1.Analysis of the Spatiotemporal Connotations of "Opening,Closing,and Pivot" in the Three Yin and Three Yang Theory of the Inner Canon of Yellow Emperor (《黄帝内经》)

Based on the different medical systems and textual contexts within Inner Canon of Yellow Emperor (《黄帝内经》), this paper argues that "opening, closing, and pivot" in the three yin and three yang theory encompasses both spatial and temporal dimensions. From the spatial perspective, the concepts of opening, closing and pivot are used to describe the dynamic movement of qi within the meridian system. In The Spiritual Pivot·Root and Knot (《灵枢·根结》), the theory primarily explains the superficial-deep and exterior-interior hierarchy of the meridians, emphasizing the movement of channel qi and the transmission of disease. By contrast, Basic Questions·Treatise on the Separation and Union of Yin and Yang (《素问·阴阳离合论篇》) primarily explains the relationships of separation and union between yin and yang among different regions and orientations on the same body-surface level. From the temporal perspective, the theory also explains the waxing and waning of yin and yang according to seasonal changes throughout the year, and has been applied to clinical practice within the theory of the five circuits and six qi. Therefore, opening, closing, and pivot in the three yin and three yang theory encompasses multiple connotations, including meridian structure, spatial distribution, seasonal qi transformation, and the annual circulation of climatic qi. Clarifying these different levels and their respective scopes of application is essential for accurately understanding the rich connotations of the opening, closing and pivot theory.
2.Analysis and Assessment of a Large-Scale Traditional Chinese Medicine Examination Based on Cognitive Diagnostic

ObjectiveTo analyze the scores of the second-tier assessment on theory of competency of Traditional Chinese Medicine (TCM) physicians based on cognitive diagnostic model, and to provide a reference for cognitive diagnostic studies of large-scale medical examinations. MethodsCognitive diagnostic analyses were conducted by selecting the responses of 16,310 candidates who took the theory examination of the second-tier assessment on the competency of TCM physicians in 2023. The degree of fit between the topological deterministic input noise and gate model (G-DINA model) and the examination data was examined; item parameter analysis and reliability analysis were conducted under the cognitive diagnostic framework; the candidates' mastery of cognitive attributes was comprehensively analysed at different levels. ResultsThe mean score of 16,310 candidates in this study was (189.76 ± 40.86), and a total of 9,994 candidates passed this examination, with a pass rate of 61.28 %. The total number of questions in the examination was 300, and among the five modules, the frequency of assessment of TCM fundamentals was the highest (132 questions, 44%), followed by TCM clinic (87 questions, 29%), Western medicine fundamentals (60 questions, 20%), and TCM classics (12 questions, 4%) and medical humanities (9 questions, 3%) was less. The results of the model fit index showed that the area under the curve (AUC) of the G-DINA model was 0.98, indicating that the model fit the study data excellently. The results of item parameter analysis showed that all the discriminations of the cognitive diagnostic model for the 300 test questions were greater than 0, with an average discrimination of 0.25, and the overall performance of the question discriminations was good. The results of the reliability analysis showed that the test level classification accuracy was 0.75, and the attribute level classification accuracy was 0.83 to 0.99, indicating that the G-DINA model has high attribute classification accuracy. The results of the cognitive diagnostic analyses showed that the proportion of candidates who had passed the exam was higher for the cognitive attributes of each module compared to those who had not passed the exam. ConclusionThe analysis of large-scale TCM examinations based on cognitive diagnosis can provide support for improving the quality of TCM talent education and cultivation.

In order to explore the treatment of Her-2 positive breast cancer patients who failed in multi-line treatments, we retrospectively analyzed the clinical data of a patient with refractory Her-2 positive breast cancer.The patient was initially diagnosed as Her-2 positive advanced breast cancer.After six line treatment in the outer hospital, the patient′s condition was basically in a progressive state.The breast tumor was broken and purulent, the lung metastasis increased, and the patient′s quality of life was poor.The patient was admitted to Department of Breast Surgery of Affiliated Suqian Hospital of Xuzhou Medical University, after MDT discussion, we gave pyrrolotinib combined with capecitabine treatment, the chest wound healed gradually, the lung metastasis gradually reduced, and the quality of life was better.A retrospective analysis of this case showed that pyrrolidine combined with capecitabine may bring hope to Her-2 positive breast cancer patients who failed to receive multi-line therapies, especially those who failed to target therapy.
4.TP and FAC effect on patients with primary tumor molecular typing luminal A, axillary lymph node metastasis three negative breast cancer

Objective To explore the application of paclitaxel or docetaxel combined with cisplatin (TP) with cyclophosphamide,pirirubicin and fluorouracil (FAC) in the primary tumor molecular typing Luminal A,axillary lymph node metastasis three negative breast cancer. Methods From January 2012 to January 2014, the clinical data of forty-nine patients with were selected. All patients were divided into two groups by balance randomization method, TP group and FAC group. Twenty-five patients were treated with TP regimen and 24 patients were treated with FAC regimen. The clinical efficacy was evaluated after six cycles of chemotherapy. Chemotherapy effects,adverse reactions and survival rates of two groups were compared. Results All patients were given intravenous chemotherapy according to the plan and were evaluated for clinical efficacy. The response rate (RR) was 64. 0% in TP group,including 4 cases of complete remission (CR),12 cases of partial remission (PR),7 cases of stable disease(SD) and 2 cases of progressive disease(PD). The adverse reactions were gastrointestinal reactions and granulocytopenia. The median progression-free survival ( PFS) and overall survival ( OS) were respectively 12. 4 months and 34. 1 months. In FAC group,the response rate ( RR) was 33. 3%,including 2 cases of CR,6 cases of PR,11 cases of SD and 5 cases of PD. The adverse reactions were gastrointestinal reactions,granulocytopenia and premature atrial contraction. The median PFS and OS were 7. 2 months and 20. 7 months respectively. The effective rate of TP group was higher than that of FAC group (χ2=4. 608,P=0. 032),and the progression-free survival time and total survival time were longer than those of FAC group (χ2 =8. 317, 8. 563, P=0. 004, 0. 003 ) . Conclusion Compared to FAC regimen, TP regimen could improve the survival rate of patients better with breast cancer of Primary tumor Luminal A and Axillary Lymph Node Metastasis Triple negative type, and adverse reactions were tolerated, it may be an optimized chemotherapy.
5.Application of urodynamics in the diagnosis and treatment of childhood nonmonosym ptomatic nocturnal enure-sis

Objective To explore the application of urodynamics in the diagnosis and treatment of nonmonosymp-tomatic nocturnal enuresis(NMNE)in children,and to provide some evidence for its clinical treatment. Methods Patients who were clinically diagnosed as NMNE at Department of Pediatrics Kidney Rheumatism and Pediatric Urology of Shengjing Hospital of China Medical University from January 2015 to August 2016 were collected. Sixty - six children with NMNE were included in the urodynamic study,and the different treatments were selected according to the results of the urodynamics and the efficacy was evaluated. Results The urodynamic results of 61 patients(92. 43%)were abnor-mal,and 5 cases(7. 57%)had no abnormalities. There were 35 cases with overactive bladder(OAB)in abnormal re-sults,with a remarkable decrease in bladder pressure capacity in 7 cases,detrusorsphincter discoordination(DSD)in 1 case,and OAB with other abnormal results in 18 cases. OAB was found in 53 cases,and the detection rate was 86. 89%(53 / 61 cases),accounting for 80. 3%(53 / 66 cases)of the total samples. The desmopressin acetate combined with Tolterodine tartrate was used to treat OAB patients,and its total effective rate was 94. 34%(50 / 53 cases). The biofeed-back treatment had effect on DSD child. The patients with a marked decrease in bladder pressure capacity and with the normal urodynamics received desmopressin acetate joint behavior training treatment,and the efficiency of the therapy was 71. 43%(5 / 7 cases)and 80. 00%(4 / 5 cases),respectively. Conclusions The urodynamics study can uncover the pathophysiological mechanism of NMNE,so this examination is essential for NMNE children to improve the treat-ment. Since the rate of OAB in NMNE is very high,it is suggested that anticholinergic medications combined with desmopressin can be used in the initial treatment of the NMNE patients to improve the rate of cure and prognosis.
6.Study on the expression of hormone receptor and human epidermal growth factor receptor 2 in breast cancer primary tumor and axillary lymph node metastasis

Objective To explore the expression of HR and Her?2 in breast cancer primary tumor and axillary lymph node metastasis. Methods Four hundred and twenty?eight female patients with unilateral breast cancer combined with axillary lymph node metastasis treated in the Affiliated Suqian Hospital of Xuzhou Medical University from January 2011 to January 2016 were selected in this study. Immunohistochemistry was used to detect the expression of ER,PR,Her?2 and Ki67 in primary tumor and axillary lymph node metastasis. Results The positive rates of ER expression were 75. 9% ( 325/428 ) and 70. 3% ( 301/428 ) respectively in primary tumor and axillary lymph node metastasis. The positive rates of PR expression were 61. 4% ( 263/428) and 56. 1% ( 240/428 ) respectively in primary tumor and axillary lymph node metastasis. The rates of Her?2 overexpression were 20. 1% ( 86/428) in primary tumor and the positive rate of Her?2 in axillary lymph node metastasis was 22. 7%( 97/428 ) . The positive rates of Ki67 expression were 45. 6%( 195/428 ) and 39. 7%(170/428) respectively in primary tumor and axillary lymph node metastasis. The expression of ER,PR,Her?2 and Ki67 in primary and axillary lymph node metastasis showed no statistical significance ( P>0. 05 ) . The molecular typing of primary tumor and axillary lymph node metastasis were not consistent in 31 patients ( 31/428,7. 24%) ,including 14 cases of primary tumor Luminal A,9 cases of Her?2 overexpression in axillary lymph node metastasis and 5 cases of triple negative breast cancer. Primary tumor Luminal B was detected in 10 cases, while 6 cases of Her?2 overexpression in axillary lymph node metastasis and 4 cases of triple negative breast cancer. Primary tumor Her?2 was overexpressed in 4 cases,while 1 case of Luminal A,3 cases of Luminal B in axillary lymph node metastasis. There were 3 cases of primary tumor triple negative breast cancer,while 2 cases of Luminal B in axillary lymph node metastasis and 1 case of Her?2 overexpression. Conclusion The expressions of ER, PR, Her?2 and Ki67 in primary tumor and axillary lymph node metastasis of some breast cancer were different. Immunohistochemistry for primary tumor and axillary lymph node metastasis of stage II?III breast cancer patients should be routinely carried out. Based on molecular typing of primary tumor and axillary lymph node metastasis,individualized treatment plan can be developed,so that patients will benefit from it.
7.Endocrine therapy for breast cancer patients with ER and PR turning positive after neoadjuvant chemotherapy

Objective To explore the effect of endocrine therapy in breast cancer patients whose estrogen receptor (ER) and progesterone receptor(PR) was preoperatively negative and turned positive after neoadjuvant chemotherapy. Methods The clinical experimental study was carried out in 97 cases of breast cancer which were divided into endocrine treatment group and control group. The follow-up time ranged from 15 to 60 months. Results In endocrine treatment group, 3 and 5-year disease-free survival were respectively 74.5% (38/51), 60.7% (31/51), and 3 and 5-year overall survival were respectively 80%(41/51), 74. 5% (38/51). In control group, 3 and 5-year disease-free survival were respectively 54.2% (26/46), 41.7%(20/46), and 3 and 5-year overall survival were 60.9%(28/46),50%(23/46),respectively. The corresponding values were significantly higher in endocrine treatment group than in control group(P<0.05).Conclusions Remedy endocrine therapy improves the disease-free and overall survival rate in breast cancer patients with the expression of ER and PR turning positive after initial neoadjuvant chemotherapy.
8.The effects of neoadjuvant endocrine therapy on expressions of ER, PR and C-erbB-2 in breast cancer patients

Objective To explore the effects of preoperative neoadjuvant endocrine therapy on expression and significance of ER, PR, C-erbB-2 in pestmenopause breast cancer patients over 60 years old. Methods 36 patients were treated with endrocrine therapy by oral tamoxifen for 60 ~ 90 days, and the expression of ER, PR and C-erbB-2 was examined before and after the endrocrine therapy. Results In 36 patients the negative expres-sion rate of ER, PR was 16.78% and 11.11% respectively before the endrocrine therapy, compared to 50% and 33.33% after the endrocrine therapy. There was significant difference(P<0.05). The expression of C-erbB-2 was 13.8% and 16.67% before and after the endrocrine therapy, and there was no significant difference between the two figures. Conclusions The neoadjuvant endocrine therapy by tamoxifen alone can down-regulate the ex-pression of ER, PR, and can inhibit tumor growth. Some patients may get partially relieved.
9.Expression and clinical significance of insulin like growth factor-1 protein in nasopharyngeal carcinoma.

OBJECTIVE:
To evaluate the expression of IGF-1R protein and its clinical signification in nasopharyngeal carcinoma.
METHOD:
SP immunohistochemical staining was performed to detect the expression of IGF-1R in 63 cases of NPC and 21 inflammatory nasopharyngeal mucosa; and the expression of IGF-1R in three NPC cell lines and chronic nasopharyngitis tissues was examined by western blots.
RESULT:
The positive rates of IGF-1R were 82.5% and 14.3% respectively in 63 cases of NPC and 21 inflammatory nasopharyngeal mucosa; it also showed stronger positive expression in Stage III-IV of NPC than that in stage I-II (P < 0 01); IGF-1R positive expression in cases of NPC with cervical lymph node metastasis was significantly higher than that of NPC without lymph node metastasis (P < 0.05); it more often appears in recurrent cases than in primary cases (P < 0.05); IGF-1R positive expression had a significantly stronger in patients whose survival time less than 5 years than that in patients whose survival time over 5 years (P < 0.01).
CONCLUSION
Over-expression of IGF-1R is related to poor differentiation of NPC, also may be linked with oncogenesis and development of NPC. The over-expression of IGF-1R may be one of a prognostic factors in NPC. The IGF-1R might be a potential therapeutic target in nasopharyngeal cancer.
