1.Effect of oral cleaning with traditional Chinese medicine preparations on hospital-acquired pneumonia in patients undergoing non-invasive mechanical ventilation

OBJECTIVE To explore the effect of oral cleaning with traditional Chinese medicine preparations on de-colonization of oropharyngeal bacteria and hospital-acquired pneumonia(HAP)in elderly patients undergoing non-inva-sive mechanical ventilation.METHODS A total of 520 elderly patients who were treated with non-invasive mechani-cal ventilation in Enze Hospital of Enze Medical Center(Group)from Jan.2022 to Dec.2024 were recruited as the research subjects and were randomly divided into the normal saline group and the traditional Chinese medicine(TCM)preparation group,with 260 cases in each group.The two groups were respectively treated with normal saline and TCM preparations for oral cleaning,twice a day in the morning and evening.The dental plaque index score,isolation rate of oropharyngeal pathogens,incidence of HAP and isolation rates of pathogens causing HAP were observed and compared between the two groups.RESULTS There were no significant differences in the dental plaque index score and the isolation rate of oropharyngeal pathogens between the two groups of patients before the first cleaning.However,the dental plaque index score and the isolation rate of oropharyngeal pathogens were low-er in the TCM preparation group than in the normal saline group after the first cleaning and the continuous clean-ing for 5 days(P<0.05).The incidence of HAP of the TCM preparation group(3.63%)was lower than that of the normal saline group(8.91%)(x2=5.872,P=0.015).The isolation rate of gram-negative bacteria from lower respiratory tract secretions of the patients with HAP was lower in the TCM preparation group than in the normal saline group(P<0.05);there were no significant differences in the isolation rates of gram-positive bacteria and fungi between the two groups.CONCLUSION As compared with the normal saline for routine oral cleaning of the elderly patients undergoing non-invasive mechanical ventilation,the TCM preparations may facilitate the decoloni-zation of oropharyngeal bacteria and remarkably reduce the incidence of HAP,which may provide a new idea for decolonization of oropharyngeal bacteria and reduction of incidence of HAP.

3.Variations of serum bone resorption biomarkers of children of the coal-burning fluorosis area

Objective:
To investigate the concentration of serum bone resorption markers NTX and TRACP-5b of children aged 8-14 years in a coal-burning fluorosis area and its relationship with age, and to provide population data for the study of pathogenesis of skeletal fluorosis.
Methods:
Totally 123 children of 8-14 schoolage in the two primary schools in Doujing Township, Shuicheng County, Liupanshui City, Guizhou Province were randomly selected as the exposed group. According to the matching principle, 64 children were randomly selected as a control from a primary school in a nondisease area Huaga Town. The dental fluorosis was investigated, and the concentrations of serum NTX and TRACP-5b were measured.
Results:
The detection rate of dental fluorosis in the fluorosis area was 94.3% and 0 in the control area. The concentrations of serum NTX in fluorosis area children were 13.04 (10.76, 15.64), 14.82 (12.15, 18.26)nmol/L in the early adolescence and middle-aged period, which lower than the control area 15.73(14.36, 18.61), 16.45(15.45, 22.02)nmol/L( P <0.05); The serum TRACP-5b levels in children with fluorosis were 276.74(237.63, 312.75), 270.14(242.82, 321.97), 305.95(259.78, 339.87)nmol/L in prepubertal, early adolescence and middle youth, lower than the control area 370.88 (304.47, 452.84), 353.30 (262.05, 393.19), 420.22 (376.96, 544.60)nmol/L( Z =-3.03, -2.66, -3.10, P <0.05). Serum NTx and TRACP-5b in fluorosis area were negatively correlated with dental fluorosis in children( r =-0.51, -0.37, P <0.01).
Conclusion
Fluorosis can reduce the concentrations of serum bone resorption markers NTX and TRACP-5b in children of different age groups. TRACP-5b may be more sensitive to fluoride exposure than NTX, but the specific mechanism remains to be further studied.
4.Evaluation of efficacy of Arndt endobronchial blocker for airway management during one-lung ventilation in pediatric patients

Objective To evaluate the efficacy of Arndt endobronchial blocker (AEB) for airway management during one-lung ventilation (OLV) in pediatric patients.Methods Thirty pediatric patients,aged 4 months-12 yr,weighing 5.6-26.0 kg,of American Society of Anesthesiologists physical status Ⅱ-Ⅳ,scheduled for elective thoracotomy under OLV requiring the use of single lumen tracheal tube assisted by AEB,were enrolled in this study.OLV was performed with the AEB placed outside the single lumen tracheal tube in pediatric patients below 2 years old and with AEB inserted through the single lumen tracheal tube in pediatric patients ≥ 2 years old.The rate of successful establishment of artificial airway at first attempt,time of establishing artificial airway,time of pulmonary collapse,AEB malpositions and airway pressure before and after OLV were recorded,and the efficacy of lung collapse was evaluated.Results The rate of successful tracheal intubation of extraluminal and endoluminal AEB placement at first attempt was 100% in 30 pediatric patients,and no high airway pressure was found after OLV.The efficacy of lung collapse was poor in one patient and good in one patient,and intraoperative AEB malpositions were found in two patients among the pediatric patients used extraluminal AEB placement.The efficacy of lung collapse was good in one patient,intraoperative AEB malposition was found in one patient,and SpO2 was decreased after OLV in one patient among the pediatric patients used endoluminal AEB placement.Conclusion Reasonably selecting the method of placing AEB followed by enhancing intraoperative airway management can be safely and effectively applied for OLV in pediatric patients.
5.Establishment and application of digital database of breast ultrasound imaging

Objective To create a large samples digital database of breast ultrasound imaging (DDBUI),which is open and can be used for scientific research,clinical training and diagnose in order to improve early diagnostic rate of breast cancer by using the database.Methods The database applies structured query language (SQL) to record the integrated resources of patients with breast masses and utilizes Brower/Server (B/S) technology to achieve universal data access.Results The database consists of 853 cases and 5780 images.So far,researchers in home and abroad have done many researches with the images in DDBUI.For example,the speckle reduction,enhancement,segmentation and classification algorithms for computer aided diagnosis (CAD).The database provides a complete shared operation platform for clinical diagnosis and teaching.Conclusions The first public database of breast ultrasound images is created which provides comprehensive information for the development and verification different CAD algorithms,and is helpful for researches,ultrasound diagnosis learning,and improving the breast cancer diagnosis.
6.Characterization of human anti-BAFF scFv-Fc that inhibits the activity of BAFF in vivo.

To investigate the effects of human anti-BAFF scFv-Fc against the hsBAFF, ICR mice were randomly divided into six groups: control, hsBAFF (1 mg x kg(-1)), hsBAFF (1 mg x kg(-1)) + Ab (1 mg x kg(-1)), hsBAFF (1 mg x kg(-1)) + Ab (2 mg x kg(-1)), hsBAFF (1 mg x kg(-1)) + human IgG (1 mg x kg(-1)) and hsBAFF (1 mg x kg(-1)) + human IgG (2 mg x kg(-1)) groups. The effects of scFv-Fc administration on the proliferation of B lymphocytes were evaluated using an MTT assay. The titres of antibody in the serum and B lymphocytes differentiation were assessed by ELISA and flow cytometry, respectively. The results showed that administration of scFv-Fc to mice injected with hsBAFF significantly prevented human BAFF-induced increases in splenic B cell numbers and serum immunoglobulin levels. Furthermore, this fully human antibody would avoid inducing the human anti-mouse antibody (HAMA) response when used in humans. These findings suggest that the compact antibody may be useful in therapeutic or diagnostic application of the BAFF-associated autoimmune diseases in human.
7.Expression and purification of Tp0751 recombinant protein of Treponema pallidum and its immuno-competence analysis

Objective To express Tp0751 laminin-binding adhesion of Treponema pallidum (T. pallidum) ,and assess the immunocompetence. Methods The Tp0751 ORF without upstream non-cod-ing region was ligated into the expression vector pET-28a( + ), and expressed in E. coli R2566. Its immuno-gen was analyzed by Western blot and ELISA. Results A fusion protein with molecular weight about 26×103 was attained after expression and purification. Western blot proved that the recombinant protein can specifically react with T. palliclum IgG positive sera. Specific humoral response were elicited after introducing recombinant protein in Zealand rabbit and the specific antibody titer was above 1:10 2400 detected by indi-rect ELISA. Conclusion The expressed recombinant protein showed excellent immunoeompetence, and the results lay the foundation for the research on its function to T. paUidum infection.
8.Study on Preparation and Identification of Immobilized Metal Ion Affinity Adsorption Gel

Sepharose 6B was activated by epichlorohydrin in the strong base condition, and then reacted with solution of iminodiacetic sodium. The arms of IDA were conjuncted to the activated Sepharose 6B. Then the products were reacted with the solution of NiSO4. The arms of IDA were chelated with Ni2+,and the chelating resin―Ni2+-IDA could be prepared. The physicochemical indexes and performance in purifying protein of the expressing product were assayed with atomic absorption method and purifying aimed protein-human B lymphocyte stimulator(hBLyS) from the expressing products in E.coli. The results indicated that the performance of made gel is very good, and its price is less than 1/10 of that of commodity gel.
9.Construction and Expression of Multi-epitopes Gene of Treponema pallidum and Immunoresponse of Recombinant Protein

OBJECTIVE To develop the strong and the specific multi-epitope antigen for the exploiting diagnosis of Treponema pallidum.METHODS The immuno-dominant epitopes of Tp0453 and Tp17 were amplified by PCR respectively,and subcloned into the expression vector pQE32 to generate multi-epitopes recombinant plasmid pQE32/Tp0453-17.The recombinant protein was expressed in Escherichia coli M15.The immunoresponse of recombinant fusion protein was analyzed by Western blot.RESULTS The multi-epitopes recombinant plasmid was successfully constructed,enzyme digestion analysis and sequencing showed that the inserted target genes were Tp0453 and Tp17 gene,compared with the gene reported by GenBank,it had 100% similarity;SDS-PAGE analysis showed the recombinant plasmid could be expressed in M15,its relatively molecular mass(Mr) of expressed product was about 52.0?103.The Western blot result showed the recombinant protein could be recognized by anti-T.pallidum positive serum.CONCLUSIONS The expressed multi-epitopes recombinant antigen showed excellent immunoresponse.The results lay the foundation for research on development of quick diagnostic kit applying to detection of T.pallidum infection.

The antibacterial peptide CM4 having potent antifungal activity on inhibitiong the cell wall regeneration of Saccharomyces cerevisiae protoplasts.When the peptide increased,the ratio of the regenerated colonies drop obviously.To study the antifungal mechanism of the antibacterial peptide,fluorescence\|labeled peptide mixted with the protoplast of yeast,then confocal laser scanning microscopy were performed.The results indicated that the peptides interactted with the protoplast membrane and damaged the structure of the membrane,then the permeation of protoplast changed.Finally the protoplasts with the peptide failed to regenerate the cell walls leading to killing the cell.
