage, and provide evidence-based guidance for treatment timing.
4.Ferroptosis: a potential new therapeutic target for myocardial injury induced by acute carbon monoxide poisoning.

Acute carbon monoxide poisoning (ACMP) is one of the most common gas poisonings in the emergency department, with tens of thousands of people seeking medical attention for carbon monoxide (CO) poisoning each year. The severity of poisoning is dependent upon environmental and human factors, with hypoxia and oxidative stress being important mechanisms of cardiac toxicity induced by CO. Myocardial involvement is common in moderate to severe ACMP, including myocardial injury, myocardial infarction, arrhythmia, and sudden death, which are associated with a high risk of death. Ferroptosis is a cell death mechanism caused by iron-dependent lipid peroxidation (LPO), although ferroptosis has been shown to play a critical role in various cardiovascular diseases, the potential mechanism by which it contributes to ACMP-induced myocardial injury is unclear. This review discusses the established link between ferroptosis and cardiovascular disease and summarizes the potential role of ferroptosis in ACMP-induced myocardial injury and the detrimental effects of ACMP on the heart. Elucidating these mechanisms could guide the development of novel therapeutic strategies that target ferroptosis to mitigate ACMP-induced myocardial injury. This review aims to provide a theoretical foundation for future research on the potential use of ferroptosis as a therapeutic target for ACMP-induced myocardial injury.

5.Preparation of anti-influenza virus nanobodies and their applications in nanobody-ELISA

Objective To develop nanobodies with broad-spectrum reactivity,specificity,and high sensitivity that can be used for detecting multiple subtypes of influenza A virus,and to establish a nanobody-based enzyme-linked immunosorbent assay(ELISA)method.Methods Gene sequences of twelve nanobodies against influenza A virus were retrieved from the National Center for Biotechnology Information(NCBI)and nanobody databases.The nanoantibodies were prepared using molecular biological techniques including gene synthesis and recombinant expression.The binding activity,specificity,sensitivity,and affinity of these nanobodies were determined by ELISA screening and Gator affinity analysis.A double-antibody sandwich ELISA assay was established by combining the selected nanobody with a traditional mouse monoclonal antibody.Results Twelve nanobodies were expressed and purified.Two nanobodies capable of binding to multiple subtypes of influenza virus including H1,H3,H5,H7,and H9 were obtained and designated as VHH54 and KV108.Both nanobodies showed no cross-reactivity with other respiratory virus antigens.Furthermore,the KV108 nanobody exhibited the highest binding affinity,with a dissociation constant of 5.94×10-9mol/L for the influenza virus nucleoprotein(NP),and the lowest detection concentration for the NP antigen reached 0.00064 μg/mL.The double-antibody sandwich ELISA,using a combination of KV108 and a mouse monoclonal antibody,could sensitively detect the five common subtypes of influenza A virus(H1N1,H3N2,H5N1,H7N9,and H9N2).The lowest detection limit reached 110-403 PFU/mL,which was higher than that of the commercial colloidal gold kitfor influenza virus detection.Conclusion This study has identified a nanobody KV108,which is capable of binding to multiple subtypes of influenza virus,and established a nanobody-based ELISA method that can detect multiple subtypes of influenza A virus.This study can facilitate the development of nanobody-based influenza detection technologies.
6.Recent advances in the role of lipid rafts in the development and progression of diabetes

Lipid rafts are microdomains in the cell membrane that are involved in cell signal transduction,metabolism,and intercellu-lar interactions.In recent years,studies have shown that lipid rafts play an important role in the pathogenesis of diabetes.Cholesterol and sphingolipids are the main lipid components in lipid rafts,and the protein components in lipid rafts include caveolin,flotillin,pal-mitoylated proteins,and glycosylphosphatidylinositol-anchored proteins.Changes in these components affect the structure and function of lipid rafts,which in turn may affect insulin signal transduction,leading to the occurrence of diabetes-related diseases.Lipid rafts are closely related to the occurrence and development of diabetes in different tissues.Pancreatic lipid rafts are closely related to insulin se-cretion,and their structural changes affect insulin synthesis and release.Changes in lipid rafts in adipose tissue are related to insulin resistance and disorders of glycolipid metabolism.Changes in lipid rafts in the liver can affect gluconeogenesis and glycogen synthesis.Lipid rafts in the kidney play a regulatory role in the progression of diabetic nephropathy.This article aims to provide a comprehensive overview of the role of lipid rafts in the pathogenesis of diabetes,offering insights into the identification of new targets for the prevention and treatment of diabetes in the future,as well as presenting a new perspective for the development of therapeutic agents for diabetes.
7.Correlation between the serum levels of aminotransferases and liver inflammation activity in pediatric chronic hepatitis B patients: An analysis of 1 267 cases

ObjectiveTo investigate the correlation of the serum levels of aminotransferases and their ratios with liver inflammation activity in pediatric chronic hepatitis B (pCHB) patients, and to provide a basis for selecting the dominant population for treatment. MethodsThis study was conducted among 1 267 pCHB patients who were admitted to The Fifth Medical Center of Chinese PLA General Hospital from January 2010 to August 2022 and these patients did not receive antiviral therapy. The patients were analyzed in terms of demographic features, blood routine, blood biochemistry, HBV serological markers, and liver biopsy data. According to liver inflammation activity based on liver biopsy, the patients were divided into no or mild inflammation activity (G0 — G1) group and significant inflammation activity (G2 — G4) group. The serum levels of aminotransferases and their ratios were compared between groups, and their correlation with liver inflammation activity in pCHB patients was analyzed. Additionally, the patients were stratified by the age, and the relationship between serum aminotransferase levels and liver inflammation activity was analyzed in each age group. For comparison of continuous data between two groups, the independent samples t-test was used when the data were normally distributed, while the Mann-Whitney U test was used when the data were not normally distributed; the chi-square test was employed for comparison of categorical data between two groups. A Spearman’s correlation analysis was performed for correlation assessment. ResultsAmong the 1 267 pCHB patients, there were 468 (36.9%) in the G0 — G1 group and 799 (63.1%) in the G2 — G4 group, and there were significant differences between the two groups in the levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), AST/ALT ratio, gamma-glutamyl transpeptidase (GGT), total bilirubin, direct bilirubin, HBeAg quantification, low-density lipoprotein, and platelet count (PLT) (all P<0.05). The correlation analysis showed that liver inflammation activity was negatively correlated with PLT and low-density lipoprotein (both P<0.05) and was positively correlated with GGT, total bilirubin, direct bilirubin, and HBeAg titer (all P<0.05), while it was not significantly correlated with ALT, AST, and AST/ALT ratio (all P>0.05). In the 0 — 12 years group, the 13 — 18 years male group, and the 13 — 18 years female group, liver inflammation activity aggravated with the increases in the serum levels of ALT and AST, and there were significant differences between groups (all P<0.05). In the 0 — 12 years group, there was a significant difference in significant liver inflammation activity between the AST/ALT ratio >1 group and the AST/ALT ratio ≤1 group (P<0.001). Among the 1 267 patients, 447 (35.28%) had an ALT level of <2×upper limit of normal (ULN), among whom 196 (43.85%) had G≥2 liver inflammation, accounting for 15.47% of all children enrolled. ConclusionLiver inflammation activity is not significantly correlated with ALT, AST, and AST/ALT ratio in pCHB patients, suggesting that the serum levels of aminotransferases cannot truly reflect liver inflammation activity in pCHB patients with an aminotransferase level of <2×ULN. In clinical practice, liver biopsy should be performed for children with an aminotransferase level of <2×ULN to clarify whether antiviral therapy should be performed.
8.Optimization of linear calibration using two reference substances under complicated chromatographic condition-taking Cassiae Semen as an example

Objective:To establish a linear calibration method using two reference substances for seven characteristic peaks of Cassiae Semen under complicated chromatographic condition,and to optimize the method.Methods:Using 15 different types of screened chromatographic columns and 2 components as reference compounds pair,the linear calibration method with 2 reference substances was established to predict the retention time of the other 5 components,and the method was verified by unknown chromatographic columns and unknown samples.Combined with column confirmation number and average coincidence rate of target peaks,the location results were compared comprehen-sively,and the method was optimized according to the defect under the influence of complicated chromatographic condition.Results:The average conformity rate of the target peak of the method before optimization was 73.3%,and the average conformity rate of the target peak of the optimized method was 98.7%.The optimized method has a high-er average peak coincidence rate and a wider range of applicability for the chromatographic column.Conclusion:The optimized linear calibration method using two reference substances can assist the localization analysis of chromato-graphic peaks in the characteristic chromatogram under complicated chromatographic condition.

10.Validity and reliability of the Negative Beliefs about Rumination Scale in Chinese college students

Objective:To evaluate the validity and reliability of Negative Beliefs about Rumination Scale(NBRS)in Chinese college students.Methods:A total of 1004 college students were selected.Exploratory factor a-nalysis,criterion validity and internal consistency reliability test were performed on sample 1(n=501),Ruminative Responses Scale(RRS),Negative Beliefs about Uncontrollability and Danger of Worry(NEG)and the Beck De-pression Inventory-Ⅱ(BDI-Ⅱ)were used as criteria for criterion validity test.Confirmatory factor analysis and measurement invariance across gender were performed on sample 2(n=503).Totally 199 college students in total sample were retested 6 weeks later.Results:The exploratory factor analysis showed that there were 11 NBRS items with 3 factors,which explained 63.75%of the total variance.The confirmatory factor analysis showed that the 3-factor model fitted well(x2/df=2.59,CFI=0.97,TLI=0.95,SRMR=0.04,RMSEA=0.06).The NBRS scores were positively correlated with the scores of RRS,NEG and BDI-Ⅱ(ICC=0.59,0.75,0.53;Ps<0.01).The Cronbach's α coefficient of the NBRS was 0.82,and the retest reliability(ICC)was 0.74.The configural,weak,strong and strict invariance of the NBRS across gender were all acceptable(△CFI<0.01,△TLI<0.01).Conclu-sion:The Negative Beliefs about Rumination Scale demonstrates good validity and reliability in Chinese college students,making it a useful tool for research and interventions related to rumination.
