Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
2.Application effect of cluster management in peri-discharge period of patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the application effect of cluster management in peri-discharge period of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Using random number table method,90 patients with AECOPD were divided into the observation group(45 cases)and the control group(45 cases).All patients underwent active treatment after admission.During the peri-discharge period,the control group received routine management,while the observation group received cluster management based on internet chronic obstructive pulmonary disease(COPD)management platform.Both groups received 12 weeks of intervention.Comparisons were made between the two groups in terms of the degree of dyspnea[modified British Medical Research Council dyspnoea scale(mMRC)dyspnoea scale],pulmonary function[forced expiratory volume in the first second as a percentage of predicted value(FEV1%pred)],exercise endurance[6-minute walk test(6MWT)],management effect(frequency of emergency infusion or hospitalization due to moderate to severe acute exacerbation,frequency of multidisciplinary outpatient follow-up,compliance with regular home-based rehabilitation and proportions of smokers before and after intervention),and levels of laboratory indices[hemoglobin(Hb)and albumin(Alb)].Results After intervention,mMRC score of the observation group decreased and was lower than that of the control group(P<0.05).The observation group showed an increase in FEV1%pred and 6 MWT distance,both of which were superior to those in the control group(P<0.05).The proportion of smokers and number of emergency infusions or hospitalization diagnosis and treatment due to moderate-severe acute exacerbation in the observation group were lower than those in the control group.The proportion of regular home-based rehabilitation and multidisciplinary outpatient follow-up visits were higher/more in the observation group than those in the control group(P<0.05).The levels of Hb and Alb in the observation group increased compared to those before intervention and were higher than those in the control group.Hb level in the control group decreased compared to that before intervention(P<0.05).Conclusion Implementing cluster management during the peri-discharge period of patients with AECOPD can effectively alleviate dyspnea,improve exercise endurance,reduce acute exacerbation risk and promote the development of healthy behaviors.

5.Exploration on the Mechanism of Yipi Yanggan Prescription for the Treatment of Liver Precancerous Lesion Based on M1 Type Macrophage Polarization-Chronic Inflammation-Liver Cell Malignant Transformation

Objective To explore the mechanism of Yipi Yanggan Prescription in the treatment of precancerous lesion of liver in rats based on M1 type macrophage polarization-chronic inflammation-liver cell malignant transformation.Methods Totally 90 Wistar rats were randomly divided into blank group,model group,Hugan Tablet group and Yipi Yanggan Prescription high-,medium-and low-dosage groups,with 15 rats in each group.The blank group was injected distilled water intraperitoneally,and the other groups were injected 5 mL/kg of diethylnitrosamine intraperitoneally at 50 mg/kg per week(twice per week)for 16 weeks to induce the precancerous lesion of liver model.Starting from the second day of modeling,Yipi Yanggan Prescription high-,medium-and low-dosage groups were orally administered with 1.2,0.6 and 0.3 g/mL Yipi Yanggan Prescription,respectively.The Hugan Tablet group was orally administered with 921 mg/kg Hugan Tablet solution,the blank group and model group were orally administered with an equal amount of physiological saline for 16 consecutive weeks.The appearance of the liver was observed,ELISA was used to detect serum ALT,AST,ALP,AFU,as well as TNF-α,IL-6,iNOS and MCP-1 content,HE staining and Masson staining were used to observe the morphology of liver tissue,immunohistochemistry was used to detect the expressions of liver cell malignancy markers OV6,CK19,CD133 and EpCAM,qPCR was used to detect the mRNA expressions of CK19,CD133 and EpCAM in liver tissue,immunofluorescence co-localization was used to detect the co-expressions of M1 type macrophage markers CD68 with IL-6 and TNF-α.Results Compared with the blank group,the liver of the model group rats was hard,with a rough surface and dull edges,and a large number of nodules were visible,the contents of serum ALT,AST,ALP,AFU,TNF-α,IL-6,iNOS and MCP-1 significantly increased(P<0.01),there were large areas of dysplasia nodules,inflammatory cell infiltration,and increased collagen fibers in liver tissue,the expressions of OV6,CK19,CD133 and EpCAM in liver tissue significantly increased,and the co-expressions of CD68 with IL-6 and TNF-α significantly increased(P<0.01).Compared with the model group,the number and size of liver nodules in each treatment group of rats decreased,the contents of serum ALT,AST,ALP,AFU,TNF-α,IL-6,iNOS and MCP-1 were significantly decreased(P<0.01),hepatocellular dysplasia and inflammatory cell infiltration were significantly improved,collagen fibers decreased,and the expressions of OV6,CK19,CD133 and EpCAM in liver tissue were significantly decreased,the co-expressions of CD68 with IL-6 and TNF-α significantly decreased(P<0.05,P<0.01).Conclusion Yipi Yanggan Prescription may alleviate inflammation by inhibiting polarization of M1 type macrophages,improve liver cell malignancy,and exert therapeutic effects on rats with precancerous lesion of liver induced by diethylnitrosamine.
6.Tiaoqi Jieyu acupuncture for treatment-resistant depression: a randomized controlled trial.

OBJECTIVE:
To observe the effect of Tiaoqi Jieyu (regulating qi and relieving depression) acupuncture on the clinical symptoms of treatment-resistant depression (TRD), and to explore the relationship between the acupuncture pain sensitivity and symptom's improvement.
METHODS:
A total of 78 patients with TRD were randomly divided into an observation group (39 cases, 3 cases dropped off) and a control group (39 cases, 4 cases dropped off). The patients in the control group were treated with medications according to the treatment plan of psychiatrists (at least one medication was 5-hydroxytryptamine reuptake inhibitor). On the basis of the control group, the patients in the observation group were treated with Tiaoqi Jieyu acupuncture, and Baihui (GV 20), Yintang (GV 24+), Yanglingquan (GB 34), Taichong (LR 3), Hegu (LI 4), Neiguan (PC 6), Yinlingquan (SP 9) and Zusanli (ST 36), etc. were selected. The acupuncture was given three times a week. Both groups were treated for 8 weeks. After 8-week treatment, the response rate of Hamilton depression scale-24 (HAMD-24) score after was evaluated in the two groups. The scores of HAMD-24 and Hamilton anxiety scale (HAMA) were compared between the two groups before treatment, after 4, 8-week treatment and 12 weeks after treatment (follow-up). After the first treatment and 8-week treatment, the visual analogue scale (VAS) score in the observation group was evaluated, and the correlation between VAS score after the first treatment and HAMD-24 score before treatment, between VAS score after the first treatment and the course of disease in the observation group was analyzed, and the correlation between difference of VAS after 8-week treatment and after the first treatment and difference of HAMD-24 score before treatment and after 8-week treatment was analyzed.
RESULTS:
After 8-week treatment, the response rate of HAMD-24 score in the observation group was 52.8% (19/36), higher than 17.1% (6/35) in the control group (P<0.001). Compared before treatment, the scores of HAMD-24 and HAMA in the two groups were decreased after 4-week treatment, 8-week treatment and in follow-up (P<0.05), and those in the observation group were superior to the control group (P<0.05). After 8-week treatment, the acupuncture pain VAS score in the observation group was (5.28±2.13) points, which was higher than (3.33±1.62) points after the first treatment (P<0.001). There was a negative correlation between VAS score after the first treatment and HAMD-24 score before treatment in the observation group (r =-0.486, P=0.003); there was no correlation between acupuncture pain VAS score after the first treatment and the course of disease in the observation group (P>0.05). After 8-week treatment, there was a positive correlation between the difference of VAS score and the difference of HAMD-24 score in the observation group (r =0.514, P=0.001).
CONCLUSION
Tiaoqi Jieyu acupuncture could improve the depression and anxiety in patients with TRD, and the symptom's improvement is related to the recovery of acupuncture pain sensitivity.

7.Formulation of expert consensus on clinical application of external ethnic medicine: taking Baimai Ointment as an example.

The concept of ethnic medicine is divided into a broad sense and a narrow sense. The broad concept refers to the traditional medicine of the Chinese nation, and the narrow concept refers to the traditional medicine of Chinese ethnic minorities. The external medicine is one of the main forms of ethnic medicine, and it is also the important content of ethnic medicine for external use, which is widely used in clinical practice. As the theory of ethnic medicine is unique, the application methods have certain characteristics, which are the key technical parts of clinical practice. However, the existing traditional Chinese medicine consensus formulation me-thods cannot meet the needs of the consensus formulation of the external ethnic medicine. Therefore, the methods suitable for expert consensus on external ethnic medicine are required. This article took Expert opinion on clinical application of Baimai Ointment as an exa-mple, and explorde a reasonable, effective, multi-dimensional, and multi-stage method to formulate expert consensus on the external ethnic medicine. In this research, three-dimensional sources of information, including ancient classics, clinical research evidence, and expert application experiences, were systematically and scientifically collected. After organization and analysis, the information was formed into comprehensive evidence. In a formal consensus meeting, part of the recommendations reached consensus. As to the issues that did not reach agreement, in-depth interviews were used to explore the reasons for the differences and resolve the disagreements. Finally, unanimous recommendations were reached. There are common problems during the formulation process of Expert opinion on clinical application of Baimai Ointment. This study is expected to provide references for the formulation of expert consensus on other external ethnic medicine.

8.Mechanism of Chaishao Liujunzi Tang in Treatment of Ulcerative Colitis Through TLR4/MyD88/NF-κB Signaling Pathway

ObjectiveTo investigate the preventive and curative effect of Chaishao Liujunzi Tang (CSLJZT) on colonic mucosal injury induced by dextran sulfate sodium (DSS) in mice with ulcerative colitis (UC) and its mechanism. MethodFifty Balb/c male mice were randomly divided into normal group, model group, CSLJZT low-dose group, CSLJZT high-dose group, and sulfasalazine group. Except for the normal group, other groups were given 2.5% DSS freely for 7 d, and were given drug intervention after successful modeling for 7 d. Bodyweight, feces, and other general physiological statuses of mice were recorded every day, and disease activity index (DAI) scores were calculated.The colon length was measured, and stained by hematoxylin-eosin (HE) staining to observe the morphological changes of the colon.The enzyme-linked immunosorbent assay (ELISA) was used to determine the content of interleukin-1β (IL-1β), myeloperoxidase (MPO), and superoxide dismutase (SOD) in the serum. Western blot was used to determine the protein expression levels of Toll-like receptor 4 (TLR4), myeloid differentiation factor 88 (MyD88), nuclear factor kappa B (NF-κB), inhibitor-kappa binding protein (IκB), Caspase-1, and nucleotide-binding oligomerization domain-like receptor 3 (NLRP3) in the colon tissues. ResultAs compared with the normal group, mice in the model group had significantly decreased body weight (P<0.01), severe diarrhea and hematochezia, and significantly increased DAI score (P<0.01). As compared with the model group, the decreasing trend of body weight was significantly alleviated in the CSLJZT groups (P<0.01), diarrhea and hematochezia were significantly improved, DAI score was significantly decreased (P<0.01), and colon length increased (P<0.05). HE staining showed that the pathological damage of colon tissue was significantly improved and the inflammatory cell infiltration was reduced in the CSLJZT groups as compared with the model group. As compared with the normal group, the serum levels of IL-1β and MPO were significantly higher (P<0.01) and SOD levels were significantly lower (P<0.01) of mice in the model group.Compared with the model group, the treated group reduced the serum IL-1β and MPO levels (P<0.01), and raised the SOD level (P<0.01). The results of Western blot showed that as compared with the normal group, the expression levels of TLR4, MyD88, NF-κB, Ccaspase-1, and NLRP3 proteins were significantly increased (P<0.01), whereas the expression level of IκB protein was significantly decreased (P<0.01) in the colonic tissue of mice in the model group. As compared with the model group, the expression levels of TLR4, MyD88, NF-κB, Caspase-1, and NLRP3 proteins were decreased (P<0.01), whereas the expression level of IκB protein was increased (P<0.01) in the colonic tissue of mice in the CSLJZT groups. ConclusionCSLJZT improves the inflammatory injury of the colon tissue in DSS-induced UC mice through TLR4/MyD88/NF-κB signaling pathway.
9.Rolling needle pricking-cupping therapy and traditional pricking-cupping therapy for cervical spondylosis of neck type: a randomized controlled trial.

OBJECTIVE:
To compare the clinical efficacy between rolling needle pricking-cupping (RNP-C) and traditional pricking-cupping (TP-C) for cervical spondylosis of neck type.
METHODS:

Currently, however, the treatment of chronic pain after SCI remains a largely unmet need. Electrical spinal cord stimulation (SCS) has been used to manage a variety of chronic pain conditions that are refractory to pharmacotherapy. Yet, its efficacy, benefit profiles, and mechanisms of action in SCI pain remain elusive, due to limited research, methodological weaknesses in previous clinical studies, and a lack of mechanistic exploration of SCS for SCI pain control. We aim to review recent studies and outline the therapeutic potential of different SCS paradigms for traumatic SCI pain. We begin with an overview of its manifestations, classification, potential underlying etiology, and current challenges for its treatment. The clinical evidence for using SCS in SCI pain is then reviewed. Finally, future perspectives of pre-clinical research and clinical study of SCS for SCI pain treatment are discussed.
