Yin-yang, visceral manifestation, and ti-yong theories are the core theories of traditional Chinese medicine (TCM) and play a crucial role in its formation and development. With the continuous evolution of Chinese philosophy, TCM philosophy and theories have undergone significant influence, resulting in differing interpretations of the visceral manifestation of liver and mingmen across various historical periods by different medical practitioners. This paper focuses on the different understandings of the position of liver and mingmen, combining relevant philosophical images and TCM anatomical illustrations to deeply explore ancient medical theorists′ concepts regarding the four spatial directions of left, right, up, and down. The research reveals that ancient medical theorists′ conceptualization of spatial theory transitioned from an initial circular motion to a vertical movement, with the focus shifting from the upper part to the lower part. The primary reasons for these differences and changes could be explained by the varying emphasis on the ti-yong theory and differing focal points within the yin-yang theory. This study systematically examines the evolution of visceral manifestation theories related to liver and mingmen, highlights the shifts in physicians′ perspectives on spatial theory, and analyzes the influence of the ti-yong theory and yin-yang theory on these changes. The findings aim to provide a theoretical guide for advancing research and clinical application of spatial theory in TCM, thereby fostering the integration of TCM philosophy with medical theory.

Pterygium, a common ocular surface disorder characterized by the abnormal growth of conjunctival tissue onto the cornea, often necessitates surgical excision as its primary treatment. While effective, pterygium surgery is frequently associated with a spectrum of postoperative complications that significantly impact patient prognosis and quality of life. This comprehensive review systematically analyzes the classification, underlying pathophysiological mechanisms, and associated risk factors of these complications, with a particular focus on less commonly explored entities such as postoperative granuloma(PPG), corneal dellen, and scleral necrosis, alongside the more prevalent issue of recurrence. We delineate these complications based on their temporal presentation(early, intermediate, and late), and provide an in-depth analysis of general and specific contributing factors, including surgical trauma, individual patient characteristics, surgical technique, and perioperative management. Furthermore, this review synthesizes advancements in preventive strategies and therapeutic interventions, encompassing refined surgical techniques [e.g., femtosecond laser-assisted pterygium surgery(FLAPS), pterygium extended removal followed by extended conjunctival transplant(P.E.R.F.E.C.T.)technique, Tissue Tuck technique], judicious application of adjuvant therapies [e.g., mitomycin C(MMC), 5-fluorouracil(5-FU), corticosteroids, anti-vascular endothelial growth factor(VEGF)agents], and optimized postoperative care protocols. By consolidating current evidence and identifying future research priorities, this review aims to provide ophthalmologists with a valuable theoretical foundation to guide individualized surgical planning, dynamic postoperative management, and ultimately minimize complications and improve patient satisfaction.

Cholangiocarcinoma is a malignant tumor with biliary epithelial features.The early diagnosis of cholangiocarcinoma is currently difficult and the treatment outcomes are poor.Its microenvironment includes abundant fibrotic mesenchyme and a variety of cell types,which promote the development and metastasis of cholangiocarcinoma by interacting with tumor cells through mechanisms such as facilitating migration,suppressing the immune response,and inducing angiogenesis and lymphangiogenesis.Immunotherapy is an important tumor treatment approach,and immunotherapy for cholangiocarcinoma has made some progress.This article reviews the characteristics of the immune microenvironment of cholangiocarcinoma,its relationship with immunotherapy,and cutting-edge therapeutic strategies.

With the research progress on the biology and pathogenesis of cancer, immune checkpoint inhibitors (ICIs) have come into being, bringing a new hope for the survival of patients with advanced cancer and opening a new era of cancer immunotherapy. However, with the wide application of immunotherapy in clinical practice, ICI-related adverse events (irAEs) have gradually emerged and are widely known by first-line clinicians. ICIs primarily activate T cells that can attack normal tissues and organs in the body and cause a variety of adverse reactions. Checkpoint inhibitor pneumonitis (CIP) is one of the rare complications with poor prognosis in irAEs. This article reviews the therapeutic mechanism of some ICIs; the incidence, risk factors, pathogenesis, and clinical and imaging manifestations of CIP; and the classification and treatment management of CIP.

Objective:To observe the peripapillary atrophy (PPA) and peripapillary choroidal vascularity index (CVI) in patients with different degrees of myopia and to analyze their correlations.Methods:A cross-sectional clinical study. From September 2021 to December 2021, 281 mypoic patients of 281 eyes treated in Eye Hospital of Wenzhou Medical University at Hangzhou were included in this study, and the right eye was used as the treated eye. There were 135 eyes in 135 males and 146 eyes in 146 females. The age was 28.18±5.78 years. The spherical equivalent refraction (SE) was -5.13±2.33 D. Objective:To study the feasibility of non-fasting blood lipid detection in patients with type 2 diabetes mellitus (T2DM) .Methods:A total of 426 diabetic patients hospitalized in the Second Hospital of Tianjin Medical University from Oct. 2018 to Apr. 2019 were selected to collect blood samples from the fasting and non-fasting state of the patients respectively. The levels of plasma three acylglycerol (TG) , total cholesterol (TC) , high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were measured at the same time, and self-control method was used. The results of two tests were compared with the wilcoxon signed-rank test.Results:Compared with the fasting results, non-fasting TG increased by 0.25 mmol/L (11.2%) , with statistical significance ( P<0.05) ; TC increased by 0.17 mmol/L (4.5%) , HDL-C increased by 0.08 mmol/L (5.7%) , and LDL-C decreased by 0.05 mmol/L (5.9%) , without significant differenc ( P>0.05) . Conclusion:Except for TG, non-fasting TC, HDL-C, LDL-C has little change compared with those in fasting state, which can replace fasting blood lipid detection as a method for detecting blood lipid profile in patients with T2DM.

Department of General practice of West China Hospital of Sichuan University has been commissioned to train general practitioner for Tibet Autonomous Region since 2015. Based on the "demand-oriented and clinical ability training", the established training framework includes four aspects: a rotation plan of general practice theory-clinical practice-community application, the closed-loop mechanism of teaching management system improvement,innovation of training assessment and teaching activities. This article summarizes the practical experience of the commissioned general practice residency training for Tibet to provide reference for the program design.

Objective To compare the difference between fasting blood lipids and non－fasting blood lipids in children in Tianjin,and to explore the feasibility of non－fasting blood lipids detection in children.Methods A total of 223 child patients were enrolled in Department of Pediatrics,the Second Hospital of Tianjin Medical University from January 2017 to February 2018,fasting and 1－6 h post－dining blood samples were collected from each patient.Total cholesterol (TC),triglyceride (TG),high－density lipoprotein cholesterol (HDL－C)and low－density lipoprotein cholesterol (LDL－C)were measured.Self－control methods were used.Wilcoxon signed rank test was used for statisti-cal analysis.Results Compared with fasting lipids,non－fasting TC increased with 0.02 mmol/L,TG increased with 0.01 mmol/L,HDL－C increased with 0.03 mmol/L,while LDL－C decreased with 0.01 mmol/L,but non－high－density lipoprotein cholesterol did not change.Among them,only the difference in HDL －C between the fasting and non－fasting was statistically significant (Z＝ －2.870,P＜0.05).The difference in the change rate of all indicators was ＜3%.According to the dyslipidemia group comparison,the differences in hypercholesterolemia (0.72 mmol/L, Z＝ －2.551,P＝0.011),hypertriglyceridemia (0.73 mmol/L,Z＝ －3. 846,P＜0.001),low high－density lipopro-tein cholesterolemia (－0.04 mmol/L,Z ＝ －8.625,P ＜0.001)and normal level HDL －C (0.1 mmol/L,Z ＝－5.040,P＜0.001)between non －fasting lipids and fasting blood lipids were statistically significant.Conclusions The change of children's blood lipid profile in fasting and non－fasting conditions is little.Lipid testing in children does not require fasting.Blood tests can be performed at any appropriate time point in the normal diet.

Objective To evaluate the repeated dose toxicity of MNT-016 in SD rats and to provide reference for toxicity evaluation.Methods MNT-016 was administered to rats at 5, 20 and 80 mg/kg for 90 days.The toxic effects on the animals were evaluated by observing the clinical signs and measuring the body weight, hematology and blood biochemistry as well as histopathological examination.NOAEL and benchmark dose lower confidence limit(BMDL) were observed by the end point of toxicity.Results Compared with the control group, the AST, TBIL, DBIL and Crea of male rats were increased in a dose-dependent manner, while TG and CHOL decreased.The body mass(before anatomy), heart, liver, thymus, epididymis of male rats in 80 mg/kg group were significantly decreased (P<0.05), while absolute organ mass of the heart and lung was increased.The body mass (before anatomy) and thymus of female rats in 80 mg/kg group were significantly decreased (P<0.05), while absolute organ mass of lungs was increased.Vacuolation of hepatocytes was observed in groups each dose, tubule atrophy was found in the kidneys of 20 and 5 mg/kg groups, and tubule basophilia was observed in 80 mg/kg group.The incidence of the above lesions was higher in male animals than in female ones.Conclusion The NOAEL of MNT-016 is lower than 5 mg/kg in male rats and 5 mg/kg in female rats.BMDL value is 2.65 mg/kg in male animals and more accurate than NOAEL, and is 9.04 mg/kg in female animals,which is slightly larger than the corresponding NOAEL.

To study chemical constituents of Eupatorium lindleyanum. Ethyl acetate extractive fractions were separated with silica gel and Sephadex LH-20 by column chromatography, and their structures were identified on the basis of spectroscopic analysis and chemical evidence. Sixteen compounds were separated and identified as scopoletin (1), 6, 7-dimethylesculetin (2), nepetin (3), eupatrin (4), luteolin (5), isoquerecitrin (6), jaceosidin (7), quceritin (8), kaempferol (9), rutin (10), cirsiliol (11), taraxasterylacetate (12), pseudotaraxasteryl acetate (13), pseudotaraxasterol (14), butanoic acid (15) and n-hexadecanoic acid (16). Of them, compounds 1-6 and 11, 13 and 15 were separated from this plant for the first time.
